Accuracy of Iodine and Calcium Concentrations in Dual Energy Computed Tomography (DECT)

Objective: The purpose of this study was to assess accuracies of iodine and calcium concentrations in dual energy computed tomography (DECT). Method: This study was performed using an in-house phantom made from polyester resin. The in-house phantom had a diameter of 16 cm and had 10 holes filled with iodine (with concentrations of 5, 7.5, 10, and 15 mg/ml), calcium (with concentrations 200, 300, 500, and 600 mg/ml), water, and air. The in-house phantom was scanned by an Ultrafast kV Switching DECT (GE Revolution) with a tube voltage of 80/140 kV, rotation time of 0.5 s, and tube current variations of 200, 250, 300, 335, and 370 mA. Images was reconstructed to a material density image (MDI). The iodine and calcium concentrations were measured using GSI Viewer software and compared with set iodine and calcium concentrations. Results: It was found that absolute percentage error (APE) of concentrations on iodine was <12% and on calcium was <25% for all concentration variations. Hence, the APE of iodine is smaller than that of calcium. It was found that increasing the tube current does not necessarily improve the accuracy of iodine and calcium concentration measurements. Conclusion: Measurements of iodine and calcium concentrations have been carried out on DECT using an in-house phantom. Overall, iodine and calcium quantification in DECT using the in-house phantom is accurate.

Impact of patient habitus and acquisition protocol on iodine quantification in dual-source photon-counting computed tomography.

Evaluation of iodine quantification accuracy with varying iterative reconstruction level, patient habitus, and acquisition mode on a first-generation dual-source photon-counting computed tomography (PCCT) system. A multi-energy CT phantom with and without its extension ring equipped with various iodine inserts (0.2 to 15.0mg/ml) was scanned over a range of radiation dose levels ( 0.5 to 15.0mGy) using two tube voltages (120, 140kVp) and two different source modes (single-, dual-source). To assess the agreement between nominal and measured iodine concentrations, iodine density maps at different iterative reconstruction levels were utilized to calculate root mean square error (RMSE) and generate Bland-Altman plots by grouping radiation dose levels (ultra-low: ; low: 1.5 to 5; medium: 5 to 15mGy) and iodine concentrations (low: ; high: 5 to 15mg/mL). Overall, quantification of iodine concentrations was accurate and reliable even at ultra-low radiation dose levels. RMSE ranged from 0.25 to 0.37, 0.20 to 0.38, and 0.25 to 0.37mg/ml for ultra-low, low, and medium radiation dose levels, respectively. Similarly, RMSE was stable at 0.31, 0.28, 0.33, and 0.30mg/ml for tube voltage and source mode combinations. Ultimately, the accuracy of iodine quantification was higher for the phantom without an extension ring (RMSE 0.21mg/mL) and did not vary across different levels of iterative reconstruction. The first-generation PCCT allows for accurate iodine quantification over a wide range of iodine concentrations and radiation dose levels. Stable accuracy across iterative reconstruction levels may allow further radiation exposure reductions without affecting quantitative results.

Iodine concentration of fortified and unfortified plant-based alternatives to milk, dairy products, eggs and fish

Animal foods, especially dairy products, eggs and fish, are the main source of iodine in the UK. However, the use of plant-based alternative products (PBAP) is increasing owing to issues of environmental sustainability. We previously measured the iodine content of milk-alternatives(1) but data are lacking on the iodine content of other plant-based products and there is now a greater number of iodine-fortified products. We aimed to compare: (i) the iodine concentration of fortified and unfortified PBAP and (ii) the iodine concentration of PBAP with their animal-product equivalents, including those not previously measured such as egg and fish alternatives.The iodine concentration of 50 PBAP was analysed in March 2022 at LGC using ICP-MS. The products were selected from a market survey of six UK supermarkets in December 2021. Samples of matrix-matched (e.g. soya/oat) fortified and unfortified alternatives to milk (n = 13 and n = 11), yoghurt (n = 2 and n = 7) and cream (n = 1 and n = 5) were selected for analysis, as well as egg- (n = 1) and fish-alternatives (n = 10). We compared the iodine concentration between PBAPs and data on their animal-product equivalents(2).The iodine concentration of fortified PBAPs was significantly higher than that of unfortified products; the median iodine concentration of fortified vs. unfortified milk alternatives was 321 vs. 0.84 µg/kg (p<0.001) and of fortified and unfortified yoghurt alternatives was 212 µg/kg vs 3.03 µg/kg (p = 0.04). The fortified cream alternative had a higher iodine concentration than the unfortified alternatives (259 vs. 26.5 µg/kg). The measured iodine concentration of the fortified products differed from that of the product label (both lower and higher); overall, the measured iodine concentration was significantly higher than that stated on the label (mean difference 49.1 µg/kg; p = 0.018).Compared to the animal-product equivalents, the iodine concentration of unfortified PBAPs was significantly lower for milk (p<0.001) and yoghurt (p<0.001), while there was no difference with fortified versions of milk (p = 0.28) and yoghurt (p = 0.09). The egg alternative had an iodine concentration that was just 0.6% of that of chicken eggs (3.38 vs. 560 µg/kg). Three (30%) of the fish alternatives had kelp/seaweed as ingredients and the median iodine concentration of these products was (non-significantly) higher than those without (126 vs 75 μg/kg; p = 0.83). However, the iodine content of all fish-alternative products was ten-times lower than that of fish (median 99 vs. 995 µg/kg; p<0.001).The majority of PBAP are not fortified with iodine but those that are fortified have a significantly higher iodine concentration than unfortified products and are closer to the value of their animal equivalents. From an iodine perspective, unfortified plant-based alternatives are not suitable replacements and consumers should ensure adequate iodine from other dietary sources. Manufacturers should consider iodine fortification of a greater number of plant-based alternatives.

In-line spectroscopy for iodine quantification in dynamic contrast-enhanced dedicated breast CT.

In 4D dynamic contrast-enhanced dedicated breast computed tomography (4D DCE-bCT), the functional properties of the breast will be characterized by monitoring the uptake and washout of iodine-based contrast agents over time. This information could be valuable in breast cancer treatment. However, prior to clinical implementation, it is crucial to validate the quantitative estimates of iodine concentrations at each time point during acquisition. To develop an in-line spectroscopy system capable of measuring iodine concentrations in a dynamic x-ray breast phantom in real-time. Potassium iodide served as the contrast agent. The system was set-up at both the entrance and exit of the phantom. It comprises a fiber-coupled green LED and collimator, which together ensure that a parallel beam passes through the sample holder. Transmitted light is captured by a collimator on the opposite side and directed through a fiber optic cable to a photodetector for intensity measurement. The relationship between 13 iodine concentrations (0-6mg I/mL) and light transmission was tested, and the system's repeatability and accuracy were determined. The system exhibited a strong correlation between iodine concentration and transmission values, achieving a root-mean-square error of 0.007. The repeated measurements had relative standard deviations of 0.04% and 0.1% for repeated water measurements at the phantom's entrance and exit, respectively. Furthermore, the accuracy measurements gave a mean error of fitting of 0.008(±0.07) mg I/mL. The in-line spectroscopy system can effectively monitor iodine concentrations in a dynamic breast phantom, providing a reliable method for quantitative validation of 4D DCE-bCT.

Dual-energy CT late arterial phase iodine maps for the diagnosis of acute non-occlusive mesenteric ischemia

PurposeTo evaluate the diagnostic accuracy of dual-energy CT (DECT) iodine maps in comparison to conventional CT series for the assessment of non-occlusive mesenteric ischemia (NOMI).Material and MethodsWe evaluated data from 142 patients (72 men; 50.7%) who underwent DECT between 2018 and 2022, with surgically confirmed diagnosis of NOMI. One board-certified radiologist performed region of interest (ROI) measurements in bowel segments on late arterial (LA) and portal venous (PV) phase DECT iodine maps as well as LA conventional series, in both ischemic and non-ischemic bowel loops, using surgical reports as reference standard, and in a control group of 97 patients. Intra- and inter-reader agreement with a second board-certified radiologist was also evaluated. Receiver operating characteristic (ROC) curve analysis was performed to calculate the optimal threshold for discriminating ischemic from non-ischemic bowel segments. Subjective image rating of LA and PV iodine maps was performed.ResultsDECT-based iodine concentration (IC) measurements showed significant differences in LA phase iodine maps between ischemic (median:0.72; IQR 0.52–0.91 mg/mL) and non-ischemic bowel loops (5.16; IQR 3.45–6.31 mg/ml) (P <.0001). IC quantification on LA phase revealed an AUC of 0.966 for the assessment of acute bowel ischemia, significantly higher compared to both IC quantification based on PV phase (0.951) and attenuation values evaluated on LA conventional CT series (0.828). Excellent intra-observer and strong inter-observer agreements were observed for the quantification of iodine concentration. Conversely, weak inter-observer agreement was noted for conventional HU assessments. The optimal LA phase-based IC threshold for assessing bowel ischemia was 1.34 mg/mL, yielding a sensitivity of 100% and specificity of 96.48%.ConclusionIodine maps based on LA phase significantly improve the diagnostic accuracy for the assessment of NOMI compared to conventional CT series and PV phase iodine maps.

Simultaneous high-pitch multi-energy CT pulmonary angiography using a dual-source photon-counting-detector CT: A phantom experiment.

A dual-source CT system can be operated in a high-pitch helical mode to provide a temporal resolution of 66ms, which reduces motion artifacts in CT pulmonary angiography (CTPA). It can also be operated in a multi-energy (ME) mode to provide iodine maps, beneficial in the evaluation of pulmonary embolism (PE). No energy-integrating detector (EID) CT can perform simultaneous ME and high-pitch acquisition. This phantom study aimed to evaluate the ability of a photon-counting-detector (PCD) CT to perform simultaneous high-pitch and ME imaging for CTPA. A motion phantom was used to mimic the respiratory motion. Two tubes filled with iodine with intravascular thrombus mimicked by injecting glue within the tubes were placed along with 5, 10, and 15mg/mL iodine samples, on a motion phantom at 20 and 30 revolutions per minute. Separate high-pitch and ME EID-CT scans and a single high-pitch ME PCD scan were acquired and virtual monoenergetic images and iodine maps reconstructed. Percent thrombus occlusion was measured and compared between static and moving images. When there was motion, EID-CT ME suffered from significant motion artifacts. The measured iodine concentrations with PCD-CT in high-pitch ME were more stable when there was a motion, with a lower standard deviation than EID-CT in ME mode. The estimated percent thrombus occlusion dropped significantly with applied motion on EID-CT, while PCD-CT high-pitch ME mode showed good agreement between measurements on static or moving images. PCD-CT with combined ME and high-pitch mode facilitates simultaneous accurate iodine quantification and assessment of intravascular occlusion.

Iodine concentration, HU accuracy, and metal artifacts evaluation on second-generation dual-layer spectral detector CT images with metal implants: a phantom study.

Metal implants may affect the image quality, iodine concentration (IC), and CT Hounsfield unit (HU) quantification accuracy. To investigate the quantitative accuracy of IC and HU from dual-layer spectral detector (DLCT) in the presence of metal artifacts. An experimental cylindrical phantom containing eight iodine inserts and two metal inserts was designed. The phantom underwent scanning at three radiation dose levels and two tube voltage settings. A set of conventional images (CIs), virtual monoenergetic images (VMIs), and iodine concentration maps (ICMs) were generated and measured for all the eight iodine inserts. Quantitative indicators of mean absolute percentage error (MAPE), artifact index (AI), contrast-to-noise ratio (CNR), signal-to-noise ratio (SNR), and standard deviation (SD) on CIs and VMIs were calculated for IC and HU. Subjective score evaluation was also conducted. The MAPEiodine values of all regions of interest across different scanning configurations were all <5%. Almost all APEiodine values were <5%, indicating that metal artifacts had little impact on IC measurements. When the tube voltage was fixed, the SD value of attenuation decreased with the increase of the tube current; this is also true when the tube current was fixed. The middle energy reconstructions seemed to give a good balance between reducing artifacts and improving contrast. VMIs from DLCT can reduce metal artifacts, the accuracy of IC quantification is not sensitive to imaging parameters. In summary, metal implants exhibit minimal impact on image quality and IC quantification accuracy in reconstructed images from DLCT.

Risks of Iodine Excess.

Iodine is a micronutrient that is required for thyroid hormone synthesis. The iodide cycle in thyroid hormone synthesis consists of a series of transport, oxidation, organification, and binding/coupling steps in thyroid follicular cells. Common sources of iodine include the consumption of an iodine-rich diet or iodine-fortified foods, the administration of amiodarone, iodine-containing supplements, or iodinated contrast media, and other miscellaneous sources. Methods to assess population iodine status include the measurement of urinary iodine concentrations, blood thyroglobulin levels, prevalence of elevated neonatal thyrotropin levels, and thyroid volume. Although excessive iodine intake or exposure is generally well tolerated, an acute iodine load may result in thyroid dysfunction (hypothyroidism or hyperthyroidism) in certain susceptible individuals due to the failure to escape from the Wolff-Chaikoff effect and to the Jod-Basedow phenomenon, respectively. In this review, we discuss the associations between excessive iodine intake or exposure, with particular focus on iodinated contrast media as a common source of excess iodine in health care settings, and risks of incident thyroid dysfunction. We also summarize the risks of iodine excess in vulnerable populations and review current guidelines regarding the screening and monitoring of iodinated contrast-induced thyroid dysfunction. Finally, we discuss the long-term potential nonthyroidal health risks associated with iodine excess and suggest the need for more data to define safe upper limits for iodine intake, particularly in high-risk populations.

Assessment of iodine nutritional status and gestational thyroid function reference ranges during the first trimester of pregnancy in Taiwan.

Iodine nutrition is critical for fetal neurodevelopment in the first trimester of pregnancy, a period associated with dramatic changes in thyroid function. The aim of this study was to evaluate iodine nutritional status and thyroid function reference ranges in the first trimester in Taiwan. Pregnant women aged 20 years and above in the first trimester were recruited in Taipei Veterans General Hospital, Taiwan from March 2019 to July 2022. Each participant provided a spot urine sample for measurement of urinary iodine concentration (UIC) and a blood sample for checkup of thyroid function and thyroid autoantibodies. A simple food frequency questionnaire was also completed. A total of 209 women with a mean age of 32.9 ± 4.4 years were enrolled. The median UIC was 160.9 μg/L (interquartile range [IQR]: 105.0-246.2 μg/L), indicating overall iodine sufficiency. The gestational thyroid function reference ranges were: thyroid stimulating hormone (TSH) (median: 0.93 [0.007-2.9] µIU/mL), free T4 (1.3 [0.93-2.2] ng/dL), free T3 (3.0 [2.3-5.0] ng/dL), total T4 (9.9 [6.4-16.9] ng/dL), and total T3 (135 [88-231] ng/dL). If the nonpregnant reference range of serum TSH was used, eight women (4.8%) would be misclassified as having subclinical hyperthyroidism, and two women (1.2%) with subclinical hypothyroidism would be missed. In multivariate analysis, nulliparous (adjusted odds ratio [OR] from model 1-3: 2.02, 2.05, 2.02; 95% CI, 1.08-3.77, 1.10-3.81, 1.11-3.66; p = 0.027, 0.023, 0.022, respectively) and multivitamin nonusers (adjusted OR from model 1-3: 1.86, 1.85, 1.78; 95% CI, 1.04-3.34, 1.03-3.32, 1.004-3.71; p = 0.038, 0.039, 0.049, respectively) had increased odds of having lower UIC levels <150 μg/L. The iodine nutritional status in the first trimester is adequate in Taiwan; however, certain subgroups such as nulliparous and multivitamin nonusers are still at risk for iodine deficiency. Gestational thyroid function reference ranges are needed for correct diagnosis of thyroid dysfunction in pregnancy.

Iodine quantification of renal lesions: Preliminary results using spectral-based material extraction on photon-counting CT.

To assess the range of quantitative iodine values in renal cysts (RC) (with a few renal neoplasms [RNs] as a comparison) to develop an expected range of values for RC that can be used in future studies for their differentiation. Consecutive patients (n = 140) with renal lesions who had undergone abdominal examination on a clinical photon-counting computed tomography (PCCT) were retrospectively included. Automated iodine quantification maps were reconstructed, and region of interest (ROI) measurements of iodine concentration (IC) (mg/cm3) were performed on whole renal lesions. In addition, for heterogeneous lesions, a secondary ROI was placed on the area most suspicious for malignancy. The discriminatory values of minimum, maximum, mean, and standard deviation for IC were compared using simple logistic regression and receiver operating characteristic curves (area under the curve [AUC]). A total of 259 renal lesions (243 RC and 16 RN) were analyzed. There were significant differences between RC and RN for all IC measures with the best-performing metrics being mean and maximum IC of the entire lesion ROI (AUC 0.912 and 0.917, respectively) but also mean and minimum IC of the most suspicious area in heterogeneous lesions (AUC 0.983 and 0.992, respectively). Most RC fell within a range of low measured iodine values although a few had higher values. Automated iodine quantification maps reconstructed from clinical PCCT have a high diagnostic ability to differentiate RCs and neoplasms. The data from this pilot study can be used to help establish quantitative values for clinical differentiation of renal lesions.

Effects of Iodinated Contrast-enhanced CT on Urinary Iodine Levels in Postoperative Patients with Differentiated Thyroid Cancer.

This study aims to observe the fluctuating urine iodine levels in patients with differentiated thyroid cancer (DTC) following iodinated contrastenhanced computed tomography (eCT) scans. The presence of iodine in iodinated contrast agents (ICAs) can impede the effectiveness of radioactive iodine treatment (RAIT) and diagnostic scans in individuals diagnosed with DTC, as it can engage in competitive interactions with 131I. According to established guidelines, it is recommended to postpone RAIT for a period of three to four months in individuals who have had prior exposure to ICAS. The measurement of spot urine iodine concentration is a valuable indicator for assessing the overall iodine content throughout the body. The objective is to identify the optimal timing for administering postoperative RAIT in DTC patients. At various time points after surgery, a cohort of 467 random urine samples (126 male samples, 341 female samples, age (45±12 years)) was obtained from 269 DTC patients. The samples were analyzed for urinary iodine and urinary creatinine levels, and the urinary iodine/urine creatinine ratio (I/Cr) was computed. All samples were divided into two groups according to whether eCT before operation: the non-enhanced CT (eCT-) group and the enhanced CT (eCT+) group. The urine samples in the eCT- group were categorized into four subgroups according to the duration of strict low iodine diet (LID): (eCT-I+) no LID; (eCT-I-2W) 2 weeks of LID; (eCT-I-4W) 4 weeks of LID; and (eCT-I-6W) 6 weeks of LID. The last three groups were merged into the eCT- and effective LID group (eCT- I-). The urine samples from the eCT+ group were categorized into five subgroups: (0.5M eCT+)0.5 month after eCT+; (1M eCT+)1 month after eCT+; (2M eCT+) 2 months after eCT+; (3M eCT+) 3 months after eCT+; (≥4M eCT+) ≥4 months after eCT+. In addition, the patients within 2 months after eCT+ were divided into 2 groups according to their LID: no effective LID group (eCT+ I+) and effective LID group (eCT+ I-). Utilizing the Kruskal-Wallis and Mann-Whitney U rank sum tests, the differences in I/Cr between groups were compared. In the eCT-group, the I/Cr ratios of eCT-I-2W, eCT-I-4W, and eCT-I-6W were significantly lower than those of eCT-I+ (χ2 values: 4.607.99, all P 0.05). However, there was no significant difference in I/Cr between eCT-I-2W, eCT- I-4W, and eCT-I-6W (2 values: 0.591.31, all P > 0.05). Significantly higher I/Cr values were observed in 0.5M eCT+ and 1M eCT+ than in eCT-I+ (χ2 values: 3.22 and 2.18, respectively, all P<0.05). There was no significant difference in I/Cr between 2M eCT+ and eCT-I+ (χ2 = 0.76, P = 0.447). The I/Cr rations of 3M eCT+, ≥4M eCT+ were not significantly different with eCT-I- (χ2 values: 1.76; 0.58; all P > 0.05). However, they were considerably lower than eCT-I+ (χ2 values: 7.03; 5.22; all P<0.05). The I/Cr for patients who underwent eCT within two months (eCT+ I-, eCT+ I+) did not differ significantly (χ2 = 1.79, P = 0.073). For patients who are considering receiving radioactive iodine therapy (RAIT) following a diagnosis of differentiated thyroid cancer (DTC), it is recommended that the interval between RAIT treatment and enhanced computed tomography [eCT] scans be conducted at least three months.

Thyroid nodule characterization using Spectral Detector Computed Tomography (SDCT) in comparison to ultrasound

ObjectiveTo characterize thyroid nodules seen on Spectral Detector computed tomography (SDCT) in respect to their Thyroid Imaging Reporting and Data System (TI-RADS) category on Ultrasound (US). MethodsWe included patients that underwent US examination for the evaluation of thyroid nodules and contrast-enhanced SDCT examination of the neck/thorax, between the years 2018–2020. The SDCT and US were performed within 6 months of each other. Only patients with a visible thyroid nodule on SDCT were included. Attenuation measurements of the nodules in Hounsfield units (HU) were performed on the conventional CT images, virtual non-contrast (VNC) images and virtual monoenergetic images of 40 keV and 100 keV. The Iodine concentration, spectral slope and enhancement estimation results of the nodules were measured. We compared the spectral results between two groups of nodules, according to the US report: TI-RADS 2–3 and TI-RADS 4–5 groups. ResultsThirty-eight nodules were included in the study, 22 nodules in the TI-RADS 2–3 group and 16 in the TI-RADS 4–5 group. The nodules of the TI-RADS 4–5 group had significantly higher Iodine concentration measurement, 4.6 ± 1.8 mg/ml, compared to 2.3 ± 1.2 mg/ml in the TI-RADS 2–3 group; significantly higher estimated enhancement, 3.9 ± 1.5, compared to 2.2 ± 0.7; and significantly higher calculated spectral slope, 5.6 ± 2.2 compared to 2.9 ± 1.5 (p < 0.001). ConclusionSpectral results of SDCT may assist in differentiating intermediate-high risk (TI-RADS 4–5) from low risk (TI-RADS 2–3) thyroid nodules.Advances in knowledge: SDCT offers additional information for the characterization of thyroid nodules.

Development of a dual energy CT based model to assess response to treatment in patients with high grade serous ovarian cancer: a pilot cohort study

BackgroundIn patients with cancer, the current gold standard for assessing response to treatment involves measuring cancer lesions on computed tomography (CT) imaging. The percentage change in size of specific lesions determines whether patients have had a complete/partial response or progressive disease, according to RECIST criteria. Dual Energy CT (DECT) permits additional measurements of iodine concentration, a surrogate marker of vascularity. Here we explore the role of changes in iodine concentration within cancer tissue on CT scans to assess its suitability for determining treatment response in patients with high grade serous ovarian cancer (HGSOC).MethodsSuitable RECIST measurable lesions were identified from the CT images of HGSOC patients, taken at 2 different time points (pre and post treatment). Changes in size and iodine concentration were measured for each lesion. PR/SD were classified as responders, PD was classified as non-responder. Radiological responses were correlated with clinical and CA125 outcomes.Results62 patients had appropriate imaging for assessment. 22 were excluded as they only had one DECT scan. 32/40 patients assessed (113 lesions) had received treatment for relapsed HGSOC. RECIST and GCIG (Gynaecologic Cancer Inter Group) CA125 criteria / clinical assessment of response for patients was correlated with changes in iodine concentration, before and after treatment. The prediction of median progression free survival was significantly better associated with changes in iodine concentration (p = 0.0001) and GCIG Ca125 / clinical assessment (p = 0.0028) in comparison to RECIST criteria (p = 0.43).ConclusionChanges in iodine concentration from dual energy CT imaging may be more suitable than RECIST in assessing response to treatment in patients with HGSOC.Trial RegistrationCICATRIx IRAS number 198179, 14 Dec 2015, https://www.myresearchproject.org.uk/.

Primary tumour iodine avidity in relation to uptake in persistent metastatic disease in papillary and poorly differentiated thyroid cancer

PurposePatients with persistent or recurrent papillary and poorly differentiated thyroid cancer can be effectively treated with radioiodine, if the tumour tissue is iodine-avid. However, iodine-avidity status is often unknown at the time of initial radioiodine treatment, limiting any adaptive approach. This study aimed to clarify the relationship between pre-therapeutic iodine avidity in primary tumour tissue, initial lymph node metastases and iodine uptake in subsequent metastases.MethodsIodine avidity was prospectively assessed pre-therapeutically in 35 patients by injection of tracer amounts of iodine-131 two days prior to surgery. Iodine concentrations in resected tissue samples were measured, enabling accurate and histologically verifiable iodine avidity data for both primary tumour and initial lymph node metastases. Iodine uptake in persistent metastatic disease was assessed by review of radiology, and treatment response was examined through journal studies.ResultsOut of data from 35 patients, 10 had persistent disease at presentation or during follow-up (range 19–46 months). Four patients had non-avid persistent metastatic disease, all with low iodine avidity in their primary tumours and initial lymph node metastases. Patients with low pre-therapeutic iodine avidity did not appear to have greater risk of persistent disease.ConclusionThe results indicate a close link between pre-therapeutically measured iodine concentrations in primary tumours with iodine avidity of any subsequent metastases.

Densification and Immobilization of AgI-Containing Iodine Waste Forms Using Spark Plasma Sintering

In this study, porous Ag-xerogel (Ag-Xero), Ag-faujasite (Ag-FAU) zeolite, and Ag-mordenite (Ag-MOR) zeolite sorbents were loaded with iodine gas [I2(g)] under saturated conditions at 150 °C for 24 h, followed by densification and consolidation into monolithic waste forms using spark plasma sintering (SPS). For Ag-Xero materials, SPS pellets were made with as-loaded samples, while others were made with preheated (PH; 500 °C for 2 h) samples to help with densification. SPS processing was conducted at 50 MPa under different temperatures (T = 200–800 °C) for different times (t = 0.5–30 min), where eleven AgI-Xero samples, five AgI-FAU, and two AgI-MOR separate samples were produced. The primary goal was to look for the optimum processing parameters for each material to yield pellets with high iodine retentions, high densities, and low porosities while preventing AgI decomposition. The Ag-Xero showed the highest iodine loadings (qe = 470 mg g–1) compared to Ag-FAU (qe = 368 mg g–1) and Ag-MOR (qe = 108 mg g–1). Measured iodine concentrations were the highest in AgI-Xero pellets without PH, followed by AgI-Xero with PH, AgI-FAU, and then AgI-MOR. Silver utilization (I/Ag on a mol % basis) values were in the order of AgI-MOR ≈ AgI-Xero (no PH) > AgI-Xero (PH) > AgI-FAU. Chemical durabilities of SPS-densified AgI-Xero (PH) pellets were very favorable, with lower releases than SPS pellets made from AgI-Xero samples without PH. These results show promise for iodine waste form production.

Application of multi-spectral CT imaging in Crohn's disease: a systematic review.

No quantitative computed tomography (CT) biomarker is actually sufficiently accurate to assess Crohn's disease (CD) lesion activity, with adequate precision to guide clinical decisions. To assess the available literature on the use of iodine concentration (IC), from multi-spectral CT acquisition, as a quantitative parameter able to distinguish healthy from affected bowel and assess CD bowel activity and heterogeneity of activity along the involved segments. A literature search was conducted to identify original research studies published up to February 2022. The inclusion criteria were original research papers (>10 human participants), English language publications, focus on dual-energy CT (DECT) of CD with iodine quantification (IQ) as an outcome measure. The exclusion criteria were animal-only studies, languages other than English, review articles, case reports, correspondence, and study populations <10 patients. Nine studies were included in this review; all of which showed a strong correlation between IC measurements and CD activity markers, such as CD activity index (CDAI), endoscopy findings and simple endoscopic score for Crohn's disease (SES-CD), and routine CT enterography (CTE) signs and histopathologic score. Statistically significant differences in IC were reported between affected bowel segments and healthy ones (higher P value was P < 0.001), normal segments and those with active inflammation (P < 0.0001) as well as between patients with active disease and those in remission (P < 0.001). The mean normalized IC at DECTE could be a reliable tool in assisting radiologists in the diagnosis, classification and grading of CD activity.

Mucosal healing assessment in Crohn’s disease with normalized iodine concentration from dual-energy CT enterography: comparison with endoscopy

ObjectivesMucosal healing (MH) is an important goal in the treatment of patients with Crohn’s disease (CD). Noninvasive assessment of MH with normalized iodine concentration (NIC) is unknown.MethodsIn this retrospective study, 94 patients with diagnosed CD underwent endoscopy and dual-energy CT enterography (DECTE) at the post-infliximab treatment review. Two radiologists reviewed DECTE images by consensus for assessing diseased bowel segments of the colon or terminal ileum, and the NIC was measured. Patients were divided into transmural healing (TH), MH and non-MH groups. The diagnostic performance of the MH and non-MH groups with clinical factors and NIC was assessed utilizing receiver operating characteristic (ROC) curve analysis.ResultsOf the 94 patients included in our study, 8 patients achieved TH, 34 patients achieved MH, and 52 patients did not achieve MH at the post-IFX treatment review. The area under the ROC curve (AUC), sensitivity, specificity, and accuracy values were 0.929 (95% confidence interval [CI] 0.883–0.967), 0.853, 0.827, and 0.837, respectively, for differentiating MHs from non-MHs, and the optimal NIC threshold was 0.448. The AUC of the combined model for distinguishing MHs from non-MHs in CD patients, which was based on the NIC and calprotectin, was 0.964 (95% CI 0.935–0.987).ConclusionsThe normalized iodine concentration measurement in DECTE has good performance in assessment MH in patients with CD. Iodine concentration from DECTE can be used as a radiologic marker for MH.

Lung dual energy CT: Impact of different technological solutions on quantitative analysis

PurposeTo evaluated the accuracy of spectral parameters quantification of four different CT scanners in dual energy examinations of the lung using a dedicated phantom. MethodMeasurements were made with different technologies of the same vendor: one dual source CT scanner (DSCT), one TwinBeam (i.e. split filter) and two sequential acquisition single source scanners (SSCT). Angular separation of Calcium and Iodine signals were calculated from scatter plots of low-kVp versus high-kVp HUs. Electron density (ρe), effective atomic number (Zeff) and Iodine concentration (Iconc) were measured using Syngo.via software. Accuracy (A) of ρe, Zeff and Iconc was evaluated as the absolute percentage difference (D%) between reference values and measured ones, while precision (P) was evaluated as the variability σ obtained by repeating the measurement with different acquisition/reconstruction settings. ResultsAngular separation was significantly larger for DSCT (α = 9.7°) and for sequential SSCT (α = 9.9°) systems. TwinBeam was less performing in material separation (α = 5.0°). The lowest average A was observed for TwinBeam (Aρe = [4.7 ± 1.0], AZ = [9.1 ± 3.1], AIconc = [19.4 ± 4.4]), while the best average A was obtained for Flash (Aρe = [1.8 ± 0.4], AZ = [3.5 ± 0.7], AIconc = [7.3 ± 1.8]). TwinBeam presented inferior average P (Pρe = [0.6 ± 0.1], PZ = [1.1 ± 0.2], PIconc = [10.9 ± 4.9]), while other technologies demonstrate a comparable average. ConclusionsDifferent technologies performed material separation and spectral parameter quantification with different degrees of accuracy and precision. DSCT performed better while TwinBeam demonstrated not excellent performance. Iodine concentration measurements exhibited high variability due to low Iodine absolute content in lung nodules, thus limiting its clinical usefulness in pulmonary applications.

Analytical validation of an inductively coupled plasma mass spectrometry method for urinary iodine concentration measurements in Taiwan

Urinary iodine concentration (UIC) measured by Sandell-Kolthoff spectrophotometric method has been used in the Nutrition and Health Surveys in Taiwan but this method is time consuming and produces toxic waste from arsenic trioxide. The aim of this study was to develop and validate an inductively coupled plasma mass spectrometry (ICP-MS) system to determine UIC in Taiwan. Samples and iodine calibrators were diluted 100-fold into an aqueous solution containing Triton X-100, 0.5% ammonia solution, and tellurium (128Te) as an internal standard. Digestion prior to analysis was not necessary. Precision, accuracy, serial dilution, and recovery tests were performed. A total of 1243 urine samples covering a wide range of iodine concentrations were measured by both Sandell-Kolthoff method and ICP-MS. Passing-Bablok regression and Bland-Altman plots were used to compare values across methods. The limit for detection and quantification by ICP-MS was 0.95μg/L and 2.85μg/L, respectively. The intra-assay and inter-assay coefficients were <10%, with a recovery range of 95%-105%. The results obtained by ICP-MS and the Sandell-Kolthoff method were highly correlated (Pearson's correlation: r=0.996, 95% confidence interval [CI]: 0.9950-0.9961, p<0.001). For UIC between 20 and 1000μg/L, the y-intercept for the Passing-Bablok regression was -1.9 (95% CI: -2.5599 to -1.3500) and the slope was 1.01 (95% CI: 1.0000-1.0206). This validated ICP-MS system can be used for measuring UIC.

Dual Energy CT for determining the severity of acute pancreatitis

We aimed to quantitatively compare the iodine concentration measured by Dual Energy Computed Tomography （CT） between acute and mild pancreatitis cases and to evaluate the accuracy of the severity assessment of acute pancreatitis using Dual Energy CT. The contrast CT grade based on the guidelines in Japan and definitive diagnoses were evaluated. Iodine concentrations and C-reactive protein （CRP） levels were compared between mild and severe acute pancreatitis. The CECT grading diagnosed five false negative cases out of 41 patients. Only when the CRP reached the highest level, there was a significant difference between severe and mild cases. There were significant differences between the iodine concentration of severe and mild cases. Our results demonstrated measuring iodine concentration using Dual Energy CT is useful for severity assessment of acute pancreatitis.
