Dyslipidemia prevalence in nonobese, nondiabetic patients with obstructive sleep apnea: does sex matter?

Dyslipidemia in obstructive sleep apnea (OSA) has been attributed to confounding obesity and/or diabetes. This study aimed to examine lipid profiles in nondiabetic, nonobese patients with OSA and identify the possible effects of age and sex. We retrospectively evaluated the lipid parameters of 3,050 adults who underwent polysomnography. A total of 2,168 patients were excluded due to obesity (body mass index ≥ 30 kg/m2), diabetes, alcoholism, untreated hypothyroidism, lipid-lowering drug use, missing sleep data, or treatment for suspected OSA. Of 882 patients (75% males, aged 46.8 ± 12.2 years) included in the study, 88.4% had OSA. Levels of total cholesterol (P = .003), low-density-lipoprotein (LDL) cholesterol (P = .005), non-high-density-lipoprotein (non-HDL) cholesterol (P = .001), and triglycerides (P = .007) were significantly higher in patients with OSA than in those without, whereas HDL-cholesterol levels did not differ. The proportion of patients with hypercholesterolemia and/or elevated non-HDL cholesterol (> 160 mg/dL) was significantly higher in OSA than in non-OSA. Correlation analyses by sex revealed stronger and more significant relationships between lipid parameters and apnea-hypopnea index in women than in men (r = .135, P < .001, vs r = .080, P = .043 for total cholesterol; r = .111, P < .001, vs r = .080, P = .046 for non-HDL cholesterol; r = .122, P < .001, vs r = .061, P = .107 for LDL cholesterol, respectively). In regression analysis, the rate of hypercholesterolemia increased with age (P < .001 for women and P = .031 for men); non-HDL- and LDL-cholesterol levels significantly increased with OSA severity (P = .035 and P = .023, respectively) and age (P = .004 and P = .001, respectively) in women. After excluding confounding obesity and diabetes, patients with OSA have an impaired lipid profile including total cholesterol, LDL cholesterol, non-HDL cholesterol, and triglycerides. The Patient Perspective: The Everyday Hazards of Fragrance Allergy.

To the Editor: In the US, 3.5% of the population has allergic contact dermatitis to fragrance.1 Based on the North American Contact Dermatitis Group data, fragrances were the fourth most common allergen.2 It can be difficult to avoid fragrance, which are virtually ubiquitous in everyday products. Even products labeled as “unscented” or “fragrance-free” may still contain a fragrance. The Contact Allergen Management Program contains a list of fragrance-free products, but it is not comprehensive. Fragrance are also one of the most common causes of airborne contact dermatitis. However, patients have limited control over, or knowledge of, the products used in public environments. Experts have proposed using barrier creams that might prevent airborne allergens from contacting skin, but there have not been formal studies demonstrating their efficacy.3 Fragrance can restrict individuals from accessing the most necessary of locations, such as public restrooms. The pervasiveness of airborne fragrances in public can necessitate meticulous planning for each outing, and new places are even more of a challenge. “Every time I go to a new location, I have to inquire what products are used,” explained Ms Brodd, a patient with allergic contact dermatitis. “Sometimes, there are power dynamics in play, such as in the workplace, and I end up having to deal with my reaction.” Similarly, Ms Ault and a friend recently encountered difficulties with newly installed fragrance machines at a health club chain. The chain's leadership and the fragrance company provided similar statements that the fragrances are “safe” because their fragrance suppliers are part of the International Fragrance Association. The International Fragrance Association is a self-regulatory global representative body of the fragrance industry. Even with assistance from her dermatologist, the general manager of the club only agreed to unplug the scent machine 15 minutes before Ault's arrival. Is this a violation of the US Americans with Disabilities Act (ADA)? The ADA defines disability as “a physical or mental impairment that substantially limits 1 or more major life activities of such individual.”4 These major life activities can include caring for oneself, concentrating, and working. The ADA prohibits discrimination based on disabilities in employment, public services, and privately owned public accommodations. However, receiving a reasonable accommodation due to their disability is challenging for individuals with fragrance allergy. The solution may not be found in the courts, but in developing fragrance-free policies, which have started to gain momentum in businesses and institutions. A fragrance-free policy may be an equitable option for a business with a patron who has a fragrance allergy. The Centers for Disease Control and Prevention has prohibited fragranced products in “all interior space owned, rented, or leased” by the Centers for Disease Control and Prevention since 2009.5 The Canadian Centre for Occupational Health and Safety provides useful fact sheets on how to implement scent-free policies.6 Steps to consider include the following: conducting an employee survey to assess the issue, creating a representative committee to draft a policy, providing education to all employees about the reasoning behind the policy, addressing concerns honestly, and being open to revision. Guidelines should stress that this policy applies to everyone and that fragrance-containing products are not limited to perfume and cologne. Other useful tips include posting a list of approved unscented products, reviewing ingredients of products used by your organization, and placing policy statement notices in visible areas. A RARE CASE OF CHRONIC COUGH

TOPIC: Lung Cancer TYPE: Medical Student/Resident Case Reports INTRODUCTION: Chronic cough is one of the most common symptoms for which outpatient care is sought. The differential diagnosis is broad, and many patients undergo extensive evaluation. We here report a rare case of chronic cough with progressive dyspnea due to Diffuse idiopathic pulmonary neuroendocrine cell hyperplasia( DIPNECH), followed by a brief literature review of DIPNECH. CASE PRESENTATION: 70-year-old female, non smoker, history of Crohn's disease presented to the Pulmonology clinic in 2005 with cough and shortness of breath. PFT's showed restrictive pattern. CT scan showed multiple bilateral pulmonary nodules. Bronchoscopy could not identify cause. On periodic CT, lung nodules remained stable. In the meantime, she continued to have chronic cough and progressively worsening shortness of breath. She received multiple trials of steroids, antibiotics, and inhalers with mild improvement. Repeat PFT's now showed severe obstructive disease. Her coughing spells made it difficult to perform on PFTs. She underwent repeat bronchoscopy with extensive testing, including bacterial, fungal, malignancy, and autoimmune causes of her symptoms, all of which came back negative. The working diagnosis was necrobiotic nodules secondary to her Crohn's disease vs. Asthma. She was put on a prolonged steroid course and developed severe side effects like muscle weakness, mood changes, tremors and worsening kidney function. The patient was hesitant to go back on steroids. She had suffered for 17 years. A trial of steroid-sparing medication CellCept was considered. However, before committing the patient to these, a wedge biopsy of the pulmonary nodule was performed and revealed neuroendocrine cells that stained positive for synaptophysin, chromogranin, and CD56.8 confirming the diagnosis of DIPNECH. The patient was subsequently referred to oncology and started on Octreotide infusions causing significant improvement in symptoms. DISCUSSION: DIPNECH is a rare condition that is becoming increasingly recognized. It is typically characterized by an insidious onset with chronic nonproductive cough, exertional dyspnoea, and wheezing, spuriously attributed to Asthma, chronic obstructive pulmonary disease, or GERD. Indeed, the diagnosis of DIPNECH is usually made several years after the onset of clinical symptoms, often following the incidental discovery of a lung nodule (or nodules) on imaging, usually computed tomography (CT). It is characterized histologically by a diffuse, bronchiolo-centric proliferation of neuroendocrine cells at the periphery of the lung, radiologically by ground-glass opacity, mosaic attenuation with air trapping, bronchial wall thickening, and pulmonary nodules and, Clinically by symptoms of airflow limitation. CONCLUSIONS: We hope this case can increase awareness of DIPNECH among clinicians as patients may otherwise be misdiagnosed and undergo unnecessary treatments or procedures causing permanent harm.
