Innovations in transdermal delivery: Exploring nicotine patches, microneedles and glucose monitoring technologies

Transdermal devices for delivering drugs have been invented in response to obstacles caused by oral administration of medications. An adhesive patch comprising medication that is applied topically for transferring a preset proportion of treatment through the skin and into the bloodstream is called a transdermal patch. The method of manufacturing for various transdermal application types, including matrix patches, is described in this review article. Nicotine substitution therapy is an assortment of products that provide nicotine and are authorized for use as a smoking cessation aid for the treatment of withdrawal. An increasing number of participants have been impacted by the global rise in diabetes in the past few years are experiencing discomfort and infections because of the intrusive design of most commercial glucose meters. Non-invasive blood glucose monitoring technology is now a hot issue for research worldwide and a novel approach that has the potential to help a lot of patients.

The role of agroforestry in solving the agricultural landscapes vulnerabilities in the Po Plain district

AbstractThe current trend of biodiversity deterioration in rural systems is a complex issue that operates across multiple spatial scales. Agroforestry practices have the potential to positively contribute towards addressing these trends by shaping the structure of agricultural landscapes and their underlying ecological functions. This study aims to test a multi-scale analytical approach to understand and account for these processes. Specifically, the study seeks to assess the contributions that agroforestry practices at the farm scale can make towards supporting biodiversity, in response to the wider-scale landscape eco-mosaic structural and functional challenges and requirements (both at the local and extra-local landscape systems). To achieve this, a series of landscape ecology analyses are conducted on an agroforestry-based rice farm located in the western Po Plain region of Northern Italy. These analyses examine various landscape structural traits (such as matrix composition, patch size, shape complexity, and diversity indices) and functional traits (including connectivity and bionomic indices), with different levels of detail for each scale of analysis. This allows for the evaluation of the current ecological status of both the extra-local and local scale landscape systems (including drivers of vulnerability and resilience) and the assessment of the farm's current contributions to biodiversity support. Based on these findings, strategic agroforestry interventions are identified at the farm scale to enhance its capacity to address the wider-scale ecological gaps. Two design scenarios are assessed, wherein functional ecological traits such as landscape diversity, connectivity, and ecological stability are improved. The results confirm the role of farm scale agroforestry management as a buffering tool, demonstrating how it contributes to the restoration of broader-scale landscape vulnerabilities. The applied approach provides cost-effective assessments of biodiversity-related ecological processes, with the accuracy of the findings dependent on the comprehensive multi-scale analysis conducted.

Formulation of Salam (Syzygium polyanthum (wight) walp) Leaves Ethanolic Extract Matrix Patch and Its Evaluation

The total flavonoid content in salam leaves has an anti-inflammatory activity at a dose of 2.1% w/w. For topical anti-inflammatory, a matrix patch has a delivery mechanism that drugs pass through in the skin in a controlled for an extended period. Polymers were the patch's main component to control the drug release. Polyvinyl alcohol (PVA) and alpha-cellulose (AC) were polymers that can increase the rate of drug diffusion and form a strong film layer. Salam leaves were extracted by maceration using ethanol 96%. Matrix patches were made with a combination of PVA and AC in ratios 1:1; 3:1; 1:3. The patches were tested for physicochemical properties. The stability test was conducted at 4, 25, and 40 temperatures for 8 h, respectively. Statistical analysis of the test data used One Way ANOVA with a confidence level of 95%. The results showed that the high concentration of PVA significantly affected the weight and moisture content but did not affect the organoleptic thickness, folding endurance, and pH of the matrix patch. The best matrix patch of salam leaves ethanolic extract was PVA and AC of 3:1 because it met the requirements for the matrix patch, including the weight of 1.96-2.06 grams, thickness of 1.15-1.18 mm, folding endurance >300 times, humidity 25.75 – 30.17% and pH 6.3 – 7.1. The patch contains flavonoids with Rf values for extract and patch of 0.95 and 0.96, respectively. Further study, in vivo anti-inflammatory test is necessary.

Composite Biosynthetic Graft for Repair of Long-Segment Tracheal Stenosis: A Pilot In Vivo and In Vitro Feasibility Study.

Pediatric patients who undergo surgery for long-segment congenital tracheal stenosis (LSCTS) have suboptimal outcomes and postsurgical complications. To address this, we propose a biosynthetic graft comprising (1) a porcine small intestinal submucosa extracellular matrix (SIS-ECM) patch for tracheal repair, and (2) a resorbable polymeric exostent for biomechanical support. The SIS-ECM patch was evaluated in vivo in an ovine trachea model over an 8 month period. Concurrently, the biosynthetic graft was evaluated in a benchtop lamb trachea model for biomechanical stability. In vivo results show that SIS-ECM performs better than bovine pericardium (control) by preventing granulation tissue/restenosis, restoring tracheal architecture, blood vessels, matrix components, pseudostratified columnar and stratified epithelium, ciliary structures, mucin production, and goblet cells. In vitro tests show that the biosynthetic graft can provide the desired axial and flexural stability, and biomechanical function approaching that of native trachea. These results encourage future studies to evaluate safety and efficacy, including biomechanics and collapse risk, biodegradation, and in vivo response enabling a stable long-term tracheal repair option for pediatric patients with LSCTS and other tracheal defects.

Arthroscopic Incomplete Rotator Cuff Repair With Patch Augmentation Using Acellular Dermal Matrix Allograft

Arthroscopic rotator cuff repair has been shown promising clinical outcomes. However, large to massive rotator cuff tears are difficult to completely repair with appropriate tension because of their size or poor tissue quality. An incomplete repair using the “hybrid technique” is one of way to solve this problem by maximizing the contact area between the tendon and the footprint of greater tuberosity. Additionally, the acellular dermal matrix patch augmentation has emerged as an adjuvant technique to enhance the biomechanical properties to promote healing of the repaired construct. This Technical Note describes arthroscopic incomplete rotator cuff repair using the “hybrid technique” with acellular dermal matrix patch augmentation.

Development and evaluation of buspirone hyrdochloride loaded transdermal patch using natural polymers

The purpose of this present study was to design and develop a Buspirone hydrochloride loaded matrix type transdermal drug delivery system (TDDS) with four natural gum polymers such as Guar gum, Gellan gum, Xanthan gum, Karaya gum. A total of twelve batch of matrix patches were formulated with above stated polymers and other excipient like Glycerin, Propylene glycol (PG), Polyethylene glycol (PEG) as plasticizer. These all batches were characterized by evaluation parameters which including drug content, flatness, thickness, uniformity of weight, folding endurance, moisture content, moisture uptake and to determine the amount of Buspirone hydrochloride present in the matrix of patches in vitro dissolution study of all twelve formulations were performed. Fourier transform infrared (FTIR) technique and Differential Scanning Colorimetry (DSC) carried out to determine interaction between Buspirone HCl and excipients which are present in patch. Permeation study of patch of Buspirone HCl was performed in Franz's diffusion cell to evaluate In-vitro skin permeation and it was found that batch G3 shows 98.89% permeability. After evaluating all batches and based on results which are obtained characterization and In-Vitro dissolution it was concluded that patches containing natural polymers are not showing any interaction and better release study.

Transdermal patches: updated review as a novel drug delivery system

Transdermal patches are a non-invasive method of drug administration. It is an adhesive patch designed to deliver a specific dose of medication through the skin and into the bloodstream throughout the body. Transdermal drug delivery has several advantages over other routes of administration, for instance, it is less invasive, patient-friendly, and has the ability to bypass first-pass metabolism and the destructive acidic environment of the stomach that occurs upon the oral ingestion of drugs. Further offering a prolonged period of regulated medication release. The preparation procedures for various transdermal patch types, including membrane matrix, drug-in-adhesive, and micro reservoir patches, are covered in this review article. The various transdermal dosage form evaluation techniques have also been studied.

Transdermal Patches Containing Opioids in the Treatment of Patients with Chronic Pain

Transdermal delivery is a non-invasive route, used as an alternative to the oral route, to administer drugs through the skin surface. One of the fields in which they are particularly used is that of pain therapy. In this treatment, transdermal patches, particularly those containing opioids, are used to complement or replace orally administered drugs. First-generation patches are constituted by reservoir systems, where the drug is dissolved in a solvent and gelled with a polymer. In contrast, the active principle is incorporated into the polymer adhesive in more recent matrix patches. In this review, the main papers related to the production and employment of transdermal patches containing the two most used opioids, i.e., fentanyl and buprenorphine, have been critically analyzed. From the analysis of the literature, it is possible to deduce that the type of drug and the amount of drug present in the patch must be chosen not according to the origin of the pain but to the age of the patient, the area where the patch is applied, and the frequency at which the patch is replaced.

Second-generation extracellular matrix patch for epicardial infarct repair

Current myocardial infarction treatments focus on improving hemodynamics rather than addressing the problem of lost myocardium impairing left ventricular function. Epicardial infarct repair with a bioactive patch placed on the ischemic area is an emerging approach to promote endogenous myocardial repair. We report the use of a second-generation CorMatrix-extracellular matrix (ECM) patch as an adjunct to surgical revascularization in treating a young patient with diffuse, multivessel coronary artery disease unamenable to PCI and a large anterior myocardial infarction. The progressive myocardial scar shrinkage and increase in left ventricular ejection fraction from 10 to 51% are generally not observed with surgical revascularization therapy alone, suggesting this new patch has adjunctive potential to current revascularization therapy.

Manufacturing Multi-Matrix Composites: Out-of-Vacuum Bag Consolidation

Abstract The formation of porosity is a major challenge in any composite manufacturing process, particularly in the absence of vacuum assistance. Highly localized injection of polymer matrix into regions of interest in a dry preform is a route to manufacturing multi-matrix fiber-reinforced composites with high filler concentrations, which are otherwise difficult to achieve. Unlike traditional composites, such multi-matrix fiber-reinforced composite systems, which combine multiple resins in continuous form, offer improved structural performance around stress concentrators and multifunctional capabilities. As the process lacks vacuum assistance, porosity becomes a primary issue to be addressed. This paper presents a rheo-kinetic coupled rapid consolidation procedure for optimizing the quality of localized matrix patches. The procedure involves manufacturing trials and analytical consolidation models to determine the best processing program for minimal voidage in the patch. The results provide a step toward an efficient manufacturing process for the optimal design of multi-matrix composites without the need for complex vacuum bag arrangements, thus reducing cost and time while opening avenues to improve overall composite performance.

Longitudinal evaluation of fentanyl concentrations in equine plasma and synovial fluid following application of transdermal fentanyl patches over one carpal joint.

To determine if transdermally delivered fentanyl can achieve greater concentrations of fentanyl in synovial fluid when applied over a synovial structure. Randomized, experimental study. Six healthy adult horses. Each horse had two 100 μg/h fentanyl matrix patches applied on the dorsal aspect of one, randomly assigned, carpometacarpal joint (CMCJ) for 48 h. Whole blood and bilateral synovial samples from the intercarpal joint were obtained at 0, 2, 6, 12, 24, 36 and 48 h. Fentanyl concentrations were measured with liquid chromatography-mass spectrometry. All subjects achieved detectable concentrations of fentanyl in both plasma and synovial fluid. Time to peak synovial and plasma concentration was 12 h. At 6 h, the synovial concentration in the untreated carpus (0.104 ng/mL ± 0.106) was lower than plasma fentanyl concentrations 0.31 ± 0.27 (p = .036). At 12 h, both treated (0.55 ng/mL ± 0.3) and untreated (0.53 ng/mL ± 0.28) synovial fluid fentanyl concentrations were lower than plasma (0.87 ng/mL ± 0.48) concentrations (p < .001 and p = .001, respectively). Synovial concentrations of fentanyl did not differbetween treated and untreated joints (p > 0.608 for all time points). Application of fentanyl matrix patches directly over the CMCJ did not result in increased fentanyl concentrations in the synovial fluid of the treated intercarpal joint in normal horses. There is likely no analgesic advantage to placing fentanyl patches directly over the affected joint, as it did not result in increased synovial concentrations at the tested site.

Dual human iPSC-derived cardiac lineage cell-seeding extracellular matrix patches promote regeneration and long-term repair of infarcted hearts

Human pluripotent stem cell-derived cardiovascular progenitor cells (hCVPCs) and cardiomyocytes (hCMs) possess therapeutic potential for infarcted hearts; however, their efficacy needs to be enhanced. Here we tested the hypotheses that the combination of decellularized porcine small intestinal submucosal extracellular matrix (SIS-ECM) with hCVPCs, hCMs, or dual of them (Mix, 1:1) could provide better therapeutic effects than the SIS alone, and dual hCVPCs with hCMs would exert synergic effects in cardiac repair. The data showed that the SIS patch well supported the growth of hCVPCs and hCMs. Epicardially implanted SIS-hCVPC, SIS-hCM, or SIS-Mix patches at 7-day post-myocardial infarction significantly ameliorated functional worsening, ventricular dilation and scar formation at 28- and 90-day post-implantation in C57/B6 mice, whereas the SIS only mildly improved function at 90-day post-implantation. Moreover, the SIS and SIS-cell patches improved vascularization and suppressed MI-induced cardiomyocyte hypertrophy and expression of Col1 and Col3, but only the SIS-hCM and the SIS-Mix patches increased the ratio of collagen III/I fibers in the infarcted hearts. Further, the SIS-cell patches stimulated cardiomyocyte proliferation via paracrine action. Notably, the SIS-Mix had better improvements in cardiac function and structure, engraftments, and cardiomyocyte proliferation. Proteomic analysis showed distinct biological functions of exclusive proteins secreted from hCVPCs and hCMs, and more exclusive proteins secreted from co-cultivated hCVPCs and hCMs than mono-cells involving in various functional processes essential for infarct repair. These findings are the first to demonstrate the efficacy and mechanisms of mono- and dual-hCVPC- and hCM-seeding SIS-ECM for repair of infarcted hearts based on the side-by-side comparison.

Tissue-Adhesive Decellularized Extracellular Matrix Patches Reinforced by a Supramolecular Gelator to Repair Abdominal Wall Defects.

Implantation of surgical meshes composed of synthetic and biological materials has been applied for abdominal wall defect repair. Despite many efforts, there are no reliable meshes that fully satisfy clinical requirements because of their lack of biodegradability, mechanical strength, and tissue-adhesive properties. Here, we report biodegradable, decellularized extracellular matrix (dECM)-based biological patches to treat abdominal wall defects. By incorporating a water-insoluble supramolecular gelator that forms physical cross-linking networks through intermolecular hydrogen bonding, dECM patches were reinforced to improve mechanical strength. Reinforced dECM patches possessed higher tissue adhesion strength and underwater stability compared with the original dECM because of enhanced interfacial adhesion strength. In vivo experiments using an abdominal wall defect rat model showed that reinforced dECM patches induced collagen deposition and the formation of blood vessels during material degradation, and the accumulation of CD68-positive macrophages was suppressed compared to nonbiodegradable synthetic meshes. Tissue-adhesive and biodegradable dECM patches with improved mechanical strength by a supramolecular gelator have enormous potential for use in the repair of abdominal wall defects.

A Review on Transdermal Drug Delivery: Design, Evaluation and Approach towards Painless Drug Delivery System

Transdermal drug delivery systems were developed in order to overcome the difficulties associated with oral drug delivery. Through an adhesive patch affixed to the skin, transdermal patches deliver medications into the bloodstream. This treatment may benefit damaged areas of the body. Unlike oral, topical, intramuscular, and intravenous drug delivery methods, transdermal drug delivery enables controlled drug release into the body. Body heat is used to melt thin layers of medication embedded in the adhesive through the transdermal patch's porous membrane. As a barrier against foreign invaders, the skin serves as a protective layer. A medication with a molecular weight less than 500 Da can penetrate the stratum corneum through the outermost layer of the skin. An overview of transdermal patches is provided in this review article, including matrix patches, reservoir patches, membrane patches, micro reservoir patches, and patches that contain drugs in adhesive forms. These dosage forms have also been evaluated using various methods.

Neuropathology Evaluation of in Utero Correction of Myelomeningocele and Complications of Late-Onset GBS Infection

Background: Myelomeningocele (MMC) causes significant morbidity and mortality. Efforts have been directed to correct this defect in utero. The neuropathology literature on antenatally repaired MMC and associated complications in humans is limited. Case report: A 12-day-old female, who underwent prenatal MMC repair via a two-layer closure (dural replacement patch, primary skin closure), was born at 34 weeks’ gestation. Her group B streptococcus positive mother received appropriate antepartum prophylactic antibiotics. She remained stable until day 11 of life when she underwent rapid clinical deterioration. Despite aggressive intervention, she expired on day 12. Review of placental pathology showed maternal and fetal inflammatory response. Autopsy revealed Gram-positive cocci and inflammation within the basilar leptomeninges and lumbosacral region. Neural and dermal elements were present within the MMC repair. Conclusion: This case documents integration of the dermal matrix patch to neural elements, adhering the spinal cord to scar tissue, the clinical implications of which remain unclear.

FORMULATION, OPTIMIZATION AND IN-VIVO ANTI-INFLAMMATORY STUDY OF OPTIMIZED TRANSDERMAL PATCHES

In the present study, an attempt was made to prepare, characterize and evaluate of transdermal matrix patches of Diclofenac Sodium for inammation and pain related disease. Based on results of various evaluation parameters like thickness, strength, elongation, better compatibility and stability the transdermal matrix patches was successfully designed and developed by trial and error method. Formulations were prepared by employing combination of HPMCK15M, PVPK30, PEG-400 and EC among penetration enhancer and patches were evaluated for uniformity of thickness, weight-variation test, folding-endurance, tensile strength, % elongation, % atness, % moistures absorption, Moistures vapor transmittance rate, assay done. Cellophane membrane employed for the diffusion study. The result revealed that formulations containing enhancer eucalyptus oil, oleic acid and tea tree oil have better anti-inammatory action as compared to formulation without it because eucalyptus oil and tea tree oil both are natural anti-inammatory remedies as well as permeation enhancer. The Formulation F 6 show highest percentage inhibition of edema as compared to others formulation.

Development, characterization and in VIVO evaluation of diffusion controlled transdermal matrix patches of a model anti-Inflammatory drug

In the current research, diffusion controlled transdermal matrix patches of Lornoxicam, an anti-inflammatory drug was developed by solvent casting method using hydrophilic and hydrophobic polymers in different ratios and tween-80 and span-80 as permeation enhancers. Formulated patches were characterized for different physicochemical parameters in terms of moisture loss, drug content, film thickness and strength, uptake and loss of moisture, transmission of water vapor etc. All of these parameters were found be satisfactory. In-vitro diffusion studies of formulated patches were performed by using Franz diffusion cells. The drug diffusion rate followed zero order kinetics with super case II transport diffusion. Based on In-vitro diffusion data, best formulation was selected and used further for the stability analysis. Developed patches were stable at different temperature and humidity settings in terms of physicochemical properties and drug content. The optimized patches were also evaluated for the in-vivoanti-inflammatory and analgesic activity using suitable animal models. The results of all the in-vitro and in-vivo studies justify the selection of transdermal route for the systemic delivery of Lornoxicam to overcome its shortcomings when administered orally.

Epicardial transplantation of autologous atrial appendage micrografts: evaluation of safety and feasibility in pigs after coronary artery occlusion

Objectives. Several approaches devised for clinical utilization of cell-based therapies for heart failure often suffer from complex and lengthy preparation stages. Epicardial delivery of autologous atrial appendage micrografts (AAMs) with a clinically used extracellular matrix (ECM) patch provides a straightforward therapy alternative. We evaluated the operative feasibility and the effect of micrografts on the patch-induced epicardial foreign body inflammatory response in a porcine model of myocardial infarction. Design. Right atrial appendages were harvested and mechanically processed into AAMs. The left anterior descending coronary artery was ligated to generate acute infarction. Patches of ECM matrix with or without AAMs were transplanted epicardially onto the infarcted area. Four pigs received the ECM and four received the AAMs patch. Cardiac function was studied by echocardiography both preoperatively and at 3-week follow-up. The primary outcome measures were safety and feasibility of the therapy administration, and the secondary outcome was the inflammatory response to ECM. Results. Neither AAMs nor ECM patch-related complications were detected during the follow-up time. AAMs patch preparation was feasible according to time and safety. Inflammation was greatly reduced in AAMs when compared with ECM patches as measured by the amount of infiltrated inflammatory cells and area of inflammation. Immunohistochemistry demonstrated an increased CD3+ cell density in the AAMs patch infiltrate. Conclusions. Epicardial AAMs transplantation demonstrated safety and clinical feasibility. The use of micrografts significantly inhibited ECM-induced foreign body inflammatory reactivity. Transplantation of AAMs shows good clinical applicability as adjuvant therapy to cardiac surgery and can suppress acute inflammatory reactivity.
