Validation of upper extremity kinematics using Markerless motion capture

Movement research has typically been performed using three-dimensional (3D) marker-based motion capture, which is considered the “gold-standard” for biomechanical assessment. However, limitations exist due to the lack of portability, extensive preparation for data collection, marker placement training, error due to marker movement, and possible skin irritation due to marker adhesives. There is inherent error due to motion artifact stemming from skin movement and differences in marker placement between testers. Markerless motion capture systems are emerging as a new method of kinematic assessment. These methods require little preparation and there is no need to alter participant clothing. Markerless motion capture has also been validated for the lower extremity in healthy older adults during gait. However, it has not been validated for other populations or for the assessment of upper extremity (UE) motion. Therefore, the purpose of this study was to examine differences in calculated UE kinematics between marker-based and a markerless motion capture system. Participants attended two data collection sessions. Marker-based and markerless motion capture data was collected simultaneously while participants completed the Box and Blocks test (BBT). Kinematic and spatiotemporal data from both systems was exported using identical time series to ensure the same conditions for comparisons. Intraclass Correlation Coefficients (ICCs) were calculated to determine between session reliability for both systems on range of motion and peak joint angular data to ensure movement variability was not affecting measurement consistency. ICCs and Bland Altman statistics were also calculated between the systems. Root mean square deviation (RMSD) values were determined between demeaned UE joint angles for the two systems to examine movement pattern differences. The resulting between-session ICCs for each system showed that the markerless system shared similar reliability during this task as the marker-based system, further supporting the effect of variability on between-session reliability. Between-system ICCs resulted in good (0.7<ICC<0.9) to excellent (ICC>0.9) agreement. Bland Altman results confirmed the existence of measurement bias between the systems. RMSD values for all UE joint angles were found to be less than 6°. Overall, the results from this study support the use of markerless motion capture in clinical settings to examine upper extremity biomechanics in children.

Development of a Low-Cost Markerless Optical Motion Capture System for Gait Analysis and Anthropometric Parameter Quantification.

Technological advancements have expanded the range of methods for capturing human body motion, including solutions involving inertial sensors (IMUs) and optical alternatives. However, the rising complexity and costs associated with commercial solutions have prompted the exploration of more cost-effective alternatives. This paper presents a markerless optical motion capture system using a RealSense depth camera and intelligent computer vision algorithms. It facilitates precise posture assessment, the real-time calculation of joint angles, and acquisition of subject-specific anthropometric data for gait analysis. The proposed system stands out for its simplicity and affordability in comparison to complex commercial solutions. The gathered data are stored in comma-separated value (CSV) files, simplifying subsequent analysis and data mining. Preliminary tests, conducted in controlled laboratory environments and employing a commercial MEMS-IMU system as a reference, revealed a maximum relative error of 7.6% in anthropometric measurements, with a maximum absolute error of 4.67 cm at average height. Stride length measurements showed a maximum relative error of 11.2%. Static joint angle tests had a maximum average error of 10.2%, while dynamic joint angle tests showed a maximum average error of 9.06%. The proposed optical system offers sufficient accuracy for potential application in areas such as rehabilitation, sports analysis, and entertainment.

Evaluation of the Margin of Stability during Gait Initiation in Young Healthy Adults, Elderly Healthy Adults and Patients with Parkinson's Disease: A Comparison of Force Plate and Markerless Motion Capture Systems.

Gait initiation (GI) is a functional task classically used in the literature to evaluate the capacity of individuals to maintain postural stability. Postural stability during GI can be evaluated through the "margin of stability" (MoS), a variable that is often computed from force plate recordings. The markerless motion capture system (MLS) is a recent innovative technology based on deep learning that has the potential to compute the MoS. This study tested the agreement between a force plate measurement system (FPS, gold standard) and an MLS to compute the MoS during GI. Healthy adults (young [YH] and elderly [EH]) and Parkinson's disease patients (PD) performed GI series at spontaneous (SVC) and maximum velocity (MVC) on an FPS while being filmed by a MLS. Descriptive statistics revealed a significant effect of the group (YH vs. EH vs. PD) and velocity condition (SVC vs. MVC) on the MoS but failed to reveal any significant effect of the system (MLS vs. PFS) or interaction between factors. Bland-Altman plot analysis further showed that mean MoS biases were zero in all groups and velocity conditions, while the Bayes factor 01 indicated "moderate evidence" that both systems provided equivalent MoS. Trial-by-trial analysis of Bland-Altman plots, however, revealed that differences of >20% between the two systems did occur. Globally taken, these findings suggest that the two systems are similarly effective in detecting an effect of the group and velocity on the MoS. These findings may have important implications in both clinical and laboratory settings due to the ease of use of the MLS compared to the FPS.

DARI Evaluation Syndesmosis

Introduction/Purpose: Return to play (RTP) assessment and decision-making protocols are among the most discussed topics in sports medicine. Given the lack of validated guidelines, physicians and rehabilitation practitioners often rely on subjective functional evaluation to guide decisions to RTP. Biomechanical assessment using motion capture may be a useful strategy to evaluate an athlete’s post-injury functional status, and to estimate their ability to RTP with reduced risk of re-injury. The purpose of this case-study was to determine the efficacy of using marker-less 3D motion capture to provide an objective functional evaluation to tailor rehabilitation and aid RTP status for a patient who underwent syndesmotic fixation. Methods: In this case-study, a National Collegiate Athletic Association Division I collegiate football offensive lineman (Height: 1.96 m, Weight: 141 kg) performed a movement screen 5 weeks after left ankle syndesmotic fixation for purely ligamentous syndesmotic injury. Testing was performed at the anticipated time of RTP. After a standardized warm-up, the patient performed a series of 14 movements consisting of upper and lower extremity actions in all three planes of motion, including bilateral and unilateral lower extremity actions. Kinematic data was captured using an 8-camera marker-less motion capture system (MLMCS). Left and right joint-specific ranges of motion were compared for symmetry and to normative data produced by the MLMCS manufacturer. Results: The participant successfully performed all 14 movements without limitation. Ankle flexion was symmetrical during bilateral and unilateral squatting actions. However, left ankle (i.e., involved side) flexion was consistently less than right ankle flexion during more dynamic actions (Table 1). Despite the asymmetries, ankle range of motion was within normal ranges for both sides in all movements. From a performance standpoint, left-side jump heights were consistently less than the right-side efforts during the unilateral countermovement jump (left: 34.5 cm versus right: 41.1 cm; -16.0%) and consecutive hops (left: 29.5 cm versus right: 33.0 cm; -10.8%). Results were shared with the athletic trainer to focus rehabilitation efforts. The patient was able to fully RTP at 6 weeks. Conclusion: In this case-study, the patient successfully performed a movement screen without limitation at the time of RTP after left ankle syndesmotic fixation. A MLMCS detected kinematic differences that would be difficult to qualitatively recognize. Specifically, the patient expressed reduced ankle flexion and jumping performance on the operative side. No baseline screening was performed, but the observed asymmetries were consistent with what would be expected from the specific injury. Further research is needed to compare baseline measures to kinematic changes. The Contribution of Lower-Body Kinematics to Pitching and Hitting Performance in Baseball: Utilizing the OpenBiomechanics Project.

As markerless motion capture systems become more affordable than ever, it is becoming far easier to collect and analyze kinematic data on baseballers. To ensure this data can be used impactfully, coaches and practitioners should possess a good understanding of specific technique characteristics that are associated with enhanced performance in pitchers and hitters. This study used the open-source data provided by Driveline Baseball's OpenBiomechanics Project to evaluate the contribution of lower-body kinematics to pitch velocity and bat speed. In addition to correlational analyses to examine the association between discrete kinematic variables and performance, statistical nonparametric mapping was used to compare slow and fast velocity groups across the entirety of pitching and hitting motions from peak knee height to ball release/contact. It was found that rotation of the trail leg and extension of the lead knee were both associated with performance in pitchers and hitters. Consequently, coaches and practitioners should ensure that individuals possess an adequate level of strength and flexibility to facilitate optimal movement of the trail hip and lead knee during pitching and hitting movements. If deficiencies exist, then training programs should be designed to address these issues and eventually bring about an improvement in performance.

Motion acquisition of gait characteristics one week after total hip arthroplasty: a factor analysis

IntroductionClinical gait analysis can be used to evaluate the recovery process of patients undergoing total hip arthroplasty (THA). The postoperative walking patterns of these patients can be significantly influenced by the choice of surgical approach, as each procedure alters distinct anatomical structures. The aim of this study is twofold. The first objective is to develop a gait model to describe the change in ambulation one week after THA. The secondary goal is to describe the differences associated with the surgical approach.Materials and methodsThirty-six patients undergoing THA with lateral (n = 9), anterior (n = 15), and posterior (n = 12) approaches were included in the study. Walking before and 7 days after surgery was recorded using a markerless motion capture system. Exploratory Factor Analysis (EFA), a data reduction technique, condensed 21 spatiotemporal gait parameters to a smaller set of dominant variables. The EFA-derived gait domains were utilized to study post-surgical gait variations and to compare the post-surgical gait among the three groups.ResultsFour distinct gait domains were identified. In postsurgical walking, Phases is statistically significantly different in patients operated with the posterior approach compared to lateral (p-value = 0.017) and anterior (p-value = 0.002) approaches. Furthermore, stance time in the posterior approach group is significantly lower than in healthy individuals (p-value < 0.001).ConclusionsThis study identified a four-component gait model specific to THA patients. The results showed that patients after THA have longer stride time but shorter stride length, wider base of support, and longer stance time, although the posterior group had a statistically significant shorter stance time than the others. The findings of this research have the potential to simplify the reporting of gait outcomes, reduce redundancy, and inform targeted interventions in regards to specific gait domains.

Comparison of kinematics and joint moments calculations for lower limbs during gait using markerless and marker-based motion capture.

Objective: This study aimed at quantifying the difference in kinematic and joint moments calculation for lower limbs during gait utilizing a markerless motion system (TsingVA Technology, Beijing, China) in comparison to values estimated using a marker-based motion capture system (Nokov Motion Capture System, Beijing, China). Methods: Sixteen healthy participants were recruited for the study. The kinematic data of the lower limb during walking were acquired simultaneously based on the markerless motion capture system (120Hz) and the marker-based motion capture system (120Hz). The ground reaction force was recorded synchronously using a force platform (1,200Hz). The kinematic and force data were input into Visual3D for inverse dynamics calculations. Results: The difference in the lower limb joint center position between the two systems was the least at the ankle joint in the posterior/anterior direction, with the mean absolute deviation (MAD) of 0.74cm. The least difference in measuring lower limb angles between the two systems was found in flexion/extension movement, and the greatest difference was found in internal/external rotation movement. The coefficient of multiple correlations (CMC) of the lower limb three joint moments for both systems exceeded or equaled 0.75, except for the ad/abduction of the knee and ankle. All the Root Mean Squared Deviation (RMSD) of the lower limb joint moment are below 18N·m. Conclusion: The markerless motion capture system and marker-based motion capture system showed a high similarity in kinematics and inverse dynamic calculation for lower limbs during gait in the sagittal plane. However, it should be noted that there is a notable deviation in ad/abduction moments at the knee and ankle.

Kinematic analysis of preparation for transferring from wheelchair to bed

ABSTRACT This study aimed to clarify the kinematics, particularly of the shoulder and hip joints, during preparation for manual wheelchair-to-bed transfer (i.e. when flipping up the arm and foot supports). This cross-sectional study included 32 able-bodied individuals. The kinematics of the shoulder and hip joints when the arm and foot supports were flipped up of manual wheelchair, were evaluated using a markerless inertial sensor-based motion capture system. We found that flipping the arm support upwards involved a large amount of abduction, internal and external rotation, flexion, and extension at the shoulder joint, whereas flipping the foot support upwards involved a large amount of flexion at the hip joint. The findings suggest that it is necessary to consider the range of motion required to flip up the arm and foot supports of manual wheelchairs, particularly in those with limited shoulder and hip range of motion such as older people, neuromuscular disorders, and orthopedic disorders.

Reliability of Markerless Motion Capture Systems for Assessing Movement Screenings.

Movement screenings are commonly used to detect unfavorable movement patterns. Markerless motion capture systems have been developed to track 3-dimensional motion. To determine the reliability of movement screenings assessed using a markerless motion capture system when comparing the results of multiple systems and multiple collection periods. Descriptive laboratory study. The inter- and intrarater reliability of a commercially available markerless motion capture system were investigated in 21 recreationally active participants aged between 18 and 22 years. A total of 39 kinematic variables arising from 10 fundamental upper and lower body movements typical of a screening procedure in sports performance were considered. The data were statistically analyzed in terms of relative error via the intraclass correlation coefficient (ICC) and absolute error via the residual standard error (RSE). Both inter- and intrarater reliability ICCs were at least moderate across all variables (ICC, >0.50), with most movements and corresponding variables having excellent reliability (ICC, >0.90). Although maximum knee valgus angles were the kinematic variables with the lowest interrater reliability (ICCs, 0.59-0.82) and moderate relative agreement, there was agreement in absolute terms with an RSE of <1.3°. Findings indicated that markerless motion capture provides reliable measurements of joint position during a movement screen, which allows for a more objective evaluation of the direction and subsequent success of interventions. However, practitioners should consider relative and absolute agreements when applying information provided by these systems. Markerless motion capture systems may assist clinicians by reliably assessing movement screenings using different systems over different collection periods.

Jumping towards field-based ground reaction force estimation and assessment with OpenCap

Low-cost and field-viable methods that can simultaneously assess external kinetics and kinematics are necessary to enhance field-based biomechanical monitoring. The aim of this study was to determine the accuracy and usability of ground reaction force (GRF) profiles estimated from segmental kinematics, measured with OpenCap (a low-cost markerless motion-capture system), during common jumping movements. Full-body segmental kinematics were recorded for fifteen recreational athletes performing countermovement, squat, bilateral drop, and unilateral drop jumps, and used to estimate vertical GRFs with a mechanics-based method. Eleven distinct performance-, fatigue-, or injury-related GRF variables were then validated against a gold-standard force platform. Across jumping movements, a total of six and three GRF variables were estimated with a bias or limits of agreement <5 % respectively. Bias and limits of agreement were between 5 and 15 % for seventeen and nineteen variables respectively. Moreover, we show that estimated force variables with a bias <15 % can adequately assess the within-athlete changes in GRF variables between jumping conditions (arm swing or leg dominance). These findings indicate that using a low-cost and field-viable markerless motion capture system (OpenCap) to estimate and assess GRF profiles during common jumping movements is approaching acceptable limits of accuracy. The presented method can be used to monitor force variables of interest and examine underlying segmental kinematics. This application is a jump towards researchers and sports practitioners performing biomechanical monitoring of jumping efficiently, regularly, and extensively in field settings.

Markerless motion capture provides accurate predictions of ground reaction forces across a range of movement tasks

Measuring or estimating the forces acting on the human body during movement is critical for determining the biomechanical aspects relating to injury, disease and healthy ageing. In this study we examined whether quantifying whole-body motion (segmental accelerations) using a commercial markerless motion capture system could accurately predict three-dimensional ground reaction force during a diverse range of human movements: walking, running, jumping and cutting. We synchronously recorded 3D ground reaction forces (force instrumented treadmill or in-ground plates) with high-resolution video from eight cameras that were spatially calibrated relative to a common coordinate system. We used a commercially available software to reconstruct whole body motion, along with a geometric skeletal model to calculate the acceleration of each segment and hence the whole-body centre of mass and ground reaction force across each movement task. The average root mean square difference (RMSD) across all three dimensions and all tasks was 0.75 N/kg, with the maximum average RMSD being 1.85 N/kg for running vertical force (7.89 % of maximum). There was very strong agreement between peak forces across tasks, with R2 values indicating that the markerless prediction algorithm was able to predict approximately 95–99 % of the variance in peak force across all axes and movements. The results were comparable to previous reports using whole-body marker-based approaches and hence this provides strong proof-of-principle evidence that markerless motion capture can be used to predict ground reaction forces and therefore potentially assess movement kinetics with limited requirements for participant preparation.

Dynamic assessment of spine movement patterns using an RGB-D camera and deep learning

In clinical practice, functional limitations in patients with low back pain are subjectively assessed, potentially leading to misdiagnosis and prolonged pain. This paper proposes an objective deep learning (DL) markerless motion capture system that uses a red–green–blue-depth (RGB-D) camera to measure the kinematics of the spine during flexion–extension (FE) through: 1) the development and validation of a DL semantic segmentation algorithm that segments the back into four anatomical classes and 2) the development and validation of a framework that uses these segmentations to measure spine kinematics during FE. Twenty participants performed ten cycles of FE with drawn-on point markers while being recorded with an RGB-D camera. Five of these participants also performed an additional trial where they were recorded with an optical motion capture (OPT) system. The DL algorithm was trained to segment the back and pelvis into four anatomical classes: upper back, lower back, spine, and pelvis. A kinematic framework was then developed to refine these segmentations into upper spine, lower spine, and pelvis masks, which were used to measure spine kinematics after obtaining 3D global coordinates of the mask corners. The segmentation algorithm achieved high accuracy, and the root mean square error (RMSE) between ground truth and predicted lumbar kinematics was < 4°. When comparing markerless and OPT kinematics, RMSE values were < 6°. This work demonstrates the feasibility of using markerless motion capture to assess FE spine movement in clinical settings. Future work will expand the studied movement directions and test on different demographics.

Agreement between a markerless and a marker-based motion capture systems for balance related quantities

Balance studies usually focus on quantities describing the global body motion. Assessing such quantities using classical marker-based approach can be tedious and modify the participant’s behaviour. The recent development of markerless motion capture methods could bypass the issues related to the use of markers. This work compared dynamic balance related quantities obtained with markers and videos. Sixteen young healthy participants performed four different motor tasks: walking at self-selected speed, balance loss, walking on a narrow beam and countermovement jumps. Their movements were recorded simultaneously by marker-based and markerless motion capture systems. Videos were processed using a commercial markerless pose estimation software, Theia3D. The centre of mass position (CoM) was computed, and the associated extrapolated centre of mass position (XCoM) and whole-body angular momentum (WBAM) were derived. Bland-Altman analysis was performed and root mean square difference (RMSD) and coefficient of correlation were computed to compare the results obtained with marker-based and markerless methods. Bias remained of the magnitude of a few mm for CoM and XCoM positions, and RMSD of CoM and XCoM was around 1 cm. RMSD of the WBAM was less than 10 % of the total amplitude in any direction, and bias was less than 1 %. Results suggest that outcomes of balance studies will be similar whether marker-based or markerless motion capture system are used. Nevertheless, one should be careful when assessing dynamic movements such as jumping, as they displayed the biggest differences (both bias and RMSD), although it is unclear whether these differences are due to errors in markerless or marker-based motion capture system.

Vertical Center-of-Mass Braking and Motor Performance during Gait Initiation in Young Healthy Adults, Elderly Healthy Adults, and Patients with Parkinson's Disease: A Comparison of Force-Plate and Markerless Motion Capture Systems.

This study tested the agreement between a markerless motion capture system and force-plate system ("gold standard") to quantify stability control and motor performance during gait initiation. Healthy adults (young and elderly) and patients with Parkinson's disease performed gait initiation series at spontaneous and maximal velocity on a system of two force-plates placed in series while being filmed by a markerless motion capture system. Signals from both systems were used to compute the peak of forward center-of-mass velocity (indicator of motor performance) and the braking index (indicator of stability control). Descriptive statistics indicated that both systems detected between-group differences and velocity effects similarly, while a Bland-Altman plot analysis showed that mean biases of both biomechanical indicators were virtually zero in all groups and conditions. Bayes factor 01 indicated strong (braking index) and moderate (motor performance) evidence that both systems provided equivalent values. However, a trial-by-trial analysis of Bland-Altman plots revealed the possibility of differences >10% between the two systems. Although non-negligible differences do occur, a markerless motion capture system appears to be as efficient as a force-plate system in detecting Parkinson's disease and velocity condition effects on the braking index and motor performance.

Detection of kinematic abnormalities in persons with knee osteoarthritis using markerless motion capture during functional movement screen and daily activities.

Background: The functional movement screen (FMS) has been used to identify deficiencies in neuromuscular capabilities and balance among athletes. However, its effectiveness in detecting movement anomalies within the population afflicted by knee osteoarthritis (KOA), particularly through the application of a family-oriented objective assessment technique, remains unexplored. The objective of this study is to investigate the sensitivity of the FMS and daily activities in identifying kinematic abnormalities in KOA people employing a markerless motion capture system. Methods: A total of 45 persons, presenting various Kellgren-Lawrence grades of KOA, along with 15 healthy controls, completed five tasks of the FMS (deep squat, hurdle step, and in-line lunge) and daily activities (walking and sit-to-stand), which were recorded using the markerless motion capture system. The kinematic waveforms and discrete parameters were subjected to comparative analysis. Results: Notably, the FMS exhibited greater sensitivity compared to daily activities, with knee flexion, trunk sagittal, and trunk frontal angles during in-line lunge emerging as the most responsive indicators. Conclusion: The knee flexion, trunk sagittal, and trunk frontal angles during in-line lunge assessed via the markerless motion capture technique hold promise as potential indicators for the objective assessment of KOA.

Evaluation of 3D Markerless Motion Capture System Accuracy during Skate Skiing on a Treadmill.

In this study, we developed a deep learning-based 3D markerless motion capture system for skate skiing on a treadmill and evaluated its accuracy against marker-based motion capture during G1 and G3 skating techniques. Participants performed roller skiing trials on a skiing treadmill. Trials were recorded with two synchronized video cameras (100 Hz). We then trained a custom model using DeepLabCut, and the skiing movements were analyzed using both DeepLabCut-based markerless motion capture and marker-based motion capture systems. We statistically compared joint centers and joint vector angles between the methods. The results demonstrated a high level of agreement for joint vector angles, with mean differences ranging from -2.47° to 3.69°. For joint center positions and toe placements, mean differences ranged from 24.0 to 40.8 mm. This level of accuracy suggests that our markerless approach could be useful as a skiing coaching tool. The method presents interesting opportunities for capturing and extracting value from large amounts of data without the need for markers attached to the skier and expensive cameras.

Design and Testing of an Active Balancing Board for Exergames

In recent years, there has been a growing recognition of the importance of balance and coordination in maintaining overall health, particularly among the elderly population. As societies around the world experience demographic shifts with an increasing number of elderly individuals, age-related declines in balance and coordination can lead to an elevated risk of falls and fall-related injuries, making it a significant public health concern. The need for effective balance rehabilitation and fall prevention strategies has become paramount. This paper presents a motion platform designed for balance rehabilitation exercises using exergames. The system consists of a 3 degrees of freedom (3DOF) platform actuated by four servomotors controlled by Omron-DeltaTau real-time motion controller. As a novelty with respect to other State of Art (SoA) actuated platforms, in the present work the movements of the user's body, which determine the platform motion, are tracked by exploiting a markerless motion capture system with an RGB camera and pose-estimation algorithm MediaPipe. The human displacement is used to generate the input for the roll rotation of the platform according to a one d.o.f. transfer function, whose parameters can be set to get different levels of difficulties in the exercise. The device offers therefore the flexibility to set different parameters to modify the level of the users’ training. The proposed device has the potential for a larger variety of rehabilitation activities and exergames. This paper describes the elements adopted for the implementation of this human-in-the-loop system, presents some preliminary tests to explore the influence of transfer function parameters on human and platform movements, and concludes with future development possibilities.

Biomechanics Analysis of Basketball Shooting Via OpenPose Motion Capture System

In basketball, it is crucial to understand the optimal shooting pattern for each individual in terms of gender and shooting distances from the basket. However, incorporating biomechanics shooting analysis quantitatively into the learning process is not preferable. Hence, the purpose of this study is to determine the quantitative aspect of basketball shooting for biomechanics analysis using the OpenPose motion capture system. This marker-less motion capture system generated the biomechanical parameter data subjected to a significant difference test for shooting performance. A total of four players – two males and two females, divided into two levels (intermediate and novice) performed ten continuous jump shots from two different shooting distances (4.57 m and 6.40 m) from the basket. Each individual’s biomechanical parameters were tested in a statistical or independent t test to determine which significant parameter has a measurable difference in shooting performance. When the effects of different body angles on missed to scored baskets were compared, only the right elbow angle (intermediate female player), the right shoulder angle (novice male player) and the left hip angle (intermediate female player and novice male player) showed a significant difference (p &lt; 0.05) towards shooting performance. In the comparison of the velocity of right upper limb key points, only the velocity of the right shoulder (intermediate male player) showed a significant difference (p &lt; 0.05) toward shooting performance. In conclusion, the findings of this study suggest that shooting analysis should be done quantitatively to demonstrate a more profound and clear understanding of biomechanics when considering an improvement in shooting performance.

Comparison of markerless and marker-based motion capture systems using 95% functional limits of agreement in a linear mixed-effects modelling framework

Biomechanics analysis of human movement has been proven useful for maintenance of health, injury prevention, and rehabilitation in both sports and clinical populations. A marker-based motion capture system is considered the gold standard method of measurement for three dimensional kinematics measurements. However, the application of markers to anatomical bony points is a time consuming process and constrained by inter-, intra-tester and session reliability issues. The emergence of novel markerless motion capture systems without the use of reflective markers is a rapidly growing field in motion analysis. However an assessment of the level of agreement of a markerless system with an established gold standard marker-based system is needed to ensure the applicability of a markerless system. An extra layer of complexity is involved as the kinematics measurements are functional responses. In this paper a new approach is proposed to generate 95% functional limits of agreement (fLoA) using the linear mixed-effects modelling framework for hierarchical study designs. This approach is attractive as it will allow practitioners to extend their use of linear mixed models to assess agreement in method comparison studies in all domains where functional responses are recorded.

Markerless motion capture technology to quantify functional performance in dementias: a systematic review

AbstractBackgroundQuantifying functional performance objectively and unobtrusively could offer vital insights for clinical decision making on the level of support required by people living with dementia and other neurodegenerative conditions. Markerless motion capture (MMC) is more suitable for deployment in clinical and real‐world settings because single sensor MMC systems are less expensive and less complex compared to marker‐based systems. The aim of this study was to complete a systematic review of published literature on the use of MMC with full‐body tracking in people with dementias.MethodWe systematically searched for relevant articles using the search terms motion capture, motion analysis and movement analysis combined with dementia, mild cognitive impairment (MCI) and Parkinson’s disease (PD) which yielded a total of 1103 results. Inclusion criteria were MMC and full‐body tracking. Studies evaluating marker‐based motion capture, body‐worn sensors or Inertial Measurement Units, virtual reality, kinetics and interventions like DBS or exercises were excluded. 134 studies were included for full article screening out of which 17 eligible studies that met the selection criteria were included in the review.ResultMMC devices used in the reviewed studies included Microsoft Kinect (n = 15) and unspecified 2D motion camera (n = 2). Patient groups included in the studies were PD (n = 12), dementia (n = 4) and MCI (n = 1). Functional performance components evaluated were gait (n = 16), sit to stand (n = 2), turning (n = 1) and stepping in place (n = 1). About half the studies used advanced machine learning methods whereas the other half used descriptive statistical methods. The lack of standardisation in the measures used prevented us from comparing and synthesizing study results.ConclusionAlthough MMC has the potential to be used for quantifying functional performance in people with dementia, MCI and PD, there appears to be significant variation in the methods used in its evaluation. Although features of mobility were measured, none of the reviewed studies analysed activities of daily living tasks, a key component of functional performance. Standardising the process for extracting features and assessing accuracy would enable more effective validation of real‐world impact. Further research is required in people with neurodegenerative conditions for MMC to be routinely used in clinical and real‐world applications.
