An Observational Study of Head and Neck Squamous Carcinomas from a Tertiary Cancer Centre of Marathwada Region, Maharashtra, India

Introduction: Head and neck cancer constitutes about one third of all the cancers in India. Head and neck squamous carcinoma is an important cause of morbidity and mortality with variability in geographical location, age, gender, habits, socio-economic status. Aim: To identify changing trends in age, gender, sites involved and histopathological grades of malignant and analyse potentially malignant lesions in head and neck region in Marathwada region of Maharashtra, India. Materials and Methods: This observational retrospective descriptive study was carried out in tertiary cancer hospital in Marathwada region of Maharashtra, India, over a period of two years which included 1008 cases. All the biopsy cases and biopsies obtained under ultrasound and also to computed tomography guidance showing invasive squamous carcinoma and potentially malignant lesions of head and neck region with their histological analysis were studied. Relevant clinical and demographic details were noted and statistical analysis was done and results were explained in tables and percentage. Results: Total 1008 cases were studied. The maximum number of cases were in the age group of 51-60 years. Males were more affected than females. The buccal mucosa (29.5%) was most common site affected and maxilla (0.8%) the least. Histologically invasive tumour were well differentiated (17.65%), moderately differentiated (79.93%) and poorly differentiated (2.42%). Rest lesions were distributed in following categories: carcinoma- in-situ, verrucous neoplasm, dysplasia, atypical squamous proliferation, leukoplakia, pseudoepitheliomatous hyperplasia. Age wise, site wise, gender wise distribution of cases and association between site of lesions and gender and grade of tumour were studied. Conclusion: The associated risk factors, lifestyle patterns, habits, addictions and geographic and demographic factors, socio-economic status and evolving Human Papilloma Virus (HPV) association and pathogenesis are still topic of debate where much progress is still to be made. It deserves closer study. Apart from invasive squamous malignancies, precursor lesions if investigated at an earliest along with increasing awareness among population in relation to healthy lifestyle changes, habits followed by thorough clinical examination and histopathology and HPV association, possibility of planning specific strategies of prevention, diagnosis and treatment help reduce morbidity and mortality.

Effect of Straw Mulching on In-situ Moisture Conservation, Productivity and Profitability of Rainfed Soybean on Farmer’s Field

Soybean is a major kharif crop grown in Marathwada region of Maharashtra under rainfed condition and is being a preferred crop by the marginal and small rainfed farmers. Though, most of the part of the region is comes under assured rainfall zone but it suffers from occurrence of frequent dryspells. The region receives mean annual rainfall of 880 mm. Rainfall in uncertain and erratic in this region. The productivity of soybean particularly in Marathwada region is uncertain due Occurrence of 3 to 4 dryspells in July to September. The crop productivity decreases with either deficiency of rainfall or its distribution or due to moisture stress in critical growth period due to dryspells occurred in July and August. Thus, to conserve the rainwater and to reduce soil evaporation, straw mulching in inter row space was advocated to farmers to avoid moisture stress and to cope with dryspells. The research demonstrations were conducted on farmer’s field for 3 year since 2019-20 to 2021-22. Results indicated that straw mulch was found effective in in-situ moisture conservation. It was observed that soybean productivity was increased by 21.77 per cent due to application of straw mulch. Similarly, GMR and NMR was increased by 22.25% and 30.84% respectively, due to application of straw mulch. Straw mulch was found effective in additional 22% moisture conservation and found effective in avoiding moisture stress particularly during dryspell period.

Clinical Outcome of Breast Conservation Surgery in Early Breast Cancer Patients: A Prospective Interventional Study

Introduction: Breast conservation surgery (BCS) has gained acceptance among the surgeons for the management of young patients with early breast cancer. Aim: To study the outcomes of breast conservation surgery in early breast cancer patients at a tertiary care centre in Marathwada region of Maharashtra, India. Materials and Methods: This prospective interventional study included 40 patients in the age group of 20-65 years with stage I or stage II breast cancer was carried out in Government Medical College and Hospital, Aurangabad, India. Once staging workup was done, all patients were evaluated for suitability of BCS. Surgical treatment consisted of wide local excision and axillary dissection with gross tumour surgical margin of 2 cm. Surgical morbidity like prolonged seroma formation, surgical site infection, haematoma, lymphoedema was assessed postoperatively. Pathological assessment included primary tumour size, histological type, margin status. Radiotherapy was given to all patients. Chemotherapy was used where indicated with the appropriate regimen. All patients were followed up at three months, six months and 12 months. Cosmetic outcome was assessed by Harris four point Likert Scale. Descriptive statistics was used and results were expressed in terms of frequency and percentages. Results: In this study; 28 (70%) patients had no postoperative wound complications. About 4 (10%) had surgical site infection, and 8 (20%) had prolonged seroma formation. Out of 40 patients, 37 (92.5%) patients postoperatively had all the margins free from the disease on histopathology, 2 (5%) had single margin positive which required revision surgery, and 1 (2.5%) had all the margins positive; hence Modified Radical Mastectomy (MRM) was performed. The cosmetic outcome was good to excellent in 34 (85%) cases. Conclusion: Breast conserving surgery had a good cosmetic outcome. Most patients had disease free margins.

Impact of medium of instruction during school education on the academic performance of medical students: An observational study

Introduction: Many of the students admitted in Indian medical colleges have had school education in vernacular medium. There exists a general opinion that such students find it difficult to cope up with medical academics in English medium as they think in one language and study in another. This study was carried out to find whether such language barrier affects their professional performance. Materials and Methods: This multi-centric retrospective observational study was carried out in 274 medical students from four government medical colleges in Marathwada region of Maharashtra, India. The medium of instruction during their school education was recorded from their 10th standard mark-sheets. The marks of final year MBBS were obtained from their academic record in academic section of the colleges. The data were analysed using one-way ANOVA. Result: There was statistically no significant difference of professional performance among the medical students with different media of instruction in their school education (P=0.1149). Conclusion: Medium of instruction of school education doesn’t have any significant effect on professional performance of medical students. Comparative study of birth weight in newborns in tribal and non-tribal area in Marathwada region of Maharashtra, India

Introduction: Birth weight is an important health indicator. The health indicator helps us to know the trend of population structure and social setup of the tribal people. The present study was conducted to study the birth weight among two population categories - tribal population of Kinwat, District Nanded and non-tribal population of Badnapur, Jalna, Maharashtra. Materials and Methods: The birth weight of 2000 neonates born from 1st April 2012 to 31st March 2014 were obtained from the registers of the maternity wards of two Hospitals; More Nursing Home, Kinwat District Nanded for tribal population and IIMSR Medical College & Noor Hospital, Badnapur, Jalna, Maharashtra, India. Results: Mean birth weight among the tribal neonates (2.74 kg) was significantly lower (p-value=0.001) than among non-tribal neonates (2.88 kg). Also, the proportion of low birth weight neonates was significantly higher among the tribal population. Similarly, for maternal age (age group 20-25 years) and parity (Para 2-4), the mean birth weight is significantly lower in tribal population as compared to the non-tribal population. Conclusion: The mean birth weight is lower in tribal population and also the proportion of low birth weight in neonates is high in this population. This signifies that the tribal population deserves attention and focused health interventions. Keywords: Birth weight, Tribal population, Low birth weight, Maternal age, Parity.

Antifungal activity of leaf extract against mycotoxin producing fungi

About 90% of the world food crops are being produced by using seeds. Healthy seeds are important for production of healthy crop and these seeds are also responsible for disease transmission. It happens either in field or in post-harvest storage condition. Due to these seed borne fungi, seed get deteriorated which may cause a great economic loss as well as several types of abnormalities occur in seeds. Major storage grain fungi contaminated with mycotoxin, mycotoxins contaminants of fungal origin occurring worldwide and characterized by its acute and chronic toxic effects on human health. In the present attempt were made to study the antifungal activity of leaf extract against mycotoxin producing fungi. Fungi isolated from stored seed grains like cereals, pulses, infected oil seeds were collected from from Marathwada region of Maharashtra, India. Antifungal activity of leaf extract of Calotropis procera, Azadirachta indica, Ocimum sanctum, Withania somnifera and Datura metel were tested against Alternaria alternata, Aspergillus flavus, Aspergillus ochraceus, Fusarium gramineorum and Penicillium citrinum by using 96-well plate method. Leaf extract showed remarkable activity against tested fungi at different concentrations as Minimum Inhibitory Concentration. The leaf extract of Calotropis procera and Azadirachta indica, have good potential antifungal properties against different mycotoxin producing fungi.

Socio-economic characteristics of chickpea growers in Marathwada region of Maharashtra

Pulses are the important sources of proteins, vitamins and minerals and are popularly known as “Poor man’s meat” and “rich man’s vegetable”, contribute significantly to the nutritional security of the country. Chickpea is the second most important grain legume in the world after dry bean. Chickpea is most important pulse crop of India in terms of area and production, widely grown for centuries and accounts for nearly 40 percent of the total pulse production in the country. Socio economic characters were calculated by using descriptive statistics mean, SD and CV, it was observed that the middle age farmers (>40 to ≤55) was 40.00 per cent, the young (>25 to ≤40) which was 12.50 per cent and old group farmers (>55 to ≤70) was 47 per cent. With respect to educational level secondary level was dominating with 39.17 per cent. About 52.50 per cent of growers belonged to medium family size. In respect of occupational level of chickpea growers, most of farmers belonged to agriculture that was 97.50 per cent followed by services 1.67 and business very negligible having 0.83 per cent.

Surgical pathology of whipple pancreaticoduodenectomy: A 3-year experience at a tertiary cancer care center of marathwada region of India

The aim was to study and analyze in detail various parameters of Whipple resection specimen. Whipple procedure is a radical surgery performed for operable tumors of ampullary-periampullary region, head of pancreas, common bile duct and duodenum. Pathologic assessment of Whipple’s resection specimen needs special attention because of peculiar complex anatomy of head of pancreas and related structures, and also because of difference of opinion about grossing protocols of dissection of the specimen and confusion about terms like ‘margins’ and ‘surfaces’A histopathologist has to be aware of these intricacies so as to diagnose and accurately evaluate factors of prognostic importance.This is the first major mono-center surgical pathology study of Whipple resections reported from Department of Oncopathology, Government Cancer Hospital, Aurangabad, a newly started state level cancer hospital serving patients from Marathwada region of Maharashtra, India. This is surgical pathology study of total 31(thirty one) cases of Whipple resections performed during 2017-19. All relevant histopathologic details pertaining to grossing, microscopic diagnosis, grading and staging, and histopathologic prognostic factors were comprehensively studied. Dissection and grossing was done as per the protocols followed at Tata Memorial Hospital MumbaiOut of total 31 Whipple resections performed at this hospital, 01(one) was classical Whipple’s PD, seventeen (17) were pylorus preserving pancreaticoduodenectomy(PPPD) andthirteen (13) were of extended pancreaticoduodenectomy resections.The patients had age ranging from 38 to 76 years. There were 20 male patients and 11 female patients (M:F-2:1). As far as site of lesion is concerned 16(52%) cases had periampullary lesion. 06(19%)hadlesion in head of pancreas,07(22%) tumors were of common bile duct and there were 02(7%) cases of duodenal carcinoma. Histopathology revealed 29(94%) cases of malignant neoplasm and 02(6%) of benign lesion which included one(01) case of Brunner's gland hyperplasia withpancreatitis in adjacent pancreas, and the remaining 01 was of necrotizing pancreatitis( acute on chronic inflammation) with pseudocyst formation involving head of pancreas.Pathological stage of most of the tumors (55%) was T3, followed by T1(25%) and T2(20%)Amongst total 29 cases of malignant neoplasm, 08(28%) had well differentiated adenocarcinoma. 17 cases (58%) were of moderately differentiated adenocarcinoma and 02(7%) each were of mucinous carcinoma and neuroendocrine carcinoma. Microscopic involvement of margin/surface (CRM) was noted in 05 (17%)cases. In three(03) cases SMA surface and in 02 posterior surface were involved.PNI was noted in 12(41%) cases and LVI in 05(17%)cases. Peripancreatic lymph node metastasis was seen in 05(17%) cases. Specimens of regional lymphnodes were received separately & were negative for deposits in all the cases. Retroperitoneal lymph nodes received along with specimen of Extended Whipple’s were negative for metastasis in all the cases.From this study we conclude that important prognostic factors were location, extension, pathologic stage, histologic grade, status of margins / surfaces, LVI, PNI, and lymph node status. Therefore pathologic assessment of surgical specimen of pancreaticoduodenectomy (Whipple resection) needs special attention to evaluate these factors. To fulfill this purpose there is need of evolving standardized grossing protocols as well as uniform terminology related to terms ‘margin’ & ‘surface’.

Hemoglobinopathy pattern among school and college students in Marathwada region of Maharashtra: A study of 500 students

Introduction: Sickle hemoglobin is highly prevalent among the tribal of central, southern, and western India which ranges from 10 to 23%. Sickle cell hemoglobinopathy a hereditary conditions that causes morbidity and mortality in school going children. Aims and Objectives: To study the prevalence of sickle cell disorder among the students. Material and Methods: All the patient presented with sign and symptoms of anemia with suspicious of sickle cell disease and visiting various schools and colleges the samples were collected for CBC, peripheral smear, HPLC examination and reported. Results: Total 500 blood samples collected from school and college students for screening by HPLC and their hemoglobinopathy status. Showing prevalence of sickle cell anemia is 5.4%. Conclusions: It can be concluded that approaching community can be possible through school going adolescent for conducting surveillance of sickle cell anaemia. As it was small study, similar study of large size can be repeated by regular and planned visit to schools.

Study to assess the knowledge regarding diabetes management amongst medical intern and nursing staff in tertiary care teaching hospital in Marathwada region of Maharashtra, India

Background: Diabetes has become a major health problem in India. Inadequate knowledge and training of frontline medical care givers like nursing staff, intern doctors and resident doctors may result in increase in both acute and chronic complications amongst diabetes patients. So, it results to high economic burden on family especially in rural parts of India. So, objective of this study was to assess knowledge about diabetes and its management amongst medical interns and nursing staff in tertiary care teaching hospital in Marathwada region of Maharashtra.Methods: This was a cross-sectional study conducted at IIMSR Medical College, Badnapur, Jalna, Maharashtra amongst 70 nursing staff and 70 medical interns by convenient sampling method during the period of June to August 2018. Knowledge on diabetes was assessed through 30 questions.Results: Knowledge related to diabetes and its management is assessed in 70 nursing staff and 70 medical interns using 50 item questionnaires. Out of 70 nursing staff mean knowledge score was 38.13 and out of 70 medical interns mean knowledge score was 40.17. Those nursing staff that had experience more than 5 years and positive family history of DM had significant greater knowledge than counterpart. There was no significant difference in knowledge level in gender, age, degree or diploma and whether in-service education opted or not amongst nursing staff. Amongst intern knowledge was more in those who had positive family history of DM. There was no significant difference in knowledge level in male and female interns.Conclusions: This study concluded that there was knowledge gap about diabetes and its management amongst frontline care givers like nursing staff as compared to medical interns suggesting requirement of additional training and educations amongst frontline health care providers.
