Modified Mandibular Lingual Arch for Orthodontic Traction of Impacted Mandibular Canine and Premolar: Case Reports

Impaction of the tooth can cause functional and esthetic problems, so early intervention is critical. This report describes two cases of orthodontic traction of impacted mandibular canine and mandibular premolar teeth using a modified mandibular lingual arch soldered with traction hooks rather than a conventional orthodontic appliance. By planning the position and shape of the traction hooks with consideration for the three-dimensional position and angulation of the impacted teeth, clinicians can apply the intended direction and magnitude of traction force. Furthermore, this appliance can be used for multiple impacted teeth in various locations within the mandibular arch by modifying the position and shape of the traction hooks.

Early orthodontic treatment in a Finnish public health centre: a retrospective cross-sectional study

Objectives The aim of this observational cross-sectional one-centre study was to assess whether the previously described national orthodontic treatment practises and international recommendations are implemented in a public health care centre in Finland. We also assessed early treatment practices and appliances used. Methods The study group comprised 801 children born in 2011 and 2012 residing in the Riihimäki health centre catchment area in Finland, representing 80% of the age cohort. The patient records were examined for data on orthodontic treatment, timing of treatment, appliances used, and occlusal traits. Results The children had been examined by four orthodontist specialists and two orthodontic postgraduate students. Mean age at occlusal examination had been 9 years. Of the children, 212 (26%) were undergoing or had undergone orthodontic treatment. An additional 4.4% were scheduled for treatment. The proportion of children deemed to need treatment was significantly different between the different orthodontists. The most frequently used appliances were quad-helix (30%), eruption guidance appliance (20%), head gear (14%), fixed appliances (10%), protraction facemask (10%), and passive mandibular lingual arch (6%). Conclusions Nearly one-third of children aged less than 12 years in the health centre were currently or had been in orthodontic treatment. Approximately half had received treatment with either quad-helix or eruption guidance appliance.

Mesioangulation of mandibular second molars: A case report

This case report describes a simple alternative for treatment of mesioangulation of mandibular second molars. An 11-year-old girl was diagnosed with unilateral posterior crossbite, moderate incisor crowding and mesioangulation of the right mandibular second molar. The ectopic second molar was uprighted using a modified lingual arch with a distal hook and elastic chains. Orthodontic traction began after appliance installation and was activated once per month using 120 g of force. After six months, the right mandibular second molar was in an upright position. Orthodontic treatment continued with teeth levelling and alignment until acceptable occlusal and aesthetic results were achieved. Adequate stability was observed one year after debonding. The modified mandibular lingual arch with a distal hook is a simple and effective alternative to manage mesioangulation of mandibular second molars with minimum adverse effects and stable outcomes.

Clinical Considerations for Dental Management of Children with Molar-Root Incisor Malformations.

Molar root-incisor malformation (MRIM) or molar-incisor malformation (MIM) is a new type of dental anomaly characterized by dysplastic roots of permanent first molars, occasionally second primary molars, and the crowns of maxillary central incisors. MRIM involving permanent first molars and second primary molars is characterized by normal crowns with short, thin, and narrow roots, whereas MRIM involving permanent maxillary central incisors exhibits constrictions of the crown in the cervical area. In the first case, we extracted the affected first permanent molars at the optimal timing to minimize space deficiencies and induce space closure. In addition, composite resin restorations were performed on the anterior central incisors. In the second case, a mandibular lingual arch was used to stabilize the affected teeth in order to mitigate discomfort by reducing rotational biting forces.

Management of the dentoalveolar effects of tongue hypertrophy in patients with Duchenne muscular dystrophy: A pilot study

Subjects affected by Duchenne muscular dystrophy (DMD) develop severe malocclusions with the progression of the disease, due to changes in orofacial musculature and function, including tongue hypertrophy. We aimed to evaluate the benefits of maintaining mandibular interarch width with the help of a simple fixed orthodontic appliance. Three adolescent DMD boys were selected consecutively to receive a passive rigid mandibular lingual arch, and followed for 4–5 years. An untreated age-matched control group was chosen and followed for a similar period. Study casts were obtained at baseline and after follow-up. Outcomes measured were overjet, overbite, maxillary and mandibular intermolar widths, mandibular arch depth, molar relationships, and the presence of lateral crossbites and anterior or lateral openbites. Changes in measurements obtained between the two time points were compared in each age-matched pair. There was a clinically important increase in the mandibular intermolar width in the non-treated children ranging from 2.5 mm to 9 mm, but not in those treated. Malocclusions generally deteriorated in untreated children while they remained stable in treated children. The use of a rigid mandibular lingual arch in boys with DMD can help slow down the rapid deterioration of the developing malocclusions that accompanies the progression of the disease.

Class II malocclusion with deep overbite: a sequential approach

OBJECTIVE: The aim of this case report was to evaluate the results of deep overbite correction by the segmented arch technique. METHODS: The first stage of the treatment was performed with this technique to intrude incisors and canines. A maxillary transpalatal bar and a mandibular lingual arch using 0.051-in archwire were employed associated to a facial bow, anchored primarily with occipital pull and, after molar relationship correction, changed to high pull. RESULTS: Upper incisors presented a great amount of intrusion and some palatal root inclination as programmed in the treatment plan. The skeletal mandibular response was favorable to achieve profile balance. Periapical radiograph showed rounded apex of incisor roots, expected in any orthodontic intrusion movement. CONCLUSION: The clinical results present the successful approach to deep overbite malocclusion.

Nonsurgical treatment of an adult patient with bilateral posterior crossbite

A woman with an Angle Class III malocclusion and bilateral posterior crossbites complaining of difficulty in chewing was treated orthodontically without surgery. The treatment comprised asymmetric extractions, a removable mandibular lingual arch constriction appliance to narrow the mandibular arch, and a standard edgewise appliance to align the teeth. Pretreatment, posttreatment, and 1-year follow-up records are shown. With this treatment strategy of constricting the mandibular arch by using a combination of removable and fixed orthodontic appliances, we achieved a good result with optimal occlusion.

Moving an incisor across the midline: A treatment alternative in an adolescent patient

A 13-year-old sought treatment for a severely compromised maxillary left central incisor and an impacted fully developed left canine. Extraction of both teeth became necessary. As the key component of the revised comprehensive treatment plan, the right maxillary central incisor was moved into the position of the left central incisor. All other maxillary teeth were moved mesially to close any gaps. Active orthodontic treatment was completed after 34 months. Frenectomy, minor periodontal surgeries, and bonded lingual retainers were used to improve aesthetics and stabilize the tooth positions. The patient was pleased with the treatment outcome. Cone-beam computed tomography provided evidence that the tooth movement was accompanied by a deviation of the most anterior portion of the median palatine suture. This observation may make relapse more likely if long-term retention cannot be ensured. Root resorption was not observed as a consequence of the major tooth movement.

The fabrication and clinical application of semi-fixed mandibular lingual arch expansion appliance

Semi-fixed mandibular lingual arch expansion appliance is composed of a mandibular molar band, a keyway and arch expansion spring. The arch expansion spring are used to expand maxillary arch symmetrically or asymmetrically when bolts of the two ends are inset into keyways. Dental arch expansion appliance for 25 patients with mandibular arch stenosis showed that semi-fixed mandibular lingual arch expansion had good effect and could be used to expand mandibular arch.

Masticatory Muscle Exercise as an Adjunctive Treatment for Open Bite Malocclusions

The article distinguishes between dental open bite and skeletal open bite malocclusions and reviews the etiologic factor and possible treatment options. The addition of light masticatory muscle exercise on two mixed detention cases is illustrated. The patients were treated with a bonded rapid palatal expander followed by a transpalatal arch and a mandibular lingual arch, high-pull headgear therapy and light masticatory muscle exercises for 1 minute five times per day. A third case illustrates an increase in the clenching exercises of at least 5 minutes per hour for 6 hours. This patient had changed her mind on orthognathic surgical treatment plans. Treatment results suggest that clenching exercises helped to control the vertical dimension and assist in closure of open bite malocclusions. More clinical research is required to ensure the optimum level of masticatory muscle exercise for the treatment of open bite malocclusions.

Stability of transverse expansion in the mandibular arch

This was a retrospective, longitudinal cephalometric and cast study of 29 white patients at pretreatment, posttreatment, and an average of 6 years 3 months postretention. The goal was to assess changes with treatment and retention with the expanding mandibular lingual arch appliance in conjunction with fixed edgewise treatment. Seven mandibular cast measurements were assessed, including arch crowding, arch perimeter, arch length, and arch width at the permanent canines, first premolars, second premolars, and first molars. Cephalometric radiographs were digitized, and 16 cephalometric measurements were made. Repeated-measures analysis of variance and 2-sample t tests were used to determine statistically significant changes. It was found that the expanding lingual arch used for less than 6 months with the mandibular fixed edgewise appliance caused an increase in both the transverse and sagittal dimensions of the mandibular dental arch. Transverse expansion was more stable in the posterior region of the mandibular dental arch than in the anterior region. Mandibular intercanine width increase could be maintained only by fixed retention. Although the maxillary and mandibular incisors were advanced and proclined, lip protrusion did not occur.

Lower arch perimeter preservation using the lingual arch

The purpose of this investigation was to determine whether the placement of a mandibular lingual arch maintained arch perimeter in the transition from the mixed to the permanent dentition, and if so, whether it was effective at preventing mesial migration of first permanent molars, or whether this migration still occurred en masse, by increased lower incisor proclination. Thirty patients were randomly assigned to either a treatment group ( N = 14, mean age = 11.5 years) or a control group ( N = 16, mean age = 11.3 years). Study models, cephalograms, and tomograms of the patients, taken at the beginning and at the end of the study period, were examined. Statistically significant differences between groups were found for positional changes of mandibular first molars and incisors, and changes in arch dimensions. The results indicate that the lingual arch can help reduce arch perimeter loss, but at the expense of slight mandibular incisor proclination. (Am J Orthod Dentofac Orthop 1997;112:449-56.)
