Insulin Pen Administration Efficacy and Safety in an Older Patient.

Objective To describe a successful pharmacist-led intervention to effectively and safely provide education and pharmacotherapy management for an older patient with uncontrolled type 2 diabetes mellitus (T2DM) who failed to remove the needle shield on insulin pens for injection. Setting: Family medicine residency clinic. Practice Description: The clinic, part of a major urban health system, consists of 27 medical residents, 15 attending physicians, and 1 ambulatory care pharmacist managing chronic diseases collaboratively. It primarily serves low-income patients in a Midwest city. Practice Innovation: A 93-year-old White female with T2DM, receiving insulin therapy, was referred to the ambulatory care pharmacist by her physician for diabetes management. The patient had been hospitalized recently for hyperosmolar hyperglycemic state with a hemoglobin A1c of 15.9%. The pharmacist identified a failure to remove the needle shield on the insulin pen resulting in ineffective insulin administration, which caused persistent hyperglycemia and subsequent hospitalizations. This also posed a safety concern for severe hypoglycemia if proper administration resumed without adjusting the inflated dosing. The pharmacist used demonstration devices and the teach-back method to provide education and implement pharmacotherapy adjustments, resulting in effective and safe insulin administration. Main Outcome Measurements: Change in diabetes medication regimen, home blood glucose readings including continuous glucose monitor data, hemoglobin A1c results, frequency of hypoglycemic episodes, and number of hospitalizations for T2DM. Results: Over seven months, dose adjustments to basal insulin, combined with proper administration technique and the addition of empagliflozin, resulted in a hemoglobin A1c below 7%, with no severe hypoglycemia or diabetes-related hospitalizations. Conclusion: Medication errors, including insulin administration errors, highlight the need for thorough education in insulin therapy management. Education and monitoring empower older patients to self-manage diabetes safely and effectively, aligning with guidelines. Further research is required to identify optimal strategies for educating older patients on self-managing T2DM with insulin therapy.

A Novel Framework for Internal Responses to Detection of Pathogens in Wastewater by Public Health Agencies.

To build on the success of wastewater surveillance during the COVID-19 pandemic, jurisdictions funded under the Centers for Disease Control and Prevention National Wastewater Surveillance System are looking to expand their wastewater programs to detect more pathogens. However, many public health agencies do not know how to use the collected wastewater data to formulate public health responses, underscoring a need for guidance. To address this knowledge gap, the Houston Health Department (HHD) developed a novel response framework that outlines an internal action plan that is tailored by pathogen type after detection of various pathogens in wastewater. In July 2023, HHD met with subject matter experts (eg, bureau chiefs, program managers) in internal departments, including epidemiology, immunization, and health education, to discuss the general outline of the response framework and each department's anticipated role after pathogen detection. The internal framework established a flow for notifications and the actions to be taken by departments in HHD, with the goals of (1) ensuring timely and efficient responses to pathogen detections, (2) creating accountability within departments for taking their assigned actions, and (3) making certain that HHD was prepared for intervention implementation when a new pathogen was detected. As more public health agencies expand their wastewater surveillance programs to target additional pathogens, development of internal action plans tailored to departmental capacity and programs is an important step for public health agencies. The information compiled in this response framework can be a model for other public health agencies to adopt when expanding the scope of their wastewater monitoring systems.

Energy implications of meeting indoor air quality and thermal comfort standards in Mediterranean schools using natural and mechanical ventilation strategies

Educational buildings often face significant challenges with indoor air quality and thermal comfort, mainly due to a lack of mechanical ventilation and air conditioning systems. Research has demonstrated that these issues negatively affect students’ learning. The main objective of this paper was to assess compliance with indoor air quality and thermal comfort standards in educational buildings, by quantifying HVAC energy implications under natural and mechanical ventilation strategies. For this purpose, an educational building model that has 4 classrooms occupied by students from different age groups was simulated in 11 Mediterranean climate zones, for 4 ventilation strategies, and with 2 building orientations. The results reveal that natural ventilation is effective only in mild Mediterranean climates, with air quality non-compliance ranging from 0 % to 2 % and thermal comfort non-compliance between 2 % and 5 %. As expected, mechanical ventilation always ensures acceptable indoor air quality and thermal comfort. It achieves average HVAC energy savings of 80 % compared to natural ventilation. When considering that students’ CO2 generation rates vary depending on the age, systems with CO2 sensors further reduce HVAC demand (8 %), while maintaining comfort levels. Building orientation was found to have a significant impact for naturally ventilated buildings. South-facing orientations can reduce HVAC energy demand by up to 42 % (1,989 kWh/m2·year) whereas mechanically ventilated buildings show minimal sensitivity to orientation (up to 36 kWh/m2·year). This research will help public authorities of the educational community and architecture and engineering sectors when they are planning, designing and retrofitting educational buildings. Educational building managers will also benefit from this research by being able to optimise building ventilation through the effective management of existing resources.

Does family motivation make a difference for management educators? An empirical study

PurposeThe present study attempts to extend our understanding of the application of self-determination theory. The study examines the mediating role of self-efficacy and self-esteem in the relationship between family motivation and academic performance.Design/methodology/approachThrough adopting a survey questionnaire, we collected 329 responses from management educators (faculty members teaching in business schools) from various universities.FindingsThe data were analyzed using the partial least square structural equation modeling technique. All the hypothesized relationships found empirical support, indicating that family motivation is directly related to academic performance. Furthermore, self-efficacy and self-esteem mediate this direct relationship.Originality/valueThe paper forwards robust theoretical and practical implications and underlines important directions for future researchers. The study makes novel contributions to relationships inviting for empirical attention.

For several years, Dr Nick has overseen the business’s environmental programs at the high-grade mineral sand mine called Kwale Operation, which is situated along Kenya’s coast. The case allows students to explore how Environmental, Social, and Governance (ESG) principles are applied in a mining company setting by using this case study. Simple assessments can be applied by students to evaluate ESG and discuss their analysis of it. The mining industry’s intricate relationship between corporate governance, business strategy, and environmental stewardship is exemplified by the Dr Nick case at Base Titanium. It provides a critical analysis of the difficulties businesses confront in attempting to strike a balance between sustainable ethical business practices and economic growth, especially in areas where governance issues are common. The Role of School Leadership in Improving the Quality Management of Education at SMA Kabupaten Kutai Barat

This study aims to analyze the role of school leadership in enhancing the quality management of education at SMAN 2 Linggang Bigung, Kutai Barat Regency. The methodology employed in this research is a qualitative approach with a descriptive design. Data were collected through in-depth interviews, observations, and documentation studies involving the principal, teachers, and representatives of the school committee. The findings indicate that the diversity of leadership styles, such as participative, transformational, and collaborative, significantly contributes to the improvement of educational quality. The strategies implemented, including teacher training, project-based learning, and parental involvement, have had a positive impact on the quality of learning and student motivation.Despite challenges such as budget constraints and resistance from some teachers, the collaborative efforts made by the principal demonstrate great potential for improving educational quality in the region. This research emphasizes the importance of inclusive leadership and adaptive strategies in creating an effective learning environment. The principal should continue to strengthen collaboration with various stakeholders and develop structured training programs for teachers to enhance their competencies in implementing innovative teaching methods. With these measures, it is hoped that SMAN 2 Linggang Bigung can continue to improve educational quality and produce graduates who are ready to compete in the global era. Modelling the blockchain technology adoption barriers in the Indian education management system

Purpose Blockchain is a nascent technology that has the ability to revolutionize the workings of the educational landscape. However, there are several barriers to the adoption of blockchain technology (BCT). So, this paper aims to determine, rank and analyse the interdependent contextual relationship among the BCT adoption barriers within the education management system (EMS) of higher education institutes. Design/methodology/approach The present research principally uses the technology-organization-environment model for the classification of BCT adoption barriers in the EMS. An integrated dual phase best-worst method and interpretive structural modelling – cross-impact matrix multiplication applied to classification (BWM-ISM-MICMAC) analysis is used for the identification, prioritization and analysis of the contextual relationships among the BCT barriers. Findings The findings of the research show that the environmental barrier among the primary barriers and the lack of government initiatives among the sub-barriers are crucial barriers to BCT adoption in the EMS. Research limitations/implications Certain barriers might not be selected during the literature review and expert opinions might be biased. Future studies should use structural equation modelling to validate the relationships between BCT barriers and conduct similar research across other business sectors to provide valuable insights for scholars and practitioners alike. Practical implications The study facilitates a better perspective of BCT barriers and explores possible solutions more comprehensively for policymakers and field managers. Consequently, it offers viable suggestions for the successful and effective implementation of BCT within the EMS. Originality/value The concept of using the blockchain in academics is a novel one. This study establishes a crucial research base for the smooth adoption of BCT in EMS.

Understanding Burnout among Surgical residents: a mixed method study

Burnout is a complex condition that can strangely affect individuals in healthcare professions, such as surgeons and surgical residents. This study aimed to assess burnout and better explain the concept of burnout in surgical residents and the clarification strategies they take with this phenomenon to solve this problem. This mixed methods study was conducted at Shiraz University of Medical Sciences in 2022–2023 with two quantitative and qualitative phases. The statistical population included 114 surgical residents. The census sampling method was used in the quantitative and purposefully in the qualitative phases. The tool of the study was a 22-item Maslach Burnout Inventory-Student Survey questionnaire. Results showed that 27.27% of the residents had no academic burnout, but most 62.73% experienced a moderate level of academic burnout; also, 10% were in mild and severe burnout levels. In the qualitative phase of the study, 1047 codes were extracted after data analysis. The results showed three themes (and nine sub-themes) that emerged from the data analysis: A) Experienced burnout, B) Coping strategies, and C) Proposed strategies to reduce academic burnout. There was a remarkable incidence of burnout in surgical residents, which was higher than predicted. Although academic burnout is not the only factor affecting the residents ' academic decline, it is one of the most essential and significant factors that threaten residents’ motivation and academic performance. The evidence from the current research shows that educational managers and stakeholders should examine the consequences of academic burnout in residents to solve their problems in the current situation through some interventions.

Relationship between procrastination, time management, personality, and psychological distress in higher education

ABSTRACT Procrastination is increasingly recognised as a significant factor influencing academic performance among university students. The objective of this study was to analyse the procrastination behaviours of a convenience sample of 910 university students from the 2018 to 2020 academic cohorts and to investigate the relationships between procrastination and students’ sociodemographic and personality traits, academic time management practices, psychological distress, and academic performance. An exploratory, non-experimental, prospective, and cross-sectional study was conducted. Instruments used included a sociodemographic questionnaire, the Procrastination Assessment Scale-Students (PASS), the Academic Time Management scale (ATM), the Brief Symptom Inventory (BSI-18), and the Big Five Inventory-10 (BFI-10). Descriptive and inferential statistics were applied. Results identified two key dimensions contributing to procrastination: inadequate planning, poor academic performance, and insufficient monitoring of academic progress, as well as higher levels of irresponsibility, emotional exhaustion, and neuroticism. 61% of the variance in procrastination is explained by the variables considered, with significant influence from poor planning and neuroticism. The findings underscore the importance of targeted interventions aimed at reducing procrastination in university settings, emphasising time management and psychological well-being.

P-1649. Nursing Home Clinician Rationales Underlying Antibiotic Decision-Making for Suspected Urinary Tract Infection

Abstract Background The factors underlying antibiotic decision-making in nursing homes (NHs) remains poorly understood. We conducted an analysis of discrete and free text responses from a mixed-methods survey presenting NH clinicians with two patient clinical vignettes on asymptomatic bacteriuria (ASB) and cystitis (CYS) designed to assess clinicians’ treatment (TX) threshold.Table 1:Clinical Vignettes Methods NH clinicians were recruited nationally via professional organizations from December 2021 to April 2022. A web-based survey tool was used to administer the vignettes (Table 1) to participants who were asked to document their TX decision and justification. Treater (TRT) and non-treater (NTRT) responses were stratified by vignette and textual responses were uploaded to NVivo. A hybrid directed and conventional content approach was utilized to develop a codebook using the constant comparison technique.Table 2:Factors Contributing to Clinicians’ Treatment Decisions by Vignette and Treatment Choice Results Among 298 survey respondents; 10.4% treated ASB while 61.1% treated CYS. Seven factors contributing to clinicians’ TX decisions were identified (Table 2). For the ASB vignette (Table 3), TRTs framed the patient’s positive test results as infection (84%) while NTRTs framed it as contamination (79%). TRTs were less likely than NTRTs (26% vs 91%) to note the absence of signs and symptoms (S/S). Both groups cited the patient’s comorbidities and/or medical history (42% vs. 54%) in their TX decision. In the CYS vignette (Table 4), TRTs focused on positive patient S/S (91%) while NTRT did not feel there were sufficient frequency or severity of symptoms to justify immediate therapy (77%). TRTs were more likely to rely on positive test results (59%) and have more concern for patients’ comorbidities and/or medical history (18%).Table 3:Asymptomatic Bacteriuria Vignette Illustrative Quotes Conclusion Both TRTs and NTRTs use urine diagnostics in their decision-making, with TRTs having more confidence in the positive predictive value of urine diagnostics. NTRTs highlighted the lack of S/S as a justification for withholding therapy in both vignettes. TRTs accurately identified S/S for CYS, however, TRTs for ASB expressed concern that S/S might be underreported. Relating digitalization and quality management in health care organizations: A systematic review.

Quality management has been established in health care organizations as an important management function for internal quality measurement and external quality reporting. Digitalization processes have recently gained momentum with uncertain outcomes for health care organizations, quality, and the role of quality management. We conduct a systematic review to answer the questions of how the introduction of digital technologies affects quality management as a management function, how quality management is affecting digitalization, which quality management topics are addressed, and which competencies are needed during digitalization. Four databases were searched from 2000 to January 2022. Both empirical and conceptual articles reporting on digital technologies and quality management as a management function, quality reporting, or risk management were included. The studies included were analyzed based on the PICOS and PRISMA approaches. A total of 20 out of 662 articles met the inclusion criteria. Results show that both quality management as a management function as well as the digitalization of quality managements' own work processes are barely visible in the existing research. Only parts of the core tasks of quality management are discussed in connection with digitalization. Core competencies relevant for digitalization processes and quality management are identified. Quality managers require knowledge of digital technologies in order to develop and implement them to support their own work processes and the work processes of health care organizations in general. Currently, research is not guiding this process, which makes it important for quality managers to be able to access local knowledge. In addition, professional associations should develop skill catalogs for quality management education.

Innovative epilepsy management: a combined figure of EEG categorization and medication mechanisms

IntroductionEpilepsy management requires precision in diagnosis and treatment, particularly when selecting antiseizure medications based on specific epilepsy syndromes. We present an innovative educational tool that integrates EEG categorization with antiseizure medication mechanisms, designed to enhance clinical decision-making in epilepsy management.MethodsThis study evaluated a cohort of neurology trainees through a pre-test and post-test design. Participants were assessed on their ability to diagnose epilepsy syndromes and select appropriate treatments based on EEG findings before and after exposure to the teaching figure. The figure aligns key EEG patterns with specific epilepsy syndromes and outlines the corresponding mechanisms of action of antiseizure medications.ResultsPost-test results demonstrated a statistically significant improvement in trainees’ ability to analyze clinical cases and make informed treatment decisions (mean pre-test score: 52.8; post-test score: 66.5; p = 0.0019). The figure facilitated a deeper understanding of the relationship between EEG findings and medication selection, particularly in complex cases.DiscussionThe integration of EEG patterns with antiseizure medication mechanisms allows for more precise epilepsy syndrome diagnosis and enhances the selection of rational polypharmacy approaches. This approach not only improves educational outcomes but also offers potential applications in clinical practice for personalized epilepsy treatment strategies.ConclusionThis innovative figure bridges the gap between EEG categorization and treatment strategies, providing a valuable tool for improving epilepsy management education and clinical outcomes.Plain language summaryThis manuscript introduces a teaching tool that helps providers better understand how brainwave patterns (EEGs) relate to epilepsy types and guides them in choosing the most effective seizure medications.

The Significance of Artificial Intelligence in Learning and Education Management in the Light of Philosophy of Education: A Critical Appraisal

The integration of Artificial Intelligence (AI) into the framework of education has spelt out a remarkable landmark in the history of educational revolution. Both in teaching-learning methodologies and management practice, AI is rapidly becoming a pivotal indispensability. Employing qualitative methods of conceptual frame, content analysis and literature review, this paper critically evaluates the significance of AI in learning and education management in the light of philosophy of education. The highlights of the paper include the concepts of AI, learning, education management, and philosophy of education. Philosophical perspectives and ethical considerations in implementing AI technology are also discussed. The capacity of AI to inform personalized learning, enhance intelligent learning, and facilitate data-driven decision making is critically examined. The paper, going forward, explores the transformational role of teachers amid the reality of the AI environmental permeation and centrality, with a focus on the need for teachers to be adequately equipped with requisite skills that promote critical thinking, creativity and innovation. In the process of the critical appraisal, it is discovered that AI has great potentials to engender harmonized and customized learning experiences. It is also discovered that it equally brings, in its wake, hordes of challenges and concerns with regard to the possibility of stripping the human face of education in terms of depersonalization, ethical concerns, danger of data privacy and algorithmic bias. On the basis of the findings, the paper recommends, among others, that stakeholders in education should adopt a balanced approach to the issue of AI in learning and education management. In this model of approach, AI is to be given an ancillary position as a source of information and knowledge in the process of education. In this way, AI will play a complementary role to the time-tested traditional values through which education strives to offer and achieve a comprehensive development of the learner. This will go a long way in addressing the challenges associated with AI, thereby harnessing its transformative potentials in learning and education management.
