Cystic adenoid carcinoma (CAC) is a slow-growing, malignant gland neoplasm, with predominance in the palate region. A 54-year-old female patient presented with an extensive ulcerated lesion, with bleeding, painless, endophytic growth, affecting the hard and soft palate, with evolution of 8 months. Image examination showed a hypodense lesion with indefinite limits, involving the palate, the entire nasal cavity, and the left maxillary sinus. After facial clinical and imaging findings were reviewed, incisional biopsy was performed in the peripheral regions of the lesion, and the patient was instructed to suspend the use of the total prosthesis. The histologic examination concluded CAC diagnosis. Because it was a malignant lesion, the patient was referred to a state referral center to continue the treatment. The role of the dentist is of paramount importance in diagnosing and conducting the primary treatment of lesions in the maxillofacial region.
