Bilateral discoid lateral menisci and unilateral discoid medial menisci

Discoid lateral meniscus is a well-studied and documented entity of the knee whereas discoid medial meniscus is an extremely rare pathology. There are no more than 25 cases of medial discoid menisci reported in the literature. Some articles report bilateral lateral menisci as well as bilateral medial menisci, but there are no publications about medial and lateral discoid menisci in the same knee with lateral discoid meniscus in the contralateral side. This is a report of a case of bilateral discoid lateral menisci and unilateral discoid medial meniscus. MRI Measurement Of Oropharyngeal Curves In Children

The shape and curve of the intubating laryngeal mask airway (ILMA) tube were derived from head and neck sagittal magnetic resonance imaging (MRI) views in 50 adults. Three sizes of the ILMA were manufactured, differing only by the size of the mask. Validation of the possibility of blind intubation with the smallest ILMA has been attempted in children, and a high number of failures have been encountered in children under 30 kg. The aim of the present study was to determine the evolution of the radius of the oropharyngeal curve in children with age and weight and to establish a relationship between the oropharyngeal curve radius and simple clinical parameters such as weight and age, in order to design a correctly fitting paediatric ILMA. The radius of the oropharyngeal curve of 52 children was measured with head and neck sagittal MRI views. By using only bony landmarks, it was easy to calculate the curve radius independently of the child's head position and of the modification of the soft pharyngeal structures in case of general anaesthesia. The radius of the oropharyngeal curve in children increased with age and correlated with age and weight. This study highlights airway modifications as a function of age, particularly the oropharyngeal curve modification. Such modifications are probably the main reason why insertion and intubation with a size 3 ILMA in children under 30 kg regularly failed in our series. Paediatric ILMA design will need more than a simple miniaturisation of the ILMA mask. Particular attention should be paid to children's anatomic differences.

Roentgenographic and magnetic resonance imaging of anterior cruciate reconstruction using a patellar tendon graft--correlations with physical findings.

The purpose of this study was to evaluate the influence of the graft positioning on the clinical outcome and magnetic resonance imaging (MRI) signal of the graft following anterior cruciate ligament (ACL) reconstruction using the central one-third of the patellar tendon. Twenty-two patients with a chronic anterior instability underwent a modified Marshall-MacIntosh procedure, while 27 with a subacute torn ACL had an ACL reconstruction using a free bone-patellar tendon-bone graft. The patients were retrospectively reviewed with a 1.8-year average follow-up (1-3 years). The clinical result was evaluated through the comparative range of motion and the residual laxity as measured with the KT 1,000 arthrometer. The roentgenographic analysis was performed from anteroposterior (AP) and mediolateral (ML) views, made first on one-leg standing with the knee at 30 degrees of flexion, and secondly at "zero" extension with active quadriceps contraction. Lines were drawn to visualise the location of the tibial and femoral tunnels in relation to the tibial plateaus and the intercondylar roof represented by Blumensaat's line. The analysis of the lateral MRI views of the graft allowed discrimination between homogenous and heterogenous graft signals. On lateral roentgenograms of normal knees, it was found that Blumensaat's line crossed the surface of the medial tibial plateau at 30% +/- 9% of its sagittal width (20%-40% range), demonstrating the variability of intercondylar roof inclination. The range of motion was normal in 34 patients (group I), 9 patients had a flexion deficit (group II), and 6 exhibited an extension deficit (group III). Cross-Sectional Area of the Stenotic Lumbar Dural Tube Measured from the Transverse Views of Magnetic Resonance Imaging

The cross-sectional area of the lumbar dural tube was calculated from transverse-slice lumbosacral magnetic resonance images (MRI) using the simple geometric formulas and computerized digitizer in 51 patients with low-back pain only, 79 with mainly radicular symptoms, and 53 with intermittent claudication, and in 39 controls with confirmed symptomatic lesions at a nonlumbar level or of a nonspinal organ. Findings suggesting developmental narrowing of the spinal canal were found not only in the intermittent claudication group, but also in the radicular pain group. Cross-sectional area of < 100 mm2 at more than two of three (L2/3, L3/4, and L4/5) intervertebral levels was highly associated with the presence of intermittent claudication. The cross-sectional area value obtained with the simplified geometric formulas was highly correlated with that calculated with the digitizer, indicating that this simple method can be used with MRI in outpatient clinics for the rapid determination of the most stenotic portion of the dural tube.

外側開窓法を施行した腰部外側ヘルニアの４症例の検討

We experienced four cases of lateral disc herniation between L4 and L5.All cases complained of severe pain in the lower extremity accompanied by disturbance in walking.All patients were evaluated with myelography, magnetic resonance imaging (MRI), discography, computed tomographic discography (CTD) and selective radiculography and block. Accurate diagnosis of intraforaminal or extraforaminal herniation was made with axial views of MRI and CTD. Selective root block was useful for detecting the level of the root lesion.We performed lateral fenestration and removed the disc herniation in all cases.The surgical outcomes were excellent in all cases.

大腿骨頭壊死症におけるＭＲＩの治療方針への有用性 治療法選択に関して

Recently, some reports demonstrate the usefullness of the magnetic resonance imaging (MRI) for early diagnosis of the avascular necrosis of the femoral head (ANF). In this report we analysed 64 hips in 40 cases using preoperative plain radiographs, tomography and MRI affected by ANF. And we gained following three conclusions.1. When rotational osteotomy of the femoral head is to be carried out, two plain views of MRI, parallel and contrary to the axis of the femoral neck, were useful for evaluation of postoperative weight bearing area.2. In 58 hips with strinct the former two plain views of MRI, atrophy of posterior area of the femoral head was recognized in 54 hips by tomography, and of them abnormal signal in posterior was revealed in 32 hips by MRI.3. In 28 hips performed anterior rotational osteotomy, atrophy of posterior area of the femoral head was recognized in 26 hips by tomography, and of them abnormal signal in posterior was revealed in 6 hips by MRI. In such cases careful follow-up should be required.
