Turbo spin-echo-based enhanced acceleration-selective arterial spin labeling without electrocardiography or peripheral pulse unit triggering and contrast enhancement for lower extremity MRA

PurposeLower extremity magnetic resonance angiography (MRA) without electrocardiography (ECG) or peripheral pulse unit (PPU) triggering and contrast enhancement is beneficial for diagnosing peripheral arterial disease (PAD) while avoiding synchronization failure and nephrogenic systemic fibrosis. This study aimed to compare the diagnostic performance of turbo spin-echo-based enhanced acceleration-selective arterial spin labeling (eAccASL) (TSE-Acc) of the lower extremities with that of turbo field-echo-based eAccASL (TFE-Acc) and triggered angiography non-contrast enhanced (TRANCE). MethodsNine healthy volunteers and a patient with PAD were examined on a 3.0 Tesla magnetic resonance imaging (MRI) system. The artery-to-muscle signal intensity ratio (SIR) and contrast-to-noise ratio (CNR) were calculated. The arterial visibility (1: poor, 4: excellent) and artifact contamination (1: severe, 4: no) were independently assessed by two radiologists. Phase-contrast MRI and digital subtraction angiography were referenced in a patient with PAD. Friedman's test and a post-hoc test according to the Bonferroni-adjusted Wilcoxon signed-rank test were used for the SIR, CNR, and visual assessment. p < 0.05 was considered statistically significant. ResultsNo significant differences in nearly all the SIRs were observed among the three MRA methods. Higher CNRs were observed with TSE-Acc than those with TFE-Acc (anterior tibial artery, p = 0.014; peroneal artery, p = 0.029; and posterior tibial artery, p = 0.014) in distal arterial segments; however, no significant differences were observed upon comparison with TRANCE (all p > 0.05). The arterial visibility scores exhibited similar trends as the CNRs. The artifact contamination scores with TSE-Acc were significantly lower (but within an acceptable level) compared to those with TFE-Acc. In the patient with PAD, the sluggish peripheral arteries were better visualized using TSE-Acc than those using TFE-Acc, and the collateral and stenosis arteries were better visualized using TSE-Acc than those using TRANCE. ConclusionPeripheral arterial visualization was better with TSE-Acc than that with TFE-Acc in lower extremity MRA without ECG or PPU triggering and contrast enhancement, which was comparable with TRANCE as the reference standard. Furthermore, TSE-Acc may propose satisfactory diagnostic performance for diagnosing PAD in patients with arrhythmia and chronic kidney disease.

Usefulness of silent magnetic resonance angiography for intracranial aneurysms treated with a flow re-direction endoluminal device.

Flow re-direction endoluminal device (FRED) is a novel dual-layer flow-diverting stent to treat cerebral aneurysms with high obliteration rates, however, it induces inevitable metal-related artifacts. We compared silent magnetic resonance angiography (MRA), a new MRA method using ultra-short time of echo and arterial spin-labeling, with conventional time-of-flight (TOF)-MRA for imaging aneurysms treated using FRED. Between May 2020 and September 2022, 16 patients with unruptured internal carotid aneurysms treated using FRED simultaneously underwent silent MRA and TOF-MRA after treatment, with 36 follow-up sessions in total. Two observers independently graded the quality of intra-aneurysmal flow and stented parent arteries under both types of MRA from 1 (not visible) to 4 (nearly equal to digital subtraction angiography [DSA]), with reference to DSA images as a standard criterion. The mean scores for intra-aneurysmal flow and stented parent arteries were significantly better for silent MRA (3.93 ± 0.21 and 3.82 ± 0.32, respectively) than for TOF-MRA (2.08 ± 0.99 and 1.92 ± 0.79, respectively) (P < 0.01). Intermodality agreements for intra-aneurysmal flow and stented parent arteries were 0.87 and 0.90, respectively. Silent MRA is superior to TOF-MRA for assessing patients treated with FRED, with potential as an alternative imaging modality to DSA.

Usefulness of silent magnetic resonance angiography (MRA) for the diagnosis of atherosclerosis of the internal carotid artery siphon in comparison with time-of-flight MRA

Background and purposeFlow visualization in 3D time-of-flight MRA (3D-TOF MRA) may be limited for internal carotid artery siphon owing to turbulent artifact. The purpose of this study was to compare the usefulness of Silent MRA and 3D-TOF MRA to assess atherosclerosis of the internal carotid artery siphon.Material and methodsA total of 106 patients with suspected cerebrovascular disease were included. All patients were scanned with Silent MRA and 3D-TOF MRA sequences and also underwent DSA examination. Two observers independently assessed the TOF MRA and Silent MRA images of atherosclerosis of the internal carotid artery siphon. The diagnostic efficacy of two MRA methods in evaluating atherosclerosis of the carotid siphon was performed by using receiver operating characteristic (ROC) curve analysis. Interobserver reliability was also assessed using weighted kappa statistics.ResultsImage of Silent MRA sequence had higher subjective evaluation scores and significantly high CNR between the carotid siphon and the background tissues than the image of 3D-TOF MRA sequence (P < 0.05). The AUC was 0.928 (95% CI 0.909–0.986) for Silent MRA, which was significantly higher than that of 3D-TOF MRA (0.671, 95% CI 0.610–0.801, P < 0.05). Silent MRA had high sensitivity, specificity and accuracy than 3D-TOF MRA for visualization of the carotid siphon.ConclusionsSilent MRA as a new angiographic modality is superior to 3D-TOF MRA for visualization of the carotid siphon, and maybe an alternative to 3D-TOF MRA in the diagnosis of atherosclerosis of the carotid siphon.

Efficacy of Angiography Examinations using 4D TRAK Contrast Enhancement Techniques

The non-enhanced 3D TOF and the contrast enhancement magnetic resonance angiography (MRA) technique are MRA examination methods used to define the carotid in cerebral vascular examination. Recently, the 4D time-resolved angiography using keyhole(TRAK) technique, which is the combination of the contrast enhancement MRA technique and the keyhole technique, has been used. This study is intended to evaluate its efficacy on the cerebrovascular diagnostic examination by comparing 4D TRAK with the existing contrast enhancement MRA technique. CE Neck MRA was used on 30 patients (15 males and 15 females) who were prescribed a Brain Neck CE MRA after receiving contrast enhancement MRA and 4D TRAK when they entered the emergency room. The same concern sectors were established on the images acquired using each technique after the injection of a contrast medium. The contrast to noise ratio was compared and analyzed through estimated values. According to the results, 4D TRAK showed stronger signal strength than the existing contrast enhancement MRA by 4.38 in CCA, 4.24 in ICA, and 2.94 in VA. The results of the matching sample T-test were statistically significant (p<0.05). When 4D TRAK and the existing contrast enhancement MRA were compared and analyzed in CE Neck MRA examination, 4D TRAK was seen as being more useful at displaying anatomical information about blood vessels and hemodynamics information. These results point towards it being a very useful method for clinical examination

Non-contrast magnetic resonance angiography/venography techniques: what are my options?

Non-contrast magnetic resonance (MR) angiography and MR venography techniques are gaining popularity for vascular imaging because they are faster, more forgiving and less costly compared with contrast-enhanced MR angiography. Non-contrast MR angiography also avoids gadolinium deposition, which is especially important in imaging children. Non-contrast MR angiography has an array of specific applications for numerous clinical indications. This review summarizes the non-contrast MR angiography methods and their relative advantages and disadvantages. The paper also guides the reader on which technique to consider when determining the optimal imaging modality for each individual patient.

Vessel-selective 4D MRA based on ASL might potentially show better performance than 3D TOF MRA for treatment evaluation in patients with intra-extracranial bypass surgery: a prospective study.

To compare bypass patency and intracranial collaterals from the external carotid artery (ECA) by four-dimensional MR angiography (MRA) based on super-selective pseudo-continuous arterial spin labeling (pCASL) combined with the keyhole and view-sharing techniques (4D-sPACK) versus three-dimensional time-of-flight (3D TOF) MRA in patients with extra-intracranial revascularization. The MR data of 45 patients administered bypass surgery were collected. The image quality of 4D-sPACK was evaluated using a 4-point grading system according to whether the diagnosis of intracranial collaterals was affected. Anastomosis patency and intracranial collateral visualization from ECA were assessed by two radiologists on 4D-sPACK and 3D TOF MRA, with digital subtraction angiography (DSA) findings as reference. Intracranial collateral assessment employed another 4-point grading system according to the number of vessels shown. Interobserver agreement was assessed with the weighted kappa statistic. Fifty hemispheres in 43 patients were included. The image quality of 4D-sPACK was good in 47 (47/50, 94.0%) hemispheres. 4D-sPACK had a higher sensitivity than 3D TOF MRA (97.73% vs 79.55%) for visualizing anastomoses. There were significant differences between 4D-sPACK (scores, 3.22 ± 1.15) and 3D TOF MRA (scores, 1.80 ± 0.67) in the visualization of intracranial collaterals from ECA (p < 0.001). The interobserver agreement was substantial for intracranial collateral assessment (κ4D-sPACK = 0.788; κ3D TOF MRA = 0.800) and almost perfect for bypass patency (κ4D-sPACK = 0.912; κ3D TOF MRA = 0.816; κDSA = 0.811). This pilot study shows that, 4D-sPACK has a better performance than 3D TOF MRA in treatment evaluation of patients after bypass surgery, and has high consistency with DSA. • 4D-sPACK is a non-contrast-enhanced dynamic MRA method for the visualization of intracranial vessels. • 4D-sPACK has higher specificity for the diagnosis of anastomosis occlusion. • 4D-sPACK is better than 3D TOF MRA in the visualization of intracranial collaterals in patients after bypass surgery.

Clinical Value of Noncontrast-Enhanced Radial Quiescent-Interval Slice-Selective (QISS) Magnetic Resonance Angiography for the Diagnosis of Acute Pulmonary Embolism Compared to Contrast-Enhanced Computed Tomography and Cartesian Balanced Steady-State Free Precession.

Free-breathing noncontrast-enhanced (non-CE) magnetic resonance angiography (MRA) techniques are of considerable interest for the diagnosis of acute pulmonary embolism (APE), due to the possibility for repeated examinations, avoidance of side effects from iodine-based contrast agents, and the absence of ionizing radiation exposure as compared to CE-computed tomographic angiography (CTA). To analyze the clinical performance of free-breathing and electrocardiogram (ECG)-gated radial quiescent-interval slice-selective (QISS)-MRA compared to CE-CTA and to Cartesian balanced steady-state free precession (bSSFP)-MRA. Prospective. Thirty patients with confirmed APE and 30 healthy volunteers (HVs). Radial QISS- and bSSFP-MRA at 1.5T. Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were computed to compare the pulmonary imaging quality between MRA methods. The pulmonary arterial tree was divided into 25 branches and an ordinal scoring system was used to assess the image quality of each pulmonary branch. The clinical performance of the two MRA techniques in accurately assessing APE was evaluated with respect to CE-CTA as the clinical reference standard. Wilcoxon signed-rank and Spearman's correlation tests were performed. Sensitivity and specificity of the MRA techniques were determined using CE-CTA as the clinical reference standard. Thrombus-mimicking artifacts appeared more frequently in lobar and peripheral arteries of patients with Cartesian bSSFP than with radial QISS-MRA (pulmonary trunk: 12.2% vs. 14.0%, P = 0.64; lobar arteries: 35.6% vs. 22.0%, P = 0.005, peripheral arteries: 74.4% vs. 49.0%, P < 0.001). The relative increases in SNR and of CNR provided by radial QISS-MRA with respect to Cartesian bSSFP-MRA were 30-35% (P-values of SNR/CNR, HVs: 0.09/0.09, patients: 0.03/0.02). The image quality of pulmonary arterial branches was considered good to excellent in 77.2% of patients with radial QISS-MRA and in 43.2% with Cartesian bSSFP-MRA (P < 0.0001). The clinical performance of radial QISS-MRA was higher than Cartesian bSSFP-MRA for grading embolism, with a total sensitivity of 86.0% vs. 80.6% and a specificity of 93.3% vs. 84.0%, respectively. Radial QISS-MRA is a reliable and safe non-CE angiographic technique with promising clinical potential compared to Cartesian bSSFP-MRA and as an alternative technique to CE-CTA for the diagnosis of APE. 1 TECHNICAL EFFICACY STAGE: 3.

Vascular magnetic resonance angiography techniques.

Magnetic resonance angiography (MRA) denotes a unique option for the evaluation of peripheral vasculature due to its noninvasive nature, lack of ionizing radiation exposure, potential for non-contrast examination, and ability for generating volumetric representations that showcase vascular pathology. The constant evolution of the available MRA techniques, however, makes understanding and determining an optimal imaging protocol difficult. Here we present a brief overview of the major MRA sequence options, their major weaknesses and strengths, and related imaging considerations. Understanding the technical underpinnings of the various MRA methods helps with recognition of common imaging issues and artifacts and rendering clinically relevant interpretations.

Selective arterial spin labeling in conjunction with phase‐contrast acquisition for the simultaneous visualization of morphology, flow direction, and velocity of individual arteries in the cerebrovascular system

In various cerebrovascular diseases the visualization of individual arteries and knowledge about their hemodynamic properties, like flow velocity and direction, can become important for an accurate diagnosis. Magnetic resonance angiography methods are intended to acquire this information, but often a single acquisition is not sufficient to retrieve all of this desired information. Using selective arterial spin labeling (ASL) methods, a single artery of interest can be tagged and visualized, whereas quantitative information about hemodynamics can be retrieved using phase-contrast techniques that are often limited regarding their selectivity. In this study, a method that allows for velocity mapping of individual arteries by incorporating phase-contrast preparation into selective ASL angiography measurements is presented. Several postprocessing steps are required to generate velocity and directional-encoded maps of selected arteries from the data acquired in a single scan. The method was successfully evaluated in healthy volunteers, and a first application in two selected patients is presented. In one patient, an aneurysm of the middle cerebral artery is investigated, and in the second patient it is used to visualize an arterio-venous malformation. Selective ASL imaging in conjunction with phase-contrast acquisition allows for investigating hemodynamic properties of individual arteries. Unenhanced respiratory-navigated NATIVE® TrueFISP magnetic resonance angiography in the evaluation of renal arteries: Comparison with contrast-enhanced magnetic resonance angiography.

To compare unenhanced three-dimensional (3D) NATIVE® true fast imaging with steady-state precession (TrueFISP) magnetic resonance (MR) angiography with the more conventional MR angiography technique obtained after intravenous administration of a gadolinium chelate in the evaluation of renal arteries and their branches in patients with suspected renal artery stenosis. A total of 39patients (25men, 14women) with a mean age of 51.4±17.5years (SD) (range: 10-82years) were included in the study. All patients with suspected renal artery stenosis underwent unenhanced 3D NATIVE® TrueFISP MR angiography and contrast-enhanced MR angiography. The two MR angiography methods were compared by two independent readers for image quality using a four-point scale, diagnostic performance and grading of renal artery stenosis on a total of 78renal arteries. For both readers image quality of unenhanced 3D NATIVE® TrueFISP MR angiography (3.12 to 3.63) was greater than that of contrast-enhanced MR angiography (1.94 to 2.71) for renal artery ostium-trunk and the left renal artery segmental branches. The sensitivity of 3D NATIVE® TrueFISP MR angiography for the diagnosis of renal artery stenosis was 100% for both readers for the right renal artery and 66% and 80% for the left renal artery for reader 1 and reader 2, respectively. Agreement between 3D NATIVE® TrueFISP MR angiography and CE-MR angiography was 95% (74/78) for reader 1 and 92% (72/78) for reader 2. Unenhanced NATIVE® TrueFISP magnetic resonance angiography can play an additional role in the evaluation of renal arteries in patients with hypertension, especially in subjects at risk of nephrogenic systemic fibrosis.

Chapter 7 - Magnetic resonance angiography: physical principles and applications

Magnetic resonance angiography (MRA) is the visualization of hemodynamic flow using imaging techniques that discriminate flowing spins in blood from those in stationary tissue. There are two classes of MRA methods based on whether the magnetic resonance imaging signal in flowing blood is derived from the amplitude of the moving spins, the time-of-flight methods, or is based on the phase accumulated by these flowing spins, as in phase contrast methods. Each method has particular advantages and limitations as an angiographic imaging technique, as evidenced in their application space. Here we discuss the physics of MRA for both classes of imaging techniques, including contrast-enhanced approaches and the recent rapid expansion of the techniques to fast acquisition and processing techniques using parallel imaging coils as well as their application in high-field MR systems such as 3T and 7T.

Clinical performance of a free-breathing spatiotemporally accelerated 3-D time-resolved contrast-enhanced pediatric abdominal MR angiography.

Pediatric contrast-enhanced MR angiography is often limited by respiration, other patient motion and compromised spatiotemporal resolution. To determine the reliability of a free-breathing spatiotemporally accelerated 3-D time-resolved contrast-enhanced MR angiography method for depicting abdominal arterial anatomy in young children. With IRB approval and informed consent, we retrospectively identified 27 consecutive children (16 males and 11 females; mean age: 3.8 years, range: 14 days to 8.4 years) referred for contrast-enhanced MR angiography at our institution, who had undergone free-breathing spatiotemporally accelerated time-resolved contrast-enhanced MR angiography studies. A radio-frequency-spoiled gradient echo sequence with Cartesian variable density k-space sampling and radial view ordering, intrinsic motion navigation and intermittent fat suppression was developed. Images were reconstructed with soft-gated parallel imaging locally low-rank method to achieve both motion correction and high spatiotemporal resolution. Quality of delineation of 13 abdominal arteries in the reconstructed images was assessed independently by two radiologists on a five-point scale. Ninety-five percent confidence intervals of the proportion of diagnostically adequate cases were calculated. Interobserver agreements were also analyzed. Eleven out of 13 arteries achieved acceptable image quality (mean score range: 3.9-5.0) for both readers. Fair to substantial interobserver agreement was reached on nine arteries. Free-breathing spatiotemporally accelerated 3-D time-resolved contrast-enhanced MR angiography frequently yields diagnostic image quality for most abdominal arteries in young children.

MRA study on variation of the circle of willis in healthy Chinese male adults.

Aim. To investigate the morphology and variation of the circle of Willis (COW) in healthy Chinese male adults. Materials and Methods. We analyzed cerebral magnetic resonance angiography (MRA) images of 2,246 healthy subjects using typical magnetic resonance imaging (MRI) and MRA. 3D-time of flight (TOF) MRA method was applied to all subjects and the classification was therefore achieved according to the integrity level of COW and the developmental situation of vessels. Results. The overall incidence of COW integrity was 12.24%, with 7.57% nonvariation integral COW. The incidences of partial integrity and nonintegrity were 70.17% and 17.59%, respectively. The integrity rate of anterior circulation was 78.58%, with a close correlation with A1 segment of the anterior cerebral artery (ACA-A1) developmental condition. The developmental variation rate of ACA-A1 was 28.23% and the variation of the right side was higher than that of the left side. The nonintegrity rate of posterior circulation was 83.93% as the hypoplasia of P1 segment of the posterior cerebral artery (PCA-P1) with an incidence rate of 15.85% for PCA-P1 variation. Conclusions. The COW variation is a common phenomenon among the healthy subjects. MRA could enable reflecting the physiological morphology of COW in a comprehensive manner.

Retrobulbar magnetic resonance angiography using binomial off-resonant rectangular (BORR) pulse.

Applying a newly developed binomial off-resonant rectangular (BORR) pulse method for high resolution three-dimensional MR angiography (MRA) on retrobulbar ocular vessels, which has not been possible with routine MRA due to background fatty tissues. BORR pulse was implemented in a gradient echo sequence by replacing the original excitation pulse, and were optimized for robust orbital fat suppression. Several other MRA methods, with or without fat suppression, were also compared, including time-of-flight, contrast enhanced MRA, and hybrid of opposite-contrast MRA. Nine healthy subjects participated with written informed consents. To reduce eye motion, the subjects were instructed to casually stare at a projected cross during each MRA scan. Major vessels were evaluated by three independent radiologists using a 4-point scale. The BORR method yielded the best MRA results for retrobulbar vessels without contrast enhancement, significantly superior than other MRA methods. BORR results had significantly higher visibility scores than all other methods for small vessels. We have successfully revealed orbital vessels in retrobulbar space for the first time using MRA, by using the BORR pulse method. With a clear depiction of the vasculature without the need for contrast enhancement, our method has the potential to provide important diagnostic information for ocular vascular diseases.

Separate pulmonary artery and vein magnetic resonance angiography by use of an arterial spin labeling method.

A separate pulmonary vein (PV) is difficult to depict with the commonly used bright-blood magnetic resonance angiography method. Until now, no study has described the depiction of peripheral PVs without the artery. Our purpose in this study was to develop an arterial spin labeling (ASL)-based magnetic resonance angiography sequence to depict the pulmonary artery (PA) and vein separately. We developed such a sequence by using two inversion recovery pulses. The first pulse was non-selective, and the second pulse was selective and was applied to the aorta and heart. All studies were conducted on a 1.5-T clinical magnetic resonance system with six different inversion times for seven healthy volunteers. For evaluation, we categorized the inversion times by using visual scoring. Then, we used the magnitude image to evaluate the PA, and we used the real image to evaluate the PV. For the PA, an inversion time of 300 ms had the lowest score (1.43), and the score changed with increasing times; an inversion time of 1,100 ms had the highest score (3.85). For the PV, an inversion time of 300 ms had the highest score (2.68), and the score decreased with increasing times. The results indicated that the PA and vein could be depicted separately by the use of an ASL-based magnetic resonance angiography method. The optimal inversion times for the PV and artery were 300 and 1,100 ms, respectively.

Feasibility of MR perfusion-weighted imaging by use of a time-spatial labeling inversion pulse

Perfusion-weighted imaging (PWI) by use of arterial spin labeling (ASL) has been introduced to the clinical setting. However, it is not widely available because it requires specialized pulse sequences. Imaging using a time-spatial labeling inversion pulse (time-SLIP), which is a magnetic resonance angiography (MRA) technique that is based on ASL, can be used in various situations. In this study, we examined the feasibility of time-SLIP PWI. Two types of time-SLIP sequences were evaluated: (1) a single inversion recovery (IR) pulse sequence, which is the same as that used in conventional time-SLIP MRA except for the timing of data acquisition, and (2) a dual IR pulse sequence, where a second, non-selective, IR pulse was added during the inflow time to suppress background signals. Subtraction processing is performed between the "on" and "off" settings of the first IR pulse (time-SLIP tag) to obtain PWI. The average signal intensity was measured in a uniform phantom as the residual of the background, and in five healthy subjects as the perfusion signal. The average signal-to-noise ratio (SNR) was also measured in the five subjects. All imaging was performed with a 1.5-T MR scanner. Images using the dual IR method showed lower background signals and higher perfusion signals compared with images using the single IR method. However, the SNR was lower in images with the dual IR method. These results demonstrate that a time-SLIP, which is an MRA method, can be used for obtaining cerebral PWI simply by adjusting the imaging parameters.

Non-contrast enhanced magnetic resonance angiography techniques in candidates for kidney transplantation: A comparative study

AimDetailed knowledge of vessel status in potential candidates for kidney transplantation is essential for the surgeon. Contrast enhanced magnetic resonance angiography has previously been used intensively for assessing this, but the discovery that use of gadolinium based contrast agents in magnetic resonance imaging can cause Nephrogenic Systemic Fibrosis in patients suffering from severe kidney disease has lead to renewed interest in non-contrast enhanced magnetic resonance angiography. The aim of this study was to find a non-contrast enhanced magnetic resonance angiography method for preoperative evaluation of the pelvic vessels prior to kidney transplantation, providing a sufficient image quality. MethodIn a prospective study we consecutively included 54 patients undergoing examinations prior to kidney transplantation. The patients were examined with the following magnetic resonance angiography sequences: A 2D Time of flight (n = 54), 3D Time of flight (n = 52) patients, 3D Phase Contrast (n = 54), 3D Balanced Steady State Free Precession (n = 52) and a 2D TRiggered Angiography Non-Contrast Enhanced (TRANCE) (a Spin Echo sequence with subtraction) (n = 48). The sequences were evaluated with respect to contrast, diagnostic performance and artefact burden. ResultsEvaluating contrast, 3D Phase Contrast was significantly better than 2D Time of flight (p < 0.001). The 2D Time of flight was significantly superior to 3D Time of flight, 3D Balanced-Turbo Steady State Free Precession and the TRANCE sequences (p < 0.001). Rating diagnostic performance, the 3D Phase Contrast ranked better than the 2D Time of flight though no significant difference was found (p > 0.2). The 2D Time of flight was significantly better than the other sequences (p < 0.001) in all cases. The artefact score was lowest for the Phase Contrast images and significantly superior to the 2D Time of flight (p < 0.005). The 2D Time of flight was significantly better than the three other sequences (p < 0.001) in all cases. ConclusionNon-contrast enhanced magnetic resonance angiography offers a safe preoperative examination for assessment of vessel status before kidney transplantation. A combination of 2D Time of flight and 3D Phase Contrast acquisitions is recommended and can be performed within a reasonable scan time.

Initial clinical evaluation of a non‐contrast‐enhanced MR angiography method in the distal lower extremities

To report the initial experience and diagnostic performance applying a novel flow-dependent non-contrast-enhanced MR angiography (NCE-MRA) method, in patients with suspected peripheral vascular disease, in comparison with established contrast-enhanced MRA (CE-MRA). The lower legs of 34 patients were imaged at 1.5 T with both NCE-MRA and CE-MRA. The NCE-MRA method consisted of a cardiac-gated balanced-SSFP sequence with controllable flow-suppression preparation. Flow-suppressed and unsuppressed datasets were subtracted to obtain angiograms. Two experienced radiologists assessed both NCE-MRA and CE-MRA images, first independently and then in consensus to resolve significant disagreements. Signal loss, vessel conspicuity, vascular disease, venous contamination, artifacts, and diagnostic confidence were assessed. Using the CE-MRA as the "gold standard," the per-segment sensitivity and specificity for detection of significant disease were 81.7% and 90.9%, respectively. Mean diagnostic confidence (scale 0-4) was 3.4 for NCE-MRA and 3.9 for CE-MRA. Most vessel segments were well visualized but the popliteal arteries often suffered some technique-related signal loss. The NCE-MRA method was able to visualize most vessel segments with good or excellent confidence, few artifacts, and excellent background suppression, giving moderate agreement with CE-MRA. However, some segments were poorly visualized, probably due to flow profile distortion in these patients.

Multimodality Imaging of Lower Extremity Peripheral Arterial Disease

Patients with symptomatic peripheral arterial disease (PAD) affecting the lower extremities are initially evaluated with an ankle-brachial index (ABI) and segmental pressure measurements. An ABI 50% stenosis) with a sensitivity and specificity of 79% and 96%, respectively.1 An ABI between 0.90 and 1.0 is considered borderline. The localization of the stenosis can be inferred by an abnormal decrease (>20 mm Hg) in segmental lower extremity pressures. In the presence of calcified atherosclerosis, arteries may become stiff and noncompressible, which results in a falsely elevated ABI (often >1.3). For patients with suspected PAD and a normal ABI at rest, it is valuable to obtain postexercise ABI measurements, which if <0.85 are consistent with PAD and is an independent predictor of mortality.2 Imaging is then needed to confirm the location and degree of stenosis before revascularization or if the diagnosis of PAD is uncertain. Characterization of PAD can be performed with noninvasive angiography using computed tomography (CTA) or magnetic resonance angiography (MRA), as well as with duplex ultrasonography (US), depending on patient specific characteristics (Table 1). Advances in both CTA and MRA provide clinicians with the opportunity to obtain a high resolution, 3-dimensional (3-D) road map of the peripheral arterial tree in patients, particularly when planning revascularization strategies. Invasive digital subtraction angiography (DSA) has been the accepted standard for evaluation of lower extremity atherosclerosis. Although DSA is a robust technique for diagnosing significant arterial stenosis or obstruction, it provides a 2-D view of the vessels, which may underestimate the degree of stenosis for tortuous vessels.3 Furthermore, there are inherent risks with arterial access, ionizing radiation, and the use of iodinated contrast media (CM).4 View this table: Table 1.
