Feature of the Postural Blocd Pressure Reflex in pregnant and lying-in women was studied. It showed marked deviations as summarized below from the normal feature :1. Pregnancy affected restoration of the blood pressure after the posture change, from lying to sitting, in that sense that its retardation in most cases and failure of restoration in many cases were observed, especially in the earliest and latest stages of the pregnancy.2. In the first half of the first week of the child-bed confinement, retardation and failure of blood pressure restoration, and also fluctuation in the restorative course were observed in many cases.Affection of the reflex restoration of the blood pressure depended on (i) the individual constitution, (ii) absence of previous experience of delivery, (iii) haemorrhage, and (iv) duration of the delivery.By the end of the first week of confinement, the feature of the postural blood pressure reflex generally regained the characteristics of normal women.3. Both in expectant and confined mothers, diastolic pressure remained unchanged by the change of the posture, as is the case in the normal person. Thus the above described change in the reflex pattern concerns with the pulse pressure.

Problems and difficulties in the clinical teaching of obstetrics and gynecology as of today

I N BYGONE years the teaching of obstetrics and gynecology was on a very casual basis. In the early French hospitals the obstetrical patients were usually found in the cellar of the hospital in order t.o assure privacy, and the whole process of care during labor was conducted in an atmosphere of secrecy. This was the time, too, when it was considered to be highly immoral for the lying-in woman to be seen or attended by a male physician. Obviously, clinical instruction under such conditions was all but impossible. In Germany, conditions were somewhat better; and when Johann Fried organized the first Prauel\-(bars later my own experience was comparable but slightly better. 7 obser\-cd four deliveries, palpated the abdomen of one pregnant woman at the time of my final examination, never did a rectal or vaginal examination, and never had a pelvimeter in my hands. From what has been said, it is obvious that the clinical teaching of obstetrics in this country was in a sad state up to a fairly recent period. As for the clinical teaching of gynecology, it was a.lmost noncxistcnt. Fortunately, however, from the early part of this century, great improvement can be recorded. Administrative Measures in Puerperal Fever

INVESTIGATIONS carried out for the Departmental Committee on Maternal Mortality have shown the importance of strains of the micro-organism known as the haemolytic streptococcus, and of carriers of this organism, in the causation of puerperal fever. The Ministry of Health, in a circular recently issued to Medical Officers of Health, therefore recommends on the occurrence of fever in a lying-in woman the isolation and separate nursing of the patient, and investigations to determine the nature of the infecting organism and possible sources of infection. The last-named include bacteriological investigation of the patient and of those who may have been in contact with her during labour and for 48 hours afterwards. As contact with hands and instruments is the most likely mode of infection of the genital tract, attention should be directed to infections or abrasions of the skin of anyone who has had contact with the patient.

ONE THOUSAND CASES IN STUDENTS' OUTDOOR OBSTETRIC PRACTICE.REPORT OF FIRST SERIES IN THE OUTDOOR OBSTETRIC DEPARTMENT OF THE UNIVERSITY COLLEGE OF MEDICINE.

I have the pleasure of submitting my report of the first series of 1,000 cases in the outdoor obstetric department of the University College of Medicine for the period of (nearly) eight years, ending Oct. 23, 1902. To be strictly accurate, a service of seventy-six months is represented, the two stations not being provided with regular attendants during four of the college vacations. Of course, the average number of patients treated annually increases year by year, as the service becomes better known. As far as I know, this work is the first attempt on the part of any medical school in the southern states to conduct a systematic and regularly organized free service for lying-in women at their own homes. A CASE OF CHRONIC INVERSION OF THE UTERUS: SPONTANEOUS REDUCTION AFTER THREE YEARS.

In bringing to your attention to-day the subject of inversion of the uterus, it is done partly because our program has never before borne such a one, and partly because it is practical; this latter because anyone attending a lying-in woman may meet with such a complication, not often, it is true, possibly not once in a lifetime, or in several lifetimes, for when we consult such records as that of the Rotunda Hospital in Dublin we find only one case in 190,000 deliveries, and in St. Petersburg no case occurred in 200,000 births. Few obstetricians have seen a case, but the subject is practical, for the unexpected does too often happen. It does not follow that because large hospitals where skilled aid is at hand, present so few cases of inverted uteri, they are so very infrequent, when we turn our attention to the methods of accouchment as followed

GONORRHEA IN THE PUERPERIUM.

In these latter days of widespread knowledge of aseptic measures for shielding the parturient woman from harm, the occurrence of puerperal sepsis is looked upon, even by the laity, as evidence that some one has blundered, and the attendant is fortunate if he escapes the charge of criminal negligence or inexcusable ignorance. If the infection of the lying-in woman happened only during labor, or her subsequent stay in childbed, the charge might be sustained with justice, but we are convinced that it happens more frequently than is apprehended that an infection has antedated childbed by weeks or months, if indeed it has not preceded conception itself and invaded territory absolutely beyond the reach of any possible antiseptic treatment. For ages the cause of Puerperal Fever (which is a symptom—not a disease) was supposed to exist in the atmosphere of the lying-in hospital or chamber; it was believed that the fever was an infectious, contagious disease —that it was not necessary for actual contact to take place. The many opportunities both on the operating table and in the dead room for studying its pathology have cleared the mystery. As a result of the recognition of its septic character within the last few years, the puerperal woman has been looked upon as a wounded woman and has been treated as such; so it is now a rare occurrence for a woman to have a bad getting up after confinement because practitioners recognize the principles of aseptic surgery and apply them intelligently to the treatment of the lying-in woman.
