Skin lesions in diseases of the chest

Summary An account is given of the cutaneous manifestations of tuberculosis, sarcoidosis and polyarteritis nodosa. Skin changes in tuberculosis may be due to the presence of tubercle bacilli living in the skin or they may be secondary to disease elsewhere in the body. In the first group the following conditions are found: primary tuberculosis of the skin, miliary tuberculosis, lupus vulgaris, tuberculosis verrucosa cutis, scrofuloderma and tuberculosis cutis orificialis. The second group consists of erythema nodosum, erythematous tuberculide, Bazin's disease, papulo-necrotic tuberculide, lichenoid tuberculides, lupus miliaris disseminatus faciei and rosaccous tuberculide. In sarcoidosis skin lesions take on a variety of forms, the principal ones being classified in one of four main groups: Boeck's sarcoid, lupus pernio, erythematous sarcoid and the subcutaneous lesions of Darier-Roussey. Lesions in Boeck's sarcoid may be either nodular or in plaques and they are often found associated with lesions of lupus pernio. In polyarteritis nodosa, skin changes are found in about 25 per cent. of all cases. The lesions may be non-specific and consist of urticaria and various forms of erythema or they may be due to damage to blood vessels in or near the skin. Nodules are the most characteristic of these changes, but h˦morrhages, livedo reticularis and ulcerations also occur. Occasionally polyarteritis nodosa is confined to the skin and in these cases livedo reticularis and ulcerations are the characteristic features.

MICROPAPULAR TUBERCULID AND ROSACEA

THIS study was prompted by the belief that resistant rosacea in many cases has been diagnosed cutaneous tuberculosis because of the term rosacea-like tuberculid. Lewandowsky's 1 classic description, which was reintroduced by MacKee and Sulzberger 2 and Wile and Grauer, 3 will not be repeated in detail here. The eruption occurs on the face and consists of purplish or brownish red papules or papulopustules. There may be distinct hyperemia and telangiectasia of varying amount. Under diascopic pressure a small amount of yellowish brown staining remains. We believe that many cases in which the diagnosis was rosacea-like may after careful analysis prove to be rosacea and not tuberculosis or, in other instances, the disease may be some other type of tuberculosis, such as lupus miliaris disseminatus faciei, papulonecrotic tuberculid or even papular sarcoid. In many cases the diagnosis of rosacea-like has been made without strict adherence to the criteria

THE MICROPAPULAR TUBERCULID

Micropapular tuberculid is the term which we have applied to a form of cutaneous tuberculosis composed of tiny papules occurring on the face. This type, while differing from both lupus miliaris disseminatus faciei and acnitis, may at times resemble rosacea. Tuberculoderms, although subject to great clinical variations, have a fairly well established course and prognosis, especially in a specific case in which the characteristics conform strictly to a well recognized type of cutaneous tuberculosis. This is true of lupus vulgaris, lupus miliaris disseminatus faciei and acnitis. There are, however, patients on whom lesions are found, particularly on the face, which have a prolonged course and a tuberculoid structure but still do not conform morphologically to any of the aforementioned types. Lewandowsky¹in 1917 first described such a type of tuberculosis of the skin, which he called rosacea-like tuberculid. His classic description is now well known and has been reintroduced

<h3>Sarcoidosis. Presented by Dr. Henry Silver.</h3> R. H., a Negress aged 25, married and childless, is presented from the clinic cf Mount Sinai Hospital for Dr. William Leifer. She was first studied in April 1936, at which time there were lesions on the face and on the extensor aspects of the extremities. A clinical diagnosis of lupus miliaris disseminatus faciei and papulonecrotic tuberculid in a patient with late asymptomatic syphilis was made. Histologic examination confirmed the diagnosis of papulonecrotic tuberculid, but examination of tissue from the face revealed a picture suggestive of sarcoid. Intradermal injections of tuberculin in dilutions of 1: 1,000,000 and 1: 100,000 gave negative reactions. The blood count was normal. The Wassermann reaction was positive. Roentgenograms of the hands and wrists showed no changes in the bones. A roentgenogram of the chest showed infiltration at the root of the right lung and enlargement of the right paratracheal

ACQUIRED SPECIFIC HYPERSENSITIVITY TO SIMPLE CHEMICALS

The various inflammatory conditions of the vermilion border of the lips are usually called with varying qualifying adjectives. Since the vermilion border is embryologically closely related to the skin, it is not unexpected that the majority of cutaneous diseases may affect the transitional membranes of the lips. The differential diagnosis of inflammatory processes of the vermilion border must include such common cutaneous diseases as lupus erythematosus, erythema multiforme, drug eruptions, pemphigus, lichen planus and lupus miliaris disseminatus faciei. While certain cutaneous diseases, such as psoriasis, seborrheic eczema and pityriasis rosea, rarely if ever localize on the lips, other diseases, including herpes simplex, bullous erythema multiforme, certain drug eruptions, angioneurotic edema and syphilitic chancres and papules, show a predilection for this region. In addition, there are certain so-called idiopathic superficial and deep pathologic alterations which usually appear solely on the lips. These include cheilitis exfoliativa and cheilitis glandularis. CLINICAL AND HISTOLOGIC FEATURES OF CERTAIN TYPES OF CUTANEOUS TUBERCULOSIS

INTRODUCTION Not only is the pathogenicity of certain forms of tuberculosis of the skin still the subject of lively debate, but even the clinical and histopathologic features of certain presumably tuberculous diseases have not as yet been definitely cataloged nosologically. Certain well-known diseases, of which lupus vulgaris and tuberculosis verrucosa cutis are prototypes, have, of course, been long recognized as being due to the invasion of the skin by the bacillus of Koch. As time went on, and investigations progressed, other skin diseases were soon proved to be of tuberculobacillary nature; among these are lichen scrofulosorum, acute disseminated miliary tuberculosis, scrofuloderma, tuberculosis ulcerosa and lupus miliaris disseminatus faciei. These diseases may be designated as genuine tuberculoses of the skin. Investigations carried on in recent years have resulted in the inclusion, in this group, of two closely related affections, namely, the erythema induratum of Bazin and the subcutaneous sarcoid of Darier.
