Effectiveness of the drug «Sulfatonic» in the treatment of patients with lumbar dorsopathy

Abstract. Prevention and medical rehabilitation of vertebrogenic pain syndromes continues to be one of the most pressing problems of rehabilitation medicine. Purpose of the study. The purpose of the study was to evaluate the effectiveness of Sulfatonic in the treatment of patients with vertebrogenic lumbar dorsopathy. Material and methods. Complex treatment was carried out on 32 patients with vertebrogenic lumbar dorsopathy with pain, musculo-tonic and neurodystrophic syndrome aged 36–60 years. Patients in the main group, against the background of the basic complex, received general hydrogen sulfide baths based on the «Sulfatonic» agent according to the developed method, patients in the control group received the basic complex. Results. It has been established that the use of general baths based on the «Sulfatonic» product in patients of the main group helps to increase the range of movements in the lumbosacral spine, reduce tension and soreness of the paravertebral muscles, regress the symptoms of tension, pain, improve the psycho-emotional status, with less pronounced dynamics of indicators in patients of the control group. Conclusion. Complex therapy of patients with dorsopathies with long-term pain syndrome using hydrogen sulfide baths based on Sulfatonic is effective. A course of use of «Sulfatonic» dissolved in water at the rate of 35 mg per 100 liters of water in the form of general baths helps to increase the range of motion in the lumbosacral spine, reduce tension, soreness of the paravertebral muscles, regression of tension symptoms, pain syndrome, improvement of well-being and mood, increased activity of patients with lumbar dorsopathy.

Polypharmacy: definition, impact on outcomes, need for correction

The review is devoted to a modern problem of polypharmacy. A universal definition and clear criteria for this concept have not yet been formed, but it is believed that this is the prescribing of at least 5 medications (M). The article discusses the frequency and main causes of polypharmacy, demonstrates its clear relationship with the age. The presence of overweight and obesity, multimorbidity, low physical activity, fragility are clearly associated with polypharmacy. Cognitive impairment, disability, long-term pain syndrome and malignant diseases also predispose to polypharmacy. The absence of a permanent attending physician, living in a nursing home, consulting with several specialists, poor management of medical records are associated with polypharmacy. It is believed that polypharmacy leads to a following number of adverse consequences: it increases the risk of falls, side effects of M, hospitalizations and even death. The main reason for this is the occurrence of various adverse interactions between M, including unpredictable ones, but the causal relationship of these phenomena with polypharmacy is not always proven. To study of adherence to prescribed therapy with polypharmacy is not an easy task, to date, there is no clear answer to the question whether polypharmacy affects adherence to drug therapy. The article presents in detail the problems of potentially irrational prescriptions, discusses the main methods of preventing and combating polypharmacy. Obviously, the most acceptable methods are the cancellation of drugs that are not indicated or contraindicated to the patient, and the prescribing of those drugs for which there are direct indications, but which the patient does not receive. The patient’s therapy should be individualized as much as possible, taking into account numerous factors related to the peculiarities of the disease course, the prognosis, the patient’s lifestyle, his physical and mental status.

Nonspecific purulent-inflammatory lesions of the spine are rare anddifficult to diagnose diseases. At the same time, there is an increase in the frequency of their occurrence and unsatisfactory treatment results remain.Aim of the study.By demonstrating these clinical cases, to emphasize the complexity of timely diagnosis of nonspecific spondylodiscites and their purulent-inflammatory complications, to show the possibilities of mini-invasive treatment options for the pathology in questionMaterial and methods.The description of these clinical cases was carried out in accordance with the recommendations and explanations of the CARE (CAse REport) and SCARE (Surgical CAse REport) groups. The article presents patients with nonspecific spondylodiscites with purulent-inflammatory complications who were treated at the State Clinical Hospital No. 25 of Volgograd. The study was carried out in accordance with the ethical norms of the Helsinki Declaration and the «Rules of Clinical Practicein the Russian Federation» approved by the Order of the Ministry of Health of the Russian Federation No.266 dated 06.19.2003. Patients gave their free informed consent to the publication of clinical cases in the open press.Results.Using the example of two clinical cases, the complexity of diagnosis and differential diagnosis of nonspecific spondylodiscites with purulent-inflammatory complications is shown, the possibility of percutaneous drainage of abscesses as an independent treatment option is substantiated.Conclusion.With a long-term pain syndrome in the back, not relieved bynonsteroidal anti-inflammatory drugs, and the presence of risk factors, an instrumental examination of the spine was performed to exclude this pathology. Sanitation anddrainage of a abscess can be an independent operation.

Osteoarthritis (OA) is the most common chronic progressive joint disease. Over the last years, it has been evidenced that OA is associated with an increased risk of death, which may be explained by low-intensity chronic inflammation underlying its pathogenesis, long-term pain syndrome, and high comorbidity. Considering the above, the FDA classified OA as a serious disease that requires more careful monitoring of patients and the earliest possible prescription of effective therapy in August 2018 [1].

The significance of FM associations for women with FM

Purpose: Living with fibromyalgia (FM) means living with a long-term pain syndrome that is invisible to others. Support and understanding from others seem to be important to managing the affected daily life. The aim of this study was to describe the significance of FM associations for women with FM. Methods: Data collection was carried out through focus group discussions with seventeen women with FM. Data were analyzed through thematic content analysis. Results: The findings show that women experienced associations for people with FM as important as they gave access to contacts with others with similar experiences. Their need of togetherness was fulfilled at the association and they described being strengthened by the support received. Because of the lack of information and knowledge about FM, the association was described as an important venue for getting and mediating information about the illness. Conclusions: At the association the women seem to be empowered, which increases their ability to manage their daily lives despite the limitations imposed by FM. Healthcare personnel could not satisfy the women’s needs and to manage to support women with FM. There is a need for communication based on a shared understanding between the women and healthcare personnel.Implications for RehabilitationThis study highlighted the need for communication based on a shared understanding between people with chronic illness and healthcare personnel to support and strengthen women with FM in their daily lives.The FM associations meet the needs for togetherness, confirmation, and information that the women with FM in this study described and healthcare personnel could not satisfy.Healthcare personnel can learn from FM associations how to empower women with FM in their everyday lives.
