IMPACT OF NYLON INSERTS FOR THE LOCATOR ATTACHMENT ON FUNCTIONAL ADAPTATION FOR MANDIBULAR OVER DENTURE WEARER (ELECTROMYGRAPHIC ANALYSIS)

Purpose: This research investigated the effect of two nylon inserts for the locator attachment with different retentive values on oral rehabilitation for the patients wearing mandibular overdenture regarding their muscular activity. Materials and methods: Six complete maxillary and mandibular overdentures were delivered to six healthy male patients. Two implants were installed in the canine region via computerized surgical stent. The mandibular overdentures were retained by locator attachment. Grouping of the study were done according to the used retentive nylon insert of the locator attachment Group 1: All the patients used blue nylon inserts for three months Group 2: the same patients used pink nylon inserts for another three months, with one month rest period interposed between the used blue &pink nylon inserts . The impact of these types of nylon inserts on the activity of masseter & temporalis muscles was investigated via electromygraphic analysis. Results: ANOVA test revealed improvement for all patients in tested muscles activity expressed as EMG records considering amplitude of these muscle during clenching and chewing hard &soft food either for blue or pink nylon inserts with statistical significance between different measuring periods. Paired t-test proved that pink insert provides significant improvement in muscular activity than the blue one at p ≤ 0.05. Conclusion: Depending on the investigation of this study , it be concluded that the retentive system used in the locator attachment has great impact on oral rehabilitation regarding the patients’ muscular activity.

Retention of mandibular implant-retained overdentures with two different attachment designs: An in vitro study

Statement of problemRetentive force and loss of retention should be considered when selecting an attachment. Studies that evaluate the retentive force and the effect of repeated cycles of insertion and removal on the TITACH attachment with a metal-to-metal interface are needed. PurposeThe purpose of this in vitro study was to measure the retention of mandibular implant-retained overdentures with 2 different attachment designs before and after 1000 insertion and removal cycles. Material and methodsTwo dental implants were inserted at the mandibular canine region bilaterally in a completely edentulous model. Sixteen mandibular implant-assisted overdentures were fabricated to form 2 groups, each with a different attachment design. One group received 8 pairs of TITACH attachments, while the other group received 8 pairs of LOCATOR attachments. A cyclic loading machine was used to perform 1000 insertion and removal cycles. A universal testing machine was used to evaluate retentive force before and after insertion and removal cycles. Data were analyzed by using the Mann-Whitney U test and Wilcoxson signed-rank test. ResultsWhen retentive forces before and after 1000 insertion and removal cycles were compared, the TITACH attachment showed significantly higher retentive force than the LOCATOR (P<.05). However, the LOCATOR attachment showed a significantly lower percentage change in retentive force (P<.05). ConclusionsThe TITACH attachment group showed favorable initial and final retentive force compared with the Zest Anchor LOCATOR attachment group. However, the LOCATOR attachment group showed favorable lower percentage change in retentive force.

Evaluation of two different attachment systems used with mandibular implant-retained overdenture

Objectives: The aim of this study was to evaluate the effect of two different attachment systems on the retention and implant stability of implant-retained mandibular overdenture. Materials and Methods: Fourteen completely edentulous patients with age ranged from 47 to 65 years planned to receive conventional complete denture. Following the treatment protocol, each patient received two implants in the mandibular anterior region, and after insurance of the osseointegration, the patients were randomly divided into two groups – Group A received Ball/O-Ring attachment and Group B received locator attachment. The retention of two groups was assessed by the digital force meter at three times (T): T0 – retention of conventional complete denture, T1 – at time of insertion of implant-retained mandibular overdenture, and T3 – retention after 3 months of insertion of implant-retained mandibular overdenture. The implant stability quotient (ISQ) was done using Magnetic Resonance Frequency Analyzer (Osstell, ISQ) at the time of loading then after 3 and 6 months. Results: The retention values before the insertion of overdenture (T0) were considerably low in comparison with those at time of insertion of overdenture (T1) and after 3 months from denture insertion (T3). Regarding the ISQ values, there was no significant difference between the two groups before and at the time of insertion of implant while there was a significant difference between the two groups with better stability results in the locator attachment group after 3 months (P Conclusion: Within the limitation of this study, both types of attachment systems, i.e., Ball/O-Ring and locator attachments are reliable modalities for improving the retention and stability of implant-retained mandibular overdenture with superior initial stability results for the locator attachment.

Effect of Two Implant-supported Partial Overdenture Attachment Design on the Periodontal Health

This study aimed to evaluate and compare the effect of locator attachment and ball and socket (BS) attachment on the peri-implant and periabutment supporting structures on clinical pocket depth and gingival index in cases of limited interarch spaces in mandibular Kennedy class I implant-supported removable partial overdentures. A comparative clinical trial was conducted among twenty partially edentulous patients aged 30-60 years. The study participants were randomly divided into two study groups based on the attachment system used: group I - locator attachment, and group II - BS attachment. Two implants were positioned in the 1st or 2nd molar area following the two-stage surgical protocol. Evaluation of the peri-implant and periabutment supporting structures was done at the time of overdenture insertion and after 6, 12, and 18 months by measuring the pocket depth and gingival index. Inter- and intragroup comparisons were done using independent-samples t test and paired-sample t test respectively. p < 0.05 was considered statistically significant. It was observed that there was a statistically significant increase in the pocket depth around the implant as well as the abutment in both groups at 6, 12, and 18 months when compared to baseline. Intergroup comparison for pocket depth and gingival index revealed nonsignificant results. However, the values were higher in the BS group. The locator attachment group had lower, though statistically not significant, pocket depth and gingival index scores around both the dental implant and the natural abutment as compared with the BS attachment. Gingival health surrounding dental implant attachments is very crucial for the long-term success of dental implant, this could help the clinician to select the proper design for implant attachment underneath the over denture prosthesis.

ELECTROMYOGRAPHIC EVALUATION OF IMPLANT OVERDENTURE RETAINED BY TWO DIFFERENT TYPES OF ATTACHMENTS (RANDOMIZED CONTROL TRIAL)

The objective of the present study was to evaluate and compare the influence of two different types of attachments on the masticatory function of complete denture wearers through studying the electromyographic (EMG) activity of the masseter and anterior fibers of temporalis muscles.Material and methods: Fourteen completely edentulous patients with problematic existing mandibular dentures received new conventional complete dentures. Two implants were installed in the mandibular canine regions. After verification of Osseointegration. Patients were randomly divided into two groups by using closed envelope according to the type of attachments used (ball and locator) were installed onto each implant. Electromyographic (EMG) activity of masseter and anterior fibers of temporalis muscles was measured during chewing hard and soft food with conventional dentures before implant placement, one month and three months after implant retained overdentures with the two types of attachments. Data was collected and statistically analyzed.Results: The results obtained from thisstudy revealed a statistically significant increase in the muscles activity after attachment of mandibular dentures onto the implants in the studied groups. The highest activity values were reported for the locator attachment group followed by ball attachment however, the least values were recorded for the conventional complete denture. Nevertheless, the comparison between the two groups at the end of follow up period revealed a statistically insignificant difference .Conclusion: Muscles activity significantly increased after implant treatment. Thus, implant treatment greatly improves oral function. The design of implant attachments did not affect significantly the muscle activity; as no significant differences were observed among the studied attachments (Ball and locator).

THE EFFECT OF MAGNETIC AND LOCATOR ATTACHMENTS ON STRAIN AROUND IMPLANTS RETAINING MANDIBULAR OVERDENTURE: AN IN VITRO STUDY

Objectives: This in-vitro strain gauge study was conducted to compare between magnetic and locator attachments regarding strain induced around two implants supporting a mandibular overdenture. Methodology: The study was performed on acrylic models of a completely edentulous mandibular arch with two implants installed bilaterally at the canine areas. The strain induced around the implants was studied during unilateral and bilateral loading at the first molar area using a universal testing machine. Results: The results of this study showed that there was no statistically significant difference between the two studied groups during unilateral loading with higher values in the locator attachment group while during bilateral loading there was a statistically significant difference between the two groups but with higher micro-strain values for the magnetic attachment group Conclusion: Within the limitation of this in-vitro study, it could be conclude that the attachment type could have an effect on the amount of load that will be transmitted to the implants supporting an over denture. Clinical studies are required to evaluate the effect of type of attachment on peri-implant crestal alveolar bone loss and correlate it with the results of the current study.

HARD TISSUE RESPONSE COMPARISON IN AN IMPLANT OVERDENTURE RETAINED BY LOCATOR OR EQUATOR ATTACHMENTS. A RANDOMIZED CLINICAL TRIAL

Introduction: Completely edentulous patients usually have many problems associated with their mandibular complete dentures. Dental implants solved many of these problems. Many attachments are used nowadays to retain mandibular overdenture to dental implants. Among these attachments are the low profile locator and equator attachments. This study was conducted to study the effect of both attachments on bone level around two implants retaining complete mandibular overdenture. Methodology: This study was conducted on 10 completely edentulous patients. Two implants were installed in the canine region bilaterally for all patients. Patients were then randomly divided into two groups where the first group received implant supported mandibular overdenture retained by locator attachment, while the other group received implant supported mandibular overdenture retained with equator attachment. The patients were then referred to oral and maxillofacial radiology department, Faculty of Dentistry, Cairo University for radiographic assessment to measure the bone height and density changes around the implant at the day of overdenture delivery and 12 months later.Results: In the current study, bone height changes in both groups was in the clinically permissible range. Regarding bone density, it increased gradually around the loaded implants in both groups. When density is compared between the two groups, the locator attachment group showed significant increase when compared to the equator attachment group after 12 months of prosthetic loading.Conclusions: Within the limitation of this study, both locator and equator attachments are viable treatment options to retain an implant supported mandibular complete overdenture regarding hard tissue response around the implants but more studies with more extended follow up is recommended.

IMPLANT STABILITY AND MARGINAL BONE LOSS IN MANDIBULAR IMPLANT RETAINED OVERDENTURE CASES: A COMPARATIVE STUDY USING TWO STUD ATTACHMENT SYSTEMS

The present study was designed to compare two stud attachment systems [Locator and Ball / O-Ring attachment] in mandibular overdenture cases retained by two implants in the inter-foraminal area. Twenty completely edentulous male patients received mandibular overdentures retained by two implants were randomly assigned into two groups; Group-I received Locator attachment while Group-II received Ball / O-Ring attachment. A comparison based on implant stability quotient [ISQ] was done using Resonance Frequency Analyzer [Osstell, ISQ] and another comparison was conducted based on the marginal bone loss using cone beam computed tomography [CBCT]. The patients were then scheduled for recall appointments up to two years follow up period starting from the loading time. The statistical analysis of the results reported no significant difference for the marginal bone loss & ISQ values between the two groups at all intervals except the third month implant stability results which showed a significant difference between the two groups with better stability results in the Locator attachment group (at p<0.05). Within the limits of the present study, it may be concluded that both studied attachment systems are successful and useful however the Locator attachment system shows superior initial stability results than the Ball attachments.
