Effect of aspirin on biomarker profile in women at high-risk for preeclampsia.

There is limited evidence in the literature regarding the temporal changes of preeclampsia-related biomarkers during pregnancy in high-risk women who develop preeclampsia despite the administration of aspirin prophylaxis. To compare the temporal changes in mean arterial pressure (MAP), uterine artery pulsatility index (UtA-PI), placental growth factor (PlGF), and soluble fms-like tyrosine kinase-1 (sFlt-1) across gestation in women screened high-risk for preterm preeclampsia receiving aspirin prophylaxis and low-risk women without aspirin treatment. This was a prospective longitudinal nested case-control study of 2007 women with singleton pregnancies who participated in the first-trimester screen-and-prevent program for preeclampsia at the Prince of Wales Hospital, Hong Kong SAR, China, between January 2020 and May 2023. The risk of developing preterm preeclampsia was determined using the Fetal Medicine Foundation triple test (maternal factors, MAP, UtA-PI and PlGF). High-risk women (adjusted risk ≥1:100) were administered a daily dose of aspirin at either 100 or 160 mg according to maternal weight, starting before 16 weeks until 36 weeks or until delivery or the onset of preeclampsia before 36 weeks. Low-risk women were matched based on maternal age, weight, and the date of the scan. The participants were followed up at 12-15+6, 20-24+6 and 30-37+6 weeks to measure MAP, UtA-PI, PlGF and sFlt-1 at each visit. The level of biomarker was expressed as multiple of median (MoM). Log10 transformation was applied to make the data Gaussian distribution prior to statistical analysis. A linear mixed-effects analysis was performed to compare the longitudinal changes of these biomarkers across gestation between the study groups. Our study involved 403 low-risk women without preeclampsia, 1471 high-risk women without preeclampsia, and 133 high-risk women who developed preeclampsia. The low-risk group had significantly lower estimated marginal mean log10 MAP MoM, log10 UtA-PI MoM, log10 sFlt-1 MoM and higher estimated marginal mean log10 PlGF MoM across gestation than the high-risk groups (p values <0.001). Among high-risk women, those who developed preeclampsia experienced a significantly higher estimated marginal mean log10 MAP MoM (0.06378 vs 0.02985; p<0.001), log10 UtA-PI MoM (0.08651 vs 0.02226; p<0.001), log10 sFlt-1 MoM (0.13204 vs 0.01234; p<0.001) and lower estimated marginal mean log10 PlGF MoM (-0.33504 vs -0.16388; p<0.001) across gestation in comparison to those without preeclampsia. In the individual gestational timepoint analysis, compared to high-risk women without preeclampsia, those who developed preeclampsia experienced higher log10 MAP MoM in all three trimesters, higher log10 UtA-PI MoM and lower log10 PlGF MoM in the second- and third-trimesters, and higher log10 sFlt-1 MoM in the third-trimester. The study has demonstrated that high-risk women who develop preeclampsia experience consistently high MAP level from the first-trimester and remain unchanged during pregnancy, high UtA-PI level and low PlGF level starting from the second-trimester, and high sFlt-1 level in the third-trimester compared to those who do not develop preeclampsia despite the administration of low-dose aspirin. These findings underscore the role of these biomarkers in further risk stratification for the development of preeclampsia among high-risk women following aspirin administration.

Promoting a sustainable diet among office workers in Sweden: A pilot Study

Abstract Background Society is facing multiple challenges, including lifestyle-related diseases, alongside global climate change. Actions are needed to promote sustainable lifestyles advocating for healthy and environmentally friendly diets. This study aimed to assess how a sustainable lifestyle, compared to conventional healthy lifestyle education at work, affects participants’ energy intake, nutrient consumption, and diet-related CO2e. Methods An 8 week two-armed randomized trial was conducted, comprising a sustainable lifestyle arm (n = 21) and a healthy lifestyle arm (n = 16) based on the theoretical framework of the Behavioral Change Wheel, in total (n = 33) completed the trial. The office workers participated in 6 workshops during working hours. Linear mixed-effects models and analysis of variance tests were used for data analysis. Changes over time are presented as arms combined, along with their respective mean differences. Results A significant decrease in mean diet-related CO2e was observed (p &amp;lt; 0.001), with a decrease on average of 0.74 and 0.71 kg CO2e/day. The mean change in daily kilocalorie intake had a significant decrease over time (p = 0.008), on average of 192 and 386 Kcal. Moreover, a significant decrease in mean protein intake was observed (p &amp;lt; 0.001), with a decrease on average of 16 and 18 grams, possibly explaining the decrease in CO2e. No significant differences were observed between the arms at any timepoint. Conclusions Promoting sustainable lifestyle habits has a similar impact to advocating for a conventional healthy lifestyle regarding diet. This supports the new Nordic Nutrition Recommendations 2023. Despite the small sample size, insights into facilitating sustainable behaviors can influence public health policies across Europe and the workplace as an arena for health promotion. Key messages • The promotion of sustainable and healthy lifestyle habits at work can promote both individual and planetary health by decreasing diet-related CO2e emissions. • Here we demonstrate that the workplace has the potential to act as an arena to combat climate change.

Moderators of the Effects of a Digital Parenting Intervention on Child Conduct and Emotional Problems Implemented During the COVID-19 Pandemic: Results From a Secondary Analysis of Data From the Supporting Parents and Kids Through Lockdown Experiences (SPARKLE) Randomized Controlled Trial.

A smartphone app, Parent Positive, was developed to help parents manage their children's conduct and emotional problems during the COVID-19 pandemic. A randomized controlled trial, Supporting Parents and Kids Through Lockdown Experiences (SPARKLE), found Parent Positive to be effective in reducing children's emotional problems. However, app effectiveness may be influenced by a range of child, family, socioeconomic, and pandemic-related factors. This study examined whether baseline factors related to the child, family, and socioeconomic status, as well as pandemic-related disruption circumstances, moderated Parent Positive's effects on child conduct and emotional problems at 1- and 2-month follow-up. This study was a secondary exploratory analysis of SPARKLE data. The data set included 646 children (4-10 years of age) with parents randomized to either Parent Positive (n=320) or follow-up as usual (n=326). Candidate baseline moderators included child age, gender, attention-deficit/hyperactivity disorder symptoms, parental psychological distress, family conflict, household income, employment status, household overcrowding, and pandemic-related disruption risk (ie, homeschooling, lockdown status, and isolation status). Child conduct and emotional problem outcomes measured at 1- (T2) and 2-months (T3) post randomization were analyzed using linear mixed-effects analysis of covariance models adjusting for baseline (T1) measure of outcome and including intervention and intervention by time point interaction terms allowing for different effects at the 2 time points. Moderation of intervention effects by baseline factors was assessed by replacing the intervention by time interaction terms with intervention by time point by baseline moderator interaction terms. Child gender was a significant moderator of the Parent Positive versus follow-up as usual effect on emotional problems (B=0.72, 95% CI 0.12-1.33; P=.02). Specifically, the effect of Parent Positive was close to significant (T2: B=-0.41, 95% CI -0.82 to 0.0004; P=.05) or significant (T3: B=-0.76, 95% CI -1.22 to -0.30; P<.001) in males only when compared with females, and males experienced a significantly larger reduction in emotional problems than females in the Parent Positive arm at the 2-month post randomization time point. None of the other investigated baseline factors moderated effects on emotional problems, and no factors moderated effects on conduct problems. This study highlights Parent Positive's potential for effectively reducing emotional problems in primary school-aged male children across a wide range of families. However, due to limited variability in the demographic background of the families, cautious interpretation is required, and replications are necessary in diverse samples with longer follow-up times. Glossed video keyword captions and L2 vocabulary acquisition: an eye-tracking study

The effect of video captions on vocabulary acquisition is a growing topic of interest among L2 researchers. Expanding on this line of inquiry, this study explores how keyword captions enhanced with first language (L1) and second language (L2) glosses (i.e. definitions or brief explanations of unfamiliar words) impact L2 learners’ vocabulary acquisition. This study involved 101 Korean undergraduate students randomly assigned to a baseline (keyword caption only), L1 gloss (keyword caption + L1 gloss), or L2 gloss (keyword caption + L2 gloss) group. The participants viewed a video that corresponded to their group, during which their eye movements were tracked and recorded. Subsequently, the participants completed two vocabulary tests assessing word form and meaning recall. Linear mixed-effects analysis revealed that keyword captions with L1 and L2 glosses resulted in distinct attentional allocation and contributed differently to lexical acquisition. Notably, although the three groups spent similar durations reading the target word forms, the L1 gloss group significantly outperformed the other two groups in word form recall scores. Furthermore, although the L2 gloss group spent three times longer reading L2 glosses compared to the time the L1 gloss group spent reading L1 glosses, the latter had significantly higher scores in the form translation test. These findings have significant implications for L2 pedagogy, particularly regarding the choice between target-language-exclusive or bilingual approaches.

P175 ESTIMATION OF SODIUM INTAKE CAN BE IMPROVED BY CORRECTION OF 24H SODIUM EXCRETION FOR 24H URINE VOLUME

Background and Objective: Long-term sodium (Na) balance studies demonstrated that 24 h sodium excretion does not reflect 24 h sodium intake, which complicates the use of 24 h urine sodium measurements for sodium intake estimation. This discrepancy has been linked to infradian rhythms of aldosterone and cortisol, which are responsible for adaptive responses targeted to (renal) water conservation. We hypothesize that correction for these parameters will improve the accuracy of sodium intake estimates. Methods: We used data from two sodium balance studies that controlled dietary salt intake for 105 (4 men) and 205 days (6 men) at levels of 6, 9 and 12 g/day. We tested whether correction of 24 h urine sodium for 24 h urine volume, 24 h urine potassium, 24 h urine urea, 24 h urine aldosterone, 24 h urine cortisol, 24 h urine cortisol/cortisone and/or urine osmolality, increased the accuracy of 24 h sodium intake estimates using forward selection linear mixed-effects analysis with a random intercept per participant. Results: The median difference between 24 h sodium intake and excretion was 26 mmol (IQR 13 - 45 mmol). 24 h urine sodium explained only 49% of the variance in 24 h sodium intake. In 7 out of 10 subjects, 24 h urine volume was of most added value to the model, after 24 h urine sodium, increasing R2 with 0.08±0.03. Including 24 h urine volume in the model significantly improved the model fit (P&lt;0.001, marginal R2 0.55). The addition of urine urea, urine aldosterone and urine osmolality also contributed significantly but the marginal R2 only further increased to 0.56. After correction of 24 h urine sodium for 24 h urine volume, sodium intake could be estimated more accurately: the 50-mmol discrepancy between 24 h sodium intake and excretion was reduced from 18% to 10% and the 25-mmol discrepancy from 52% to 43% (P&lt;0.001, Figure 1). Conclusions: In healthy young males, estimation of sodium intake can be improved by correcting 24 h urine sodium for 24 h urine volume.

Neighborhood in Chinese lexicon: A megastudy analysis of lexical decision and naming of two-character Chinese words.

The present study examines the impact of neighborhood size (number of other two-character words sharing the same character at the same position) on Chinese lexical processing, along with its joint effects with variables such as character frequency, word frequency, and semantic transparency. Previous factorial experiments have yielded conflicting results that are difficult to reconcile with existing models (Li et al., 2015, 2017). To provide high-powered tests for these theoretically important effects on visual word recognition, we leveraged the megastudy approach and used linear mixed-effect analyses to investigate lexical decision and naming responses to a large pool of two-character Chinese words (N > 17,000) sourced from Tse et al.'s (2017, 2023) database. In all analyses we controlled for extraneous orthographic (e.g., stroke count), phonological (e.g., consistency), and semantic (e.g., transparency) variables. In addition to evaluating Li et al.'s (2015, 2017) models, we also investigated whether the parallel dual-route mechanism, which entails lexical access via whole-word or character decomposition-then-composition, could account for neighborhood size effect and its interactions in lexical decision and naming. Finally, we discuss the implications of our findings on the specificity of lexical effects with regard to character position and lexical processing task. Biomarkers in the prediction of complications in pregnancy after assisted reproductive technology.

To compare the temporal changes in mean arterial pressure (MAP), uterine artery pulsatility index (UtA-PI), placental growth factor (PlGF), and soluble fms-like tyrosine kinase-1 (sFlt-1) across gestation between assisted reproductive technology (ART) pregnancies complicated with great obstetrical syndromes (GOS) or gestational diabetes (GDM) ± large-for-gestational-age (LGA) fetus, and uncomplicated ART pregnancies. This was a prospective longitudinal study of 143 women with singleton pregnancies who conceived through ART at the Department of Obstetrics and Gynecology, Prince of Wales Hospital, the Chinese University of Hong Kong, Hong Kong SAR between December 2017 and January 2020. The participants were followed up at 6-6+3, 11-13+6, 20-24+6, 30-34+6, and 35-37+6 weeks for the measurement of MAP, UtA-PI, PlGF, and sFlt-1. A linear mixed-effects analysis was performed to compare the biomarkers in the GOS, GDM ± LGA, and uncomplicated groups across gestation. Thirty-three (23.1%) and fifty-five (31.5%) women were diagnosed with GOS and GDM ± LGA, respectively. The GOS group had higher estimated marginal mean log10 MAP mulitples of the median (MoM) across gestation, compared with the uncomplicated group (0.00771 vs -0.02022; P < 0.001), when adjusting for clinical visits and days of embryo transfer. The absolute mean log10 MAP MoM in the GOS group was found to be significantly higher than that of the uncomplicated group at all clinical visits from 6 weeks onwards. Furthermore, the estimated marginal mean log10 PlGF MoM was significantly lower in the GOS group across gestation, compared with the uncomplicated group (-0.04226 vs 0.05566; P = 0.010). The significant difference in log10 PlGF MoM was observed from 11-13+6 to 30-34+6 week of gestation (P < 0.05). However, no significant differences in the estimated marginal means of log10 UtA-PI MoM and log10 sFlt-1 MoM between GOS and uncomplicated groups were observed. GDM ± LGA group had a lower estimated marginal mean log10 PlGF MoM throughout pregnancy compared with the uncomplicated group (-0.01536 vs 0.05572; P = 0.032). In the individual visit analysis, the significant difference was observed at the 20-24+6 and 35-37+6 weeks visits (P < 0.05). There were no significant differences in estimated marginal mean log10 MoM of MAP, UtA-PI, and sFlt-1 between GDM ± LGA and uncomplicated groups during pregnancy. Our study has revealed that among pregnancies conceived through ART, GOS is associated with higher MAP and lower PlGF from early gestation until late third trimester, while GDM ± LGA is associated with lower PlGF during the second half of pregnancy. The same degree of differences in MAP and PlGF persists from early until late gestation in the GOS group and these findings highlight the importance of early screening during the first trimester to identify women who are at risk for developing GOS following ART procedures. Lastly, the potential of PlGF in predicting the development of GDM from the second trimester of pregnancy requires further investigation.

Usefulness of Transbronchial Lung Cryobiopsy When Starting Antifibrotic Treatment and Predicting Progressive Fibrosing Interstitial Lung Disease: Descriptive Research.

Although transbronchial lung cryobiopsy (TBLC) is widely used in diagnostic algorithms for various interstitial lung diseases (ILDs), its real-world utility in the therapeutic decision-making strategy for ILD patients remains unclear, in particular, when judging the time to start antifibrotic agents. We analyzed medical records of 40 consecutive patients with idiopathic or fibrotic hypersensitivity pneumonitis who underwent TBLC. A TBLC-based usual interstitial pneumonia (UIP) score was used to assess three morphologic descriptors: patchy fibrosis, fibroblastic foci, and honeycombing. In our 40 patients with ILD, the most frequent radiological feature was indeterminate for UIP (45.0%). Final diagnosis included idiopathic pulmonary fibrosis (22.5%), fibrotic nonspecific interstitial pneumonia (5.0%), fibrotic hypersensitivity pneumonitis (35.0%), and unclassifiable ILD (37.5%). Linear mixed-effects analysis showed that declines in the slopes of %FVC and %DLCO in patients with TBLC-based UIP "Score ≥ 2" were significantly steeper than those of patients with "Score ≤ 1." During follow-up of patients with Score ≥ 2 (n = 24), more than half of them (n = 17) received an antifibrotic agent, with most patients (n = 13) receiving early administration of the antifibrotic agent within 6 months after the TBLC procedure. TBLC-based UIP Score ≥ 2 indicated the increased possibility of a progressive fibrosis course that may prove helpful in predicting progressive pulmonary fibrosis/progressive fibrosing ILD even if disease is temporarily stabilized due to anti-inflammatory agents. Patients may benefit from early introduction of antifibrotic agents by treating clinicians.

Exploring the Interpersonal Outcomes of Mindfulness-Based Stress Reduction Training Among Police Employees

Police employees have an essential role in sustaining safety and harmony within communities, and they need to stay well as individuals in order to achieve this. Yet, their stressful work environment does not always afford optimal interpersonal interactions. In collaboration with the Dutch police force, the present project explored the effects of a mindfulness-based stress reduction (MBSR) training on the fostering of constructive interpersonal responses as well as individual well-being among employees of the Dutch police. In total, 133 police employees participated in a non-randomized waitlist controlled MBSR intervention. Participants filled out four online questionnaires spread evenly over a period of six months on constructive interpersonal responses and individual well-being. Linear mixed-effects analyses showed increases on various indicators of constructive interpersonal responding and individual well-being from pre-to post-training. These results were largely maintained at two-month follow-up. Interestingly, the positive effects of mindfulness intervention did not generalize across all measures of constructive interpersonal responding. Moreover, more frequent home practice predicted training benefits. We discuss the theoretical and practical implications of these findings.

Altered fronto-limbic-motor response to stress differs between functional and epileptic seizures in a TBI model

Background and ObjectivesPsychogenic nonepileptic (functional) seizures (FS) clinically resemble epileptic seizures (ES) with both often preceded by traumatic brain injury (TBI). FS and ES emergence and occurrence after TBI may be linked to aberrant neurobehavioral stress responses. We hypothesized that neural activity signatures in response to a psychosocial stress task would differ between TBI + FS and TBI + ES after controlling for TBI status (TBI-only). MethodsIn the current multicenter study, participants were recruited prospectively from Rhode Island Hospital, Providence Rhode Island Veterans Administration Medical Center, and the University of Alabama at Birmingham Medical Center. Previous diagnoses of TBI, ES, and FS were verified based on data collected from participants, medical chart and record review, and, where indicated, results of EEG and/or video-EEG confirmatory diagnosis. TBI + ES (N = 21) and TBI + FS (N = 21) were matched for age and sex and combined into an initial group (TBI + SZ; N = 42). A TBI-only group (N = 42) was age and sex matched to the TBI with seizures (TBI + SZ) group. All participants completed an fMRI control math task (CMT) and stress math task (SMT) based on the Montreal Imaging Stress Task (MIST). ResultsThe TBI + SZ group (n = 24 female) did not differ in mood or anxiety severity compared to TBI-only group (n = 24 female). However, TBI + FS group (n = 11 female) reported greater severity of these symptoms compared to TBI + ES (n = 13 female). The linear mixed effects analysis identified neural responses that differed between TBI-only and TBI + SZ during math performance within the left premotor cortex and during auditory feedback within bilateral prefrontal cortex and hippocampus/amygdala regions. Additionally, neural responses differed between TBI + ES and TBI + FS during math performance within the right dorsolateral prefrontal cortex and bilateral amygdala during auditory feedback within the supplementary motor area. All tests comparing neural stress responses to psychiatric symptom severity failed to reach significance. DiscussionControlling for TBI and seizure status, these findings implicate specific nodes within frontal, limbic, and sensorimotor networks that may maintain functional neurological symptoms and possibly distinguish FS from ES. This study provides class II evidence of differences in neural responses to psychosocial stress between ES and FS after TBI.

Socioeconomic disparities and green space associated with myopia among Chinese school-aged students: A population-based cohort study.

There is increasing evidence on the link between environmental factors and myopia in children and adolescents, yet with inconsistent conclusions. We investigated the associations between socioeconomic inequalities and green space with myopia in school-aged students participating in the Tianjin Child and Adolescent Research of Eye (TCARE) study. We obtained data from a population-based dynamic cohort study conducted in Tianjin, China, in 2021 and followed up in 2022. We included 1 245 271 participants from 16 districts with an average age of 11.6 years (standard deviation = 3.3) in our analysis. We synthesized their area-level SES through a prediction model that combined economic, educational, and health care variables and assessed the greenness levels surrounding the school using the Normalized Difference Vegetation Index (NDVI) based on data obtained through satellite remote sensing. We performed generalised linear mixed effects analyses for each myopia outcome separately, with adjustments for students' sex, years of education completed, and the school's geographical location. We observed that students living in low SES areas had the highest prevalence of myopia (60.7%) in the last screening in 2022, as well as a higher incidence of one-year myopia (26.4%) compared to those residing in middle SES areas (22.7%). With a 0.1 increase in the 250, 500, and 1000 m buffer NDVI, the prevalence of myopia dropped by 6.3% (odds ratio (OR) = 0.937; 95% confidence interval (CI) = 0.915, 0.960), 7.7% (OR = 0.923; 95% CI = 0.900, 0.946), and 8.7% (OR = 0.913; 95% CI = 0.889, 0.937), respectively. The interaction analysis showed that low SES and low greenness exacerbate the prevalence of myopia. Findings from longitudinal analyses consistently demonstrated a correlation between higher values of NDVI and a slower progression of myopia. These findings remained robust across sensitivity analyses, including for variables on parental myopia and students' behaviors. Exposure to green spaces could play a crucial role in slowing the progression of myopia among school-aged students. Myopia control policies should prioritise young populations residing in low SES areas with limited access to green spaces, as they face the highest potential risks.

Patient Perceptions of Standardized Risk Language Used in ACR Prostate MRI PI-RADS Scores

IntroductionProstate MRI reports use standardized language to describe risk of clinically significant prostate cancer (csPCa) from “equivocal” (Prostate Imaging Reporting and Data System [PI-RADS] 3), “likely” (PI-RADS 4), to “highly likely” (PI-RADS 5). These terms correspond to risks of 11%, 37%, and 70% according to American Urological Association guidelines, respectively. We assessed how men perceive risk associated with standardized PI-RADS language. MethodologyWe conducted a crowdsourced survey of 1,204 men matching a US prostate cancer demographic. We queried participants’ risk perception associated with standardized PI-RADS language across increasing contexts: words only, PI-RADS sentence, full report, and full report with numeric estimate. Median perceived risk (interquartile range) and absolute under/overestimation compared with American Urological Association standards were reported. Multivariable linear mixed-effects analysis identified factors associated with accuracy of risk perception. ResultsMedian perceived risks of csPCa (interquartile range) for the word-only context were “equivocal” 50% (50%-74%), “likely” 75% (68%-85%), and “highly likely” 87% (78%-92%), corresponding to +39%, +38%, and +17% overestimation, respectively. Median perceived risks for the PI-RADS-sentence context were 50% (50%-50%), 75% (68%-81%), and 90% (80%-94%) for PI-RADS 3, 4, and 5, corresponding to +39%, +38%, and +20% overestimation, respectively. Median perceived risks for the full-report context were 50% (35%-70%), 72% (50%-80%), and 84% (54%-91%) for PI-RADS 3, 4, and 5, corresponding to +39%, +35%, and +14% overestimation, respectively. For the full-report-with-numeric-estimate context describing a PI-RADS 4 lesion, median perceived risk was 70% (50%-%80), corresponding to +33% overestimation. Including numeric estimates increased correct perception of risk from 3% to 11% (P < .001), driven by men with higher numeracy (odds ratio 1.24, P = .04). ConclusionMen overestimate risk of csPCa associated with standardized PI-RADS language regardless of context, especially for PI-RADS 3 and 4 lesions. Changes to PI-RADS language or data-sharing policies for imaging reports should be considered.

Ophthalmic artery Doppler in women with hypertensive disorders of pregnancy: relationship to blood pressure control and renal dysfunction at 6-9 weeks postnatally.

To examine the postnatal course of ophthalmic artery (OA) Doppler in women with hypertensive disorders of pregnancy (HDP) and to evaluate the correlation between OA Doppler parameters and poor postnatal blood pressure control and renal dysfunction at 2-3 weeks and 6-9 weeks postnatally. This was a prospective cohort study of women with a singleton pregnancy and HDP seen at a tertiary pregnancy hypertension clinic between 2019 and 2021. Three visits were included: Visit 1, the last visit to the antenatal hypertension clinic within 2 weeks prior to delivery; Visit 2, at 2-3 weeks postnatally; and Visit 3, at 6-9 weeks postnatally. At each visit, maternal demographic characteristics, medical history, blood pressure and OA Doppler were obtained. In addition, fetal growth and fetal Dopplers were examined antenatally and, at 6-9 weeks postnatally, estimated glomerular filtration rate and proteinuria were quantified. Study participants were divided into four hypertension groups, according to longitudinal changes in blood pressure at the three visits. For the postnatal visits, hypertension was defined as systolic blood pressure (SBP) ≥ 140 mmHg and/or diastolic blood pressure (DBP) ≥ 90 mmHg in the absence of antihypertensive medication, and SBP ≥ 130 mmHg and/or DBP ≥ 80 mmHg whilst taking antihypertensives. Group 1 was hypertensive at all three visits; Group 2 was hypertensive at Visits 1 and 2 but normotensive at Visit 3; Group 3 was hypertensive at Visits 1 and 3 but normotensive at Visit 2; and Group 4 was hypertensive at Visit 1 but normotensive at Visits 2 and 3. The longitudinal changes in mean arterial pressure (MAP), peak systolic velocity (PSV) 1, PSV2 and the ratio of PSV2/PSV1 over the three timepoints were examined by a repeated-measures, multilevel, linear mixed-effects analysis, controlling for maternal age, weight at presentation and use of antihypertensive medication. In addition, we examined the longitudinal change in OA Doppler parameters in women with different degrees of postnatal blood pressure control and in those with and those without renal dysfunction at 6-9 weeks postnatally. A total of 108 women were recruited to the study, of whom 86 had new-onset hypertension and 22 had chronic hypertension. When controlling for maternal age, weight at presentation and use of antihypertensive medication, a significant decline in log10 MAP (P < 0.001), log10 PSV1 (P < 0.001) and log10 PSV2 (P = 0.01) was seen between Visits 1 and 3. Log10 PSVR did not change with time. When assessing OA Doppler against hypertension group, log10 PSV1 and log10 PSV2 did not differ between the hypertension groups, whilst Group 4 had a lower log10 PSVR compared with Group 1 (P < 0.01), Group 2 (P = 0.03) and Group 3 (P < 0.01). At 6-9 weeks postnatally, log10 PSVR was lower in those without compared to those with renal dysfunction (-0.021, P = 0.01), whilst log10 MAP, log10 PSV1 and log10 PSV2 values did not differ. Log10 PSVR did not change with time and remained at -0.12 (95% CI, -0.13 to -0.11) across the three visits. In women with HDP, the OA-PSVR was significantly higher in those with labile or persistently raised blood pressure postnatally compared to women whose blood pressure normalized. Similarly, the OA-PSVR at 6-9 weeks postnatally was significantly higher in women with renal dysfunction vs those without dysfunction. © 2023 International Society of Ultrasound in Obstetrics and Gynecology.

Mine for life: Charting ownership effects in memory from adolescence to old age.

This study investigates the self-reference effect (SRE) with an ownership memory task across several age groups, providing the first age exploration of implicit ownership memory biases from adolescence to older adulthood (N = 159). Using a well-established ownership task, participants were required to sort images of grocery items as belonging to themselves or to a fictitious unnamed Other. After sorting and a brief distractor task, participants completed a surprise one-step source memory test. Overall, there was a robust SRE, with greater source memory accuracy for self-owned items. The SRE attenuated with age, such that the magnitude of difference between self and other memory diminished into older adulthood. Importantly, these findings were not due to a deterioration of memory for self-owned items, but rather an increase in memory performance for other-owned items. Linear mixed effects analyses showed self-biases in reaction times, such that self-owned items were identified more rapidly compared with other owned items. Again, age interacted with this effect showing that the responses of older adults were slowed, especially for other-owned items. Several theoretical implications were drawn from these findings, but we suggest that older adults may not experience ownership-related biases to the same degree as younger adults. Consequently, SREs through the lens of mere ownership may attenuate with age.

What influences the comprehensibility of L2 writers' opinion texts by L2 readers? Interactions between textual characteristics and readers’ profiles

This study explored how L2 readers perceive the comprehensibility (i.e., ease of understanding) of L2 opinion texts in terms of reader profiles and text characteristics. We investigated how 100 Japanese learners of L2 English evaluated the comprehensibility of 16 variants of two opinion essays, which differed in lexico-grammatical accuracy, coherence, and rhetorical organization (inductive vs. deductive structure). To delve into the evaluators' backgrounds, exploratory factor analysis was conducted with a view to streamlining background variables. These variables were then compared through a t-test of two groups of readers identified by cluster analysis based on their degree of leniency or strictness. We further identified the influence of specific linguistic features on their evaluations by conducting linear mixed-effects analyses. Our findings reveal that: (1) Some L2 readers were significantly stricter than the L1 readers, whereas others were more lenient; (2) The lenient readers typically began learning English earlier, used it more extensively outside academic settings, and engaged more frequently in online communication compared to their stricter counterparts; and (3) Though disrupted sequences of ideas (coherence errors) were universally detrimental to comprehensibility, lexico-grammatical errors significantly impacted only the strict readers, not the lenient ones.

Examining Linguistic Differences in Electronic Health Records for Diverse Patients With Diabetes: Natural Language Processing Analysis.

Individuals from minoritized racial and ethnic backgrounds experience pernicious and pervasive health disparities that have emerged, in part, from clinician bias. We used a natural language processing approach to examine whether linguistic markers in electronic health record (EHR) notes differ based on the race and ethnicity of the patient. To validate this methodological approach, we also assessed the extent to which clinicians perceive linguistic markers to be indicative of bias. In this cross-sectional study, we extracted EHR notes for patients who were aged 18 years or older; had more than 5 years of diabetes diagnosis codes; and received care between 2006 and 2014 from family physicians, general internists, or endocrinologists practicing in an urban, academic network of clinics. The race and ethnicity of patients were defined as White non-Hispanic, Black non-Hispanic, or Hispanic or Latino. We hypothesized that Sentiment Analysis and Social Cognition Engine (SEANCE) components (ie, negative adjectives, positive adjectives, joy words, fear and disgust words, politics words, respect words, trust verbs, and well-being words) and mean word count would be indicators of bias if racial differences emerged. We performed linear mixed effects analyses to examine the relationship between the outcomes of interest (the SEANCE components and word count) and patient race and ethnicity, controlling for patient age. To validate this approach, we asked clinicians to indicate the extent to which they thought variation in the use of SEANCE language domains for different racial and ethnic groups was reflective of bias in EHR notes. We examined EHR notes (n=12,905) of Black non-Hispanic, White non-Hispanic, and Hispanic or Latino patients (n=1562), who were seen by 281 physicians. A total of 27 clinicians participated in the validation study. In terms of bias, participants rated negative adjectives as 8.63 (SD 2.06), fear and disgust words as 8.11 (SD 2.15), and positive adjectives as 7.93 (SD 2.46) on a scale of 1 to 10, with 10 being extremely indicative of bias. Notes for Black non-Hispanic patients contained significantly more negative adjectives (coefficient 0.07, SE 0.02) and significantly more fear and disgust words (coefficient 0.007, SE 0.002) than those for White non-Hispanic patients. The notes for Hispanic or Latino patients included significantly fewer positive adjectives (coefficient -0.02, SE 0.007), trust verbs (coefficient -0.009, SE 0.004), and joy words (coefficient -0.03, SE 0.01) than those for White non-Hispanic patients. This approach may enable physicians and researchers to identify and mitigate bias in medical interactions, with the goal of reducing health disparities stemming from bias.

Anxiety, depression and distress outcomes from the Health4Life intervention for adolescent mental health: a cluster-randomized controlled trial

Mental disorders are a leading cause of disease burden worldwide. As onset typically occurs in adolescence, prevention during this period is critical. The Health4Life-school-based multiple health behavior change (MHBC) intervention targets six lifestyle risk factors: diet, sleep, physical activity, screentime, alcohol use and smoking. Health4Life has been evaluated in a cluster-randomized controlled trial in 71 Australian schools (6,639 grade seven students). This study presents intervention effects on secondary outcomes of depressive, anxiety and psychological distress symptoms. Generalized linear mixed-effect analyses of data from baseline, post-intervention (7 weeks), 12 months and 24 months showed that the Health4Life intervention was no more effective than an active control in reducing depressive, anxiety or psychological distress symptoms at a 24 or 12 month follow-up; however, there were short-term benefits for psychological distress and depressive symptoms immediately post-intervention. This study offers new evidence that multiple health behavior change interventions may improve adolescent mental health, but future research should explore methods to address anxiety and sustain effects over the longer term. Longitudinal Changes in Psychosocial Adjustment Before and During the COVID-19 Pandemic for Adolescents with Differential Patterns of Solitude and Sociability.

Previous research has lacked a comprehensive, longitudinal analysis of characteristics of solitude and sociability, and how they are associated with changes in psychosocial adjustment before and during the pandemic. The current study surveyed 1071 adolescents (Mage = 10.6, SD = 1.69, 49.86% female, age range = 8-14 years at Year 1) over six years (three years before pandemic, three years during pandemic). Piecewise linear mixed-effects analysis showed that adolescents with higher solitude and lower sociability reported improvements in adjustment during the pandemic, whereas adolescents with lower solitude and higher sociability reported declines in adjustment. The findings highlight the importance of considering multiple characteristics of solitude and sociability, as well as contextual factors (e.g., pandemic), to better understand the implications of solitude on adolescent adjustment.

Exploring the Benefits and Acceptance of Blended Positive Psychotherapy as an Adjunctive Treatment for Clients with Residual Depressive Symptoms: A Mixed-Method Study

A proof-of-concept study was conducted to explore the acceptability and potential benefits of a blended positive psychotherapy intervention for clients with residual depressive symptoms. A single-arm pilot study was conducted in 2022 and 2023 with 24 Dutch adults experiencing residual depressive symptoms after treatment. Clients who had recently received an evidence-based treatment for depressive disorder were approached to participate in this study through opportunity sampling. The intervention consisted of nine sessions with a therapist and a six-week self-guided digital positive psychology intervention. Acceptability was examined using semi-structured interviews (n = 15). Participants filled out questionnaires pre- (n = 21), mid- (n = 14) and post-intervention (n = 8). Potential benefits were assessed in terms of changes in mental well-being (MHC-SF), depression (PHQ-9) and personal recovery (QPR). Quantitative data and qualitative data were analysed using linear mixed-effects models and framework analysis, respectively. The analyses were primarily based on Sekhon’s theoretical framework of acceptability. Linear mixed-effects analyses showed changes over time in most mental health indicators, including mental well-being (Hedge’s g = 1.58), depression (g = 1.43) and personal recovery (g = 1.96). Most of the interviewed participants considered blended positive psychotherapy a valuable adjunctive treatment; it connected well with their wish to become more positive in their daily life without ignoring difficult experiences. For some participants, shifting towards a positive treatment approach was difficult, resulting in early dropout. This study’s findings suggest that blended positive psychotherapy is acceptable to most people with residual depressive symptoms after treatment. Its impact is yet to be established in larger samples of studies involving more robust designs.

Long-Term Longitudinal Course of Cognitive and Motor Symptoms in Patients With Cerebral Small Vessel Disease.

Patients with cerebral small vessel disease (SVD) show a heterogenous clinical course. The aim of the current study was to investigate the longitudinal course of cognitive and motor function in patients who developed parkinsonism, dementia, both, or none. Participants were from the Radboud University Nijmegen Diffusion Tensor and Magnetic Resonance Cohort study, a prospective cohort of patients with SVD. Parkinsonism and dementia were, respectively, diagnosed according to the UK Parkinson's Disease Society brain bank criteria and the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, criteria for major neurocognitive disorder. Linear and generalized linear mixed-effect analyses were used to study the longitudinal course of motor and cognitive tasks. After a median follow-up of 12.8 years (interquartile range 10.2-15.3), 132 of 501 (26.3%) participants developed parkinsonism, dementia, or both. Years before diagnosis of these disorders, participants showed distinct clinical trajectories from those who developed none: Participant who developed parkinsonism had an annual percentage of 22% (95% CI 18%-27%) increase in motor part of the Unified Parkinson's Disease Rating Scale score. This was significantly higher than the 16% (95% CI 14%-18%) of controls, mainly because of a steep increase in bradykinesia and posture and gait disturbances. When they developed dementia as well, the increase in Timed Up and Go Test time of 0.73 seconds per year (95% CI 0.58-0.87) was significantly higher than the 0.20 seconds per year increase (95% CI 0.16-0.23) of controls. All groups, including the participants who developed parkinsonism without dementia, showed a faster decline in executive function compared with controls: Annual decline in Z-score was -0.07 (95% CI -0.10 to -0.05), -0.09 (95% CI -0.11 to -0.08), and -0.11 (95% CI -0.14 to -0.08) for participants who developed, respectively, parkinsonism, dementia, and both parkinsonism and dementia. These declines were all significantly faster than the annual decline in Z-score of 0.07 (95% CI -0.10 to -0.05) of controls. A distinct pattern in deterioration of clinical markers is visible in patients with SVD, years before the diagnosis of parkinsonism and dementia. This knowledge aids early identification of patients with a high risk of developing these disorders.
