Lichen Aureus with Dermoscopic Findings: A case report

Lichen aureus is a rare variant of pigmented purpuric dermatosis, which often affects young adults and is localized mainly on the lower extremities. We present here a 31-year-old female who had a solitary golden-brown-color patch on her right leg for 1 year. Punch biopsy of the lesion revealed lymphohistiocytic band-like infiltrate of the papillary dermis with erythrocyte extravasation and hemosiderin deposits. The diagnosis of Lichen aureus was made based on clinicopathologic findings. The dermoscopic findings that support this diagnosis were brownish yellow diffuse coloration of the background, round to oval red dots, globules and patches and twisted red loops. Herein, we present the dermoscopic findings of a 31-year-old female patient with a diagnosis of lichen aureus established with clinical and histopathological features. Diagnostic usefulness of dermoscopy in differentiating lichen aureus from nummular eczema.

Lichen aureus (LA) is a variant of pigmented purpuric dermatosis that is characterized clinically by rust macules, papules or plaques, mainly on the legs. In some cases, LA can be difficult to be distinguished from nummular eczema (NE) with the naked eye. Dermoscopy can be applied to skin lesions, revealing additional features that can be highly valuable for correct diagnosis. This study was conducted to investigate characteristic dermoscopic findings of LA and to identify distinctive features that can differentiate it from NE. Fourteen LA patients and 14 NE patients diagnosed by skin biopsy were enrolled. Skin lesions were evaluated via polarized dermoscopy. On dermoscopy, "coppery orange diffuse coloration of background", "round to oval red globules", "gray dots", and "networks of brownish to gray interconnected lines" were more commonly seen in LA (100%, 92.9%, 42.9% and 64.3%, respectively) compared with NE. "Scales", "shiny yellow clods" and "irregularly distributed brownish-red globules" were more commonly seen in NE (100%, 85.7% and 57.1%, respectively) compared with LA. Dermoscopy provides valuable information for diagnosis of LA and aids in differentiating it from NE.

Lichen aureus: clinicopathological features in a Chinese series.

Lichen aureus (LA) is a rare variant of pigmented purpuric dermatosis. There are few Chinese cases of LA documented in the literature in English. We aimed to investigate the clinical and histopathological characteristics of LA in a series of Chinese patients. This was a retrospective monocentric study conducted in the Department of Dermatology, Peking Union Medical College Hospital. A total of 25 patients were included in the study; 15 male and 10 female patients. Unilateral distribution of the lesions was found in 88.0% of the patients, while bilateral distribution was found in 12.0%. A segmental distribution was observed in 12 patients. The most commonly involved areas were the upper extremities (n=10; 40.0%) and lower extremities (n=10; 40.0%). Fifteen cases (60%) presented with an inflammatory reaction pattern of a band-like lymphocytic infiltrate. Periadnexal inflammation and perineural inflammation were found in 12 patients and five patients, respectively. A segmental pattern of distribution of the lesion is very common in LA. Both upper limbs and lower limbs may be equally affected. Band-like lymphocytic infiltrate is the most common inflammatory reaction pattern. Periadnexal inflammation and perineural inflammation are relatively common in LA.

Lichen aureus (LA) presents an inflammatory infiltrate with a lichenoid pattern, which mainly localizes to the superficial dermis, and it is admixed with extravasated erythrocytes. We present the case of a 12-year-old male who had a 1.3-cm golden-brown-color patch on the right upper back for 2 weeks. Histopathologic examination revealed a lichenoid, superficial, and deep perivascular lymphocytic infiltrate with perineural and periappendageal involvement, extravasated erythrocytes, papillary dermal edema, and hemosiderophages. The perineural and periappendageal infiltrate is unusual for LA and suggestive of lichen striatus (LS). In addition, the presence of perineural inflammation is not a common feature for either LA or LS. However, given the clinical findings, taken together, these features were interpreted as being most consistent with LA. To the best of our knowledge, no previous case of LA has been reported with histological features of perineural inflammation and LS.

Lichen Aureus

A possible association between lichen aureus (LA) and mycosis fungoides (MF) has been suggested in the past. We evaluated the clinicopathologic features of LA and its relationship to MF. Data from 23 patients with a clinicopathologic diagnosis of LA were reviewed. Lesions were asymmetrically localized on 1 area of the body (mostly 1 extremity) and were characterized histologically by dense, bandlike lymphocytic infiltrates. A monoclonal T-cell population was detected in half of the cases. After a mean follow-up of 102.1 months, 14 patients had no sign of skin disease, 7 patients had unmodified skin lesions, and 2 other patients with unmodified skin lesions had died of unrelated conditions. Treatment modalities did not affect the outcome. There was no relationship between the presence or absence of monoclonality and patient status at follow-up assessments. Conclusion Patients with classic lesions of LA do not show progression to MF.

Lichen aureus is a rare asymptomatic dermatosis of unknown origin that is now classified in the group of pigmented purpuric dermatoses. The eruption consists of asymptomatic, roundish or irregular, lichenoid erythematous-purpuric papules with a tendency to coalesce in patches, most prevalent on the limbs. No meaningful laboratory abnormalities have been found. Histologically, the epidermis is normal, with a lymphohistiocytic, bandlike infiltrate with extravasated blood red cells and hemosiderin deposits observed in the dermis. During the last 20 years we have followed eight new patients, which represent 0.05% of our hospitalized patients. In five of eight patients the disease resolved in two to four years. No therapy has been carried out on these children, since lichen aureus has a tendency for slow, spontaneous improvement and resolution.

<h3>To the Editor.—</h3> Lichen aureus is a chronic and benign capillaritis of unknown cause. The typical lesion is a single golden patch, often localized on the lower extremity. We describe a patient with lichen aureus in a segmental distribution on the left side of the chest and the left upper extremity. The first lesion developed about one month after a surgical procedure on the distal third of the left forearm. Lichen aureus: a case report

Lichen aureus (LA) is a distinctive, rare disease occurring predominantly in males, characterized by solitary rust- or copper-coloured patches of lichenoid micropapules, often present on fingers and calves. A case of LA in a 41-year-old male is presented. Biopsy revealed the characteristic pattern of LA, including a dense dermal infiltrate of lymphocytes and histiocytes containing haemosiderin. No epidermal changes were present.

A Report of Two Patients With Lichen Aureus

Two patients with lichen aureus are described. The clinical and histologic features of our patients were compared with previously reported cases of lichen aureus in an effort to classify this entity properly among the pigmented purpuric dermatoses. We regard lichen aureus as a distinct entity. Although this disorder has been described principally in adult patients it began at the age of 14 years in one of our patients.
