Grease-gun injury of the orbit: two cases report and literature review

BackgroundThe grease-guns injury is an uncommon injury to the orbit. We present the twelfth and thirteenth cases of grease-gun injury to the orbit to be reported in the English language literature since 1964. Here we discus and review the presentation, investigation, and treatment of this unusual trauma.Case presentationCase 1 was a 29-year-old man who presented 1 day after a grease-gun injury of the left orbit with severe pain, marked periorbital swelling, and proptosis. Computed tomography (CT) revealed penetration of grease into his left orbit. Following surgical removal, proptosis decreased. The limitation of extraocular movement and loss of visual acuity to finger count was discovered after the initial surgery. Motility gradually returned. Visual acuity recovered after phacoemulsification, capsular tension ring and intraocular lens implantation for traumatic cataract and subluxation. Case 2 was a 6-year-old boy who was referred 2 months after a grease-gun injury for worsening swelling with sinus, necrosis and slight ptosis of the upper left eyelids. This is a case of orbital chronic inflammation from grease-gun injuries masquerading as orbital cellulitis. The imaging findings of CT and magnetic resonance imaging (MRI) are not typical. Surgical exploration and debridement was inevitable and actually relieved the symptoms.ConclusionsGrease-gun injuries can damage the orbit in different degrees. Careful history inquiry and taking is important to establish the diagnosis. Imaging examinations using CT or MRI are helpful to determine depth of trauma and foreign bodies in the orbit at diagnosis. We suggest that surgical exploration and debridement is a key step in the management.

Primary versus secondary intraocular lens implantation in traumatic cataract after open-globe injury in pediatric patients

To compare the visual and refractive outcomes and complications of primary and secondary intraocular lens (IOL) implantation after open-globe injury in pediatric patients. Sadguru Netra Chikitsalaya, Madhya Pradesh, India. Retrospective case series. Patients had primary cataract extraction with IOL implantation with globe repair (Group A) or repair followed by secondary IOL implantation (Group B). Preoperative and surgery data, visual outcomes, and postoperative complications were recorded. The study included 139 patients. Thirty (49%) of 61 patients in Group A and 47 (60%) of 78 patients in Group B achieved a corrected distance visual acuity (CDVA) of 20/40 or better. In both groups, a central corneal scar and amblyopia were the main reasons for not achieving a CDVA better than 20/40. The mean spherical equivalent (SE) was 1.81 diopters (D) in Group A and 1.55 D in Group B. Forty patients (66%) in Group A and in 60 patients (77%) in Group B achieved a desirable refractive outcome (SE <2.00 D). A large wound was the greatest risk factor for not obtaining desirable refractive outcomes. Fibrinous uveitis was the most common postoperative complication in both groups. Pupillary optic capture and IOL decentration were significantly more common in Group A than in Group B (P=.02), while strabismus was significantly more common in Group B (P=.04). Both approaches had satisfactory and comparable visual and refractive outcomes. However, primary IOL implantation can be considered in cases with small peripheral corneal lacerations with the goal of providing early visual rehabilitation and avoiding repeat general anesthesia.

Comparison of outcomes of primary versus secondary intraocular lens implantation for traumatic cataract in children

Objective To assess and compare the outcomes of primary versus secondary intraocular lens (IOL) implantation for traumatic cataract in children.Methods 134 cases (134 eyes) of pediatric traumatic cataract in our hospital were randomly divided into two groups:group A (64 eyes,treated by primary IOL implantation) and group B (70 eyes,treated by secondary IOL implantation).Follow up was from 6 to 24 months.Results The recovery rate from blindness and disability were 90.6％ and 62.5％ in group A and 91.5％ and 80.0％ in group B at 6 months after surgery.There was no significant difference in recovery rate from blindness between two groups (x2 =0.026,P ＞ 0.5).The recovery rates from disability weresignificantly different between two groups (χ2 =5.03,P ＜ 0.05).The postoperative complications were severer in group A than group B in early and late stage.The incidences of pupil capture of IOL,posterior capsular opacity and secondary glaucoma were higher in group A than group B.The differences were statistically significant.Conclusion Traumatic cataract in children treated by secondary IOL implantation can have better visual outcomes and less surgical complications compared with primary IOL implantation. Effect of secondary intraocular lens implantation in traumatic cataract with posterior capsular rupture

Objective To explore the effect of secondary intraocular lens (IOL) implantation in traumatic cataract with posterior capsular rupture.Methods 72 cases of traumatic cataract with posterior capsular rupture were divided into two groups:group A (36 eyes,36 cases) and group B (36 eyes,36 cases).Patients in group A were treated by traumatic cataract removal and primary IOL implantation.Patients in group B underwent traumatic cataract removal and secondary IOL implantation.The visual acuities and surgical complications were compared between two groups.Results There were 22 eyes (61.6％) in group A and 27 eyes (75.0％) in group B with visual acuities better than 0.5 at 8 weeks after surgery and the difference was statistically significant (x2 =8.175,P ＜ 0.05).The different degrees of inflammatory reaction were observed in two groups.The incidences of surgical complications in group B were much lower than that in group A (x2 =10.125,P ＜ 0.05).Conclusion The traumatic cataract with posterior capsular rupture treated by secondary IOL implantation can have better visual outcomes and less surgical complications compared with primary IOL implantation. The clinical analysis on intraocular lens implantation for traumatic cataract

Objective To investigate the surgical methods and skills of intraocular lens implantation in traumatic cataract. Methods 72 cases （73 eyes） of traumatic cataract underwent surgery of intraocular lens implantation, traditional method and fixation method with sutures were selected according to the injury of capluse. Results After 3 months of following up, correct vision was 〈0.5 in25 eyes （34.5%）,0.5-1.0 in32 eyes （43.83%）, 〉 1.0 in 16 eyes （21.92%）. Conclusion It was proved effective method by using different methods of intraocular lens implantation in traumatic cataract according to the injury of len capulse. Intraocular lens implantation for traumatic cataract in children in East Africa

Purpose: To review the visual outcomes and complications after intraocular lens (IOL) implantation in children with traumatic cataract in sub-Saharan Africa, where contact lenses for unilateral aphakia are impractical in most patients. Setting: PCEA Kikuyu Eye Unit, Nairobi, Kenya, East Africa. Methods: All children younger than 16 years having IOL implantation for traumatic cataract between February 1993 and December 1998 (215 eyes) were retrospectively reviewed. Complications and visual outcomes were evaluated. Results: The study group comprised 147 boys and 68 girls. The median interval between injury and cataract surgery was 8 weeks. The most common causes of injury were stick (36.3%) and thorn (10.7%). Extracapsular cataract extraction with IOL implantation was performed in all patients. The most frequent early complication was fibrinous uveitis in 110 eyes (51.2%). One hundred sixty-seven eyes (77.7%) had 1 month or more follow-up. Of those, 108 eyes (64.7%) had a best corrected visual acuity of 20/60 or better. Twenty-one eyes (12.6%) had a visual acuity of worse than 20/200, with the most common causes being amblyopia (9/21) and retinal detachment (5/21). Amblyopia was found in 42 of 108 (38.9%) children aged 8 years or less at the time of injury. Eyes with the IOL in the capsular bag were significantly less likely to require subsequent capsulotomy ( P < .01) during the 2-year follow-up. Conclusion: The results indicate that posterior chamber IOLs can be safely implanted by experienced surgeons in most children older than 2 years with traumatic cataract and should be the standard of care throughout the world.

Intraocular lens implantation in traumatic cataract in children

Thirty-six cases of intraocular lens implantation in children who had cataracts of traumatic origin are presented. Shearing lenses were preferred for implantation in some of these cases. Our results generally support intraocular lens implantation in children whose cataracts are traumatically induced.
