Maxillary Anterior Teeth Defect Caused by Trauma-A Case of Implantation after Bone Augmentation-

A 52 year-old man (born in 1949) presented on January 22, 2002 wishing to receive implant treatment. Approximately 20 years before presentation, the patient was hit directly on the face by a ball while playing baseball and four maxillary anterior teeth were avulsed, resulting in extremely strong resorption of the bone. Although he was using a conus denture, he visited our clinic requesting implant treatment. According to a panorama radiograph and CT scans (transverse section and cross section, it was not possible to place implants in the left maxillary anterior tooth region where bone defect was extensive. The patient desired not only to receive prosthetic treatment for the defect, but also to restore the contour of the gingiva to its original shape. The treatment plan was to place two implants in the right maxillary anterior tooth region where bone was present, as well as to conduct bone grafting in the region with extensive bone defect, and placing implants after the bone volume has been augmented.

真菌塊の排出を伴った上顎洞アスペルギルス症の１例

We report a characteristic case of aspergillosis of the maxillary sinus in which natural discharge of a mass of mycotoxin and choanal flow occurred during follow-up after presentation. The patient was a 47-year-old woman who presented with discomfort and mild tenderness of the left cheek region and discomfort of the left maxillary molar region. The clinical diagnosis was maxillary sinus mycosis. Radical Caldwell-Luc operation was performed on the left maxillary sinus with the patient under general anesthesia. Histological examinations revealed sinus aspergillosis. After operation, the patient's progress has been good, without relapse.

The effect of different vasoconstrictor containing local anaesthetic solutions on plasma glucose in normal and diabetic-volunteers

In this study, the change in plasma glucose half an hour after injection of different vasoconstrictors contained in local anaestheticsolution where measured both in normal and diabetic volunteers. Single cartridge contain (1.8) ml of plain (2%) Xylocain or that with (1:80000) concentration of a adrenaline or (3%) prilocain with (0.03) IU of felypressin injected at labial to left maxillary lateral incisor region. The result showed that half an hour after injection of a adrenaline containing solution, a very highly significant increase in plasma glucose noticed in both study groups from pre-injection base line value (p<0.001). Increase in plasma glucose in diabetic group were significantly greater than that in normal control group (p<0.05). Within diabetic volunteers, this increase were significantly greater in insulin dependent diabetic group than that in non insulin dependent volunteers. After injection of plain solution or that containing felypressin, reduction in plasma glucose noticed. In conclusion, adrenaline contained within local anaesthetic solution produce hyperglycemic effect even in dose of single dental cartridge. This response enhanced in diabetic patient in general with greater enhancement noticed in insulin dependent patient.

上顎洞に発生した髄外性形質細胞腫の１例

In this article, a recent case of extramedullary plasmacytoma originating in the left maxillary sinus is reported. The patient was a 67-year-old man who presented with swollen gingiva in the left maxillary molar region. Under a suspected diagnosis of malignant tumor of the maxillary sinus, a biopsy was performed. Extramedullary plasmacytoma was suspected. Hematological examination revealed MP (+) of IgG (λ) type. In addition, BJP (-) was indicated by urinalysis. On the basis of the results of whole body autoradiographic examination, plasmacytoma originating in the maxillary sinus was finally diagnosed. Partial excision of the maxilla and postoperative irradiation were given as treatment. The patient's progress has been good.

Reconstruction of an alveolar cleft for orthodontic tooth movement

Bone grafting to repair an alveolar cleft has long been an integral part of the treatment of persons with unilateral and bilateral clefts of the lip and alveolus. The presence of the cleft places a limitation on the orthodontist who would like to move teeth in the area of the cleft. Various grafting materials have been placed in alveolar clefts in an attempt to solve this problem. The case to be presented is a patient with a Class II, Division 2, malocclusion with a left unilateral alveolar cleft and a repaired cleft lip. Ten months after initiating orthodontic treatment, a free gingival graft procedure was performed because of insufficient vestibular depth and the narrow width of the keratinized attached gingiva at the left maxillary lateral and central incisor region. Two months after periodontal surgery, a mix of decalcified freeze-dried bone allograft and a granular bioactive glass graft material (1:1) were applied subperiostally on the buccal aspect of the edentulous cleft region. Six months later, the teeth adjacent to the grafted alveolar cleft were orthodontically moved into the edentulous area. The treatment results indicated that orthodontic, periodontal, and surgical interventions resulted in a successful closure of the alveolar cleft as well as improved periodontal conditions of the teeth adjacent to the cleft area. From the orthodontic point of view, tooth movement can be achieved successfully into a bone graft made of freeze-dried bone and bioactive glass. (Am J Orthod Dentofacial Orthop 2000;117:156-63)

放線菌による上顎骨骨髄炎の１例

Approximately 60% of all actinomycotic infections occur in the cervicofacial area. The submandibular region is the most frequent site, and actinomycosis of bone is rare. We report a rare case of actinomycosis arising in the maxilla.The patient was a 51-year-old woman who presented with a diffuse swelling and purulent fistulas in the left maxillary molar region. Radiography revealed an irregular radiopaque mass surrounded by a radiolucent area in the left maxillary molar region. The left maxillary sinus was cloudy. Computed tomography showed hone destruction appearing to be a sequestrum in the left maxillary molar region with severe mucosal thickening of the left maxillary sinus.The clinical diagnosis was chronic osteomyelitis of the maxilla with spontaneous sequestration and chronic maxillary sinusitis. The sequestrum was removed, and radical antrum operation was performed with the patient under general anesthesia. The histopathological diagnosis was actinomycosis with a sequestrum.

Trigeminal Neuralgia Associated with Tentorial Agenesis and Temporal Lobe Herniation. Case Report.

A 22-year-old female presented with an extremely rare case of trigeminal neuralgia associated with tentorial agenesis. The pulsating pain in her left maxillary region persisted for an abnormally long time and had no trigger zone. The pain later spread to the periorbital region. Coronal magnetic resonance imaging revealed left medial temporal lobe herniation caused by tentorial agenesis. The herniated temporal lobe, which had distorted the superior cerebellar artery, was causing compression of the trigeminal nerve. Her condition improved following microvascular decompression surgery. Tentorial agenesis should be considered as a cause of atypical pulsating facial pain, especially in younger patients.

Dermoid cyst is a congenital cyst with an appendage of the skin caused by impaction of the embryonal ectoderm. It usually appears in soft tissue and is extremely rare in the jaw.We recently encountered one case of dermoid cyst with numerous hairs in the left maxillary incisor region in a 17-year-old boy. Here we describe this case along with a discussion based on the literature.

Parosteal osteosarcoma of the maxilla.

We describe the case of a 47-year-old female who presented with a small nodule in the buccal sulcus in the left maxillary incisor region. Clinical and radiographic examination revealed a small sclerotic neoplasm with a short pedicle. Histopathological examination confirmed a diagnosis of parosteal osteosarcoma. The contribution of CT to the diagnosis and management is discussed.

Herpetic Trigeminal Trophic Syndrome

REPORT OF A CASE An 87-year-old white man came for treatment of an arciform ulcer of the left side of the nose, nasolabial fold, and maxillary area that had been present for 10 weeks. The lesion had begun as an itchy growth that progressively expanded and broke down. The patient admitted to picking at the affected area. Previous treatment by another dermatologist with topical and oral antibiotics and oral acyclovir (200 mg by mouth five times a day) had been unsuccessful; a superficial biopsy was nondiagnostic. Twenty years previously, the patient had undergone a left frontal sinus operation and repair of a deviated nasal septum and subsequently developed recurrent, severe, left trigeminal neuralgia, requiring repeated nerve blocks. He also described a peculiar sensation of constant drainage in the nasopharynx. Physical examination demonstrated a crescentic, uniformly deep ulcer on the left ala nasi, maxillary region, and nasolabial fold (Figure 1). There

Peripheral ameloblastoma: report of a case with malignant aspect

Peripheral ameloblastoma is a very rare tumour with histological characteristics similar to those of the centrally located ameloblastoma. It appears in the gum and in the alveolar process of the jaws. We report a case of tumour of the left maxillary region, near the canine area. Histologically it presented itself with follicles and nests of cells with a trabecular architectural pattern. High mitotic index and clear images of perineural infiltration were present. The diagnosis was peripheral ameloblastoma with malignant aspects. At 12-month follow-up no local recurrence was noted.

Prenatal ultrasound diagnosis of an intracranial teratoma influencing management: case report and review of the literature

Of seventeen cases of prenatally diagnosed intracranial teratomas, fourteen babys were delivered through a cesarean section. All children died before or shortly after delivery. In this report another case is presented of ultrasound diagnosis of an intracranial teratoma. The teratoma filled the whole fetal cranium, and eroded through the skull base into the left maxillary region. In full agreement with the wishes of the parents, conservative management resulted in the spontaneous vaginal delivery of a fetus that died during labor as a result of brain compression and acidosis during the second stage. In contrast with other cases in the literature, no encephalocentesis was performed. In order to minimize maternal morbidity, it is advised to induce or accept labor as soon as the diagnosis is clear and to pursue a vaginal delivery.

Removal of an arteriovenous malformation in the optic chiasm: A case of bonnet-dechaume-blanc syndrome without retinal involvement

A rare case of an arteriovenous malformation involving the optic chiasm associated with vascular anomaly in the left maxillary region is presented. The angiogram demonstrated an arteriovenous malformation arising from the anterior circle of Willis (internal carotid, posterior communicating, and proximal anterior cerebral arteries), and located in the region of the optic chiasm and hypothalamus. The case was diagnosed as the Bonnet-Dechaume-Blanc syndrome without intraorbital and retinal arteriovenous malformations. The suprasellar arteriovenous malformation was removed, except for a small portion in the hypothalamus, with satisfactory results.

手挙大に発育した上顎化骨性腺維腫の１例

Ossifying fibroma of the jaw is histologically very similar to fibrous dysplasia. This is a case report of ossifying fibroma which was diagnosed as fibrous dysplasia for a long time. A 24-year-old man, who had a large swelling in left maxillary region, visited our clinic. Previously this lesion which he found 9 years ago was diagnosed histologically as fibrous dysplasia and treated four times. The tumor, however, had grown gradually to fist size. It was diagnosed here as ossifying fibroma from both clinical and histological findings. Tumor was excised under general anesthesia and postoperative course was not eventful for 2 years. Tumor was ball-like mass with dimensions of 66×64×72mm and weighed 180mg. It seemed that CT finding was very useful for differential diagnosis between these to lesions.

Mesenchymal chondrosarcoma (MCS) was originally reported by Lichtenstein and Bernstein in 1959. A limited number of cases, however, has been reported to the present. Histological findings of several parts of this tumor are similar to those of the hemangiopericytoma (HP), reticulum cell sarcoma, and other small-cell tumors.The present report demonstrates a 23-year-old female with MCS of the left maxillary region. The histological diagnosis was considered to be HP based on the findings of a small biopsied specimen. Partial maxillectomy was performed under general anesthesia. The histological findings of the various parts of the resected tumor, however, provided a diagnosis that the tumor was MCS with similar histological features of HP in some parts.In this report, we have discussed the difference in clinical findings between MCS and HP.

Distal oxidative metabolic responses induced by epidermal injury

Gingival wounds were created at the left maxillary molar region in young rats. Following injury, contralateral and gingival wound specimens were removed over a 7 day experimental period. Distal (contralateral) and wound gingival oxidative metabolism was activated within two days' after injury. Cytochrome oxidase (mitochondrial), NADH cytochrome c reductase (mitochondrial, microsomal) and NADPH cytochrome c reductase (soluble) activities were significantly greater at the distal site two days after injury than at the control sham site. Similar trends were also noted at the wound site at the same time intervals. The enzymatic activities returned to normal levels between the (2‐3) days postoperatively at the distal site and between (2‐5) days postoperatively at the wound site.

Odontodysplasia

Odontodysplasia is a rare dental deformity which manifests itself clinically as one or more quadrants of hypoplastic, hypocalcified, and late-erupting teeth. Histologically, the teeth affected by odontodysplasia present abnormalities in all derviatives of the tooth germ. To date, no particular etiologic factor has been commonly agreed upon as the cause of this dental malformation. Two cases of odontodysplasia are presented in this article. In the first case the condition manifested itself throughout the entire maxillary left quadrant. In the second case the condition was contained within the left maxillary central incisor region. Histologic examination of both cases revealed many similarities. The possible etiologies of the two cases are discussed, and the conclusion is that, despite the differences between regional and singular odontodysplasia, the histologic similarities support a common etiology.

Symphosarcomatous infiltration of the cauda equina. Myelography in an unusual case.

Recent writings on the subject of lymphoma and leukemia record an appreciable incidence of central nervous system involvement. 1-5 Infiltration of the spinal leptomeninges, cord, and spinal roots is, however, one of the less common modes by which these disorders produce prominent neural symptoms. Case reports in which this condition has been verified stress the resemblance of the clinical manifestations to those of infectious neuronitis, encephalomyelitis, and meningitis. In the case to be described extensive lymphosarcomatous invasion of spinal roots resulted in a clinical syndrome predominantly characterized by lumbosacral radiculopathy. The motor deficits were much more evident than the sensory, although pain was a prominent symptom. The appearance of the cauda equina at surgery was striking but was easily correlated in retrospect with an interesting myelogram. A CASE OF UPPER-JOW CHONDROSARCOMA

1) A case of chondrosarcoma was reported, which involved the left maxillary region of a 3 year-old boy, probably originating from the ethmoid bone.2) Biopsy specimen revealed a somewhat variant degree of malignancy as they were obtained at different time and from different sites.3) X-ray therapy was ineffective.4) Surgical intervention was only alleviative.5) No metastasis was clinically demonstrated during his life in spite of the considerably large size of the tumor.6) Patient died 18 months after the onset of disease.

MICROSPORON INFECTION OF THE PALPEBRAL AND CILIARY REGIONS

REPORT OF A CASE S. A. S. was a Negro schoolboy, born in Brazil and residing in Bello Horizonte. His family and personal history were of no particular interest. The patient stated that about six months prior to examination an eruption appeared on his scalp; afterward this spread to the forehead, cheeks, eyelids and upper and lower eyelashes on the right side and to both external ears. A dog was the transmitter of the disease, characterized by loss of hair. Dermatologic Examination .—There were ten patches of microsporosis on the patient's scalp, five in the occipital region, one in the right parietal region (fig. 1) and four in the left parietal region (fig. 2). On the face the patches were distributed as follows: one on the forehead (fig. 3), one on the left malar region (fig. 2), one on the left maxillary region (fig. 2), and one at the angle
