Age and Vascular Burden Determinants of Cortical Hemodynamics Underlying Verbal Fluency.

BackgroundAging processes and several vascular burden factors have been shown to increase the risk of dementia including Alzheimer's disease. While pathological alterations in dementia precede diagnosis by many years, reorganization of brain processing might temporarily delay cognitive decline. We hypothesized that in healthy elderly individuals both age-related neural and vascular factors known to be related to the development of dementia impact functional cortical hemodynamics during increased cognitive demands.MethodsVascular burden factors and cortical functional hemodynamics during verbal fluency were assessed in 1052 non-demented elderly individuals (51 to 83 years; cross-sectional data of the longitudinal TREND study) using functional near-infrared spectroscopy (fNIRS). The prediction of functional hemodynamic responses by age in multiple regressions and the impact of single and cumulative vascular burden factors including hypertension, diabetes, obesity, smoking and atherosclerosis were investigated.ResultsReplicating and extending previous findings we could show that increasing age predicted functional hemodynamics to be increased in right prefrontal and bilateral parietal cortex, and decreased in bilateral inferior frontal junction during phonological fluency. Cumulative vascular burden factors, with hypertension in particular, decreased left inferior frontal junction hemodynamic responses during phonological fluency. However, age and vascular burden factors showed no statistical interaction on functional hemodynamics.ConclusionBased on these findings, one might hypothesize that increased fronto-parietal processing may represent age-related compensatory reorganization during increased cognitive demands. Vascular burden factors, such as hypertension, may contribute to regional cerebral hypoperfusion. These neural and vascular hemodynamic determinants should be investigated longitudinally and combined with other markers to advance the prediction of future cognitive decline and dementia.

Motivation by potential gains and losses affects control processes via different mechanisms in the attentional network

Attentional control in demanding cognitive tasks can be improved by manipulating the motivational state. Motivation to obtain gains and motivation to avoid losses both usually result in faster reaction times and stronger activation in relevant brain areas such as the prefrontal cortex, but little is known about differences in the underlying neurocognitive mechanisms of these types of motivation in an attentional control context. In the present functional magnetic resonance imaging (fMRI) study, we tested whether potential gain and loss as motivating incentives lead to overlapping or distinct neural effects in the attentional network, and whether one of these conditions is more effective than the other.A Flanker task with word stimuli as targets and distracters was performed by 115 healthy participants. Using a mixed blocked and event-related design allowed us to investigate transient and sustained motivation-related effects. Participants could either gain money (potential gain) or avoid losing money (potential loss) in different task blocks.Participants showed a congruency effect with increased reaction times for incongruent compared to congruent trials. Potential gain led to generally faster responses compared to the neutral condition and to stronger improvements than potential loss. Potential loss also led to shorter response times compared to the neutral condition, but participants improved mainly during incongruent and not during congruent trials. The event-related fMRI data revealed a main effect of congruency with increased activity in the left inferior frontal gyrus (IFG) and inferior frontal junction area (IFJ), the pre-supplementary motor area (pre-SMA), bilateral insula, intraparietal sulcus (IPS) and visual word form area (VWFA). While potential gain led to increased activity in a cluster of the IFJ and the VWFA only during incongruent trials, potential loss was linked to activity increases in these regions during incongruent and congruent trials. The block analysis revealed greater activity in gain and loss blocks compared to the neutral condition in most of these regions but no differences in the direct comparison of gain and loss blocks.These findings show that potential monetary gain and loss rely on different mechanisms: Gain was more effective in reducing the reaction time compared to potential loss. Brain data indicate that in the gain context attentional control is executed specifically in incongruent trials, whereas the loss context induces an unspecific increase of attentional control. These findings extend previous studies by providing evidence for diverging neural mechanisms for the effects of different types of motivation on attentional control, specifying the underlying activity patterns in task- and stimulus-related regions.

Reduced cortical activation in inferior frontal junction in Unverricht–Lundborg disease (EPM1) – A motor fMRI study

Unverricht-Lundborg disease (EPM1) is characterized by stimulus-sensitive and action-activated myoclonus, tonic-clonic seizures and ataxia. Several disease-related alterations in cortical structure and excitability have been associated with the motor symptoms of EPM1. This study aimed to elucidate possible alterations in cortical activation related to motor performance in EPM1. Fifteen EPM1-patients and 15 healthy volunteers matched for age and sex underwent motor functional MRI. Group differences in activations were evaluated in the primary and supplementary motor cortices and sensory cortical areas. Furthermore, in EPM1 patients, the quantitative fMRI parameters were correlated with the severity of the motor symptoms. The EPM1-patients exhibited decreased activation in the left inferior frontal junction (IFJ) during right hand voluntary motor task when compared with controls. In the quantitative analysis, EPM1-patients had significantly weaker activation than controls in the hand knob and supplementary motor areas (SMA). The volume of activation in M1 decreased with age and duration of disease in the patient group, whereas the volume increased with age in controls. Negative correlations were observed between fMRI parameters of SMA and disease duration or age in patients but not in controls. The weaker motor fMRI activation observed in EPM1 patients parallels previous neurophysiological findings and correlates with the motor symptoms of the disease. Thus, the observed decrease in IFJ activation in EPM1 patients may be associated with the difficulties in initiation or termination of motor execution, a typical clinical symptom in EPM1. The fMRI findings reflect the progressive nature of this disease.

Modulation of executive control in dual tasks with transcranial direct current stimulation (tDCS)

Executive processing in dual tasks is primarily associated with activation of the lateral prefrontal cortex (lPFC), which is demonstrated in functional imaging studies (e.g., Szameitat et al., 2006). However, a causal relation between lPFC activity and executive functions in dual tasks has not been demonstrated so far. Here, we used anodal transcranial direct current stimulation (atDCS [1mA, 20min] vs. sham stimulation [1mA, 30s]) over the left inferior frontal junction under conditions of random and fixed task order in dual tasks as well as in single tasks in healthy young individuals (Experiment 1). We found that atDCS, if administered simultaneously to the task, improved performance in random-order dual tasks, but not in fixed-order dual tasks and single tasks. Moreover, dual-task performance under random-order conditions did not improve if atDCS was applied prior to the task performance. The identical procedure in Experiment 2 showed no difference in dual-task performance under random-task order conditions when we compared cathodal tDCS (ctDCS) with sham stimulation. Our findings suggest that dual-task performance is causally related to lPFC activation under conditions that require task-order decisions and high demands on executive functioning. Subsequent studies may now explore if atDCS leads to sustained improvements parallel to the training of dual tasks.

Transcranial magnetic stimulation dissociates prefrontal and parietal contributions to task preparation.

Cognitive control is thought to rely upon a set of distributed brain regions within frontoparietal cortex, but the functional contributions of these regions remain elusive. Here, we investigated the disruptive effects of transcranial magnetic stimulation (TMS) over the human prefrontal and parietal cortices in task preparation at different abstraction levels. While participants completed a task-switching paradigm that assessed the reconfiguration of task goals and response sets independently, TMS was applied over the left inferior frontal junction (IFJ) and over the left intraparietal sulcus (IPS) during task preparation. In Experiment 1, TMS over the IFJ caused interference with the updating of task goals, while leaving the updating of response sets unaffected. In Experiment 2, TMS over the IPS created the opposite pattern of results, perturbing only the ability to update response sets, but not task goals. Experiment 3 furthermore revealed that TMS over the IPS interfered with task goal updating when the pulses are delivered at a later point in time during preparation. This dissociation of abstract and action-related components not only reveals distinct cognitive control processes during task preparation, but also sheds new light on how prefrontal and parietal areas might work in concert to support flexible and goal-oriented control of behavior.

Effects of multitasking‐training on gray matter structure and resting state neural mechanisms

Multitasking (MT) constitutes engaging in two or more cognitive activities at the same time. MT‐training improves performance on untrained MT tasks and alters the functional activity of the brain during MT. However, the effects of MT‐training on neural mechanisms beyond MT‐related functions are not known. We investigated the effects of 4 weeks of MT‐training on regional gray matter volume (rGMV) and functional connectivity during rest (resting‐FC) in young human adults. MT‐training was associated with increased rGMV in three prefrontal cortical regions (left lateral rostral prefrontal cortex (PFC), dorsolateral PFC (DLPFC), and left inferior frontal junction), the left posterior parietal cortex, and the left temporal and lateral occipital areas as well as decreased resting‐FC between the right DLPFC and an anatomical cluster around the ventral anterior cingulate cortex (ACC). Our findings suggest that participation in MT‐training is as a whole associated with task‐irrelevant plasticity (i.e., neural changes are not limited to certain specific task conditions) in regions and the network that are assumed to play roles in MT as well as diverse higher‐order cognitive functions. We could not dissociate the effects of each task component and the diverse cognitive processes involved in MT because of the nature of the study, and these remain to be investigated. Longitudinal changes in brains of patients with fluent primary progressive aphasia

Primary progressive aphasia (PPA) is a rare clinical dementia syndrome with predominant, progressive language impairment. Clinical symptoms, linguistic impairment and the course of the disease may vary considerably between patients. In order to capture these aspects, longitudinal assessments of neurofunctional changes in PPA including their relationship to behaviour and clinical symptoms are mandatory, ideally at intervals shorter than 1year. Here, we report a longitudinal fMRI study investigating the development of lexical processing and their neural basis in PPA patients over 1year. Four logopenic PPA patients and four matched controls were scanned 3 times (T1, T2, T3, at 6months intervals) while performing a visual lexical decision task on German words and pseudowords. Group differences for the lexicality effect (pseudowords>words) were assessed at time point T1 and its longitudinal changes in the BOLD signal associated with the lexicality effect were analysed. Brain atrophy was assessed with a high-resolution MPRAGE sequence and analysed using deformation based morphometry (DBM). From the very beginning of the study, PPA patients showed reduced left-hemispheric and increased right-hemispheric activations compared to controls. During the progression of the disease, activation increased predominantly in left posterior middle temporal gyrus (pMTG) and inferior frontal junction area, whereas the same regions decreased in activity in control brains. Interestingly, DBM data showed that this increase in activation in PPA patients was accompanied by progressing atrophy in the same regions. At a behavioural level, the accuracy in the lexical decision task was comparably high for both groups during the whole period of examination, despite some large variability between patients. To conclude, the dissociation between (i) maintained high performance, (ii) increased activity in regions involved in lexical access such as pMTG, and (iii) progressive atrophy of the very same regions supports the notion of a compensatory mechanism in brains of PPA patients for maintaining language while brain atrophy is progressing. The activity increase within a left-lateralised fronto-temporal network seems vital for high-level performance, whereas initial right-hemispheric recruitment of homologue language regions, which is reminiscent of that in vascular aphasics, has no continuous impact on lexical performance.

Relationships between brain metabolism decrease in normal aging and changes in structural and functional connectivity

Normal aging is characterized by brain glucose metabolism decline predominantly in the prefrontal cortex. The goal of the present study was to assess whether this change was associated with age-related alteration of white matter (WM) structural integrity and/or functional connectivity. FDG-PET data from 40 young and 57 elderly healthy participants from two research centers (n=49/48 in Center 1/2) were analyzed. WM volume from T1-weighted MRI (Center 1), fractional anisotropy from diffusion-tensor imaging (Center 2), and resting-state fMRI data (Center 1) were also obtained. Group comparisons were performed within each imaging modality. Then, positive correlations were assessed, within the elderly, between metabolism in the most affected region and the other neuroimaging modalities. Metabolism decline in the elderly predominated in the left inferior frontal junction (LIFJ). LIFJ hypometabolism was significantly associated with macrostructural and microstructural WM disturbances in long association fronto-temporo-occipital fibers, while no relationship was found with functional connectivity. The findings offer new perspectives to understand normal aging processes and open avenues for future studies to explore causality between age-related metabolism and connectivity changes.

Common and distinct neural mechanisms of attentional switching and response conflict

The human capacities for overcoming prepotent actions and flexibly switching between tasks represent cornerstones of cognitive control. Functional neuroimaging has implicated a diverse set of brain regions contributing to each of these cognitive control processes. However, the extent to which attentional switching and response conflict draw on shared or distinct neural mechanisms remains unclear. The current study examined the neural correlates of response conflict and attentional switching using event-related functional magnetic resonance imaging (fMRI) and a fully randomized 2×2 design. We manipulated an arrow-word version of the Stroop task to measure conflict and switching in the context of a single task decision, in response to a common set of stimuli. Under these common conditions, both behavioral and imaging data showed significant main effects of conflict and switching but no interaction. However, conjunction analyses identified frontal regions involved in both switching and response conflict, including the dorsal anterior cingulate cortex (dACC) and left inferior frontal junction. In addition, connectivity analyses demonstrated task-dependent functional connectivity patterns between dACC and inferior temporal cortex for attentional switching and between dACC and posterior parietal cortex for response conflict. These results suggest that the brain makes use of shared frontal regions, but can dynamically modulate the connectivity patterns of some of those regions, to deal with attentional switching and response conflict.

Executive deficits are related to the inferior frontal junction in early dementia.

Executive functions describe a wide variety of higher order cognitive processes that allow the flexible modification of thought and behaviour in response to changing cognitive or environmental contexts. Their impairment is common in neurodegenerative disorders. Executive deficits negatively affect everyday activities and hamper the ability to cope with other deficits, such as memory impairment in Alzheimer's disease or behavioural disorders in frontotemporal lobar degeneration. Our study aimed to characterize the neural correlates of executive functions by relating respective deficits to regional hypometabolism in early dementia. Executive functions were assessed with two classical tests, the Stroop and semantic fluency test and various subtests of the behavioural assessment of the dysexecutive syndrome test battery capturing essential aspects of executive abilities relevant to daily living. Impairments in executive functions were correlated with reductions in brain glucose utilization as measured by [18F]fluorodeoxyglucose positron emission tomography and analysed voxelwise using statistical parametric mapping in 54 subjects with early dementia, mainly Alzheimer's disease and frontotemporal lobar degeneration, and its prodromal stages: subjective and mild cognitive impairment. Although the analysis revealed task-specific frontoparietal networks, it consistently showed that hypometabolism in one region in the left lateral prefrontal cortex—the inferior frontal junction area—was related to performance in the various neuropsychological tests. This brain region has recently been related to the three component processes of cognitive control—working memory, task switching and inhibitory control. Group comparisons additionally showed hypometabolism in this area in Alzheimer's disease and frontotemporal lobar degeneration. Our study underlines the importance of the inferior frontal junction area for cognitive control in general and for executive deficits in early dementia.

Anti-Saccade Performance Predicts Executive Function and Brain Structure in Normal Elders

To assess the neuropsychological and anatomical correlates of anti-saccade (AS) task performance in normal elders. The AS task correlates with neuropsychological measures of executive function and frontal lobe volume in neurological diseases, but has not been studied in a well-characterized normal elderly population. Because executive dysfunction can indicate an increased risk for cognitive decline in cognitively normal elders, we hypothesized that AS performance might be a sensitive test of age-related processes that impair cognition. The percentage of correct AS responses was evaluated in 48 normal elderly subjects and associated with neuropsychological test performance using linear regression analysis and gray matter volume measured on magnetic resonance imaging scans using voxel-based morphometry. The percentage of correct AS responses was associated with measures of executive function, including modified trails, design fluency, Stroop inhibition, abstraction, and backward digit span, and correlated with gray matter volume in 2 brain regions involved in inhibitory control: the left inferior frontal junction and the right supplementary eye field. The association of AS correct responses with neuropsychological measures of executive function was strongest in individuals with fewer years of education. The AS task is sensitive to executive dysfunction and frontal lobe structural alterations in normal elders.

Strategy-effects in prefrontal cortex during learning of higher-order S–R rules

All of us regularly face situations that require the integration of the available information at hand with the established rules that guide behavior in order to generate the most appropriate action. But where individuals differ from one another is most certainly in terms of the different strategies that are adopted during this process. A previous study revealed differential brain activation patterns for the implementation of well established higher-order stimulus-response (S-R) rules depending on inter-individual strategy differences (Wolfensteller and von Cramon, 2010). This raises the question of how these strategies evolve or which neurocognitive mechanisms underlie these inter-individual strategy differences. Using functional magnetic resonance imaging (fMRI), the present study revealed striking strategy-effects across regions of the lateral prefrontal cortex during the implementation of higher-order S-R rules at an early stage of learning. The left rostrolateral prefrontal cortex displayed a quantitative strategy-effect, such that activation during rule integration based on a mismatch was related to the degree to which participants continued to rely on rule integration. A quantitative strategy ceiling effect was observed for the left inferior frontal junction area. Conversely, the right inferior frontal gyrus displayed a qualitative strategy-effect such that participants who at a later point relied on an item-based strategy showed stronger activations in this region compared to those who continued with the rule integration strategy. Together, the present findings suggest that a certain amount of rule integration is mandatory when participants start to learn higher-order rules. The more efficient item-based strategy that evolves later appears to initially require the recruitment of additional cognitive resources in order to shield the currently relevant S-R association from interfering information.

The implementation of verbal instructions: An fMRI study

In everyday life, our actions are often guided by verbal instructions. Usually, we can implement such instructions immediately without trial and error learning. This raises the fundamental question how verbal instructions are transformed into efficient motor behavior. The aim of this study was to gain deeper insights into the implementation of verbal instructions both on a neural and a cognitive level. To this end, we devised an fMRI experiment in which participants were required to permanently implement new stimulus-response (S-R) mappings and object-color (O-C) mappings. This enabled us to test whether there are brain areas that are specific to the implementation of newly instructed S-R mappings or whether newly instructed rules are represented independently from the specific content. Furthermore, we could test which brain areas are involved in the processing of S-R mappings when compared with O-C mappings. Our results suggest that only one brain area, the left inferior frontal junction (IFJ), was sensitive to the novelty of instructions regardless of whether these instructions conveyed S-R or O-C mappings. Furthermore, our results show that instructions conveying S-R mapping involve a network of brain areas, including pre-PMd, M1, and IPS that was not sensitive to the novelty of the instructions. Therefore, we conclude that the implementation of verbal instructions results from an interplay of a brain areas that represent novel rulelike information in domain general terms and brain areas that are specific to S-R rules.

Differential Neural Activation for Updating Rule versus Stimulus Information in Working Memory

Establishing what information is actively maintained in working memory (WM) and how it is represented and controlled is essential to understanding how such information guides future behavior. WM has traditionally been investigated in terms of the maintenance of stimulus-specific information, such as locations or words. More recently, investigators have emphasized the importance of rules that establish relationships between those stimuli and the pending response. The current study used a mental arithmetic task with fMRI to test whether updating of numbers (i.e., stimuli) and updating of mathematical operations (i.e., rules) in WM relies on the same neural system. Results indicate that, while a common network is activated by both types of updating, rule updating preferentially activates prefrontal cortex while number updating preferentially activates parietal cortex. The results suggest that both numbers and rules are maintained in WM but that they are different types of information that are controlled independently.

USING THE STROOP TASK TO INVESTIGATE THE NEURAL CORRELATES OF SYMPTOM CHANGE IN SCHIZOPHRENIA

This study examined brain activation during a cognitive inhibition task in patients with schizophrenia following changes in their positive symptoms. A Stroop task was used during functional magnetic resonance imaging in 11 patients with schizophrenia (patient group) and 9 healthy volunteers (control group). At baseline, the patient group showed significantly attenuated activation within the anterior cingulate gyrus, left pre-/postcentral gyrus and inferior frontal junction. At follow-up, there was a significant increase in activation in the left inferior frontal junction associated with a decrease in positive symptoms, suggesting this region plays a role in the development of these symptoms.

Neural system for updating object working memory from different sources: Sensory stimuli or long-term memory

Working memory (WM) is the active maintenance of currently relevant information so that it is available for use. A crucial component of WM is the ability to update the contents when new information becomes more relevant than previously maintained information. New information can come from different sources, including from sensory stimuli (SS) or from long-term memory (LTM). Updating WM may involve a single neural system regardless of source, distinct systems for each source, or a common network with additional regions involved specifically in sensory or LTM processes. The current series of experiments indicates that a single fronto-parietal network (including supplementary motor area, parietal, left inferior frontal junction, middle frontal gyrus) is active in updating WM regardless of the source of information. Bilateral cuneus was more active during updating WM from LTM than updating from SS, but the activity in this region was attributable to recalling information from LTM regardless of whether that information was to be entered into WM for future use or not. No regions were found to be more active during updating from SS than updating from LTM. Functional connectivity analysis revealed that different regions within this common update network were differentially more correlated with visual processing regions when participants updated from SS, and more correlated with LTM processing regions when participants updated from the contents of LTM. These results suggest that a single neural mechanism is responsible for controlling the contents of WM regardless of whether that information originates from a sensory stimulus or from LTM. This network of regions involved in updating WM interacts with the rest of the brain differently depending on the source of newly relevant information.

Who Comes First? The Role of the Prefrontal and Parietal Cortex in Cognitive Control

Cognitive control processes enable us to adjust our behavior to changing environmental demands. Although neuropsychological studies suggest that the critical cortical region for cognitive control is the prefrontal cortex, neuro-imaging studies have emphasized the interplay of prefrontal and parietal cortices. This raises the fundamental question about the different contributions of prefrontal and parietal areas in cognitive control. It was assumed that the prefrontal cortex biases processing in posterior brain regions. This assumption leads to the hypothesis that neural activity in the prefrontal cortex should precede parietal activity in cognitive control. The present study tested this assumption by combining results from functional magnetic resonance imaging (fMRI) providing high spatial resolution and event-related potentials (ERPs) to gain high temporal resolution. We collected ERP data using a modified task-switching paradigm. In this paradigm, a situation where the same task was indicated by two different cues was compared with a situation where two cues indicated different tasks. Only the latter condition required updating of the task set. Task-set updating was associated with a midline negative ERP deflection peaking around 470 msec. We placed dipoles in regions activated in a previous fMRI study that used the same paradigm (left inferior frontal junction, right inferior frontal gyrus, right parietal cortex) and fitted their directions and magnitudes to the ERP effect. The frontal dipoles contributed to the ERP effect earlier than the parietal dipole, providing support for the view that the prefrontal cortex is involved in updating of general task representations and biases relevant stimulus-response associations in the parietal cortex.

Regional profiles of cortical gray matter volume deficits in schizophrenia and alcoholism

The antisaccade task is a prominent tool to investigate the response inhibition component of cognitive control. Recent theoretical accounts explain performance in terms of parallel programming of exogenous and endogenous saccades, linked to the horse race metaphor. Previous studies have tested the hypothesis of competing saccade signals at the behavioral level by selectively slowing the programming of endogenous or exogenous processes e.g. by manipulating the probability of antisaccades in an experimental block. To gain a better understanding of inhibitory control processes in parallel saccade programming, we analyzed task-related eye movements and blood oxygenation level dependent (BOLD) responses obtained using functional magnetic resonance imaging (fMRI) at 3T from 16 healthy participants in a mixed antisaccade and prosaccade task. The frequency of antisaccade trials was manipulated across blocks of high (75%) and low (25%) antisaccade frequency. In blocks with high antisaccade frequency, antisaccade latencies were shorter and error rates lower whilst prosaccade latencies were longer and error rates were higher. At the level of BOLD, activations in the task-related saccade network (left inferior parietal lobe, right inferior parietal sulcus, left precentral gyrus reaching into left middle frontal gyrus and inferior frontal junction) and deactivations in components of the default mode network (bilateral temporal cortex, ventromedial prefrontal cortex) compensated increased cognitive control demands. These findings illustrate context dependent mechanisms underlying the coordination of competing decision signals in volitional gaze control.
