Diagnosis and management of Auricular Lobe Keloid: A case report

Background: Keloid is an abnormal wound-healing process characterized by the formation of benign fibroproliferative tissue following trauma or injury. The auricular area is the most common area affected by keloid. Despite various treatments available, auricular keloid remains recalcitrant. Objective: To report a successful multimodal treatment of ear lobe keloid. Case Report: A 21-year-old male patient presented with bilateral keloids in the right and left earlobe areas sized 2 x 3 cm each. The keloid mass has been slowly growing for the last five months. He had a history of ear piercing in both ear lobes two years ago. The patient underwent bilateral keloid excision, followed by serial intralesional triamcinolone injections on days 3, 7, and 14 after surgery. The histopathological examination of resected tissue was also consistent with keloid tissue. Clinical question: What is the optimal management of auricular lobe keloid? Methods: We performed evidence-based literature reviews regarding optimal management of auricular lobe keloid. Result: Combination therapy is the most widely chosen treatment approach for auricular lobe keloids. This approach provides more aesthetic outcomes and lower recurrence rates than monotherapy. Conclusion: Surgical excision followed by serial intralesional triamcinolone injection is effective for managing auricular lobe keloid. Nevertheless, patients should be advised to avoid body piercing and elective cosmetic procedures to prevent the formation of keloids in the future.

Proprioceptive acuity for locating and controlling movements of a hand-held tool

We investigated proprioceptive acuity for location and motion of a never seen hand-held tool (30 cm long rod) and the accuracy of movements to place tool parts in the location of remembered visual targets. Ten blindfolded right-handed subjects (5 females) reached with the tool held in the right hand to touch the tip and midpoint to the stationary and moving left index-tip, to the right and left ear lobes and to remembered visual target locations. We also tested accuracy of left hand rod reaches to the ear lobes to determine if rod dimensions and control of tool movements experienced during right hand tool use could be used to accurately localize the rod parts when held in the left hand. Errors for right hand-held rod-tip movements to touch the stationary and moving left index-tip averaged only about 1 cm larger than observed previously for right hand movements to touch its index-tip to the left index-tip. The tool-tip was localized with lower mean distance errors (about 1 cm) than the tool-midpoint (5.5–6.5 cm) when reaching to touch the ear lobes with the rod in right and left hands. Right hand reaches to place the tool- tip and midpoint in remembered visual target locations were inaccurate with large overshoots of close targets and undershoots of far targets, similar to previous reports for reaching with the right hand to remembered visual targets. These results support the distalization hypothesis, that tool the tool endpoint becomes the effective upper limb endpoint when using the tool.

The effects of transcutaneous auricular vagus nerve stimulation (taVNS) on cholinergic neural networks in humans: A neurophysiological study

ObjectiveThe mechanisms of actions of transcutaneous auricular vagus nerve stimulation (taVNS) are still unclear, however the activity of the cholinergic system seems to be critical for the induction of VNS-mediated plasticity. Transcranial Magnetic Stimulation (TMS) is a well-suited, non-invasive tool to investigate cortical microcircuits involving different neurotransmitters. Herein, we evaluated the effect of taVNS on short-latency afferent inhibition (SAI), a TMS paradigm specifically measuring cholinergic neurotransmission. MethodsFifteen healthy subjects participated in this randomized placebo-controlled double-blind study. Each subject underwent two different sessions of 1-hour exposure to taVNS (real and sham) separated by a minimum of 48 h. Real taVNS was administered at left external acoustic meatus, while sham stimulation was performed at left ear lobe. We evaluated SAI bilaterally over the motor cortex before and after exposure to taVNS. ResultsNo side effects were reported by any of the participants.Statistical analysis did not show any significant effect of taVNS on SAI. ConclusionsOur study demonstrated that cholinergic circuits explored by SAI are different from circuits engaged by taVNS. SignificanceSince the influence of VNS on cholinergic neurotransmission has been exhaustively demonstrated in animal models, further studies are mandatory to understand the actual impact of VNS on cholinergic circuits in humans.

An extremely rare case of atypical fibroxanthoma – case report

The first clinical case of atypical fibroxanthoma was documented in 1958, and to date, approximately 3,000 patients have been described in the literature with this diagnosis. Furthermore, the recurrence rate or potential for metastasis in atypical fibroxanthoma is estimated to be as low as 2%. It is noteworthy that fibroxanthoma most commonly arises in the head and neck region. Therefore, the clinical case presented here of a patient diagnosed with atypical fibroxanthoma on the left earlobe margin, with suspected disease recurrence after approximately one year, is extremely rare, so we believed it is noticeable to report.

Passive leg raising modulates low-frequency oscillation propagation in peripheral atherosclerosis: A pilot study.

Low-frequency oscillations (LFOs) observed in the periphery may reflect physiological processes. The aim of this study was to investigate these processes' effects on LFOs and the differences between healthy subjects and those with peripheral arteriosclerosis disease (PAD). 14 PAD patients and 25 healthy controls were studied in resting (RS) and passive leg raising (PLR) states. We simultaneously measured LFOs at the peripheral left earlobes (LE), right earlobes (RE), left fingertips (LF), right fingertips (RF), left toes (LT), and right toes (RT), along with coherence and phase shift analysis processing. The coherence coefficients in the PAD group were lower than those in the healthy group (p < .01), and the phase shifts in the PAD group were higher than those in the healthy group (p < .01) in a resting state. Mild to moderate PAD patients had greater coherence coefficients and smaller phase shifts than severe PAD patients. 0.05Hz PLR LFOs originating in the LT can be observed in other peripheral positions. The proportion of occurrence times for 0.05Hz PLR LFOs peaks observed at different peripheral positions was different in healthy subjects, patients with bilateral multiple lower limb arteriosclerosis, and those with left or right lower limb arteriosclerosis. The coherence coefficient and phase shift characteristics of LFOs were different between healthy subjects and PAD patients. LFOs have the potential to provide valuable physiological process information associated with atherosclerosis in the periphery.

Effects of transcutaneous auricular vagus nerve stimulation at left cymba concha on experimental pain as assessed with the nociceptive withdrawal reflex, and correlation with parasympathetic activity.

The aim of this study was to clarify the effects of transcutaneous auricular vagus nerve stimulation (taVNS) to the left cymba concha on the pain perception using nociceptive withdrawal reflex (NWR), which is known to be associated with chronic pain, and to investigate whether there is a relationship between taVNS-induced suppression of the NWR and parasympathetic activation. We applied either 3.0mA, 100 Hz taVNS for 120 s on the left cymba concha (taVNS condition) or the left earlobe (Sham condition) for 20 healthy adults. NWR threshold was measured before (Baseline), immediately after (Post 0), 10min (Post 10) and 30 min after (Post 30) stimulation. The NWR threshold was obtained from biceps femoris muscle by applying electrical stimulation to the sural nerve. During taVNS, electrocardiogram was recorded, and changes in autonomic nervous activity measured by heart rate variability (HRV) were analyzed. We found that the NWR thresholds at Post 10 and Post 30 increased compared with baseline in the taVNS group (10min after: p = .008, 30 min after: p = .008). In addition, increased parasympathetic activity by taVNS correlated with a greater increase in NWR threshold at Post 10 and Post 30 (Post 10: p = .003; Post 30: p = .001). The present results of this single-blinded study demonstrate the pain-suppressing effect of taVNS on NWR threshold and suggest that the degree of parasympathetic activation during taVNS may predict the pain-suppressing effect of taVNS after its application.

Differences in symmetrical low-frequency oscillations among healthy subjects, and those with stroke or peripheral arterial disease

ObjectiveLow-frequency oscillations (LFOs) observed in near-infrared spectroscopy (NIRS) reflect autonomic physiological processes, and may serve as useful indicators for detecting and monitoring circulatory dysfunction. The aim of this study was to reveal whether LFOs can be used as vascular perfusion biomarkers to differentiate different types and degrees of vascular lesions based on clinical patient data. Materials and Methods: In this study, healthy controls, ischemic stroke patients and peripheral atherosclerosis patients completed a resting-state LFO detection experiment. LFOs were collected simultaneously at peripheral right and left earlobes, fingertips and toes, along with coherence and phase shift analyses processing. Results: The results showed that the coherence coefficients of symmetric peripheral positions and the absolute value-phase shifts of fingers and toes can be used to distinguish healthy individuals, ischemic stroke patients and peripheral atherosclerosis patients. The symmetric earlobes’ absolute value-phase shifts could be used to differentiate mild and severe ischemic stroke patients; the coherence coefficients and absolute value-phase shifts of the symmetric toes could be used to differentiate mild and severe peripheral arteriosclerosis patients. The accuracy of differentiating between types of patients was 70%; those with different degrees of peripheral atherosclerosis was 85%, and those with different degrees of ischemic stroke was 72%. Conclusions: LFOs can serve as vascular perfusion biomarkers to differentiate types and degrees of vascular lesions. Therefore, LFOs have the potential to provide valuable patient information to assist researchers and clinicians in identifying specific peripheral circulatory damage subgroups.

The Effects of Transcutaneous Vagus Nerve Stimulation on Functional Connectivity Within Semantic and Hippocampal Networks in Mild Cognitive Impairment.

Better treatments are needed to improve cognition and brain health in people with mild cognitive impairment (MCI) and Alzheimer's disease (AD). Transcutaneous vagus nerve stimulation (tVNS) may impact brain networks relevant to AD through multiple mechanisms including, but not limited to, projection to the locus coeruleus, the brain's primary source of norepinephrine, and reduction in inflammation. Neuropathological data suggest that the locus coeruleus may be an early site of tau pathology in AD. Thus, tVNS may modify the activity of networks that are impaired and progressively deteriorate in patients with MCI and AD. Fifty patients with MCI (28 women) confirmed via diagnostic consensus conference prior to MRI (sources of info: Montreal Cognitive Assessment Test (MOCA), Clinical Dementia Rating scale (CDR), Functional Activities Questionnaire (FAQ), Hopkins Verbal Learning Test - Revised (HVLT-R) and medical record review) underwent resting state functional magnetic resonance imaging (fMRI) on a Siemens 3T scanner during tVNS (left tragus, n = 25) or sham control conditions (left ear lobe, n = 25). During unilateral left tVNS, compared with ear lobe stimulation, patients with MCI showed alterations in functional connectivity between regions of the brain that are important in semantic and salience functions including regions of the temporal and parietal lobes. Furthermore, connectivity from hippocampi to several cortical and subcortical clusters of ROIs also demonstrated change with tVNS compared with ear lobe stimulation. In conclusion, tVNS modified the activity of brain networks in which disruption correlates with deterioration in AD. These findings suggest afferent target engagement of tVNS, which carries implications for the development of noninvasive therapeutic intervention in the MCI population.

Meditative-based diaphragmatic breathing vs. vagus nerve stimulation in the treatment of fibromyalgia—A randomized controlled trial: Body vs. machine

Vagus nerve innervation via electrical stimulation and meditative-based diaphragmatic breathing may be promising treatment avenues for fibromyalgia. Explore and compare the treatment effectiveness of active and sham transcutaneous vagus nerve stimulation (tVNS) and meditative-based diaphragmatic breathing (MDB) for fibromyalgia. Participants enrolled from March 2019-October 2020 and randomly assigned to active tVNS (n = 28), sham tVNS (n = 29), active MDB (n = 29), or sham MDB (n = 30). Treatments were self-delivered at home for 15 min/morning and 15 min/evening for 14 days. Follow-up was at 2 weeks. Outpatient pain clinic in Oslo, Norway. 116 adults aged 18-65 years with severe fibromyalgia were consecutively enrolled and randomized. 86 participants (74%) had an 80% treatment adherence and 107 (92%) completed the study at 2 weeks; 1 participant dropped out due to adverse effects from active tVNS. Active tVNS is placed on the cymba conchae of the left ear; sham tVNS is placed on the left earlobe. Active MDB trains users in nondirective meditation with deep breathing; sham MDB trains users in open-awareness meditation with paced breathing. Primary outcome was change from baseline in ultra short-term photoplethysmography-measured cardiac-vagal heart rate variability at 2 weeks. Prior to trial launch, we hypothesized that (1) those randomized to active MDB or active tVNS would display greater increases in heart rate variability compared to those randomized to sham MDB or sham tVNS after 2-weeks; (2) a change in heart rate variability would be correlated with a change in self-reported average pain intensity; and (3) active treatments would outperform sham treatments on all pain-related secondary outcome measures. No significant across-group changes in heart rate variability were found. Furthermore, no significant correlations were found between changes in heart rate variability and average pain intensity during treatment. Significant across group differences were found for overall FM severity yet were not found for average pain intensity. These findings suggest that changes in cardiac-vagal heart rate variability when recorded with ultra short-term photoplethysmography in those with fibromyalgia may not be associated with treatment-specific changes in pain intensity. Further research should be conducted to evaluate potential changes in long-term cardiac-vagal heart rate variability in response to noninvasive vagus nerve innervation in those with fibromyalgia. https://clinicaltrials.gov/ct2/show/NCT03180554, Identifier: NCT03180554.

A two-week course of transcutaneous vagal nerve stimulation improves global sleep: Findings from a randomised trial in community-dwelling adults.

Short sleep duration and poor sleep quality are common in the general population. This study tested if a 2-week course of daily transcutaneous vagal nerve stimulation (tVNS) improves sleep in community-dwelling adults. Participants were n=68 men and women aged 18-75years randomised into four groups: early and sham tVNS and late and sham tVNS. Early groups underwent daily 4h stimulation between Day 0 and 13, while late groups underwent daily 4h stimulation between Day 14 and 28. tVNS was performed with transcutaneous electrical nerve stimulation (TENS) on the left tragus, and sham tVNS (control conditions) was applied on the left earlobe. Sleep was measured with the Pittsburgh Sleep Quality Index. Analysis of prespecified contrasts (C), based on linear mixed modelling, revealed that for tVNS there were significant improvements in global sleep scores over time between Day 0 and Day 13 in the early stimulation phase (C=-1.90; 95% CI=-2.87 to -0.94), and between Day 14 and Day 28 in the late phase (C=-0.87; 95% CI=-1.41 to -0.32). No such differences were found under sham tVNS (applied early or late). However, global sleep scores showed no significant improvement under tVNS when compared against control groups during both the early (χ2=0.83, p=0.36), or late stimulation phase (χ2=0.24, p=0.63). We showed that two weeks of tVNS improves global sleep scores, but the change in sleep was not significantly different to control groups. Further studies are warranted to test the utility of tVNS in alleviating sleep complains in community-dwelling adults.

Clinical Improvement in Patients with Morbus Hansen Borderline Tuberculoid Lesions with 3-Month Multibacillary Regimen Therapy

Background/Aim: Morbus Hansen (MH) is a chronic infectious disease caused by Mycobacterium leprae. The disease has an average incubation period of 3-5 years. Morbus Hansen (MH) has various clinical, microbiological and histopathological features. In the following, a new case of MH is reported with a clinical manifestation of Borderline Tuberculoid (BT) type MH. The patient was treated with the MDT MB regimen for 3 months and had clinical improvement. Case Report: A 23-year-old boy with complaints of sudden feeling of heat, tingling and cramping from the tip of his right foot to the sole of his right foot. He also complaining about black lesions on both hands, right elbow, face, chest and back. The diagnosis of gout, fungus and dermatitis can be ruled out after microscopic examination of AFB on the right, left ear lobe, and foot lesions showed negative results and histopathological examination results showed Morbus Hansen type BT. The risk factor for contracting Morbus Hansen is the patient has lived in a dormitory for 2.5 years and it is known that there are people who suffer from a similar disease in that environment. The patient was treated using a multibacillary regimen for 3 months and the lesions improved and the numbness decreased. Treatment plan during the case for 12-18 months. This case is unique due to the clinical manifestation has not significant characteristic for MH, AFB is not found from ear lobe smear. MH established after skin biopsy done. Conclusion: It was reported that one MH case with TB for 3 months had clinical improvement, with plans to continue treatment for 12-18 months. No evidence for a modulating effect of continuous transcutaneous auricular vagus nerve stimulation on markers of noradrenergic activity.

Although transcutaneous auricular vagus nerve stimulation (taVNS) is thought to increase central noradrenergic activity, findings supporting such mechanism are scarce and inconsistent. This study aimed to investigate whether taVNS modulates indirect markers of phasic and tonic noradrenergic activity. Sixty-six healthy participants performed a novelty auditory oddball task twice on separate days: once while receiving taVNS (left cymba concha), once during sham (left earlobe) stimulation. To maximize potential effects, the stimulation was delivered continuously (frequency: 25Hz; width: 250μs) at an intensity individually calibrated to the maximal level below pain threshold. The stimulation was administered 10min before the oddball task and maintained throughout the session. Event-related pupil dilation (ERPD) to target stimuli and pre-stimulus baseline pupil size were assessed during the oddball task as markers of phasic and tonic noradrenergic activity, respectively. Prior to and at the end of stimulation, tonic pupil size at rest, cortisol, and salivary alpha-amylase were assessed as markers of tonic noradrenergic activity. Finally, we explored the effect of taVNS on cardiac vagal activity, respiratory rate, and salivary flow rate. Results showed a greater ERPD to both target and novelty compared to standard stimuli in the oddball task. In contrast to our hypotheses, taVNS did not impact any of the tested markers. Our findings strongly suggest that continuous stimulation of the cymba concha with the tested stimulation parameters is ineffective to increase noradrenergic activity via a vagal pathway.

Pilomatricoma of Pinna- A Maze that may Misdirects

Pilomatricoma is a rare cutaneous epidermal appendageal tumour having differentiation in the direction of hair cortex cells. It usually presents as single, less than 3 cm size, subcutaneous lesion, commonly seen in children, more commonly in head, neck and upper extremities. It can mimic a variety of other lesions clinically as well as cytologically, thereby posing a diagnostic challenge. The ear lobe is a very rare site for it to occur. The authors report a case of a 61-year-old female presented with history of non tender nodule on left ear lobe for six months. The lesion was clinically suspected as keloid, sebaceous cyst and chondroma, later diagnosed as pilomatricoma on fine needle aspiration cytology, confirmed further on histopathological examination. Sebaceous cyst has been ruled out due to the presence of characteristic findings in both cytology and histopathology. However, due to the inherent disadvantage of cytology, predominance of one component over the other would have mislead the diagnosis. It is being reported for its rare location, unusual age of presentation and to report its mimics on cytology.

Validation of a new device for photoplethysmographic measurement of multi-site arterial pulse wave velocity

Pulse wave velocity (PWV) is commonly used for assessing arterial stiffness and it is a useful and accurate cardiovascular mortality predictor. Currently, many techniques and devices for PWV measurement are known, but they are usually expensive and require operator experience. One possible solution for PWV measurement is photoplethysmography (PPG), which is convenient, inexpensive and provides continuous PWV results. The aim of this paper is validation of a new device for PPG sensor-based measurement of multi-site arterial PWV using a SphygmoCor XCEL (as the reference device) according to the recommendations of the Artery Society Guidelines (ASG). In this study, 108 subjects (56 men and 52 women, 20–91 years in 3 required age groups) were enrolled. The multi-site PWV was simultaneous measured by 7 PPG sensors commonly used in pulse oximetry in clinical settings. These sensors were placed on the forehead, and right and left earlobes, fingers and toes. Pulse transit time (PTT) was measured offline as the difference of time delay between two onsets of the pulse wave determined by the intersecting tangent method. The PWV was calculated by dividing the distance between PPG sensors by PTT. During PPG signals measurement, reference carotid to femoral PWV (cfPWV) was performed with a SphygmoCor XCEL system. The Pearson correlation coefficient (r) between the obtained PWV results was calculated. The Bland-Altman method was used to establish the level of agreement between the two devices. Mean difference (md) and standard deviation (SD) were also calculated. The multi-site PWV was highly correlated with accuracy at the ASG-defined level of “Acceptable” (md < 1.0 m/s and SD ≤ 1.5 m/s) with cfPWV: forehead - right toe (r = 0.75, md = 0.20, SD = 0.97), forehead - left toe (r = 0.79, md = 0.18, SD = 0.91), right ear - right toe (r = 0.79, md = 0.11, SD = 0.96), left ear - left toe (r = 0.75, md = 0.43, SD = 0.99), right ear - left toe (r = 0.78, md = 0.40, SD = 0.93), left ear - right toe (r = 0.78, md = 0.11, SD = 0.96), right finger - right toe (r = 0.66, md = 0.95, SD = 1.29), left finger - left toe (r = 0.67, md = 0.68, SD = 1.35). This study showed that PWV measured with the multi-site PPG system, in relation to the obtained numerical values, correlated very well with that measured using the commonly known applanation tonometry method. However, it should be noted, that the measured PWV concerns the central and muscular part of the arterial tree while the cfPWV is only for the central one. The best results were obtained when the proximal PPG sensor was placed on the head (ear or forehead) and the distal PPG sensor on the toe. PPG sensors can be placed in many sites at the same time, which provides greater freedom of their configuration. Multi-site photoplethysmography is an alternative method for PWV measurement and creates new possibilities for the diagnostics of cardiovascular diseases.

Technical Note: Modulation of fMRI brainstem responses by transcutaneous vagus nerve stimulation

Our increasing knowledge about gut-brain interaction is revolutionising the understanding of the links between digestion, mood, health, and even decision making in our everyday lives. In support of this interaction, the vagus nerve is a crucial pathway transmitting diverse gut-derived signals to the brain to monitor of metabolic status, digestive processes, or immune control to adapt behavioural and autonomic responses. Hence, neuromodulation methods targeting the vagus nerve are currently explored as a treatment option in a number of clinical disorders, including diabetes, chronic pain, and depression. The non-invasive variant of vagus nerve stimulation (VNS), transcutaneous auricular VNS (taVNS), has been implicated in both acute and long-lasting effects by modulating afferent vagus nerve target areas in the brain. The physiology of neither of those effects is, however, well understood, and evidence for neuronal response upon taVNS in vagal afferent projection regions in the brainstem and its downstream targets remain to be established.Therefore, to examine time-dependent effects of taVNS on brainstem neuronal responses in healthy human subjects, we applied taVNS during task-free fMRI in a single-blinded crossover design. During fMRI data acquisition, we either stimulated the left earlobe (sham), or the target zone of the auricular branch of the vagus nerve in the outer ear (cymba conchae, verum) for several minutes, both followed by a short ‘stimulation OFF’ period. Time-dependent effects were assessed by averaging the BOLD response for consecutive 1-minute periods in an ROI-based analysis of the brainstem.We found a significant response to acute taVNS stimulation, relative to the control condition, in downstream targets of vagal afferents, including the nucleus of the solitary tract, the substantia nigra, and the subthalamic nucleus. Most of these brainstem regions remarkably showed increased activity in response to taVNS, and these effect sustained during the post-stimulation period. These data demonstrate that taVNS activates key brainstem regions, and highlight the potential of this approach to modulate vagal afferent signalling. Furthermore, we show that carry-over effects need to be considered when interpreting fMRI data in the context of general vagal neurophysiology and its modulation by taVNS.

Ear Acupuncture Points in Neonates From Drug-Dependent Mothers: A Prospective Study.

Aim: The aim of the study was to determine the presence or absence of ear acupuncture points (EAP) in newborn children with or without neonatal abstinence syndrome (NAS) and to confirm the hypothesis that neonates with NAS have more EAP than healthy neonates.Methods: We conducted a prospective case control study with ethical consent at the University Children's Hospital, Division of Neonatology Bern and the Department of Gynecology and Obstetrics Inselspital Bern in Switzerland. We determined the EAP in n = 26 newborn children born to drug-dependent mothers compared with n = 50 healthy newborns. For the detection of EAP, we used an ear point detection pen. EAP are present only if weakness exists in the corresponding area.Results: Twenty-six neonates who were born to drug-dependent mothers and developed NAS were screened on the 5th day after delivery (range 1–22). The median Finnegan Score was 12 points (range 6–18) on the day of examination. Twenty-four active EAP were detected on the left earlobe and 25 were detected on the right earlobe. There was no significant difference between the right and left lobes (p = 0.9285, two tailed test) and the number of acupuncture points. The correlation between the Finnegan Score and the number of EAP was highly significant (p = 0.0001). The most common active points were the psycho-vegetative rim of the reflex zone of sympathicus and parasympathicus. Organic points were also commonly detected. The urinary bladder, kidney and hip points were detected with a frequency of 12–15%. The shen men pain point was found in three neonates, and the point of desire as a psychological point, was also detected. The correlation between sex and active EAP was highly significant (p = 0.0093, Mann-Whitney test for the left earlobe and p = 0.0025 for the right earlobe). Boys had a significantly higher number of EAP than girls. All NADA points were detected in the neonates born to drug-dependent mothers, and the most frequent point was the vegetative point. Healthy neonates showed only the vegetative point in the vegetative rim 1/3 among the NADA points. A comparison of newborns born to drug-dependent mothers and 50 healthy neonates showed that the former group had statistically significantly more active points. For the left earlobe, the difference between neonates born to drug-dependent mothers and controls was statistically significant (p = 0.0008, Mann-Whitney test). Highly similar results were found for the right earlobe (p = 0.0001, Mann-Whitney test).Discussion: Our current work confirms that neonates born to drug-dependent mothers with high Finnegan scores and NAS have more EAP than healthy neonates. The vegetative rim is the most common point as shown in our previous studies. Our observations showed that twins had similar but not identical points; each individual had unique points depending on health status. Newborn boys with NAS had a higher number of EAP than newborn girls in the neonatal intensive care unit. This findings may be attributed to the reserve of newborns with NAS. Newborn girls are considered more robust than boys in the neonatal care setting. EAP in neonates might potentially be used for diagnosis and therapeutic opinions in neonates in the future.

The effects of transcutaneous auricular vagal nerve stimulation on pupil size

ObjectiveMechanisms of action and optimal stimulation parameters of transcutaneous auricular vagus nerve stimulation (taVNS) are currently unknown. Pupil size has gained attention as a promising biomarker of vagal activation in different studies on animal models. The aim of this study is to investigate the effects of taVNS on pupil diameter in healthy subjects. MethodsAll subjects received taVNS at the left external acoustic meatus and control stimulation at the left earlobe during the same experimental session. Different intensities (0.5 mA; 1.0 mA; 2.0 mA; 3.0 mA) for both conditions were tested. Tonic pupil size was recorded in both eyes at baseline and during each stimulation using an infrared-automated pupillometer in three different illuminance conditions (scotopic, mesopic, photopic). ResultsIn scotopic illuminance condition, a significant interaction between intensity and condition (real vs control) was found for the left eye. Post-Hoc analysis showed that during real taVNS at 2 mA, pupil size was significantly larger in comparison to baseline and 2 mA control stimulation. ConclusionsOur study demonstrates that taVNS induces pupil dilation under specific illuminance conditions and at specific stimulation intensity. SignificanceThe effects of taVNS are strictly dependent on technical aspects, such as stimulation parameters and experimental set-up.

Folliculosebaceous Cystic Hamartoma: A Rare Entity

Folliculosebaceous Cystic Hamartoma (FSCH) is an uncommon cutaneous hamartomatous lesion. It is basically a tumour like malformations exhibiting abnormal overgrowth of biphasic elements i.e., epithelial and mesenchymal components that are normally found in the skin. Few other hamartomas of pilosebaceous origin are trichofolliculoma, trichodiscoma, fibrofolliculoma and pilar sheath acanthoma. Though distinct, this entity shares similar histological features to Sebaceous Trichofolliculoma (STF). Hence, a proper histological examination is must in differentiating it from STF and other clinical and histological entities. Authors hereby, describes a case of FSCH in a 37-year-old male who presented with a slow growing cystic mass on his left earlobe, an uncommon location for such lesion. The mass was excised, and a detailed histological evaluation showed a characteristic multinodular growth pattern with lobules of pilosebaceous glands forming nodules around cystically dilated follicular infundibular structures. No hair shafts were seen in the cystic cavity ruling out differential diagnosis of sebaceous hyperplasia which could have been a close mimic. The intervening stroma revealed dense collagen, sheets and lobules of adipocytes, many vascular channels filled with fibrinous deposits. Perivascular adipocytes were also noted along with few nerve tissues (perineurium). A detailed clinicopathological work-up helped in arriving at a final diagnosis of FSCH with neural component. The patient was doing well till last six months of follow-up. No recurrence of similar lesion was noted in the patient.

Radiotherapy for Recurrent Keloid: A Case Report

Background: Keloid is a benign growth of connective tissue. There are several risk factors that play a role in keloid growth. Excision surgery is one of the modalities in the treatment of keloids. However, excision surgery alone has a recurrence rate of 45-100%.Case: Male, 37 years old, with complaints arising from a keloid lesions in the left earlobe since three years ago with a history of previous injuries. The lesions is then operated on but always grows back postoperatively. The number of surgeries that have been carried out three times with further treatment in the form of corticosteroid injection. However, keloid still recurrence. Finally it was decided to undergo treatment with surgery followed by radiotherapy within a period of no more than 24 hours postoperatively. Follow-up after six months gave good results without recurrence.Discussion: Keloid has a high recurrence rate after excision surgery. Surgery followed by radiotherapy has a low recurrence rate compared to surgery or surgery followed by administration of corticosteroid injections. Radiotherapy as adjuvant therapy for postoperative keloid excision has the role of sterilizing the connective tissue stem cell active fibroblasts and acute inflammatory cells that grow in the early postoperative period. A study states that administration of postoperative radiation with electrons in keloids in the earlobe at a dose of 15 Gy in three fractions gives a low recurrence rate and a low risk of side effects in the surrounding soft tissue.

Reduced Frontal Nogo-N2 With Uncompromised Response Inhibition During Transcutaneous Vagus Nerve Stimulation-More Efficient Cognitive Control?

We have previously shown invasive vagus nerve stimulation to improve attention and working memory and alter emotion-attention interaction in patients with refractory epilepsy, suggesting that VNS might be useful in the treatment of cognitive impairment. The current research focuses on whether non-invasive, transcutaneous vagus nerve stimulation (tVNS) has similar effects to VNS. Furthermore, we aimed to assess whether tVNS has an impact on cognitive control in general or on underlying brain physiology in a task that mimics everyday life demands where multiple executive functions are engaged while encountering intervening emotional stimuli. Event-related potentials (ERP) evoked in such a task, specifically centro-parietal P3 and frontal N2 were used as biomarkers for attention allocation and cognitive control required to carry out the task. A single-blinded, sham-controlled, within-subject study on healthy subjects (n = 25) was conducted using Executive Reaction Time Test (RT-test), a Go/NoGo task engaging multiple executive functions along with intervening threat-related distractors while EEG was recorded. tVNS at the left tragus and sham stimulation at the left ear lobe was alternately delivered throughout the task. To assess the impact of tVNS on neural activity underlying attention and cognitive control, centro-parietal P3 and frontal N2 peak amplitudes were measured in Go and NoGo conditions. Task performance was assessed with RTs and different error types reflecting cognitive control in general and distinct executive functions, such as working memory and response inhibition.No significant effects due to tVNS on performance in the Executive RT-test were observed. For N2 there was a main effect of stimulator status and a significant interaction of trial type (Go, NoGo) and stimulator status. Post hoc analysis revealed that tVNS resulted in a significant reduction of frontal N2 only in the NoGo condition. No significant effects were observed for P3 nor were there any effects of emotion. Diminished NoGo-N2 potential along with unaltered task performance during tVNS suggests fewer cognitive control resources were required to successfully withhold a prepotent response. Though caution is warranted, we suggest that tVNS may lead to more efficient neural processing with fewer resources needed for successful cognitive control, providing promise for its potential use in cognitive enhancement.
