Challenges and opportunities in achieving secure hospital clinical mobility management: An illustrative use case

Objective: To qualitatively describe a use case at Yale New Haven Health System (YNHHS) illustrating the need for and effective deployment of innovative technologies to manage an enterprise-owned shared device (EOSD) management program. EOSD management provides clinicians with secure, rapid access to enterprise mobile devices and applications, maintains devices in functional, use ready condition for clinicians, and enables enterprise tracking and reduced loss of devices.Methods: Executive leaders in clinical information technology and informatics management at YNHHS were interviewed through written and telephonic communication. Qualitative data was gathered through communications between clinical and information technology executives and the implementation support team of a leading identity and access management (IAM) solutions and EOSD management solution provider. Use case information was gathered, integrated and shared with health system executives and health IT/informatics leaders to verify the description of unmet needs, solution objectives and impact/value delivered after implementation of the EOSD management solution.Results: Benefits realized from implementation of an enterprise-shared mobility management solution included establishment of a cohesive and comprehensive enterprise-owned, shared device management strategy. This included effective monitoring and dynamic management of the system’s mobile device fleet, and better IT resource management with reduced mobile device loss. The IT administrative burden was reduced. While not surveyed systematically, improved clinician experience and satisfaction were reported to IT leaders anecdotally. EOSD management solution deployment was rapid, as was the time to improved clinician mobile experience and clear demonstration of value.Conclusions: A leading US health system was able to rapidly deploy a shared mobile device management solution that enabled effective monitoring and dynamic management of the enterprise mobile device fleet, with easier and faster clinician device access and workflows, and reduced IT administrative demand and costs. While the complexities associated with increased clinical mobility in healthcare will likely continue to grow, issuing future device and mobile management challenges that require effective hospital system response, technologies have emerged that enable more effective, efficient and satisfactory organizational mobility performance.

Traction with fraction: Strategic IS management in SMEs through Fractional CIOs

Small and medium-sized enterprises (SMEs) increasingly need to manage information technology (IT) effectively in order to remain competitive. However, compared to larger organizations, SMEs often face challenges in terms of resources and employer attractiveness, and regularly do not have the need to employ a Chief Information Officer (CIO) on a full-time basis. To address this issue, a growing number of global experts have begun to provide CIO services on a part-time basis for multiple clients simultaneously. This approach allows SMEs to tap into the expertise of experienced IT leaders at a fraction of the cost and without committing to long-term arrangements. While these professionals, known as “Fractional CIOs”, have proven their value in the field, there has been a lack of academic research on this emerging trend. Therefore, we carried out a comprehensive research project between 2020 and 2023, involving 62 Fractional CIOs from 10 countries. The research produced a definition, different types of engagements, and success factors for Fractional CIOs and their engagements. This paper summarizes these findings for a wider audience of academics and practitioners.

The New Cyberlinktech, Inc. Resilience to Creating Growth Model

The New Cyberlinktech, Inc. (TNCI) operates in a highly dynamic and evolving market, requiring the need for a strategic business plan to sustain growth and adaptability. The New Cyberlinktech, Inc. (TNCI) is one of the leading information technology providers for Private Emission Testing Centers (PETCs) under the Department of Transportation and the Land Transportation Office in the Philippines. TNCI faces significant challenges in the current market landscape, including increasing regulatory pressure against PETCs, the emergence of the Motor Vehicle Inspection System (MVIS) as an alternative mode of vehicle testing, and the frequent leadership changes and abrupt policy shifts at the Land Transportation Office (LTO) and the Department of Transportation (DOTr). To ensure sustainability and adaptability, this dissertation aims to develop a comprehensive business plan for The New Cyberlinktech, Inc. (TNCI) that addresses the challenges posed by the rapidly changing market landscape including a 5-year roadmap. The research design employs a mixed methods approach. The qualitative phase involves conducting interviews with key stakeholders within TNCI, including senior management, employees, and selected industry experts. Through these qualitative interactions, the study aims to identify the specific challenges faced by TNCI in the evolving market and explore potential strategic approaches to address them. In the quantitative phase, the focus is on using surveys among the current clients of the company to determine the preferred IT projects that they want the company to implement in the future. The quantitative analysis provides a comprehensive assessment of TNCI’s current performance, identifies potential risks and opportunities, and informs the development of strategic recommendations. The integrated analysis phase synthesizes the findings from the qualitative and quantitative data. By comparing and contrasting the insights gained from both approaches, a holistic understanding of the challenges and opportunities facing TNCI is achieved. The strategic business plan is formulated based on the integrated analysis, considering the specific needs and goals of TNCI. The plan encompasses strategies to address regulatory challenges, navigate the MVIS landscape, manage leadership changes, adapt to policy shifts, and recommend potential IT projects that the company can adapt and implement. Potential findings may include insights into the specific regulatory pressures faced by PETCs, the implications of the MVIS on TNCI’s business model, and strategies to mitigate the effects of leadership and policy changes. The implications of the research findings can inform TNCI’s decision-making process and help guide the company in sustaining growth and adaptability in the rapidly evolving market. This dissertation contributes to the body of knowledge by providing a comprehensive strategic business plan tailored to TNCI’s context. The findings and recommendations have practical implications for TNCI and other IT companies operating in similar environments. Future research opportunities may involve evaluating the implementation and effectiveness of the proposed business plan and exploring additional strategies to enhance TNCI’s competitive advantage in the PETC industry. She’s worth IT: challenges for female CIOs in ensuring IT security compliance

PurposeIT security compliance is critical to the organization’s success, and such compliance depends largely on IT leadership. Considering the prevalence of unconscious gender biases and stereotyping at the workplace and growing female leadership in IT, the authors examine how the internalization of stereotype beliefs, in the form of the employee’s gender, impacts the relationships between leadership characteristics and IT security compliance intentions.Design/methodology/approachA controlled experiment using eight different vignettes manipulating Chief Information Officer (CIO) gender (male/female), Information Technology (IT) expertise (low/high) and leadership style (transactional/transformational) was designed in Qualtrics. Data were gathered from MTurk workers from all over the US.FindingsThe findings suggest that both CIOs' and employees' gender play an important role in how IT leadership characteristics – perceived expertise and leadership style – influence the employees' intentions and reactance to comply with CIO security recommendations.Research limitations/implicationsThis study's findings enrich the security literature by examining the role of leadership styles on reactance and compliance intentions. They also provide important theoretical implications based on gender stereotype theory alone: First, the glass ceiling effects can be witnessed in how men and women employees demonstrate prejudice against women CIO leaders through their reliance on perceived quadratic CIO IT expertise in forming compliance intentions. Secondly, this study's findings related to gender role internalization show men and women have a prejudice against gender-incongruent roles wherein women employees are least resistive to transactional male CIOs, and men employees are less inclined to comply with transactional female CIOs confirm the findings related to gender internationalization from Hentschel et al. (2019).Practical implicationsThis study highlights the significance of organizations and individuals actively promoting gender equality and fostering environments that recognize women's achievements. It also underscores the importance of educating men and women about the societal implications of stereotyping gender roles that go beyond the organizational setting. This research demonstrates that a continued effort is required to eradicate biases stemming from gender stereotypes and foster social inclusion. Such efforts can positively influence how upcoming IT leaders and employees internalize gender-related factors when shaping their identities.Social implicationsThis study shows that more work needs to be done to eliminate gender stereotype biases and promote social inclusion to positively impact how future IT leaders and employees shape their identities through internalization.Originality/valueThis study redefines the concept of “sticky floors” to explain how subordinates can hinder and undermine female leaders, thereby contributing to the glass ceiling effect. In addition, the study elucidates how gender roles shape employees' responses to different leadership styles through gender stereotyping and internalization.

Enhancing patient safety: A national standard for cyber resiliency in healthcare.

With support from Public Safety Canada's Cyber Security Cooperation Program, HealthCareCAN and the Digital Governance Council developed a new standard to support cyber resiliency in Canada's healthcare system. With a clear framework and enhanced cybersecurity capabilities, healthcare organizations will be better protected from cybercrime, allowing them to respond more effectively to evolving threats and defend critical infrastructure. Health and information technology leaders can derive practical guidance and next steps from this three-year national project to enhance cyber resilience and improve safety within their organizations.

Managing cybersecurity risk in healthcare settings.

Cybersecurity attacks have been steadily increasing in the healthcare sector over the past decade. Health data is a valuable source of reliable and permanent personal information making it an attractive target. Institutions that have faced limited cybersecurity funding must now augment their approach to combat this threat. The Internet of Things (IoT) refers to the connection of physical operational devices to digital networks allowing for communication between devices. In the healthcare setting, this includes patient monitoring, diagnostics, and even robotic surgery devices. This increased connectivity increases the importance of agile and robust cybersecurity measures. A progressive approach must involve collaboration between information technology, clinical, and administrative leaders to be successful. Adequate protection of patient data and the integrity of digital infrastructure must be a priority mandate at the enterprise level.

Association between Electronic Health Record Implementations and Hospital-Acquired Conditions in Pediatric Hospitals.

Implementing an electronic health record (EHR) is one of the most disruptive operational tasks a health system can undergo. Despite anecdotal reports of adverse events around the time of EHR implementations, there is limited corroborating research, particularly in pediatrics. We utilized data from Solutions for Patient Safety (SPS), a network of 145+ children's hospitals that share data and protocols to reduce harm in pediatric care delivery, to study the impact of EHR implementations on patient safety. Determine if there is an association between the time immediately surrounding an EHR implementation and hospital-acquired conditions (HACs) rates in pediatrics. A survey of information technology leaders at pediatric institutions identified EHR implementations occurring between 2012 and 2022. This list was cross-referenced with the SPS database to create an anonymized dataset of 27 sites comprising monthly HAC and care bundle compliance rates in the 7 months preceding and succeeding the transition. Six HACs were analyzed: central-line associated bloodstream infection (CLABSI), catheter-associated urinary tract infection (CAUTI), adverse drug events, surgical site infections (SSIs), pressure injuries (PIs), and falls, in addition to four associated care bundle compliance rates: CLABSI and CAUTI maintenance bundles, SSI bundle, and PI bundle. To determine if there was a statistically significant association with EHR implementation, the observation period was divided into three eras: "before" (months -7 to -3), "during" (months -2 to +2), and "after" go-live (months +3 to +7). Average monthly HAC and bundle compliance rates were calculated across eras. Paired t-tests were performed to compare rates between the eras. No statistically significant increase in HAC rates or decrease in bundle compliance rates was observed across the EHR implementation eras. This multisite study detected no significant increase in HACs and no decrease in preventive care bundle compliance in the months surrounding an EHR implementation.

Building information modelling (BIM) interoperability for architecture and engineering (AE) of the structural project: A case study

The Architecture, Engineering (AE), and Construction (AEC) industry is a sector in accepting change and adopting new technological approaches. Building Information Modelling (BIM) is increasingly adopted as the leading information technology tool emerging in the AEC industry. Thanks to its advantages, especially the generation of a virtual model centralising all the information concerning the different trades involved and the effective interoperability, BIM has greatly helped construction projects reach a mature performance level. However, some phases in the AE lack interoperability and information exchange efficiency. This inefficiency is much more critical in the case of design and structural projects. In this respect, this study aims to conduct and integrate BIM into an AE process to analyse and improve the degree of efficiency of a structural project. For this purpose, a real case study on a high-standing villa was conducted. The BIM process was implemented based on the spirit of collaboration and interoperability with both Revit® and Robot® software. The results describe the BIM process’s ease of modelling, verification, transfer, exchange, and error detection. Thus, the speed, ease of modification, and decision-making. Thus, the rate of improvement and decision-making. The paper joins the body of studies seeking to reposition the AEC industry. It develops a Framework between Revit and Robot at the professional level, valid for all structural projects. For the scientific community, the study proposes innovation in AEC as a field of research to achieve the desired performance and sustainability. The work is limited to architecture and structural engineering and does not include MEP.

IT Leaders’ Experiences of Resilience during the Pandemic : An Appreciative Inquiry

Purpose : The COVID-19 pandemic highlighted that leaders need to have the capacity for resilience. When leaders are confronted with disruptions that are now more of a certainty than a possibility, they must have the ability to bounce back, recover, be agile, and be resilient. Objective : To explore the resilience experiences of leaders in the information technology (IT) industry (both mid-level and senior leaders) during the outbreak of the COVID-19 pandemic, guided by the theory of self-leadership. Method : Qualitative exploratory study using an appreciative inquiry approach based on 19 mid- and senior-level leaders, both men and women, in the IT Industry in India, chosen purposively. The data in the form of in-depth interviews were analyzed thematically. Findings : Participants’ experience of resilience during the pandemic was expressed as a capability of leaders, which is inherent in the IT industry, given its dynamism. Various facets of resilience and its impact were highlighted. Practical Implications : The IT industry disruption is characteristic of the pandemic. Hence, understanding how leaders handled the crisis will help in training and development, talent acquisition, self-development, and a guiding pathway for budding leadership. Resilient leaders can positively impact self-leadership by inspiring individuals to develop self-leadership skills, fostering a culture within their organizations, and increasing team self-efficacy. Originality : The resilience capabilities of the leaders in the IT industry in India has not been explored qualitatively before, especially during the pandemic.

Thought Leader Perspectives on the Benefits, Barriers, and Enablers for Routinely Collected Electronic Health Data to Support Professional Development: Qualitative Study.

Hospitals routinely collect large amounts of administrative data such as length of stay, 28-day readmissions, and hospital-acquired complications; yet, these data are underused for continuing professional development (CPD). First, these clinical indicators are rarely reviewed outside of existing quality and safety reporting. Second, many medical specialists view their CPD requirements as time-consuming, having minimal impact on practice change and improving patient outcomes. There is an opportunity to build new user interfaces based on these data, designed to support individual and group reflection. Data-informed reflective practice has the potential to generate new insights about performance, bridging the gap between CPD and clinical practice. This study aims to understand why routinely collected administrative data have not yet become widely used to support reflective practice and lifelong learning. We conducted semistructured interviews (N=19) with thought leaders from a range of backgrounds, including clinicians, surgeons, chief medical officers, information and communications technology professionals, informaticians, researchers, and leaders from related industries. Interviews were thematically analyzed by 2 independent coders. Respondents identified visibility of outcomes, peer comparison, group reflective discussions, and practice change as potential benefits. The key barriers included legacy technology, distrust with data quality, privacy, data misinterpretation, and team culture. Respondents suggested recruiting local champions for co-design, presenting data for understanding rather than information, coaching by specialty group leaders, and timely reflection linked to CPD as enablers to successful implementation. Overall, there was consensus among thought leaders, bringing together insights from diverse backgrounds and medical jurisdictions. We found that clinicians are interested in repurposing administrative data for professional development despite concerns with underlying data quality, privacy, legacy technology, and visual presentation. They prefer group reflection led by supportive specialty group leaders, rather than individual reflection. Our findings provide novel insights into the specific benefits, barriers, and benefits of potential reflective practice interfaces based on these data sets. They can inform the design of new models of in-hospital reflection linked to the annual CPD planning-recording-reflection cycle.

Using information technology to integrate tobacco use treatment in routine oncology care: Lessons learned from the U.S. Cancer Center Cessation Initiative Cancer Centers.

Cancer patients who receive evidence-based tobacco-dependence treatment are more likely to quit and remain abstinent, but tobacco treatment programs (TTPs) are not consistently offered. In 2017, the U.S. National Cancer Institute, through the Cancer Moonshot, funded the Cancer Center Cessation Initiative (C3I). C3I supports 52 cancer centers to implement and expand evidence-based tobacco treatment in routine oncology care. Integration into routine care involves the use of health information technology (IT), including modifying electronic health records and clinical workflows. Here, we examine C3I cancer centers' IT leadership involvement and experiences in tobacco-dependence treatment implementation. This qualitative study of C3I-funded cancer centers integrated data from online surveys and in-person, semistructured interviews with IT leaders. We calculated descriptive statistics of survey data and applied content analysis to interview transcripts. Themes regarding IT personnel included suggestions to involve IT early, communicate regularly, understand the roles and influence of the IT team, and match program design with IT funding and resources. Themes regarding electronic health record (EHR) modifications included beginning modifications early to account for long lead time to make changes, working with IT to identify and adapt existing EHR tools for TTP or designing tools that will support a desired workflow developed with end-users, and working with IT personnel to make sure TTPs comply with system and state policies (e.g., privacy laws). The experiences of C3I cancer centers regarding the use of health IT to enhance tobacco-dependence treatment program implementation can guide cancer centers and community oncology practices to potentially enhance TTP implementation and patient outcomes.

Whither digital agriculture in India?

Agriculture is central to the Indian economy and suffers from widespread operational inefficiencies that could be corrected by the use of digital agriculture technologies (DA). We review and synthesise available literature concerning digital agriculture in India and anticipate its transformative potential in the coming decade. Although the initial growth of DA was more conspicuous in the downstream sectors and high-value crops, reaching smallholder farmers upstream is slowly emerging despite significant obstacles such as small fragmented holdings, inadequate data infrastructure and public policy, and unequal access to digital infrastructure. Agri-tech enables innovation at many locations within value chains, and a steady shift is occurring in change from individual farms to the whole value chain. Technology in the sector is progressing from information and communication technology-based solutions to Internet of Things and artificial intelligence–machine learning-enabled services. India’s public policy shows signs of a longstanding investment and collaboration in the sector, with an explicit focus on data infrastructure development. We find smallholder predominance, diversity in production systems, the predominance of commodity crops, proximity to urban markets, and public policy as the major factors of DA’s success in India. A stocktake of the available technologies and their applications by the public sector, tech giants, information technology leaders and agri-food tech startups in India strongly indicates a digital transformation of Indian agriculture. However, given the federal structure of governance and agriculture being a state (province) subject, we need to wait to see how DA policies are rolled out and taken up across the country.

A Review of the Leading Information Technology Companies’ Modern Slavery Statements

Modern slavery is an insidious problem in many sectors of the global economy, and it poses a challenge for the information technology industry. However, the issue of modern slavery in the information technology industry has received little or no attention in the business and management literature and this paper looks to provide an exploratory review of one of the ways in which the leading information and communication technology companies have addressed this issue, by reviewing their modern slavery statements. The findings reveal that five interlinked themes, namely, corporate commitment; risk assessments and due diligence in supply chains: codes of conduct; awareness, capability building, and training; and future plans and key performance indicators illustrated the spirit of the selected companies’ modern slavery statements. Identifying and tackling the risk of modern slavery within supply chains presents the major challenges for the information technology companies, and the paper concludes that the leading technology companies’ modern slavery statements can be best seen as a work in progress. Firm financial outcomes: The impact of technology leaders

As the importance of Information Technology (IT) has exploded in business, its prominence in senior leadership teams has become pervasive. However, the success of these companies' efforts is mixed. This research uses Return on Assets, Tobin's q, and Market‐to‐Book value, to evaluate the impact of adding IT leaders into the C‐Suite (CxOs). We examine instances of a CxO as a member of the top‐five compensated executives from 1992 and 2018, identifying 2056 observations, and find that firms with a CxO have better financial performance than their non‐CxO peers. Second, different CxO titles lead to different financial impact on the firm.

Clinico Epidemiological Profile of Repetitive Stress Injuries in Computer Professionals of West Coast of India

Repetitive Stress Injury can be regarded as a modern-day epidemic, a gift of industrialization. We have conducted a study to find the prevalence of musculoskeletal injuries in computer professionals with the aim to present data on the problem. The initial data was collected by a self-administered questionnaire enquiring about years of work and per day working hours, symptomatology with their duration and severity, their own appreciation of work output. These individuals were called up according to symptoms, assessed according to their symptomatology, relevant investigations and different modalities of treatment were advised. The study was conducted in one of the leading information technology company at our city. A total of 1159 questionnaires were collected. Six hundred and twenty-four (53.8%) had at least one positive symptom. 46.6% had stage 1 symptoms, 38.5% had stage 2 and 8.1% had stage 3 symptoms at the time of presentation. Two hundred and six patients were examined clinically by the senior author and a definitive clinicoanatomical diagnosis could be made only in 43 patients. Median age in our study group was 24 years. Ninety-eight (22.17%) of positive patients concluded decrease work output due to their symptoms, but only 50(11.3%) patients had consulted a doctor. Two hundred four (46.15%) of patients contributed their symptoms to an ill designed workstation. We conclude that repetitive stress injuries are a definitive problem in computer professionals. The median age in our study group was lower than many international studies. Nearly half of these patients were in stage 2 and stage 3 at presentation.

Overall, the paper provides important spring of dexterity and is efficacious for managers, stakeholders and groundwork.

Immersive Virtual Reality Teaching in Colleges and Universities Based on Vision Sensors

With the progress of society and the development of economy, people pay more and more attention to education, and traditional teaching methods are gradually unable to meet the modern teaching system. As a leader in modern information technology, virtual reality technology has developed rapidly in recent years, and virtual reality technology has also been introduced into many fields, such as teaching. Based on the immersive and extended characteristics of virtual reality, this paper proposes a virtual reality active visual interaction method based on the visual sensor. Based on virtual teaching, after 3 months of learning, the average, standard deviation, and average standard error of the experimental group’s performance are higher than those of the control group. Compared with the control group, the experimental group’s performance has increased by 8.25%. The difference is statistically significant. Learning significance ( P < 0.05 ), immersive virtual reality teaching has played a significant role in the effect, which can greatly improve the cognitive experience of students and achieve a good learning experience and effect.

Automatic Assessment and Identification of Leadership in College Students

As one of the 21st Century skills, fostering leadership competencies in college students has been a focus of research in the behavioral education field. While leaders are expected to have general leadership traits such as communication and resource management, leading teams in particular expertise areas requires additional skills specifically tailored to the tasks involved. For example, in information technology (IT), the roles of leaders are different from those in other non-IT fields due to the characteristics of IT projects. IT leaders, in particular, are required to have not just managerial and interpersonal skills but also necessary technical and practical capabilities. Understanding the key elements that contribute to the emergence and development of student leaders necessitates the ability to assess and recognize leadership in college students. However, current approaches for measuring leadership qualities entail administering questionnaires, which have coverage constraints and may mislabel future leaders if the survey is administered too early in a student&#x2019;s educational phase, when their leadership talents may not have emerged yet. Addressing this problem, this paper introduces <i>LAIGA</i>, a machine learning-based framework for automatically and passively assessing and identifying leaders in college students from their academic profiles and behavior in the learning management system (LMS). The proposed technique can capture students&#x2019; actions that indicate general leadership qualities and technical competence as evidenced by their academic performances. Specifically, we address two problems: leadership assessment and leader identification tasks. A case study of senior-year college students in the IT major is used to validate the proposed method, yielding 5.87% MAPE in the leadership assessment task and 83.2% F1 in the leadership identification task. Furthermore, the results also show that the proposed method can forecast leadership assessment and identification after graduation, knowing as little as students&#x2019; information during their first year of study. The findings of this study not only shed light on the ability to use machine learning technologies to automatically assess and identify leadership in college students but also allow educational organizations to understand factors that help develop leadership skills and design mechanisms that promote the growth of student leaders. The proposed method is highly generalizable to other fields of study, as long as students&#x2019; grades and LMS activities are available.

Two facets of pride and knowledge hiding: an empirical analysis

Purpose This study aims to investigate whether or not the two facets of pride, hubristic and authentic, are associated with knowledge hiding. Design/methodology/approach This study collects survey data (N = 343) from one of the leading information technology (IT) companies in Myanmar at two stages with a two-month interval. This study uses multiple regression analyses to test this study’s hypotheses. Findings Results reveal that hubristic pride is positively related to knowledge hiding, whereas the relationship between authentic pride and knowledge hiding is negative. These relationships are contingent upon the level of employees’ self-efficacy. Research limitations/implications This study suggests that managers should include measures for moral emotions in their recruitment and selection criteria. Furthermore, the authors suggest that managers should design strategies to induce moral emotions at the workplace and enhance personal resources (e.g. self-efficacy), which have an instrumental effect in maximizing the prosocial facet of pride (i.e. authentic pride) as well as minimizing adverse experiences of the antisocial facet of pride (i.e. hubristic pride), thereby reducing knowledge hiding. Originality/value The findings shed light on the significance of the inclusion of emotional variables in understanding employees’ knowledge hiding. To the best of the authors’ knowledge, this study is the first empirical study to examine the combined effect of emotive and cognitive variables in predicting knowledge hiding by demonstrating that hubristic pride only mitigates knowledge hiding behavior among high self-efficacious employees.

Implementation of Patient Engagement Tools in Electronic Health Records to Enhance Patient-Centered Communication: Protocol for Feasibility Evaluation and Preliminary Results.

BackgroundPatient–physician communication during clinical encounters is essential to ensure quality of care. Many studies have attempted to improve patient–physician communication. Incorporating patient priorities into agenda setting and medical decision-making are fundamental to patient-centered communication. Efficient and scalable approaches are needed to empower patients to speak up and prepare physicians to respond. Leveraging electronic health records (EHRs) in engaging patients and health care teams has the potential to enhance the integration of patient priorities in clinical encounters. A systematic approach to eliciting and documenting patient priorities before encounters could facilitate effective communication in such encounters.ObjectiveIn this paper, we report the design and implementation of a set of EHR tools built into clinical workflows for facilitating patient–physician joint agenda setting and the documentation of patient concerns in the EHRs for ambulatory encounters.MethodsWe engaged health information technology leaders and users in three health care systems for developing and implementing a set of EHR tools. The goal of these tools is to standardize the elicitation of patient priorities by using a previsit “patient important issue” questionnaire distributed through the patient portal to the EHR. We built additional EHR documentation tools to facilitate patient–staff communication when the staff records the vital signs and the reason for the visit in the EHR while in the examination room, with a simple transmission method for physicians to incorporate patient concerns in EHR notes.ResultsThe study is ongoing. The anticipated completion date for survey data collection is November 2021. A total of 34,037 primary care patients from three health systems (n=26,441; n=5136; and n=2460 separately recruited from each system) used the previsit patient important issue questionnaire in 2020. The adoption of the digital previsit questionnaire during the COVID-19 pandemic was much higher in one health care system because it expanded the use of the questionnaire from physicians participating in trials to all primary care providers midway through the year. It also required the use of this previsit questionnaire for eCheck-ins, which are required for telehealth encounters. Physicians and staff suggested anecdotally that this questionnaire helped patient–clinician communication, particularly during the COVID-19 pandemic.ConclusionsEHR tools have the potential to facilitate the integration of patient priorities into agenda setting and documentation in real-world primary care practices. Early results suggest the feasibility and acceptability of such digital tools in three health systems. EHR tools can support patient engagement and clinicians’ work during in-person and telehealth visits.
