Anatomic assessment of femoral tunnel by transtibial drilling technique in double-bundle anterior cruciate ligament reconstruction: an in vivo study

ObjectiveTo evaluate the anatomy of femoral tunnels created by simulated transtibial technique in double-bundle anterior cruciate ligament (ACL) reconstruction. MethodsTwo tibial tunnels, anteromedial (AM) and posterolateral (PL), were drilled 45° and 55° to tibial plateau respectively. On the femoral side, the AM and PL tunnels were drilled through anteriomedial portal. After the four tunnels were established, the shaft of a reamer was introduced into the joint through tibial tunnel and reached against the lateral wall of intercondylar notch. The position that the reamer shaft can reach was marked and recorded. ResultsNeither femoral AM nor PL tunnel opening can be fully or partially reached by the reamer shaft through the tibial AM tunnel in all cases. The evaluation through the tibial PL tunnel showed that only in 8 of 50 cases (16%) the femoral AM tunnel opening and in 4 cases (8%) the PL opening can be fully reached. On the other hand, in 12 cases (24%) the femoral AM tunnel opening and in 10 cases (20%) the PL opening can be partially reached by the shafts through the tibial PL tunnel. ConclusionThe result strongly suggests that transtibial technique is not well competent for femoral tunnel drilling in anatomic double-bundle ACL reconstruction as we have hypothesized.

Proportional evaluation of anterior cruciate ligament footprint size and knee bony morphology.

The purpose of this study was to reveal the correlation in size between the native anterior cruciate ligament (ACL) footprint and the femoral intercondylar notch and the tibia plateau, and to calculate the proportion in size between the ACL footprint and knee bony morphology. Twenty-six non-paired human cadaver knees were used. All soft tissues around the knee were resected except the ACL. The ACL was cut in the middle, and the femoral bone was cut at the most proximal point of the femoral notch. The ACL was carefully dissected, and the periphery of the ACL insertion site was outlined on both the femoral and tibial sides. An accurate lateral view of the femoral condyle and an axial view of the tibial plateau were photographed with a digital camera, and the images were downloaded to a personal computer. The size of the femoral and tibial ACL footprints and the area of the lateral wall of the intercondylar notch and the tibia plateau were measured with Image J software (National Institution of Health). The sizes of the native femoral and tibial ACL footprints were 69.8 ± 25 and 133.8 ± 31.3 mm(2), respectively. The areas of the lateral wall of the intercondylar notch and the tibia plateau were 390.5 ± 70.5 and 2,281.7 ± 377.3 mm(2), respectively. The femoral ACL footprint area and the area of the lateral wall of the femoral intercondylar notch (Pearson's correlation coefficient = 0.603, p = 0.001), and the tibial ACL footprint area and the area of the tibia plateau (Pearson's correlation coefficient = 0.452, p = 0.02) both showed significant correlation. The femoral ACL footprint was 17.8 ± 4.9 %, the size of the lateral wall of the femoral intercondylar notch, and the tibial ACL footprint was 5.9 ± 1.3 %, the size of the tibia plateau. For clinical relevance, the femoral ACL footprint is approximately 18 %, the size of the intercondylar notch, and the tibial ACL footprint is approximately 6 %, the size of the tibia plateau. It might be possible to predict the size of the ACL measuring these parameters preoperatively.

A knee-specific finite element analysis of the human anterior cruciate ligament impingement against the femoral intercondylar notch

This work presents a finite element analysis of anterior cruciate ligament (ACL) impingement against the intercondylar notch during tibial external rotation and abduction, as a mechanism of noncontact ACL injuries. Experimentally, ACL impingement was measured in a cadaveric knee in terms of impingement contact pressure and six degrees-of-freedom tibiofemoral kinematics. Three-dimensional geometries of the ACL, femur and tibia were incorporated into the finite element model of the individual knee specimen. A fiber-reinforced model was adopted, which accounts for the anisotropy, large deformation, nonlinearity and incompressibility of the ACL. With boundary conditions specified based on the experimental tibiofemoral kinematics, the finite element analysis showed that impingement between the ligament and the lateral wall of intercondylar notch could occur when qthe knee at 45° was externally rotated at 29.1° and abducted at 10.0°. Strong contact pressure and tensile stress occurred at the impinging and nonimpinging sides of the ligament, respectively. The impingement force and contact area estimated from the model matched their counterparts from the corresponding cadaver experiment. The modeling and experimental approach provides a useful tool to characterize potential ACL impingement on a knee-specific basis, which may help elucidate the ACL injury mechanism and develop more effective treatments.
