Analysis of laser Doppler flowmetry long-term recordings for investigation of cerebral microcirculation during neurointensive care.

Cerebral blood flow is monitored in the neurointensive care unit (NICU) to avoid further brain damage caused by secondary insults following subarachnoid hemorrhage and brain trauma. Current techniques are mainly snap-shot based and focus on larger vessels. However, continuous monitoring of the smaller vessels may help detect the onset of secondary insults at an earlier stage. In this study, long-term measurements of brain microcirculation with laser Doppler flowmetry (LDF) were performed and evaluated. The aim was to identify and describe physiological signal variations and separate these from movement artifacts. Fiberoptic probes for subcortical LDF recordings of perfusion and total light intensity (TLI) were implanted in three patients with subarachnoid hemorrhage. Data were successfully collected and visualized in real-time over 4 days, resulting in 34, 12, and 8.5 h per patient. Visual observation, wavelet transforms, moving medians, and peak envelopes were used to identify and describe movement artifacts and physiological changes. Artifacts occurred in <5% of the total recording time and could be identified through signal processing. Identified physiological signal patterns included a slowly increasing perfusion trend over hours, vasomotion mainly at 2 cycles/min both in the perfusion and the TLI, and rapid, synchronized changes in the TLI and the perfusion on 38 occasions. Continuous LDF recordings indicating changes in the microvascular blood flow can increase the understanding of the microcirculation in the injured brain. In the long run, this may become a complement for the detection of secondary insults at an earlier stage than possible with today's techniques.

Assessment of Pulpal Status in Primary Teeth Following Direct Pulp Capping in an Experimental Canine Model.

(1) Background: This study aimed to assess the pulpal response of primary teeth by pulse-oximetry (PO) in a canine model, following direct pulp capping (DPC). (2) Methods: Forty-eight primary teeth from eight canine subjects were divided into three treatment groups, based on the DPC material—calcium hydroxide (CH), MTA, BiodentineTM)—and three corresponding control groups. Data from PO pulp testing were correlated with laser Doppler flowmetry (LDF) testing, computer tomographic (CT) densitometry and histological analysis; the experiment lasted 14 days. (3) Results: SpO₂ recordings revealed statistically significant differences (p = 0.002, <0.05) between the treatment and control groups, and no significant differences (p = 0.257, >0.05) were observed between treatment groups. LDF recordings showed significant differences (p = 0.002, <0.05) between the treatment and control groups and identified significant differences between materials (p = 0.001, <0.05). CT densitometry indicated vital pulps in all teeth, with pulpal inflammation detected in 6/8 CH-capped teeth and 2/8 MTA-capped teeth. Histologic evaluation confirmed vital pulp in all specimens, with different degrees of inflammation. (4) Conclusions: Within its limitations, the present study confirms the diagnostic value of PO evaluation of pulpal status in primary teeth with histologic means after pulp-capping procedures in a canine model. However, various degrees of pulpal inflammation elicited by different pulp-capping materials seem not to correlate with the obtained PO values.

Parameter Sensitivity and Experimental Validation for Fractional-Order Dynamical Modeling of Neurovascular Coupling.

Goal: Modeling neurovascular coupling is very important to understand brain functions, yet challenging due to the complexity of the involved phenomena. An alternative approach was recently proposed where the framework of fractional-order modeling is employed to characterize the complex phenomena underlying the neurovascular. Due to its nonlocal property, a fractional derivative is suitable for modeling delayed and power-law phenomena. Methods: In this study, we analyze and validate a fractional-order model, which characterizes the neurovascular coupling mechanism. To show the added value of the fractional-order parameters of the proposed model, we perform a parameter sensitivity analysis of the fractional model compared to its integer counterpart. Moreover, the model was validated using neural activity-CBF data related to both event and block design experiments that were acquired using electrophysiology and laser Doppler flowmetry recordings, respectively. Results: The validation results show the aptitude and flexibility of the fractional-order paradigm in fitting a more comprehensive range of well-shaped CBF response behaviors while maintaining a low model complexity. Comparison with the standard integer-order models shows the added value of the fractional-order parameters in capturing various key determinants of the cerebral hemody-namic response, e.g., post-stimulus undershoot. This investigation authenticates the ability and adaptability of the fractional-order framework to characterize a wider range of well-shaped cerebral blood flow responses while preserving low model complexity through a series of unconstrained and constrained optimizations. Conclusions: The analysis of the proposed fractional-order model demonstrates that the proposed framework yields a powerful tool for a flexible characterization of the neurovascular coupling mechanism.

Mechanisms of Venoarteriolar Reflex in Type 2 Diabetes with or without Peripheral Neuropathy.

Simple SummaryPostural changes induce arteriolar vasoconstriction in response to an increase in venous pressure in the limbs known as the venoarteriolar reflex (VAR). Previous studies reported that skin blood perfusion is impaired during the VAR in persons with type 2 diabetes mellitus (T2DM) with or without peripheral neuropathy, compared to control subjects. The aim of this study is to investigate the underlying mechanisms of the VAR in T2DM, with and without peripheral neuropathy. This study provides evidence that confirmed neuropathy alters the VAR by an alteration of the neurogenic response to leg dependency.The aim of this study is to investigate the underlying mechanisms of the venoarteriolar reflex (VAR) in type 2 diabetes mellitus (T2DM), with and without peripheral neuropathy. Laser Doppler flowmetry (LDF) recordings were performed on the medial malleus and dorsal foot skin, before and during leg dependency in healthy controls, in persons with obesity, in those with T2DM, in those with T2DM and subclinical neuropathy, and in those with T2DM and confirmed neuropathy. LDF recordings were analyzed with the wavelet transform to evaluate the mechanisms controlling the flowmotion (i.e., endothelial nitric oxide-independent and -dependent, neurogenic, myogenic, respiratory and cardiac mechanisms). Skin blood perfusion decreased throughout leg dependency at both sites. The decrease was blunted in persons with confirmed neuropathy compared to those with T2DM alone and the controls. During leg dependency, total spectral power increased in all groups compared to rest. The relative contribution of the endothelial bands increased and of the myogenic band decreased, without differences between groups. Neurogenic contribution decreased in controls, in persons with obesity and in those with T2DM, whereas it increased in subclinical- and confirmed neuropathy. In conclusion, this study provides evidence that confirmed diabetic neuropathy alters the VAR through the neurogenic response to leg dependency.

Minimizing the duration of laser Doppler flowmetry recordings while maintaining wavelet analysis quality: A methodological study

Recording oscillations in cutaneous blood perfusion using laser Doppler flowmetry (LDF) is of common use in clinical research, due, mainly, to its non-invasive nature. Detailed information about the vascular tone in different frequency bands and the related physiologic compartment can be inferred from wavelet analysis (WA) of an LDF signal. However, little is known about the minimal recording duration required to generate a proper representation of the frequency content of such signals. To address this question, the three lowest frequency bands (i.e. myogenic, neurogenic and endothelial) of the LDF wavelet power spectrum were studied to define the shortest recording duration needed to perform a quality WA. Sixty-five healthy participants were included for 20 min of basal LDF recording on forearm and dorsum of the foot. Recordings were short cut every minute and compared to the 20-min reference recording that is generally used in the literature.Our results indicate that minimum 10 min recordings are required for analysis of myogenic events, whatever the anatomical site. On the forearm, LDF recordings lengths should be at least 15 min and 10 min for the neurogenic and endothelial bands, respectively. On the foot, 13 min of LDF recording is required to assess neurogenic and endothelial bands.Depending on the aim of any given study involving healthy subjects, LDF recordings need to last at least 15 min on forearm and 13 min on the foot to maintain a quality WA on low-frequency bands.

Wavelet Analysis of the Temporal Dynamics of the Laser Speckle Contrast in Human Skin.

Spectral analysis of laser Doppler flowmetry (LDF) signals has been widely used in studies of physiological vascular function regulation. An alternative to LDF is the laser speckle contrast imaging method (LSCI), which is based on the same physical principle. In contrast to LDF, LSCI provides non-scanning full-field imaging of a relatively wide skin area and offers high spatial and temporal resolutions, which allows visualization of microvascular structure. This circumstance, together with a large number of works which had shown the effectiveness of temporal LSCI analysis, gave impetus to experimental studies of the relation between LDF and LSCI used to monitor the temporal dynamics of blood flow. Continuous wavelet transform was applied to construct a time-frequency representation of a signal. Analysis of 10 minute LDF and LSCI output signals recorded simultaneously revealed rather high correlation between oscillating components. It was demonstrated for the first time that the spectral energy of oscillations in the 0.01-2Hz frequency range of temporal LSCI recordings carries the same information as the conventional LDF recordings and hence it reflects the same physiological vascular tone regulation mechanisms. The approach proposed can be used to investigate speckle pattern dynamics by LSCI in both normal and pathological conditions. The results of research on the influence of spatial binning and averaging on the spectral characteristics of perfusion monitored by LSCI are of considerable interest for the development of LSCI systems optimized to evaluate temporal dynamics.

High-Resolution Laser Doppler Measurements of Microcirculation in the Deep Brain Structures: A Method for Potential Vessel Tracking

Background: Laser Doppler flowmetry (LDF) can be used to measure cerebral microcirculation in relation to stereotactic deep brain stimulation (DBS) implantations. Objective: To investigate the microcirculation and total light intensity (TLI) corresponding to tissue grayness in DBS target regions with high-resolution LDF recordings, and to define a resolution which enables detection of small vessels. Methods: Stereotactic LDF measurements were made prior to DBS implantation with 0.5-mm steps in the vicinity to 4 deep brain targets (STN, GPi, Vim, Zi) along 20 trajectories. The Mann-Whitney U test was used to compare the microcirculation and TLI between targets, and the measurement resolution (0.5 vs. 1 mm). The numbers of high blood flow spots along the trajectories were calculated. Results: There was a significant difference (p < 0.05) in microcirculation between the targets. High blood flow spots were present at 15 out of 510 positions, 7 along Vim and GPi trajectories, respectively. There was no statistical difference between resolutions even though both local blood flow and TLI peaks could appear at 0.5-mm steps. Conclusions: LDF can be used for online tracking of critical regions presenting blood flow and TLI peaks, possibly relating to vessel structures and thin laminas along stereotactic trajectories.

Noninvasive examination of endothelial, sympathetic, and myogenic contributions to regional differences in the human cutaneous microcirculation

The aim of this study was to examine whether there are regional differences in the cutaneous microvascular responses of the forearm and the leg. Utilizing a non-invasive measure (spectral analysis),we looked at the influence of the endothelial, sympathetic, and myogenic function between regions at thermoneutral conditions (33°C) and in response to local skin warming (42°C) using laser-Doppler flowmetry (LDF). We recruited 18 young, healthy participants, who visited the lab on 2 separate occasions. Participants were instrumented with LDF probes and local skin heater probe-holders, placed on the forearm or the leg. Blood pressure was recorded by oscillometry. At both 33°C and during local skin warming to 42°C, skin vasomotion for the forearm and leg were evaluated using spectral analysis of the LDF recordings. There were significant differences among all frequencies of interest between the forearm and the leg. At 33°C the leg presented with higher (P=0.003) activity for endothelial (0.009–0.021Hz), sympathetic (P=0.002) (0.021–0.052Hz), and myogenic (P=0.004) (0.052–0.145Hz) activity when compared to the forearm. In contrast, following 35min of local skin warming, the forearm had greater endothelial (P=0.019), sympathetic (P=0.006), and myogenic (P=0.005) vasomotion than the leg. These outcomes indicate regional differences in the cutaneous microcirculation. The current results are similar to our previous work using invasive methods and pharmacological agents, indicating that non-invasive analyses may be useful in the diagnoses and understanding of the mechanisms that control the microvascular function of pathological conditions.

Altered skin vasomotion in hypertensive humans (677.1)

Endothelium‐dependent cutaneous vasodilation (VD) is attenuated in essential hypertensive (HTN) humans. Using spectral analysis (fast Fourier transformation) we aimed to characterize vasomotion in laser Doppler flowmetry (LDF) recordings during local heating (LH)‐induced VD before and after inhibition of nitric oxide (NO)‐dependent signaling (NOS‐I: L‐NAME) in HTN and normotensive (NTN) humans. We hypothesized that HTN would have lower total power spectral densities (PSD), and in the frequency intervals (FI) associated with endothelial and neurogenic vascular control. LDF recordings during LH experiments from 18 HTN (MAP: 108±2mmHg) and 18 NTN (MAP: 88±2mmHg) men and women were analyzed. LH‐induced VD increased total PSD for all FI (all p&lt;0.001). HTN had a lower total PSD (HTN:30±6AU2, NTN:14±3AU2, p=0.03) and neurogenic FI (HTN:6±1AU2, NTN:17±3AU2, p&lt;0.01) versus NTN. As a percentage of total PSD, HTN had reduced neurogenic (HTN:41±3%, NTN:51±1%, p&lt;0.001) and higher myogenic contributions (HTN:29±3%, NTN:24±1%, p=0.04) to the total spectrum. NOS‐I decreased total PSD (p&lt;0.001) for both groups, but HTN exhibited lower endothelial (p&lt;0.01), neurogenic (p&lt;0.05), and total PSD (p&lt;0.001) compared to NTN. These data suggest that HTN results in altered neurogenic and NO‐dependent control of skin vasomotion and support the use of spectral analysis as a non‐invasive technique to study vasoreactivity.Grant Funding Source: Supported by R01‐HL093238‐02

Altered skin flowmotion in hypertensive humans

Essential hypertensive humans exhibit attenuated cutaneous nitric oxide (NO)-dependent vasodilation. Using spectral analysis (fast Fourier transformation) we aimed to characterize the skin flowmotion contained in the laser-Doppler flowmetry recordings during local heating-induced vasodilation before and after concurrent pharmacological inhibition of nitric oxide synthase (NOS) in hypertensive and age-matched normotensive men and women. We hypothesized that hypertensive subjects would have lower total power spectral densities (PSDs), specifically in the frequency intervals associated with intrinsic endothelial and neurogenic control of the microvasculature. Furthermore, we hypothesized that NOS inhibition would attenuate the endothelial frequency interval. Laser-Doppler flowmetry recordings during local heating experiments from 18 hypertensive (MAP: 108±2mmHg) and 18 normotensive (MAP: 88±2mmHg) men and women were analyzed. Within site NO-dependent vasodilation was assessed by perfusion of a non-specific NOS inhibitor (NG-nitro-l-arginine methyl ester; l-NAME) through intradermal microdialysis during the heating-induced plateau in skin blood flow. Local heating-induced vasodilation increased total PSD for all frequency intervals (all p<0.001). Hypertensives had a lower total PSD (p=0.03) and absolute neurogenic frequency intervals (p<0.01) compared to the normotensives. When normalized as a percentage of total PSD, hypertensives had reduced neurogenic (p<0.001) and augmented myogenic contributions (p=0.04) to the total spectrum. NOS inhibition decreased total PSD (p<0.001) for both groups, but hypertensives exhibited lower absolute endothelial (p<0.01), neurogenic (p<0.05), and total PSD (p<0.001) frequency intervals compared to normotensives. These data suggest that essential hypertension results in altered neurogenic and NOS-dependent control of skin flowmotion and support the use of spectral analysis as a non-invasive technique to study vasoreactivity.

Fisher information and Shannon entropy for on-line detection of transient signal high-values in laser Doppler flowmetry signals of healthy subjects

Laser Doppler flowmetry (LDF) is an easy-to-use method for the assessment of microcirculatory blood flow in tissues. However, LDF recordings very often present TRAnsient Signal High-values (TRASH), generally of a few seconds. These TRASH can come from tissue motions, optical fibre movements, movements of the probe head relative to the tissue, etc. They often lead to difficulties in signal global interpretations. In order to test the possibility of detecting automatically these TRASH for their removal, we process noisy and noiseless LDF signals with two indices from information theory, namely Fisher information and Shannon entropy. For this purpose, LDF signals from 13 healthy subjects are recorded at rest, during vascular occlusion of 3 min, and during post-occlusive hyperaemia. Computation of Fisher information and Shannon entropy values shows that, when calibrated, these two indices can be complementary to detect TRASH and be insensitive to the rapid increases of blood flow induced by post-occlusive hyperaemia. Moreover, the real-time algorithm has the advantage of being easy to implement and does not require any frequency analysis. This study opens new fields of application for Fisher information and Shannon entropy: LDF ‘denoising’.

Implant stability during osseointegration in irradiated and non‐irradiated minipig alveolar bone: an experimental study

Primary implant stability is related to local bone density. After insertion of an implant, implant stability is subject to changes due to bone remodeling. In patients who have undergone radiotherapy in the head and neck region, implant stability is impaired because irradiation reduces bone vitality. The current study was designed to monitor and test implant stability immediately after implant placement and during osseointegration in irradiated and non-irradiated minipig alveolar bone. All maxillary and mandibular premolars and molars of six adult Göttingen minipigs were extracted. The maxilla and mandible of three minipigs received three irradiation exposures at a total dose of 24 Gy. After irradiation, five initial implant holes were drilled in the residual alveolar ridge of each edentulous site. In order to assess bone vascularity, laser Doppler flowmetry recordings were carried out in the initial holes. A total of 120 implants were placed in the six minipigs. Subsequently, and at 8, 16, and 24 weeks after implant placement, implant stability was recorded by resonance frequency analysis (RFA). RFA values were expressed as an implant stability quotient (ISQ). ISQ values recorded immediately after implant placement showed no differences between irradiated and non-irradiated minipigs. Repeated measurements at the four recording moments showed a decrease of ISQ values in all minipigs, being more pronounced in irradiated bone, when compared with non-irradiated bone. The results at the third and fourth recording moments showed a stabilization or even a slight increase of ISQ values. The results document the negative effect of irradiation on bone vascularity and hence on implant stability.

Comparison of gingival blood flow during healing of simplified papilla preservation and modified Widman flap surgery: a clinical trial using laser Doppler flowmetry

This prospective randomized-controlled clinical trial compared the gingival blood flow responses following simplified papilla preservation (test) versus modified Widman flap (control). Twenty contra-lateral upper sites with pocket depth > or = 5 mm after initial treatment in 10 chronic periodontitis patients were randomly assigned to either test or control treatment, using a split-mouth design. Laser Doppler flowmetry recordings were performed pre-operatively, following anaesthesia, immediately post-operatively and on days 1, 2, 3, 4, 7, 15, 30 and 60, at nine selected sites per flap. Significant ischaemia was observed at all sites following anaesthesia and immediately post-operatively. At the mucosal flap basis, a peak hyperaemic response was observed on day 1, which tended to resolve by day 4 at the test sites, but persisted until day 7 at the control sites. The buccal and palatal papillae blood perfusion presented the maximum increase on day 7 in both groups and returned to baseline by day 15. Both surgical modalities yielded significant pocket depth reduction, recession increase and clinical attachment gain. Periodontal access flaps represent an ischaemia-reperfusion flap model. The simplified papilla preservation flap may be associated with faster recovery of the gingival blood flow post-operatively compared with the modified Widman flap.

Gingival blood flow changes following periodontal access flap surgery using laser Doppler flowmetry

To investigate the pattern of gingival blood flow changes following periodontal access flap surgery by laser Doppler flowmetry (LDF). Fourteen patients with chronic periodontitis presenting upper anterior sites with pocket depth >or=5 mm after initial treatment were included in the study. Periodontal access flap surgery was performed on the experimental areas and LDF recordings were taken at baseline, following anaesthesia, immediately postoperatively and on days 1, 2, 3, 4, 7, 15, 30 and 60 of healing, at nine predetermined sites per flap. Significant ischaemia was observed at all flap sites following anaesthesia and immediately postoperatively. At the alveolar mucosal sites, a peak increase of the gingival blood flow was observed on postoperative day 1 (p<0.001), which persisted until day 7 (p=0.012) and resolved by day 15. The mucosal sites close to the flap periphery presented higher blood perfusion compared with the sites located centrally in the flap. The microcirculatory perfusion of the buccal and palatal papillae was maximum on postoperative day 7 (p=0.013 and <0.001, respectively) and returned to baseline by day 15. Topographically distinct areas of the periodontal access flap consistently present different patterns of microvascular blood flow alterations during the wound-healing period.

Comparison between Hilbert–Huang transform and scalogram methods on non-stationary biomedical signals: application to laser Doppler flowmetry recordings

A significant transient increase in laser Doppler flowmetry (LDF) signals is observed in response to a local and progressive cutaneous pressure application on healthy subjects. This reflex may be impaired in diabetic patients. The work presents a comparison between two signal processing methods that provide a clarification of this phenomenon. Analyses by the scalogram and the Hilbert–Huang transform (HHT) of LDF signals recorded at rest and during a local and progressive cutaneous pressure application are performed on healthy and type 1 diabetic subjects. Three frequency bands, corresponding to myogenic, neurogenic and endothelial related metabolic activities, are studied at different time intervals in order to take into account the dynamics of the phenomenon. The results show that both the scalogram and the HHT methods lead to the same conclusions concerning the comparisons of the myogenic, neurogenic and endothelial related metabolic activities—during the progressive pressure and at rest—in healthy and diabetic subjects. However, the HHT shows more details that may be obscured by the scalogram. Indeed, the non-locally adaptative limitations of the scalogram can remove some definition from the data. These results may improve knowledge on the above-mentioned reflex as well as on non-stationary biomedical signal processing methods.

Evaluation of gingival blood flow by the use of laser Doppler flowmetry following periodontal surgery. A pilot study

The aim of this pilot study was to evaluate the applicability of laser Doppler flowmetry (LDF) in recording the gingival blood flow following periodontal surgery. Five patients suffering from advanced chronic generalized periodontitis were included in the study. After completion of basic periodontal therapy, full-mouth plaque score, bleeding on probing, gingival recession and clinical attachment level were recorded. The upper anterior areas with a pocket probing depth of 6 mm or more were treated with an access flap. LDF recordings were performed in both the buccal and palatal aspect of the operated areas with the aid of an individual acrylic stent at the day of the surgery, prior to local anaesthesia, 3 min following anaesthesia, immediately after the operation and at days 1, 2, 3, 4, 7, 15, 30 and 60 following operation. Overall, the blood flow decreased immediately following anaesthesia and remained in lower values compared to baseline immediately following operation. The gingival blood flow presented an overall increase in comparison to baseline values until the 7th day following surgery at the buccal and palatal interdental sites, as well as at the alveolar mucosa sites. By the 15th day, as well as at the following observation periods of 30 and 60 days, the gingival blood flow values at the palatal and alveolar mucosa sites were very similar to baseline. Increased blood flow changes were observed at 30 and 60 days following operation at the buccal interdental sites. The results of the present pilot study suggest that the LDF might present clinical applicability in recording changes in gingival blood flow following periodontal surgery.

Quantitative estimates of angiogenic and anti-angiogenic activity by laser Doppler flowmetry (LDF) and near infra-red spectroscopy (NIRS)

The use of laser Doppler flowmetry (LDF) and near-infra-red spectroscopy (NIRS) for non-invasive in vivo measurements of angiogenic and anti-angiogenic activity in nude mice was evaluated. Angiogenic foci were induced in the skin by implantation of slow release pellets containing 200 ng basic fibroblast growth factor (bFGF). LDF and NIRS recordings from induced foci were significantly higher than placebo implants (P<0.05) and controls (P<0.001), proving that LDF and NIRS provide measures of angiogenic activity. Correspondingly, by these methods, an anti-angiogenic activity was significantly demonstrated in bFGF-stimulated animals treated with either the specific anti-angiogenic compound TNP-470 (P<0.05) or the anti-inflammatory agent dexamethasone (P<0.001). We conclude that LDF and NIRS, alone or in combination, are useful non-invasive tools for early evaluation of angiogenic and anti-angiogenic activity in vivo.

Baroreceptor-mediated reduction of jejunal mucosal perfusion, evaluated with endoluminal laser Doppler flowmetry in conscious humans

Reduction of central blood volume elicits a peripheral vasoconstrictor reflex in various tissues including skin, skeletal muscle and the hepatomesenteric region. The aim of the present study was to investigate whether this reaction includes a decreased perfusion of the jejunal mucosa in man. Laser Doppler flowmetry (LDF) was used to monitor jejunal mucosal and skin perfusion simultaneously in eleven healthy volunteers. LDF recordings were performed during quiescent (phase 1) periods of the migrating motor complex. Seven subjects demonstrated cycling changes of jejunal mucosal perfusion (vasomotion). The average minimum jejunal flux value was 72±6 perfusion units. The average intraindividual coefficient of variation was 18±2%. Lower body negative pressure (LBNP) was used to elicit controlled reductions of central blood volume. LBNP of 10 mm Hg induced a 12±4% ( P<0.05) decrease in jejunal perfusion and a 43±11% ( P<0.001) decrease in cutaneous perfusion. Corresponding responses to LBNP of 20 mm Hg were 17±5% ( P<0.01) and 37±10% ( P<0.01) reductions in jejunal mucosal and skin perfusion, respectively. Cardiac index was significantly reduced by the LBNP procedure, whereas heart rate remained unchanged and blood pressure changes were minor and inconsistent. These findings indicate that the reflex vasoconstriction induced by mild central hypovolemia includes a significant reduction of jejunal mucosal perfusion in supine resting humans. This reflex may provide one mechanism for the intestinal ischemia often occurring in critically ill patients.

Comparison of the Effects of Cetirizine and Ebastine on the Skin Response to Histamine Iontophoresis Monitored with Laser Doppler Flowmetry

Background: The administration of histamine with iontophoresis is an alternative method to skin prick tests or intradermal injections. Skin reactions obtained with this method can be recorded with laser Doppler flowmetry (LDF) and previous studies with this method have shown histamine-induced laser Doppler changes in the wheal area. Objective: In order to compare the influence of two H₁ receptor antagonists (cetirizine 10 mg vs. ebastine 10 mg) on the skin vascular responses to histamine introduced by iontophoresis, we designed a double-blind, randomized, two-period crossover trial in which 18 volunteers were randomized. Methods: Before and 2, 5 and 7 h after drug administration, iontophoresis (30 s, 1.4 mA/cm²) of histamine 10% was performed and followed by (1) monitoring of skin vascular responses with LDF at the administration site and at 1 cm from it, and (2) wheal and flare area measurements. Results: 2, 5 and 7 h after intake of the antihistaminic drug, there were significant differences between both drugs. Concerning LDF recordings, we noted at the histamine administration site an increase in perfusion unit (PU) values which is an effect known to be in proportion to the degree of inhibition of wheal reaction, and at 1 cm distal to the histamine administration site, there was a decrease in PU values. These changes were more marked under cetirizine. A greater suppressive effect of cetirizine on the wheal and flare reaction was consistently observed at all time points during the study, demonstrating its superior efficacy. Conclusion: We conclude that (1) cetirizine demonstrated a stronger antihistaminic effect compared to ebastine at all time points; (2) iontophoresis appears to be an appropriate method to study specific microvascular changes at the delivery site of histamine and hence to detect the earliest changes occurring at the site of agonist-antagonist competition in the skin.

Intraoperative improvement of somatosensory evoked potentials and local spinal cord blood flow in patients with syringomyelia.

This report is the first to correlate data concerning intraoperative somatosensory evoked potentials (SSEPs) and local spinal cord blood flow (ISCBF) in patients with syringomyelia. In a consecutive study, bilateral median nerve SSEPs were recorded intraoperatively in 13 patients undergoing a syrinx shunt to the posterior fossa cisterns (syringocisternostomy). ISCBF was measured in five of these patients using laser doppler flowmetry (LDF) calibrated in arbitrary units (AU). SSEP recordings obtained 30 min after syrinx decompression demonstrated a slight but consistent reduction of N20 latencies (mean change: 0.53 ms right, p < 0.003; 0.58 ms left, p < 0.001) concurrent with a similar but less consistent increase of N20 amplitudes (0.16 mV right, p = 0.256; 0.29 mV left, p = 0.03). Prior to shunting, LDF recordings from the spinal cord overlying syrinxes revealed very low ISCBF values in five of five patients (mean LDF, 13.2 AU +/- 15.3 SD). Immediately after shunting, there was a dramatic rise of ISCBF (mean LDF, 241.2 AU +/- 106.3 SD) associated with visualized hyperemia of the spinal cord and pial vessels. The ISCBF fell to intermediate levels after 2 min (157.2 AU +/- 33.0 SD) and remained at these levels during the interval of recording (5 min). Hyperventilation testing in two patients prior to shunting revealed no change in ISCBF consistent with a loss of CO2 vascular reactivity and a paradoxical increase of ISCBF in one patient 5 min after shunting. Each patient in this study experienced neurological improvement in the immediate postoperative period associated with collapse or disappearance of the syrinx on magnetic resonance imaging scans. Because syrinx shunting results in an acute decompression of the distended spinal cord, it is possible that the rapid improvement of SSEPs reflects a relief of mechanical factors such as stretching and compression of nervous tissue. However, the LDF findings in this study suggest that distended spinal cord cavities are also capable of producing regional ischemia. A significant reduction of ISCBF is a possible contributing cause of neurological injury and SSEP abnormalities. Intraoperative improvement of SSEPs and ISCBF were found to correlate well with neurological recovery following syringocisternostomy. Our results indicate that SSEP monitoring can provide useful information during surgical procedures for syringomyelia and that further experience with LDF monitoring may provide insights into the pathophysiology of this condition.
