A 9-month-old child had a large liver mass and elevated serum alpha-fetoprotein. Computed tomography demonstrated a large lobulated liver mass with calcification that on biopsy proved to be a hepatoblastoma. Bone imaging identified increased uptake in the liver at the site of the hepatoblastoma. Calcification is commonly seen on CT imaging of hepatoblastomas but is very rarely identified on isotope bone imaging. Hepatoblastoma only rarely metastasizes to bone; most often it metastasizes to the lungs. Neuroblastoma commonly shows uptake on bone imaging and may metastasize to the liver. This differential diagnosis was excluded by the biopsy.
