Autologous ear cartilage is relatively easy to procure in suitable form and sufficient quantity. At the donor site, there are, as a rule, no complications to be expected. Autologous ear cartilage without conjunctival coverage is of good use for replacing and lengthening the inner layer of an eyelid, not only to reconstruct large eyelid-defects, but also to give additional support and length. The cartilage graft will grow on well, provided the wound is well vascularized. This must not be expected with scarring or poor blood perfusion. The brittleness of the cartilage demands an adapted technique. Precautions to protect the cornea and to prevent adhesions in the fornix are to be taken.

Histology of Partial Thickness Double Composite Eyelid Graft

Double composite grafting is a technique used in the repair of large (greater than 75%) eyelid defects due to tumor resection, trauma, or congenital coloboma. The procedure utilizes two partial thickness composite grafts derived from two unaffected lids. The skin defect is covered by a pedicle skin graft to provide an adequate blood supply. This is thought to be superior to revascularization occurring from the margin of the graft alone. We examined histologically the lid margins of two such grafts 2 and 9 months postoperatively. Each demonstrated scarred tarsus and absent meibomian glands. One of the two grafts had loss of cilia. The architecture of the graft tissue otherwise was preserved.

Free Autogenous Grafts in Eyelid Tarsoconjunctival Reconstruction

Classical techniques of reconstruction of large eyelid defects require temporary occlusion of the eye. Use of a free autogenous tarsoconjunctival graft with a skin-muscle flap is described. Twelve patients were operated with ten good, two fair, and no poor results. This method can be used to repair medial, lateral, and central defects of the upper and lower eyelids.
