Abstract 047: Solumbra ‘PINCHING’ technique for medium vessel occlusions (MeVOs): A Single Center Experience

Introduction Various techniques have been evolved and used by different operators for mechanical thrombectomies for improved procedural performance, lower intraoperative times and better recanalization rates The widely used, isolated stent‐retriever and contact aspiration technique for medium vessel occlusions (MeVOs) carries a high risk for clot fragmentation, spasm and distal migration. The stent–aspiration (Solumbra) technique uses a large bore aspiration catheter with a stent retriever device for potential synergistic effects. However, its use is limited in distal thrombectomies. Hereby, we assess the efficacy and safety of solumbra ‘Pinching’ technique by using a smaller profile aspiration catheter for MeVO thrombectomy. Methods We retrospectively analyzed our prospective institutional database for medium vessel occlusions (M2, M3 MCA/ A2, A3 ACA/ P2 PCA) who underwent endovascular mechanical thrombectomy using stent retriever or aspiration or combined (solumbra) technique. The technique involves deploying a stent retriever through microcatheter and large bore aspiration catheter like ACE 68 (Penumbra, Inc.), followed by both microcatheter and aspiration removal, clot localization and blind navigation of a correctly sized lower profile (3MAX or 4MAX, Penumbra, Inc.) aspiration catheter over stent bare wire to “pinch” the primal end of the clot. Besides various demographic parameters, we collected final recanalization rates with first pass effect [using modified Thrombolysis in Cerebral Infarction (TICI) score]. All the intra‐procedural complications (haemorrhage, vasospasm) were analysed. Clinical outcome was analysed using modified ranking score (mRS) on 7th day post procedure/ discharge(whichever is earlier) and at 90 days. Results Total 26 MeVOs were successfully treated in twenty‐five patients with solumbra ‘Pinching’ technique since January 2022 (Male: female 16:7, mean age 58.6 years, mean NIHSS 9.1). Majority of the procedures were done for primary MeVOs and 72% consisted of M2 MCA occlusions. Whether to use a 3MAX or 4MAX aspiration catheter was determined by eyeballing the appropriate calibre for target vessels. Final first pass mTICI 2c/3 recanalization was achieved in 24 patients (88%). Minor Intra‐procedural complications were observed in 13 patients. Favorable modified ranking score (mRS 0‐2) was achieved in approximately 80% patients at the end of 7 days/discharge and 14 out of 16 follow up patients achieved good clinical outcome at 90 days Conclusion Solumbra ‘Pinching’ technique, using a low profile aspiration catheter like 3 MAX or 4 MAX (Penumbra, Inc.) with a stent retriever, can be effectively used in management of acute ischaemic stroke secondary to a MeVOs with significantly enhancing the first‐pass recanalization with lesser risk of intimal injury, dissection and secondary embolization.

Abstract 248: Structural Analysis of Aspiration Catheters and Procedural Outcomes: An Analysis of the SVIN Registry

Introduction Rapid expansion of mechanical thrombectomy and swift manufacturing development has translated into significant evolution of large bore catheter technology. The objective of this study was to evaluate the association among diverse structural components of large bore aspiration catheters on procedural performance. Methods Retrospective analysis of a prospectively maintained mechanical thrombectomy consortium (SVIN Registry) treated with stand‐alone contact aspiration for the first pass in the MCA M1 or intracranial ICA occlusions from 2012‐2021. Catheters were stratified based on construction materials, tip technology, catheter sizing, and catheter lining. Factors associated with first pass effect (FPE ‐ first‐pass eTICI2c‐3 reperfusion) as well as speed of clot engagement were analyzed. Results We identified 983 patients with proximal occlusion and aspiration as the first pass technique. FPE was observed in 34% and associated with age (OR:1.016;95%CI:1.006‐1.027), cardioembolic stroke etiology (OR:1.685;95%CI:1.77 ‐2.41), MCA M1(OR:2.737;95%CI:1.091 ‐1.867), non‐general anesthesia (OR:0.546;95%CI:0.389 ‐0.767), as well as with 0.070” (OR:2.038;95%CI:1.099 ‐3.779) and 0.088” (OR:3.899;95%CI:1.582 ‐9.606) distal catheter inner diameter in the adjusted analysis. Median time from arterial access to clot contact was 17 minutes with faster times observed in younger patients (OR:0.986;95%CI:0.975 ‐0.996) as well as with the use of aspiration catheters with shorter length of distal outer hydrophilic coating (18‐30cm) on multivariable regression (OR:0.303; 95%CI:0.113‐0.816). Conclusion Larger aspiration catheter distal inner diameter was associated with higher rates of FPE. Aspiration catheters with reduced distal tip hydrophilic coating were associated with faster arterial access to clot contact time.

Improved first-pass effect in acute stroke thrombectomy using Solitaire-X compared to Solitaire-FR.

In acute ischemic stroke (AIS), successful endovascular thrombectomy (EVT) of large vessel occlusion (LVO) necessitates the most suited device. Solitaire-X has longer and larger diameter pusher wires than Solitaire-FR.As the role of a larger pusher-wire diameter is uncertain, we aim to compare procedural, clinical, and radiological outcomes for AIS patients undergoing EVT using either type of Solitaire device. Procedures were performed using the Solumbra technique, which combines a large-bore aspiration catheter with a stentriever. The primary outcome was to compare rates of successful first-pass recanalization (defined as TICI 2b/3 score). The secondary objectives were procedural (rates of successful recanalization), clinical (post-procedural NIHSS and days of hospitalization), and radiological (post-procedural ASPECT score and hemorrhagic transformation) outcome measures. Consecutive AIS patients undergoing EVT for LVO were recruited into a prospective multicenter database at our academic center. We have used Solitaire-FR until October 2020 and Solitaire-X ever since. We retrospectively analyzed our prospective consecutive registry. Included in our analysis are patients undergoing EVT using Solitaire only; patients with tandem lesions or underlying stenosis requiring emergent stenting during the procedure were excluded. The cohort of patients treated with Solitaire-X was compared with a cohort consisting of the most recent consecutive cases undergoing EVT with the Solitaire-FR. A total of 182 (71.9 ± 14, 61% male patients) AIS patients were included in the analysis with both groups (n = 91 each) sharing similar demographic characteristics, premorbid conditions, and stroke characteristics (time from symptom-onset, NIHSS, ASPECTS, occlusion site, and rates of intravenous-tPA treatment). The Solitaire-X group had a higher rate of first-pass recanalization (65.9% vs. 50.5%, p = 0.049). On 24-h post-procedural head-CT, the Solitaire-X group had higher ASPECT scores (6.51 ± 2.9 vs. 5.49 ± 3.4, p = 0.042) and lower post-procedural average bleeding volumes (0.67 ± 2.1 vs. 1.20 ± 3.4 mL, p = 0.041). The Solitaire-X group had shorter duration of hospitalization (16.6 ± 13.1 days vs. 25.1 ± 23.2, p = 0.033). On multivariate analysis, using Solitaire-X was the sole independent predictor of first-pass recanalization (OR 2.17, 95% CI 1.12-4.26, p = 0.023). In our study, the use of the Stentriever-X with a larger pusher-wire diameter was associated with a higher likelihood of first-pass effect and improved procedural, clinical, and radiological outcomes in AIS patients.

First experience with Walrus balloon guide catheter in a whole-body flow model

PurposeFlow arrest using a balloon guide catheter (BGC) in mechanical thrombectomy (MT) due to large vessel occlusion has been associated with better outcomes. Known limitations of currently commercially available BGCs are incompatibility with large bore aspiration catheters (AC) and lack of distal flexibility. Walrus presents variable stiffness and compatibility with large bore AC. The goal of this study is to describe the first experience with Walrus in a realistic stroke simulation model.MethodsA full-length modular vascular model under physiological conditions was used. 8F+-Walrus inner-diameter (ID) 0.087in 95 cm combined with 6F-Sofia AC ID 0.070in 131 cm and an 8F-Flowgate2 BGC ID 0.084in 95 cm with a 5F-Sofia AC ID 0.055in 125 cm were used to perform aspiration MT. User surveys, access to target and occlusion site, technique, time of delivery, anatomical change, and catheter kick-back were assessed.ResultsSeven neuroradiologists with average of 10 years-experience in MT performed primary aspiration using the above-mentioned combinations in three different anatomies (N = 41). All operators would likely (29%) or very likely (71%) use again Walrus in combination with large bore AC and the majority (86%) found its navigability easier than with other BGCs. Time to reach final BGC position and catheter kick-back did not differ significantly among anatomies or catheter combinations (p > 0.05). However, Walrus was more likely to reach ICA petrous segment (p < 0.05) and intracranial occlusion with AC (p < 0.01).ConclusionThe Walrus combined with large bore AC presented significantly better distal access and navigability for primary aspiration in an in vitro stroke model.

Improved catheter delivery for aspiration thrombectomy using Tenzing 7 ledge reducing catheter and FreeClimb 70.

Previous comparative mechanical thrombectomy device trials reported a substantial crossover rate from first-line aspiration to stent-retriever thrombectomy. A specialized delivery catheter may help track large-bore aspiration catheters to target occlusions. We report our multicenter experience of aspiration thrombectomy of intracranial large vessel occlusions using the FreeClimbTM 70 and Tenzing® 7 delivery catheter (Route 92, San Mateo, CA). After local Institutional Review Board approval, we retrospectively reviewed the clinical, procedural, and imaging data of patients who underwent mechanical thrombectomy with the FreeClimb 70 and Tenzing 7. FreeClimb 70 was successfully delivered using Tenzing 7 to target occlusion in 30/30 (100%) patients (18 M1, 6 M2, 4 ICA-terminus, and 2 basilar artery occlusions), without the use of a stent-retriever for anchoring. In 21/30 (70%) cases, a leading microwire was not needed to advance the Tenzing 7 to the target. Median (interquartile range) time from groin puncture to first pass was 12 (interquartile range 8-15) minutes. Overall first pass effect, or first pass effect (modified thrombolysis in cerebral ischemia 2C-3), was achieved 16/30 (53%). For M1 occlusions, first pass effect was 11/18 (61%). Successful reperfusion (modified thrombolysis in cerebral ischemia ≥ 2B) was achieved in 29/30 (97%) cases after a median of 1 pass (interquartile range 1-3). Median groin puncture to reperfusion time was 16 (interquartile range 12-26) minutes. There were no procedural complications or symptomatic intracranial hemorrhage. Average improvement in National Institutes of Health Stroke Scale at discharge was 6.6 ± 7.1. There were three patient deaths (renal failure, respiratory failure, and comfort care). Initial data support the use of Tenzing 7 with FreeClimb 70 catheter for reliable access to rapid, effective, and safe aspiration thrombectomy of large vessel occlusions.

Porównanie in vivo cewników aspiracyjnych o różnej średnicy u pacjentów z zawałem mięśnia sercowego z uniesieniem odcinka ST

WstępTrombektomia aspiracyjna jest zalecana jedynie u wybranych pacjentów z zawałem mięśnia sercowego z uniesieniem odcinka ST (&lt;i&gt;ST elevation myocardial infarction&lt;/i&gt; – STEMI). Analiza z wykorzystaniem optycznej tomografii koherencyjnej wykazała ograniczenia niektórych cewników aspiracyjnych w usuwaniu skrzepliny. Ponadto testy &lt;i&gt;in vitro&lt;/i&gt; wskazują na istotne różnice w ich skuteczności zależnie od konstrukcji końcówki dystalnej oraz wewnętrznej średnicy. Celem pracy jest porównanie skuteczności przezskórnej interwencji wieńcowej (&lt;i&gt;percutaneous coronary intervention&lt;/i&gt;&lt;i&gt;&lt;b&gt;&lt;/b&gt;&lt;/i&gt; – PCI) z zastosowaniem trombektomii aspiracyjnej oraz bez jej użycia, a także sprawdzenie skuteczności różnych cewników w zależności od ich wewnętrznej średnicy.Materiał i metodyBadaniem objęto 773 chorych ze STEMI: 586 chorych tylko po PCI (grupa Ctrl) oraz 187 chorych po PCI i trombektomii. Cewniki podzielono na te o dużym otworze (&lt;i&gt;large-bore&lt;/i&gt; – LB), zastosowane u 146 pacjentów (grupa LB), oraz te o wąskim otworze (&lt;i&gt;small-bore&lt;/i&gt; – SB) – u 41 pacjentów (grupa SB). Porównano perfuzję mięśnia sercowego, frakcję wyrzutową lewej komory (&lt;i&gt;left ventricular ejection fraction&lt;/i&gt; – LVEF) i długoterminową śmiertelność z jakiejkolwiek przyczyny. Analizę statystyczną oparto na nieparametrycznej analizie przeżycia ANOVA oraz Kaplana i Meiera.WynikiGrupy charakteryzowała stronniczość wyboru w odniesieniu do danych demograficznych oraz prezentacji angiograficznej zawału mięśnia sercowego. W porównaniu z grupą Ctrl grupy LB i SB były młodsze, prezentowały wolniejszy przepływ nasierdziowy i większy materiał zakrzepowy w klasyfikacji TIMI (Thrombolysis in Myocardial Infarction). W grupie SB, w porównaniu z grupą Ctrl, obserwowano nieistotny trend w kierunku gorszej perfuzji mięśnia sercowego. Grupa LB miała niższą LVEF przed wypisem w porównaniu z grupą Ctrl. Długoterminowa umieralność z przyczyn ogólnych była porównywalna we wszystkich grupach.WnioskiNie mamy wystarczających dowodów na to, że cewniki SB są mniej skuteczne. Pozytywną obserwacją jest fakt, że długoterminowa śmiertelność z jakiejkolwiek przyczyny w grupie pacjentów po trombektomii aspiracyjnej jest porównywalna z tą w grupie kontrolnej, wymaga ona jednak potwierdzenia w przyszłych badaniach.

Initial Results of a Direct Aspiration First-Pass Technique to Treat Acute Ischemic Stroke Patients in Nepal.

Objective Endovascular therapy has become the mainstay of treatment of acute ischemic stroke (AIS) due to large vessel occlusion. A direct aspiration first-pass technique (ADAPT) using large bore aspiration catheters has been introduced as a rapid, simple method for achieving good revascularization and good clinical outcomes. The aim of this study was to assess the safety and efficacy of ADAPT in the treatment of AIS due to large-vessel occlusion in the Nepali patient population. Materials and Methods Retrospective data were collected for all consecutive patients treated for AIS with ADAPT from March 2019 through January 2021 at two hospitals. Outcomes were successful revascularization (modified thrombolysis in cerebral infarction score of 2b-3), time to revascularization, procedural complications, and good clinical outcome (modified Rankin Scale score of 0 to 2) and mortality at 90 days. Statistical Analysis Retrospective data were collected and descriptive statistics were calculated. Results Sixty-eight patients treated for AIS with ADAPT were included. The median National Institutes of Health Stroke Scale score at presentation was 13 (IQR 10-13.25). The median time from arterial puncture to revascularization was 40 minutes (IQR 30-45). Successful revascularization was achieved in 54 patients (79.4%). No cases of symptomatic intracranial hemorrhage occurred. At 90-day follow-up, good clinical outcome was achieved in 57 patients (83.8%), and 4 patients died (5.9%). Conclusion A direct aspiration first pass technique appears to be a fast, simple, safe, and effective method for the management of AIS in the Nepali patient population.

Abstract Number ‐ 73: Successful Endovascular Treatment of Medically Refractory CVST Using a Large Bore Distal Platform Thrombo‐aspiration Catheters.

Introduction Cerebral venous sinus thrombosis (CVST) is a rare (0.5‐2/100,000 incidence) but potentially fatal (5% mortality rate) neurological condition. The diagnosis may be delayed due to the non‐specific presenting symptoms such as headache, altered mental status, or seizures. Anticoagulation has long been the standard therapy for CVST. However, a subset of patients do not respond to medical therapy, and may develop complications such as ischemic stroke, hemorrhage, and cerebral edema. In the ISCVT study, 13% of patients experienced poor outcomes despite anticoagulation. Recently, it has been shown that patients with extensive clot burden, cerebral edema, elevated ICP, and altered mentation may achieve better outcomes with endovascular thrombectomy (EVT) compared to anticoagulation. We present a patient with CVST who failed therapeutic anticoagulation but improved with thrombectomy using the novel Zoom 088 Aspiration system, that further demonstrating the efficacy of combination therapy with anticoagulation and EVT. Methods A case report of successful CVST treatment using large bore aspiration catheters. Results A 37‐year‐old woman with a history of migraine and current oral contraceptive use presented with acute, persistent, and severe right hemicranial and retro‐orbital headache. Non‐contrast CT head suggested thrombosis of the right transverse sinus. MR venogram confirmed CVST involving the right transverse sinus, sigmoid sinus, and internal jugular vein. We started therapeutic anticoagulation, but the patient’s headache progressively worsened. Repeat MR venogram revealed extension of the CVST. Given clinical and radiological worsening, we proceeded with EVT procedure using a Zoom 088 aspiration system, resulting in successful recanalization of the involved sinuses. However, there was persistent severe headache following the procedure. She developed a new long segment obstructive thrombus in the left transverse sinus and partial re‐occlusion of the right sigmoid sinus on repeat MR venogram. A second EVT procedure was performed using the Zoom 088 Aspiration system, with complete recanalization. Postoperatively, her symptoms resolved and at 3‐month follow‐up she remained largely asymptomatic without neurological deficits. To the best of our knowledge, this is the first reported successful clinical use of the Zoom 088 aspiration device for CVST. The Zoom 088 Aspiration system is a newer generation large distal platform thrombo‐aspiration catheter. Studies have established efficacy of contact aspiration and stent retriever technique for arterial thrombi. Venous sinus clots are loosely bound to the vessel wall, perhaps making them more readily removable through aspiration. However, its standard application for CVST, including its efficacy and complication rates, remain to be analyzed in larger prospective studies. Conclusions This case highlights the pivotal role of EVT, especially large‐bore catheter based aspiration approach, in the management of medically refractory CVST. Thrombo‐aspiration technology is evolving at an accelerated rate with versatile and navigable devices such as the Zoom 088 aspirator. Larger studies are warranted to establish its technical efficacy and safety profile.

In Vitro Comparison of Several Thrombus Removal Tools.

Although the routine use of thrombus aspiration is not recommended, the thrombectomy technique still might be considered for a selected population of patients. Therefore, the assessment of the effectiveness of commercially available thrombectomy devices is still clinically relevant. Here, we present an in vitro comparison of several different types of catheters that can be used for thrombus aspiration or removal. Through the removal of 6 h and 24 h human blood clots in an in vitro model, four catheters were compared: the Launcher, Pronto V4, Vasco+ and the stent-retriever Catchview. The aspiration efficacy was expressed as a percentage of the initial thrombus weight. The effectiveness of the patient's aspiration was dependent on the time of thrombus formation and was significantly higher for a thrombus formed over 24 h (58.5 ± 26.5%) than for one formed over 6 h (48.0 ± 22.5%; p < 0.001). In the presented in vitro model, Pronto V4 and Launcher showed the highest efficiency. Large-bore aspiration catheters were found to be more effective than narrow-bore catheters or stent-retrievers in an in vitro model of thrombus removal. The thrombus aspiration efficacy increases with longer thrombus formation times.

Abstract 98: The Impact Of Aspiration Catheter Size On Thrombectomy Outcomes Using Adapt Technique-analysis From The STAR Registry

Introductions: Clinical trials have shown that aspiration thrombectomy is as safe and effective as stent-retriever thrombectomy. Multiple improvements have been made to the aspiration technique over the last few years. In this study, we aim to assess the effect of aspiration catheter bore size on the outcomes of A direct aspiration first pass technique (ADAPT) thrombectomy. Methods: We included patients who underwent ADAPT thrombectomy for M1 or internal carotid artery terminus (ICA-T) occlusions in the Stroke Thrombectomy and Aneurysm (STAR) database. Patients included between July 2016 and July 2022. We compared baseline characteristics, procedural metrics and outcomes between patients who underwent thrombectomy using small bore (0.035”-0.060”), medium bore (0.062”-0.068”) and large bore (0.070”-0.074”) catheters. Results: A total of 1158 patients were included; 576 (49.7%) females, 645 (70%) White, and 464 (40.6%) received IV-tPA. No difference was noticed in age, sex, and vascular risk factors between the 3 different groups. There was higher rate of IV-tPA in the small-bore catheter group (48.8%) compared to the medium and large bore catheter groups (38.4% and 36.7%, respectively) (P=0.03). Procedure duration was shorter when using medium (20 min) and large (18 min) compared to small bore catheters (30 min) (P=0.01). Both medium and large bore catheters were associated with higher rate of successful recanalization (88.9% and 87.9%, respectively) compared to small bore catheters (81.6%) (P=0.010). However, the difference in successful recanalization or procedure duration between medium and large bore catheters was not significant. No difference was noted in the rate of symptomatic hemorrhagic transformation (sICH) (4.7%, 5.3%, and 7.1%; P=0.345), 90-day favorable outcome (modified Rankin Scale 0-2) (41.8%, 39.3%, 40.8%; P=0.766) or 90-day mortality (18.1%, 23.5%, 24.4%; P=0.111) between the groups. Conclusions: Higher rate of successful recanalization and shorter procedure duration were observed when using medium and large bore aspiration catheters compared with small bore catheters in ADAPT technique. However, these procedural benefits were not observed when comparing large bore to medium bore catheters.

Contact aspiration and stent retriever versus stent retriever alone following mechanical thrombectomy for patients of acute ischemic stroke: A recanalization success analysis

ObjectiveSecond-generation catheters used in mechanical thrombectomy have different advantages and disadvantages. The objective of this study was to evaluate the effectiveness and safety of the combination of contact aspiration and stent retriever technique on the rate of reperfusion after mechanical thrombectomy for large vessel occlusion. MethodsPatients who underwent contact aspiration alone (CAA cohort, n = 150), stent retriever alone (SRA cohort, n = 129), or combined contact aspiration and stent retriever (CSR cohort, n = 122) techniques following mechanical thrombectomy were included in the analysis. A balloon guide catheter was used for all thrombectomies. Digital subtraction angiography was used to identify thrombolysis in cerebral infarction. ResultsThe number of patients with thrombolysis in cerebral infarction score of ≥ 2c (near complete or complete antegrade reperfusion) was significantly higher in the CSR cohort than those in the CAA cohort (101 [83%] vs. 90 [60%], p < 0.0001) and those of SRA cohort (101 [83%] vs. 77 [59%], p = 0.0001). Arterial perforation was higher in patients in the CSR cohort than in those in the CAA (p < 0.0001) and SRA (p = 0.015) cohorts. Intracerebral hemorrhage was lower in patients in the CSR cohort than in those in the CAA (p = 0.0001) and SRA (p = 0.0353) cohorts. All-cause mortality at 1 year was fewer in the CSR cohort than in the CAA cohort (p = 0.018). ConclusionsThe combination of thrombo aspiration by large bore aspiration catheter and stent retriever is the most effective technique but has some related risks. Level of evidenceIV. Percutaneous Large-Bore Pulmonary Thrombectomy with the FlowTriever Device: Initial Experience in Intermediate-High and High-Risk Patients

ObjectivesThis retrospective cohort study investigates outcomes of patients with intermediate-high and high-risk pulmonary embolism (PE) who were treated with transfemoral mechanical thrombectomy (MT) using the large-bore Inari FlowTriever aspiration catheter system.Material and MethodsTwenty-seven patients (mean age 56.1 ± 15.3 years) treated with MT for PE between 04/2021 and 11/2021 were reviewed. Risk stratification was performed according to European Society of Cardiology (ESC) guidelines. Clinical and hemodynamic characteristics before and after the procedure were compared with the paired Student’s t test, and duration of hospital stay was analyzed with the Kaplan–Meier estimator. Procedure-related adverse advents were assessed.ResultsOf 27 patients treated, 18 were classified as high risk. Mean right-to-left ventricular ratio on baseline CT was 1.7 ± 0.6. After MT, a statistically significant reduction in mean pulmonary artery pressures from 35.9 ± 9.6 to 26.1 ± 9.0 mmHg (p = 0.002) and heart rates from 109.4 ± 22.5 to 82.8 ± 13.8 beats per minute (p < 0.001) was achieved. Two patients died of prolonged cardiogenic shock. Three patients died of post-interventional complications of which a paradoxical embolism can be considered related to MT. One patient needed short cardiopulmonary resuscitation during the procedure due to clot displacement. Patients with PE as primary driver of clinical instability had a median intensive care unit (ICU) stay of 2 days (0.5–3.5 days). Patients who developed PE as a complication of an underlying medical condition spent 11 days (9.5–12.5 days) in the ICU.ConclusionIn this small study population of predominantly high-risk PE patients, large-bore MT without adjunctive thrombolysis was feasible with an acceptable procedure-related complication rate.

Stand-Alone Large Bore Aspiration Catheter (0.072 Inch) for Both Guide Support and Clot Aspiration in Transradial Posterior Circulation Stroke Thrombectomy: Technical Series.

The large size of guide catheters in the traditional triaxial configuration can prove limiting during transradial vertebrobasilar thrombectomy. This is especially important for the direct aspiration technique because of the large aspiration catheters that can reach an inner diameter of 0.072 in. A strategy that strikes a balance between stable proximal vessel support and distal navigation for aspiration is conceptually attractive. To describe a series of transradial posterior circulation thrombectomy procedures in which the aspiration catheter served a dual role of guide support and clot aspiration in a coaxial configuration, thus obviating a larger guide catheter. Patients selected in the series underwent radial artery access and direct over-the-wire navigation of the aspiration catheter into the vertebral artery. With coaxial microcatheter navigation, the aspiration catheter reached distal enough to ingest the clot successfully. Along with clinical and angiographic data, imaging features such as angle of vertebral artery origin were calculated. Five patients underwent a stand-alone aspiration catheter technique for basilar artery occlusion through transradial access. All procedures resulted in thrombolysis in cerebral infarction 3 recanalization. The mean time to basilar artery recanalization was 10 minutes. No access site complications or vertebral artery dissection were noted. The mean subclavian artery-vertebral origin angle was 84.06° (range 78.2-90.2°). For patients selected properly based on vascular anatomy and a careful technique, a large bore aspiration catheter can fulfil a stand-alone dual-role, thus obviating the need for a guide catheter. This can potentially improve the technical feasibility and success of transradial vertebrobasilar thrombectomy.

Effectiveness of Anchoring with Balloon Guide Catheter and Stent Retriever in Difficult Mechanical Thrombectomy for Large Vessel Occlusion.

ObjectiveA distal navigation of a large bore aspiration catheter during mechanical thrombectomy (MT) is important. However, delivering a large bore aspiration catheter is difficult to a tortuous or atherosclerotic artery. We report the experience of anchoring with balloon guide catheter (BGC) and stent retriever to facilitate the passage of an aspiration catheter in MT. MethodsWhen navigating an aspiration catheter failed with a conventional co-axial microcatheter delivery, an anchoring technique was used. Two types of anchoring technique were applied to facilitate distal navigation of a large bore aspiration catheter during MT. First, a passage of aspiration catheter was attempted with a proximal BGC anchoring technique. If this technique also failed, another anchoring technique with distal stent retriever was tried. Consecutive patients who underwent MT with an anchoring technique were identified. Details of procedure, radiologic outcomes, and safety variables were evaluated. ResultsA total of 67 patients underwent MT with an anchoring technique. Initial trial of aspiration catheter passage with proximal BGC anchoring technique was successful for 35 patients (52.2%) and the second trial with distal stent retriever anchoring was successful for 32 patients (47.8%). Overall, navigation of a large bore aspiration catheter was successful for all patients (100%) without any procedure related complications. ConclusionOur study showed the usefulness of anchoring technique with proximal BGC and distal stent retriever during MT, especially in those with an unfavorable anatomical structure. This technique could be an alternative option for delivering an of aspiration catheter to a distal location.

A Prospective, Multicenter, Single-Group Target-Value Clinical Trial to Evaluate the Safety and Efficacy of a Large Bore Aspiration Catheter System for the Endovascular Treatment of Acute Ischemic Stroke.

ObjectiveThis study was conducted to determine the safety and efficacy of the Esperance® Distal Access Catheter (0.071”), a novel large bore aspiration catheter in treating acute ischemic stroke (AIS) with large vessel occlusion (LVO).MethodsA prospective multicenter clinical trial involving 15 stroke centers was performed. Baseline characteristics, procedural data, and angiographic and clinical outcomes of all acute stroke procedures (from May 2020 to March 2021) using the novel large bore aspiration catheter were analyzed.ResultsDuring the study period, 160 consecutive patients were recruited. The mean age and median baseline NIHSS were 65.4 years and 16, respectively. Successful reperfusion was achieved in 147 (91.9%) cases [at least modified Thrombolysis in Cerebral Infarction (mTICI) 2b] with aspiration alone, with complete reperfusion (mTICI 3) in 94 (58.8%) cases. Successful/complete first pass reperfusion was achieved in 104 (64.60%) cases, including mTICI 2b in 34 (21.1%) cases and mTICI 3 in 70 (43.5%) cases. The time from groin puncture to successful reperfusion was 44 (33, 62) min. There were 16 (9.9%) cases requiring rescue therapy using stent-retriever. Procedure-related complications included 2 (1.3%) cases of arterial perforation, 2 (1.3%) cases of arterial dissection, 12 (7.5%) cases of distal embolization, and 1(0.6%) case of puncture site infection. The rate of symptomatic intracranial hemorrhage (sICH) was 3.8% and mortality rate was 13.8%. A total of 99 (62.3%) cases had a favorable outcome (mRS 0–2) at 90 days.ConclusionsIn current practice, the first-line aspiration approach with the Esperance® Distal Access Catheter is safe and efficacious. This device may achieve high reperfusion rates with lower instances of rescue stent retriever therapy.

Experimental evaluation of the performance of large bore aspiration catheters

Background and purposeMechanical thrombectomy using an aspiration catheter (AC) is widely performed in patients with acute ischemic stroke due to large vessel occlusion. AC diameter directly impacts aspiration performance, which has led device companies to develop large bore ACs. The purpose of this study was to evaluate the in vitro performance of several commercially available large bore ACs. Materials and methodsWe conceived an experimental set up to assess tracking force, aspiration flow rate, and distal end tensile force pre- and post-thrombectomy of ACs including Sofia 6 (Microvention Inc., California, USA), React 71 (Medtronic Neurovascular, Irvine, California, USA), Jet 7 (Penumbra Inc, Alameda, California, USA), Catalyst 7 (Stryker Neurovascular, Kalamazoo, Michigan, USA) and Embovac 071 (Johnson & Johnson Medical Ltd, UK). ResultsReact 71 and Sofia 6 had significantly lower trackability force compared to the other ACs tested. Distal AC segment tensile force was highest for React 71 and lowest for Jet 7. Jet 7 had the highest internal diameter and the highest aspiration flow rate. ConclusionThe data from this comparative analysis may assist clinicians in selecting the appropriate AC and highlights the need for key performance criteria during the development of next-generation large-bore ACs for optimal AC performance.

JET 7 catheter for direct aspiration in carotid T occlusions: preliminary experience and literature review.

We report our preliminary experience with the Penumbra JET 7 reperfusion catheter (JET 7), a new large-bore (0.072″) aspiration catheter, in patients with acute ischemic stroke (AIS) due to carotid T occlusion. Data of all eligible patients who received A Direct Aspiration First Pass Technique (ADAPT) for AIS due to carotid T occlusion at our center from March 2018 through June 2020 were retrospectively reviewed. The safety and performance of JET 7 cases and smaller large-bore catheters (LBCs) were compared. JET 7 was used in 19 patients, and smaller LBCs were used in 41 patients. Median puncture to revascularization time was significantly different between the JET 7 and the smaller LBCs (16 vs. 27min; P = 0.011). The rate of patients who received rescue therapy with a stent retriever was also significantly different between the JET 7 cases and the smaller LBCs cases (5.3% vs. 22.0%; P = 0.046). Successful revascularization (TICI ≥ 2b) was achieved in 94.7% of JET 7 cases and 75.6% of smaller LBCs cases (P = 0.148). Good functional outcome (mRS 0-2) at 90days occurred in 63.2% of JET 7 cases and 46.3% of smaller LBCs cases (P = 0.274). In this early experience, ADAPT with JET 7 could be considered as one of the possible first-line therapies in carotid T occlusion, showing good rate of vascularization and lower rate of rescue therapy in comparison with smaller LBCs.

Validity about Diameter of Guiding Catheter for Posterior Circulation Derived by MRA Study.

The development of large-bore aspiration catheters (ACs) has advanced the treatment of mechanical thrombectomy (MT) and their use requires larger guiding catheters (GCs). However, due to the small vessel diameter of the vertebral artery (VA), it can be difficult to cannulate large-bore GC to the VA. This study aims to determine the percentage of VAs that are amenable to GC placement based on the use of a large-bore AC and to clarify the diameters of VAs in the general population using neck MRA. Left and right VA diameters were measured in 1394 consecutive adult patients who underwent neck MRA at our hospital between April 2020 and June 2021. Sex and left/right differences in the VA diameters, as well as the conformity ratios of GCs (6, 7, and 8 French) to right and left VAs, were examined. The patients ranged in age from 18 to 98 years (mean 70.8 ± 13.5 years), with 770 (55.2%) males. The left and right VA mean diameters were 2.82 ± 0.75 mm (range 0-5.1 mm) and 2.65 ± 0.75 mm (range 0-5.3 mm), respectively. The conformity ratios of 6, 7, and 8 French GC to left and right VAs were 85.3% and 79.9%, 74.9% and 68.4%, and 60.9% and 53.7%, respectively. When performing MT for the posterior circulation system, a large-bore AC of 0.060 inches or larger is usually required, and GC placement of 7-French or larger is necessary. The results of this study showed that 7-French GC placement is achievable in approximately 70% of these cases.

Biomarkers of Technical Success After Embolectomy for Acute Stroke.

Stent retrievers and large-bore aspiration catheters have doubled substantial reperfusion rates compared to first-generation devices. This has been accompanied by a 3-fold reduction in procedural time to revascularization. To measure future thrombectomy improvements, new benchmarks for technical efficacy are needed. This review summarizes the recent literature concerning biomarkers of procedural success and harm and highlights future directions. Expanded Treatment in Cerebral Ischemia (eTICI), which incorporates scores for greater levels of reperfusion, improves outcome prediction. Core laboratory-adjudicated studies show that outcomes following eTICI 2c (90%-99% reperfusion) are superior to eTICI 2b50 and nearly equivalent to eTICI 3. Moreover, eTICI 2c improves scale reliability. Studies also confirm the importance of rapid revascularization, whether measured as first pass effect or procedural duration under 30 minutes. Distal embolization is a complication that impedes the extent and speed of revascularization, but few studies have reported its per-pass occurrence. Distal embolization and emboli to new territory should be measured after each thrombectomy maneuver. Collaterals have been shown to be an important modifier of thrombectomy benefit. A drawback of the currently accepted collateral grading scale is that it does not discriminate among the broad spectrum of partial collateralization. Important questions that require investigation include reasons for failed revascularization, the utility of a global Treatment in Cerebral Ischemia scale, and the optimal grading system for vertebrobasilar occlusions. Emerging data support a lead technical efficacy endpoint that combines the extent and speed of reperfusion. Efforts are needed to better characterize angiographic measures of treatment harm and of collateralization.
