Mapping and ranking outcomes for the evaluation of seasonal influenza vaccine efficacy and effectiveness: a delphi study

ABSTRACT Background Protection provided by seasonal influenza vaccination (SIV) may be measured against numerous outcomes, and their heterogeneity may hamper decision-making. The aim of this study was to explore outcomes used for estimation of SIV efficacy/effectiveness (VE) and obtain expert consensus on their importance. Research design and methods An umbrella review was first conducted to collect and map outcomes considered in systematic reviews of SIV VE. A Delphi study was then performed to reach expert convergence on the importance of single outcomes, measured on a 9-point Likert scale, in principal target groups, namely children, working-age adults, older adults, subjects with co-morbidities and pregnant women. Results The literature review identified 489 outcomes. Following data reduction, 20 outcomes were selected for the Delphi process. After two Delphi rounds and a final consensus meeting, convergence was reached. All 20 outcomes were judged to be important or critically important. More severe outcomes, such as influenza-related hospital encounters and mortality with or without laboratory confirmation, were generally top-ranked across all target groups (median scores ≥8 out of 9). Conclusions Rather than focusing on laboratory-confirmed infection per se, experimental and observational VE studies should include more severe influenza-related outcomes because they are expected to exercise a greater impact on decision-making.

Antidepressant and anxiolytic efficacy and tolerability of agomelatine in the treatment of depression after COVID-19 in daily clinical practice (non-interventional study TELESPHOR)

Depression and anxiety are among the most common disorders observed in patients after COVID-19 coronavirus infection.Objective: to evaluate the antidepressant efficacy and tolerability of agomelatine in patients with a depressive episode (DE) after COVID-19 infection in everyday clinical practice.Material and methods. The multicenter observational study TELESPHOR included outpatients aged 18 to 65 years who developed DE (Hamilton Depression Rating Scale (HAMD-17) score of 8–24) within 3 months of the resolution of a laboratory-confirmed COVID-19 infection and started monotherapy with agomelatine. Patients were assessed at enrolment and subsequently at weeks 2, 4 and 8 of the observation period. The primary endpoint was the change in HAMD-17 scores by week 8 of the observation in patients taking agomelatine as an antidepressant. The main secondary outcome variables were the dynamics of scores for items 10 (psychic anxiety) and 11 (somatic anxiety) of the HAMD-17 scale, the proportion of responders and remitters (according to the HAMD-17 scale) and the impact on quality of life (QOL) according to the SF-36 questionnaire. The tolerability of agomelatine was assessed at each visit. Data from all patients who participated in the study and took agomelatine at least once were included in the efficacy and safety analysis. The final analysis included data from 103 patients, including 73 women (70.9%).Results. Treatment with agomelatine was associated with a significant improvement in depression severity: the mean HAMD-17 total score decreased statistically significantly at weeks 2, 4 and 8 from 16.3±4.7 at study inclusion to 13.7±4.5, 9.5±4.9 and 5.4±3.6 points – by 2.6±3.3, 6.7±5.3 and 10.9±4.9 points, respectively, compared to baseline (p<0.0001 for all time points). The average score for item 10 (psychic anxiety) and item 11 (somatic anxiety) of the HAMD-17 scale also decreased significantly from 1.4±0.9 and 1.3±0.8 at baseline to 0.5±0.6 and 0.6±0.6 points by the eighth week of observation (p<0.0001). The indicators of the scales for physical functioning and mental health of the SF-36 questionnaire improved significantly compared to the initial values – from 39.7±8.4 to 50.5±5.7 points (p<0.0001) and from 37.3±8.6 to 58.7±6.9 points (p<0.0001) respectively, by the eighth week. The proportion of responders was 81.4 %, the proportion of remitters was 71.6%. Agomelatine was well tolerated during the entire observation period. No adverse events were recorded that led to discontinuation of the drug. One patient showed inefficacy of the treatment, so he discontinued participation in the study.Conclusion. The treatment of patients with DE after COVID-19 with agomelatine is characterized by significant antidepressant and anxiolytic efficacy, improved QOL and good tolerability.

Resurgence of pediatric Mycoplasma pneumoniae infections in Southeast Texas, Nov 2023-June 2024.

We conducted a retrospective review of children tested for Mycoplasma pneumoniae from January 1, 2020-June 30, 2024. M. pneumoniae PCR positivity increased starting in November 2023, peaking at 18.3% in June 2024. During the resurgence, children with laboratory-confirmed infection often had severe respiratory disease or extrapulmonary manifestations.

Serological response to avian influenza viruses among occupationally exposed population in Taiwan, a longitudinal cohort study, 2021-2023.

The Taiwan Centers for Disease Control (TCDC) initiated longitudinal sero-epidemiological surveillance of avian influenza viruses (AIVs) targeting poultry workers exposed on affected farms since 2021. Convalescent-phase serum samples were collected from these workers 21-28 days post-outbreak identification. TCDC recontacted participants in subsequent years for repeat serological sampling. Hemagglutination inhibition (HI) assays were used to test against representative strains of AIV A/H5 2.3.4.4c and A/H5 2.3.4.4b. By the end of 2023, a 2-year cohort of 59 participants and a 3-year cohort of 61 participants were established. None of them had laboratory-confirmed infection. HI titers against AIV A/H5 2.3.4.4c remained low from 2021 to 2023. For AIV A/H5 2.3.4.4b, HI titer remained low from 2021 to 2022 but increased in more than half of the participants (60%, 72/120) in 2023. This change parallels the increasing AIV A/H5 2.3.4.4b burden indicated by increased outbreaks and warrants continuous monitoring.

Elevated Soluble ACE2 Activity in Children and Adults After SARS-CoV-2 Exposure Irrespective of Laboratory-Confirmed Infection.

The pivotal role of the cell entry receptor ACE2 for SARS-CoV-2 infection is well-established. When ACE2 is shed from cell surface into plasma as soluble ACE2 (sACE2), it can effectively neutralize SARS-CoV-2. This longitudinal prospective cohort study analyzed sACE2 activity in 1192 participants, aged 4 months to 81 years, 3 and 12 months after SARS-CoV-2 household exposure. Following SARS-CoV-2 exposure, participants exhibited significantly elevated sACE2 activity, irrespective of confirmed infection, with the highest levels observed in exposed children. Longitudinal analysis revealed a decline in sACE2 levels over time, reaching levels comparable to age- and sex-matched pre-pandemic controls. An increase in sACE2 activity was also confirmed in vitro in Calu-3 (human lung) cells within hours of SARS-CoV-2 exposure, providing a direct link between SARS-CoV-2 exposure and elevated sACE2. This study, therefore, challenges the dichotomy of categorizing SARS-CoV-2 exposed participants as infected or not infected solely on currently established diagnostic assays. It demonstrates lasting host responses independent of B- and T-cell memory and may help to keep SARS-CoV-2 infections in balance and contribute to successful virus clearance in children and adults lacking humoral and cellular immune responses following SARS-CoV-2 exposure. Baseline Characteristics of Covid-19 Infected Spontaneous Abortion Cases

Background: The COVID-19 pandemic has raised concerns about the potential impact of SARS-CoV-2 infection on pregnancy outcomes. However, there is limited evidence regarding the association between COVID-19 and spontaneous abortion cases, particularly in Bangladesh.Methods: This retrospective observational study investigated the baseline characteristics of 48 cases of spontaneous abortion in pregnant women with laboratory-confirmed COVID-19 infection at Enam Medical College Hospital, Savar, Dhaka, Bangladesh between March and July 2020. Participants were included if they experienced fetal demise at 11 or more weeks of gestation, and potential confounding factors, such as fetal malformations or other obstetric disorders, were excluded. Data on demographic characteristics, gestational age, parity, gravidity, and COVID-19 severity were collected and analyzed.Results: The age distribution of participants ranged from 18 to 33 years, with 50% in the 26-30 year age group. A majority (58.33%) were obese, and 83.33% exhibited mild COVID-19 symptoms, while 16.67% were asymptomatic. Regarding gestational age at the time of spontaneous abortion, 50% were between 11-15 weeks, 41.67% were between 16-20 weeks, and 8.33% were between 21-25 weeks. All participants had at least one previous pregnancy, with 33.33% experiencing their second pregnancy, 41.67% their second pregnancy with a previous live birth, and 25% their third pregnancy with at least one previous live birth.Conclusions: This study provides insights into the baseline characteristics and maternal outcomes of COVID-19 infected spontaneous abortion cases in Bangladesh. The findings suggest that severe COVID-19 illness may not be a prerequisite for adverse pregnancy outcomes and highlight the need for further investigations into the potential mechanisms of intrauterine SARS-CoV-2 transmission and the role of placental factors. Larger-scale studies are warranted to better understand the impact of COVID-19 on pregnancy outcomes and develop appropriate management strategies.

PREVALENCE OF PARVOVIRUS INFECTION IN THE NORTHWESTERN FEDERAL DISTRICT OF RUSSIA

Abstract Here, we analyze the spread of parvovirus B19 infection in the Northwestern Federal District from the years 2018 to 2023. It was shown that cases of infection were recorded in 10 out of 11 territories, excepting the Nenets Autonomous Okrug. Overall, IgM antibodies to parvovirus B19 (B19V) were detected in 295 samples out of 2079 tested (14.1%). The prevalence of parvovirus B19 infection varied over the years of the study: in 2023, an increase in incidence was detected in the Northwestern Federal District, with an indicator of 0.93 per 100 thousand population, which significantly exceeded the long-term average level of 0.43 per 100 thousand population in the district. Children under 14 years of age predominated in the age pattern. The virus circulated most actively in St. Petersburg and the Leningrad region, which is due to the high population density in these territories and the many municipal institutions where long-term close contacts between people take place. The winter-spring seasonality of the infection has been confirmed. An analysis of primary diagnoses in patients with laboratory-confirmed parvovirus infection (2022) showed that the correct clinical diagnosis was not established in any case. The most common clinical diagnostic errors were: “Acute respiratory infection”; “measles” and “rubella”; herpes virus infections. In 30% of cases, parvovirus B19 infection was assessed by clinicians as a non-infectious disease. Errors in the initial diagnosis of parvovirus infection can contribute to the spread of infection, including in risk groups. Thus, while examining patients at a dialysis center, laboratory methods revealed latent parvovirus B19 circulation in this risk group confirmed by the detection of B19V DNA and virus-specific IgM antibodies in some samples tested. The seroprevalence established among patients not exceeding 76.9% accounts for an opportunity of infection spread among patients seronegative for parvovirus B19 with worsening of disease course and prognosis. Considering the widespread distribution of parvovirus infection, when identifying infectious diseases and respiratory diseases that occur with unclear exanthema manifestations among people at risk and/or from organized groups, it is advisable to conduct targeted serological monitoring for laboratory markers of parvovirus B19 infection. One of the directions for further qualitative study of parvovirus infection is to solve an issue of its identification and recording in the epidemiological surveillance system.

Abstract 4135466: Feasibility of Using Existing Infection Surveillance Methods for Percutaneous Coronary Intervention-related Infections

Background: Percutaneous coronary intervention (PCI) is a treatment that requires attention in terms of infection control, as it necessitates aseptic procedures similar to surgical operations. However, surveillance practices for PCI-related infections are uncommon, and no standardized surveillance program has been established. Purpose: To assess the feasibility of using existing surveillance methods to detect PCI-related infectious diseases. Method: To identify PCI-related bloodstream infections of local access sites, four existing case definitions, namely, laboratory-confirmed bloodstream infections (LCBI), clinical sepsis (CSEP), local access site infection (LASI), and exit site infection (ESI), were retrospectively applied to 1,037 patients who underwent PCI between April 2018 and March 2021. Of these, 167 experienced fever, underwent blood culture testing, and/or received antibiotics within 6 days of PCI. To confirm consistent evaluation, two independent infection control experts evaluated each case record according to the four case definitions. Results: Twenty-three patients (2.2%) were diagnosed with PCI-related infections, including two patients according to LCBI, 21 according to CSEP, and none according to LASI and ESI. Thirteen patients (LCBI: 1, CSEP: 12) received circulatory assist devices such as intra-aortic balloon pumping. Of the remaining 11 patients, 10 underwent emergency PCI, and one underwent elective PCI ( P =0.0001). The average length of hospital stay was 16.1 days for patients with PCI-related infections and 4.7 days for non-infected patients ( P <0.05). Conclusion: The positive diagnosis of PCI-related infections was predominantly based on CSEP. Many of these positive patients underwent emergency PCI and/or received circulatory assist devices. The results of the present study warrant further investigations with larger patient cohorts.

Dengue fever in immunocompromised patients: A systematic review and meta-analysis

ObjectivesGiven the complex role of immunity in dengue severity, we aimed to review the clinical course of dengue infection in immunocompromised patients. MethodsWe conducted a systematic review of studies reporting outcomes among immunocompromised patients with laboratory-confirmed dengue infection. Meta-analysis using the Mantel-Haenszel method (fixed effects) was performed for studies with control groups. We registered the study with PROSPERO (No. CRD42021258930). ResultsWe included 115 studies. Among these, 30 studies compared immunocompromised (cases) and nonimmunocompromised (control) patients, focusing mainly on children (n = 22 studies) with malnutrition (n=18). Immunocompromised patients had a higher likelihood of dengue complications (OR 1.87; 95% CI: 1.04-3.35]) but a lower likelihood of severe dengue (OR 0.83; 95% CI: 0.69-1.00]. No significant difference in mortality was observed. In the 85 studies focused solely on immunocompromised patients, severe dengue and mortality rates were 9% and 4%, respectively, mostly among adult solid organ transplant recipients and those with inflammatory diseases. Immunosuppressive treatment alterations and temporary graft dysfunction were reported. ConclusionImmunocompromised patients have an increased risk of dengue-related complications. However, definitive conclusions about the comparative severity of dengue across different immunocompromised patient groups are limited by a lack of robust data, highlighting the need for well-designed future studies.

Influenza Vaccine Effectiveness Against Illness and Asymptomatic Infection in 2022-2023: A Prospective Cohort Study.

Previous estimates of vaccine effectiveness (VE) against asymptomatic influenza virus infection based on seroconversion have varied widely and may be biased. We estimated 2022-2023 influenza VE against illness and asymptomatic infection in a prospective cohort. In the HEROES-RECOVER cohort, adults at increased occupational risk of influenza exposure across 7 US sites provided weekly symptom reports and nasal swabs for reverse transcription-polymerase chain reaction (RT-PCR) influenza testing. Laboratory-confirmed influenza virus infections were classified as symptomatic (≥1 symptom) or asymptomatic during the week of testing. Participants reported demographic information and vaccination through surveys; most sites verified vaccination through medical record and immunization registry review. Person-time was calculated as days from the site-specific influenza season start (September-October 2022) through date of infection, study withdrawal, or season end (May 2023). We compared influenza incidence among vaccinated versus unvaccinated participants overall, by symptom status, and by influenza A subtype, using Cox proportional hazards regression adjusted for site and occupation. We estimated VE as (1 - adjusted hazard ratio) × 100%. In total, 269 of 3785 (7.1%) participants had laboratory-confirmed influenza, including 263 (98%) influenza A virus infections and 201 (75%) symptomatic illnesses. Incidence of laboratory-confirmed influenza illness among vaccinated versus unvaccinated participants was 23.7 and 33.2 episodes per 100 000 person-days, respectively (VE: 38%; 95% CI: 15%-55%). Incidence of asymptomatic influenza virus infection was 8.0 versus 11.6 per 100 000 (VE: 13%; 95% CI: -47%, 49%). Vaccination reduced incidence of symptomatic but not asymptomatic influenza virus infection, suggesting that influenza vaccination attenuates progression from infection to illness.

Assessment of neutralizing antibody response as a correlate of protection against symptomatic SARS-CoV-2 infections after administration of two doses of the CoronaVac inactivated COVID-19 vaccine: A phase III randomized controlled trial

The emergence of variants of concerns of SARS-CoV-2 highlights the need for comprehensively elucidating the correlates of protection for different COVID-19 vaccine types. Inactivated COVID-19 vaccines are currently amongst the most widely administered vaccines globally. However, investigations into the correlates of protection for inactivated COVID-19 vaccines are relatively rare. Data from a phase III double-blind, randomized, placebo-controlled clinical trial (NCT0445659) that evaluated the efficacy and safety of the CoronaVac vaccine in healthcare professionals were utilized in this secondary analysis. Additionally, the correlation between neutralizing antibody levels measured by micro-cytopathic effect (CPE) neutralization assay and the occurrence of laboratory-confirmed infections was assessed using neutralizing antibodies measured in blood samples collected on day 28 after receiving two doses of the vaccine. Finally, the protective threshold required to provide 50% protection against symptomatic illness and virus infections was estimated. The risk of infection was negatively correlated with the levels of post-vaccination neutralizing antibodies measured on day 28 after the second dose. A neutralization titer of 30 (95% CI: 2-56) was predicted to provide 50% efficacy against symptomatic infection, whilst a titer of 42 (95% CI: 24-62) was predicted to provide 50% efficacy against total infection. Lastly, a neutralization titer of 247 (95% CI: 139-506) or higher was required to achieve 80% or higher protection against symptomatic infections. The results highlight the value of neutralizing antibody response as a correlate of protection, which can be used to inform future vaccine development and implementation. Further studies of immune correlates of protection for other vaccines are warranted.

Respiratory Syncytial Virus Risk Profile in Hospitalized Infants and Comparison with Influenza and COVID-19 Controls in Valladolid, Spain, 2010-2022.

We aimed to describe the risk profile of RSV infections among children aged ≤ 24months in Valladolid from January 2010 to August 2022 and to compare them with influenza and COVID-19 controls. We conducted a retrospective cohort study of all laboratory-confirmed RSV, influenza, and COVID-19 infections. We analyzed risk factors for RSV hospitalization and severity (length-of-stay ≥ 8 days, intensive-care-unit admission, in-hospital death or readmission < 30days) and compared severity between hospitalized RSV patients vs. influenza and COVID-19 controls using multivariable logistic regression models. We included 1507 patients with RSV (1274 inpatient), 32 with influenza, and 52 COVID-19 controls. Hospitalized RSV (mean age 5.3months) and COVID-19 (4months) were younger than influenza (9.1months) patients. Sixteen percent of patients had RSV within the first month of life. Most infants did not have comorbidities (74% RSV, 56% influenza, and 69% COVID-19). Forty-one percent of patients with RSV and influenza were coinfected vs. 27% COVID-19 (p = 0.04). Among RSV, hospitalization risk factors were prematurity (adjusted OR 3.11 [95% CI 1.66, 4.44]) and coinfection (2.03 [1.45, 2.85]). Risks for higher severity were maternal smoking (1.89 [1.07, 3.33]), prematurity (2.31 [1.59, 3.34]), chronic lung disease (2.20 [1.06, 4.58]), neurodevelopmental condition (4.28 [2.10, 8.73]), and coinfection (2.67 [2.09, 3.40]). Breastfeeding was protective against hospitalization (0.87 [0.80, 0.95]) and severity (0.81 [0.74, 0.88]), while complete vaccination schedule was protective against severity (0.51 [0.27, 0.97]). RSV had 2.47 (1.03, 5.96) higher risk of experiencing any severe outcome compared to influenza and did not show significant differences vs. COVID-19. RSV hospitalizations were more frequent and severe than influenza, while severity was comparable to the early pandemic COVID-19. Currently, both influenza and COVID-19 vaccines are included in the maternal and childhood Spanish immunization schedule between the ages of 6 and 59months. RSV monoclonal antibody is recommended for ≤ 6months but a third of patients were aged 6-24months. Maternal RSV vaccination can protect their children directly from birth and indirectly through breastfeeding.

Bloodstream infections in a Brazilian Pediatric Tertiary Oncology Hospital: 30-month analysis regarding the classification of the infection, distribution of microorganisms and their resistance profile

Abstract Background Infections are a major cause of morbidity and mortality in pediatric oncology patients during their treatment. Bloodstream infections are the main site of infection in these patients. Bacteremia in these patients can be secondary to a primary infectious focus, result from central line associated bloodstream infections (CLABSIs) or mucosal barrier injury laboratory-confirmed bloodstream infection (MBI-LCBI). Methods Descriptive study of bloodstream infections in pediatric patients undergoing oncological treatment and hematopoietic stem cell transplantation in a tertiary pediatric oncology hospital in Brazil from January 1st, 2021 to June 30th, 2023. Infections were classified according to the Centers for Disease Control and Prevention/ National Healthcare Safety Network (CDC/NHSN) criteria into CLABSIs, MBI-LCBIs and secondary bloodstream infections. Among the bloodstream infections that met CDC/NHSN criteria for CLABSI, were analyzed separately those who also met the criteria of catheter-related bloodstream infection (CRBSI) according to Infectious Diseases Society of America (IDSA). Results During this period were diagnosed 335 episodes of bloodstream infection (123 episodes in 2021, 150 in 2022 and 62 episodes in the first 6 months of 2023). There were 378 microorganisms isolated. These infections were classified into: 35% CLABSIs, 27% MBI-LCBIs, 23% CRBSIs, and 15% secondary bloodstream infections. Among the CLABSIs, the most common microorganisms were non-fermenting gram negative bacilli (NFGNB) 38%, enterobacteriaceae 35% and coagulase-negative staphylococcus (CoNS) 11%. Between CRBSIs, there were 31% enterobacteriaceae, 22% CoNS and 19% NFGNB. Among the MBI-LCBIs there were 66% enterobacteriaceae and 23% group viridans streptococci (VGS). Streptococcus pneumoniae (33%) was the most common microorganism in secondary infections followed by Staphylococcus aureus (17%). Assessing all infections together, the main enterobacteriaceae were Klebsiella spp. (40%) and only 50% of them were sensitive to cefepime while 10% were carbapenems resistant. Among NFGNB, 13% of Pseudomonas spp. and 5% of Acinetobacter spp. were carbapenems resistant. Between the gram-positive bacteria, 77% of the S. aureus were methicillin sensitive, the same percentage of the VGS were sensitive to cefepime. Conclusion Among bloodstream infections, there were a predominance of CLABSI followed by MBI-LCBIs. Antimicrobial resistance between gram-positive bacteria, is of little concern in our institution. In the case of gram-negative bacteria, resistance to cefepime is becoming worrying among enterobacteriaceae.

Prefilled Saline Syringes versus Those Prepared at Bedside for Flushing Central Venous Catheters in Neonatal ICU

Abstract Background A central line-associated bloodstream infection (CLABSI) is defined as a laboratory-confirmed bloodstream infection not related to an infection at another site that develops within 48 hours of a central line placement. CLABSI is the most common type of nosocomial infection in this population and is a leading cause of morbidity and mortality in NICU. Therefore, more efforts are needed to reduce the incidence of CVC-related infections among NICU patients, and to enhance their survival and prognosis. Aim of the Work To compare the effect of using of prefilled saline syringes prepared in the laminar flow under complete aseptic conditions vs using ones prepared at bedside for central venous access flushing regarding risk of CLABSI, catheter permanence time, risk of thrombosis, and length of stay in our NICU. Patients and Methods This prospective randomized controlled trial in Ain Shams University Hospital NICU over a 6-month period from January 2023 till July 2023. They were selected through simple random sampling method to be divided into 2 groups: Group 1 (cases): 50 patients had their CVCs flushed by 3ml syringe prefilled with saline 0.9% prepared in the laminar flow on the same day. Group 2 (controls): 50 patients had their CVC flushed through manually prepared 3 ml syringe prepared at bedside from a common bottle of saline 0.9%. Results It was found that Group 1 patients had significantly lower incidence of CLABSI than Group 2 patients (4 versus 13 neonates) (p-value &amp;lt;0.0001). The incidence of CVC thrombosis was shown to be less in Group 1, however, this difference was non-significant (2 versus 1 neonate) (p-value 0.557). This study showed there was no statistically significant difference between both groups regarding the catheter permanence time (8.56 ± 6.52 versus 10.46 ± 6.29) (p-value 0.146). There was no statistically significant difference between both groups regarding the length of hospital stay in studied patients, 26.5 (14 – 37) in Group 1 versus 25 (12 – 32) in Group 2 (p-value 0.704). Conclusion The use of prefilled saline syringes for flushing of central venous catheters in neonates has shown to significantly reduce the risk of CLABSI, reduce incidence of catheter- related thrombosis, and flushing time, thus improve quality of care in neonatal ICU.

A Study to Evaluate the Effectiveness of Health Education Program on Knowledge Regarding CLABSI on B.SC. Nursing Students Studying in Sharda University, Greater Noida, Uttar Pradesh

Introduction: Central line-associated bloodstream infection (CLABSI) When a central artery is infected with pathogens or viruses, it can be quite serious. Healthcare providers may put a central line into one of the larger veins to give medicines, fluids, or blood products, or draw blood for lab tests that people need over a long period. A CLABSI is a laboratory-confirmed bloodstream infection not found to be present or incubating on admission and the patient has had a central line in place for &gt; =2 calendar days by the day of the event, with that same organism also being RECOGNIZED from one other sterile site. Aim: The study aimed to determine the effectiveness of the health education program or knowledge regarding CLABSI. Methodology: A quasi-experimental pretest one-group and post-test design was adopted. In this situation, the population of the study, and the inclusion, and exclusion criteria were applied. Use of sample size, sampling techniques, validation, and data collection participants Population of the study included all B.Sc. Nursing undergraduate students at Sharda University School of Nursing. Research incurred from May to June 2024. A purposive sampling strategy was used in the selection and determination of the sample size of the study which was 60 students using the independent t-test formula. Result: The study shows that 68.3% of the participants must have a poor level of knowledge. 23.3% had an average level of knowledge 8.3% had a good level of knowledge regarding the pretest, a maximum of 63.3% they were having a good level of knowledge, about 26.7% of them are having an average level of knowledge, 10% of them had a poor level of knowledge regarding it. From the pre-test to the post-test the mean knowledge score has significantly increased, in the pre-test was 9.83 S. D= 4.00 and in the post-test, it iwas 15.65 and S. D= 3.99. The Paired T-test value is -8.85 and the p-value is less than 0.01. Conclusion: According to the findings, nursing students need to have a comprehensive grasp of CLABSI to build an understanding of the illness state and to receive proper treatment to prevent the development of other life-threatening disorders and sepsis.

Vaccination against dengue fever for travellers.

Dengue fever, endemic to most tropical and subtropical countries, is a major cause of illness in travellers, but severe dengue, hospitalisation and death are considered rare in this population. Two vaccines against dengue fever, Dengvaxia®and Qdenga®, are available. While there is no recommendation for the use of Dengvaxia®in travellers, Qdenga®has been licensed for travellers in many European countries since December 2022, most recently (29 July 2024) in Switzerland by Swissmedic. The Swiss Expert Committee for Travel Medicine (ECTM), having assessed available data on the Qdenga®vaccine, issues the following recommendations: (1) Vaccination against dengue fever virus with Qdenga®is not recommended for persons with no previous dengue fever infection. (2) Vaccination with Qdenga®may be recommended for travellers aged 6 years and older who have evidence of previous dengue infection, defined as (a) a laboratory-confirmed dengue infection (PCR, antigen or seroconversion)or(b) a compatible history of dengue infection with a positive IgG serological test AND expected exposure to a region with significant dengue transmission. Travel medicine advisors should provide clear information in accessible language on the complexity of dengue vaccines and the risk/benefit evaluation for their use in travellers.

The TIRS trial: Enrollment procedures and baseline characterization of a pediatric cohort to quantify the epidemiologic impact of targeted indoor residual spraying on Aedes-borne viruses in Merida, Mexico.

Aedes mosquito-borne viruses (ABVs) place a substantial strain on public health resources in the Americas. Vector control of Aedes mosquitoes is an important public health strategy to decrease or prevent spread of ABVs. The ongoing Targeted Indoor Residual Spraying (TIRS) trial is an NIH-sponsored clinical trial to study the efficacy of a novel, proactive vector control technique to prevent dengue virus (DENV), Zika virus (ZIKV), and chikungunya virus (CHIKV) infections in the endemic city of Merida, Yucatan, Mexico. The primary outcome of the trial is laboratory-confirmed ABV infections in neighborhood clusters. Despite the difficulties caused by the COVID-19 pandemic, by early 2021 the TIRS trial completed enrollment of 4,792 children aged 2-15 years in 50 neighborhood clusters which were allocated to control or intervention arms via a covariate-constrained randomization algorithm. Here, we describe the makeup and ABV seroprevalence of participants and mosquito population characteristics in both arms before TIRS administration. Baseline surveys showed similar distribution of age, sex, and socio-economic factors between the arms. Serum samples from 1,399 children were tested by commercially available ELISAs for presence of anti-ABV antibodies. We found that 45.1% of children were seropositive for one or more flaviviruses and 24.0% were seropositive for CHIKV. Of the flavivirus-positive participants, most were positive for ZIKV-neutralizing antibodies by focus reduction neutralization testing which indicated a higher proportion of participants with previous ZIKV than DENV infections within the cohort. Both study arms had statistically similar seroprevalence for all viruses tested, similar socio-demographic compositions, similar levels of Ae. aegypti infestation, and similar observed mosquito susceptibility to insecticides. These findings describe a population with a high rate of previous exposure to ZIKV and lower titers of neutralizing antibodies against DENV serotypes, suggesting susceptibility to future outbreaks of flaviviruses is possible, but proactive vector control may mitigate these risks.

Neisseria gonorrhoeae treatment failure to the recommended antibiotic regimen-Québec, Canada, 2015-19.

To describe Neisseria gonorrhoeae treatment failure to the recommended antimicrobial regimens (azithromycin, cefixime and ceftriaxone). Our study was a longitudinal analysis of treatment failures from an observational open cohort of gonococcal infection cases collected in Québec, Canada (n = 2547) between September 2015 and December 2019. Epidemiological and clinical data were collected using a self-administered questionnaire, direct case interviews and chart reviews. Antimicrobial susceptibility testing was performed using the agar dilution method. To be retained as a treatment failure, cases must have had (i) a laboratory-confirmed gonococcal infection; (ii) a documented treatment; (iii) a positive test of cure (TOC) performed within a defined period and (iv) no sexual contact (vaginal, oral or anal), even protected with a condom, between the beginning of treatment and the positive TOC. A broader definition, including suspected cases, was also examined. Among 1593 cases where a TOC was performed, 83 had a positive TOC: 11 were retained as treatment failure, and 6 were considered suspected cases (overall = 17/1593; 1.1%). Possible explanations for retained or suspected treatment failure included resistance to the antibiotics used for treatment (n = 1), pharyngeal infection (n = 9, of which 5 had been treated with ceftriaxone and 4 with other regimens); and azithromycin monotherapy (n = 1). Some cases had more than one potential explanation. Treatment failure occurred in 1.1% of cases of Neisseria gonorrhoeae infection for which a TOC was performed, including some cases of pharyngeal infection treated with ceftriaxone.

Characteristics of inpatient and outpatient respiratory syncytial virus mortality in Gavi-eligible countries

The current understanding of the RSV-related mortality age distribution in low- and lower-middle-income countries (LMICs) relies on a limited number of disease incidence studies reporting wide age bands, and lacking specificity to Gavi-eligible countries. Understanding the age distribution of RSV-related deaths is crucial for the implementation of RSV interventions in LMICs that rely on support from Gavi. This study aims to provide the age profile of RSV mortality specifically in Gavi-eligible countries.Utilizing data from the RSV GOLD project, an ongoing global online mortality registry focusing on children under the age of 5 with laboratory-confirmed RSV infection, we employed two models (Complete Data Model and Prospective Data Model) to estimate the age profiles. To mitigate biases related to age group representation, we applied post-stratification weighting in our analysis.We included 423 pediatric deaths, including 145 from the community, under 2 years of age from 15 Gavi-eligible countries. Both models identified a peak age at 1 month and found that the majority of RSV-related mortality cases (59–77 %) from Gavi-eligible countries occur before 6 months of life. However, the models exhibited disparities in other age-related metrics. We present fitted age-at-time-of-death probability distributions to aid impact and cost-effectiveness studies.We expect that implementing infant RSV immunization strategies, such as maternal vaccination or infant immunoprophylaxis, will have high impact on RSV-related mortality in Gavi-eligible countries. The divergent results from the two models underscore the importance of carefully considering potential biases in retrospective and surveillance data when interpreting the age profile of RSV mortality cases in future research.

Rotavirus Vaccine Effectiveness Against Severe Acute Gastroenteritis: 2009-2022.

Rotavirus was the leading cause of acute gastroenteritis among US children until vaccine introduction in 2006, after which, substantial declines in severe rotavirus disease occurred. We evaluated rotavirus vaccine effectiveness (VE) over 13 years (2009-2022). We analyzed data from the New Vaccine Surveillance Network using a test-negative case-control design to estimate rotavirus VE against laboratory-confirmed rotavirus infections among children seeking care for acute gastroenteritis (≥3 diarrhea or ≥1 vomiting episodes within 24 hours) in the emergency department (ED) or hospital. Case-patients and control-patients were children whose stool specimens tested rotavirus positive or negative, respectively, by enzyme immunoassay or polymerase chain reaction assays. VE was calculated as (1-adjusted odds ratio)×100%. Adjusted odds ratios were calculated by multivariable unconditional logistic regression. Among 16 188 enrolled children age 8 to 59 months, 1720 (11%) tested positive for rotavirus. Case-patients were less often vaccinated against rotavirus than control-patients (62% versus 88%). VE for receiving ≥1 dose against rotavirus-associated ED visits or hospitalization was 78% (95% confidence interval [CI] 75%-80%). Stratifying by a modified Vesikari Severity Score, VE was 59% (95% CI 49%-67%), 80% (95% CI 77%-83%), and 94% (95% CI 90%-97%) against mild, moderately severe, and very severe disease, respectively. Rotavirus vaccines conferred protection against common circulating genotypes (G1P[8], G2P[4], G3P[8], G9P[8], and G12[P8]). VE was higher in children <3 years (73% to 88%); protection decreased as age increased. Rotavirus vaccines remain highly effective in preventing ED visits and hospitalizations in US children.
