Ki-1 positive large cell lymphoma of the orbit

Anaplastic large cell lymphoma (ALCL) positive for the cell marker Ki-1/CD30 is a recently recognized high grade lymphoma. The authors present a case of a three-year-old female child with a rapidly growing sino-orbital lesion with central nervous system extension, suggestive of rhabdomyosarcoma. The child presented a clinical challenge for prompt diagnosis and treatment.Histologically, the unusual tumor showed a mixture of large malignant lymphocytes and histiocytes. Immunohistochemically, the malignant lymphocytes showed positive reactivity to LCA, Ki-1, CD3, UCHL-1. The histiocytes were positive for KP-1 and lysozyme.There was complete resolution of the lymphoma. To the authors' knowledge, this is the first case of Ki-1+ ALCL lymphoma involving the orbit of a child.

Malignant lymphoma particularly of T phenotype can be associated with specific or non specific cutaneous lesions. These cutaneous manifestations can occur at the onset of the disease being sometimes the revealing sign or they can appear during the course of the lymphoreticular malignancies. Glomerulonephritis was also described in lymphoma. Ki-1 positive large cell lymphoma was recently identified. A new case is reported with lymphadenopathy and intestinal localisation revealed by cutaneous and mucosal ulcerations principally in the mouth and a focal segmental glomerulonephritis with endo- and extracapillary proliferation. The absence of lymphoma in cutaneous and renal lesions and the clinical presentation support the hypothesis of paraneoplastic manifestations, may be related to a vasculitis.

Ki-1 positive large cell lymphoma. A morphologic and immunologic study of 19 cases.

In this report, we analyze the clinical, morphologic, and immunologic findings of 19 patients with Ki-1+ large cell lymphoma, a recently described malignant lymphoma which is usually of T-cell phenotype. Most patients in this selected series of 14 children and adolescents and five adults presented with peripheral lymphadenopathy or skin lesions. No patient had bone marrow involvement. Distinctive morphologic features of Ki-1+ large cell lymphoma are tumor cell pleomorphism, sinus infiltration, fibrosis, partial lymph node involvement, sparing of B-cell regions, and a prominent plasma cell infiltrate. Seventy-two percent of cases were of T-cell phenotype; the remaining cases expressed neither B- nor T-cell-specific markers. Virtually all cases were positive for Ia (HLA-DR), Tac (interleukin-2 receptor), and T9 (transferrin receptor), indicating that this lymphoma is frequently a tumor of activated T-cells. We conclude that a diagnosis of Ki-1+ large cell lymphoma should be considered in any pleomorphic tumor with the features described in this report.
