Characteristics and six-month outcome of patients who use suicide threats to seek hospital admission.

Retrospective review of patients' charts and other records was used to collect data on diagnoses, psychosocial characteristics, and subsequent suicide attempts of 45 patients who made contingent suicide threats, defined as threatened suicide or exaggerated suicidality reported to increase the likelihood of hospital admission, and 92 suicidal patients who did not make such threats. Patients who made contingent suicide threats were more likely to be substance dependent, antisocial, homeless, unmarried, and in legal difficulty. Subsequent suicide attempts were uncommon in both groups. The authors suggest that hospitalization should not be used as a substitute for social services, substance abuse treatment, and legal assistance for patients who make contingent suicide threats.

Anatomy and function of the orbital frontal cortex, II: Function and relevance to obsessive-compulsive disorder.

The authors review neurophysiological, neurobehavioral, and neuropsychological investigations of the orbital frontal cortex (OFC) in human and non-human primates. The article critically examines the role of the OFC in 1) recognition of reinforcers; 2) stimulus-reinforcer learning; 3) modulation of responses based on changes in reinforcement contingencies; 4) emotions, social behavior, and autonomic regulation; 5) mnemonic functions; and 6) rule learning. Examining these functional areas with reference to the OFC's anatomical and neurophysiological properties, the authors suggest ways in which the OFC might contribute to obsessive-compulsive disorder.

Partial hospitalization at night: the Brussels Nighthospital.

This paper describes the Brussels Nighthospital, which has offered partial hospitalization to young chronic psychiatric patients since 1978. The hospital's rehabilitation program provides evening treatment and nighttime accommodations to approximately 44 individuals, who live in the facility for about a year. During the day patients are engaged in useful activities outside the facility. The night hospital has developed a network of such placements. Psychotherapy and family support groups are also offered. A follow-up study of 120 patients (mean time after discharge = 4.2 years) showed that most maintained their achieved level of adaptation and continued to live in the community engaged in useful activities.

Commentary on “Clients' Perceptions of Treatment for Depression: I and II”

The process of reviewing manuscripts submitted to psychotherapy journals is becoming more complex these days as editors call upon reviewers who have relevant expertise but who are not committed to the research paradigm subscribed to by its author(s). Such has been the case regarding one of these articles, Study I1 (Levy, Glass, Ankoff, Gershefski, & Elkin, 1996). I was given for review the original and the revised manuscript of that article, presumably because I am both an advisory editor of the journal and knowledgeable about the subject matter of the study. It also happens that, unlike the authors, I am a qualitative researcher.The editor concluded that the revision should be allowed to stand because it is coherent with the natural science paradigm it represents and that this coherence includes the style of presentation–includes, as I would put it, its rhetoric (Bazerman, 1988; Nelson, Megill, & McCloskey, 1987; Rennie, 1995a).
