Recommendations regarding the admission, infection prevention and control of pediatric patients during coronavirus disease 2019 outbreak in Shanghai China.

Since December 2019, the number of coronavirus disease 2019 (COVID-19) cases and severe cases caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in China has been increasing rapidly (1,2). A total of 80,926 confirmed and 349 suspected cases have been reported by March 10, 2020. Epidemiological data showed that all people were susceptible to SARS-CoV-2. Respiratory droplets and close contact are main means of transmission, and aerosol transmission occur in a closed environment exposed to high concentrations of aerosols (3). The clinical symptoms of COVID-19 were atypical (4-6). Provinces and municipalities in China have launched first-level response to this major public health emergency. It has been one month since the lockdown were launched in all cities in Hubei Province. The increasing number of people returning home during the Spring Festival across the country has posed a new challenge to prevention. There are still medical demands for patients due to the diseases other than COVID-19. There is a need to attend to pediatric inpatients by caregivers. Currently, diagnosis of suspected cases is nucleic acid detection, but cannot be used as a universal screening method. COVID-19 symptoms are atypical with infectious possibility during incubation period, it is necessary to inquire about the epidemiological history and related symptoms for the prevention and control of outbreak in children’s hospitals or pediatric wards at general hospitals. In order to prevent the occurrence and spread of COVID-19 infection, Children’s Hospital of Fudan University formulated this expert recommendation, “Recommendations on the admission and management of pediatric patients during COVID-19 outbreak (1st edition)” (referred to as Recommendations). This study is based on a study first published on Chinese Journal of Evidence-based Pediatrics in Chinese (7). We aimed to prevent transmission of COVID-19 infection.

Individualized dextrose dosing for neonatal hypoglycemia

Newborn infants at risk for hypoglycemia are typically managed with feedings or the administration of dextrose gel. Despite these measures, many infants require admission to a special care nursery or neonatal intensive care unit (NICU) for intravenous dextrose. Unfortunately, NICU admission separates mothers and infants, and may lead to the unintended consequence of difficulties in establishing breastfeeding. Therefore, it is important to investigate methods of management of hypoglycemia that decrease separation of mothers and infants and reinforce breastfeeding. In this volume of The Journal of Pediatrics, Sen et al describe shorter NICU stays after a protocol change that utilized admission glucose levels to titrate the initial dose of intravenous dextrose for infants with hypoglycemia. As the study was not a clinical trial, more research on the safety and effectiveness of this approach is needed to support translation into widespread practice. However, the results of this study will be good news for neonatologists who struggle with the unintended consequences when otherwise healthy newborn infants are brought into the NICU with low blood sugar. How to publish in pediatrics journals with impact factor: Role of open access journals

To publish articles in prestigious scientific journals is not a simple task, particularly because of three main reasons: the difficulty for designing and performing competitive and high quality research, the high rates of rejection in most high-impact journals, and the absence of systematized training in the methodology of biomedical publications in the curricular programs. If to this is added the progressive complexity of the instructions for authors and the formal requirements that most journals impose, it is logical that there is discouragement among potential authors. On the other hand, the pressure and the demand for authorship of scientific articles to be able to get academic and professional positions of a certain level are increasing. However, what at first glance seems a gloomy perspective, it is not so much if some key aspects related to the structure and writing of manuscripts and the systematics of the editorial process of the journals are known and applied, which, in short, continue being in force since the aphorism «publish or perish» became popular at the beginning of the last century. As described in this article, the steps to follow are straightforward, logical, and interrelated, so getting off to a good start and completing the various stages properly and in the right order always represents a clear advantage in ensuring the final success of having your paper accepted.

A Critical Analysis of Heart Diseases Research Literature Based on the Scopus Database during the Period 2009-2018

The study aims to examine heart diseases research productivity during 2009-2018as indexed in the Scopus database. The study examines various scientometric parameters. It was found that a total of 5056 publications received 65165 citations and the maximum 645 publications were came in the year 2016. The relative growth rate (RGR) was shown in decreasing trends (0.85 to 0.14) while the Doubling time (Dt) is depicted in increasing trends (0.81 to 5.34) during the period of study. The average degree of authors’ collaboration, collaboration coefficient and the collaborative index was recorded (0.98), (0.77) and (4.70) respectively. Saxena, A was the most productive author from All India Institute of Medical Sciences (AIIMS) New Delhi with 55 publications in heart diseases research; 29 h-index and a total 3608 citations, followed by Talwar, S with the same institution contributed 50 research papers; 17 h-index; and 1407 citations and All India Institute of Medical Sciences, New Delhi is also most productive institution with 431 contributions published in Indian Journal of Pediatrics, followed by Postgraduate Institute of Medical Education and Research with 175 contribution. Medicine is the most famous subject area in heart diseases research with 3401 publications, followed by Biochemistry, Genetics and Molecular Biology with 857 publications. The most significant keywords are Human in 3695 publication, followed by Article keyword in 2787 publications. Indian Heart Journal was the most popular journal with 208 publications and the journal cite score was (0.79), followed by Journal of Clinical and Diagnostic Research with 153 publications while the particular journal cites score is (0.86). The most famous funding agency name is Indian Council of Medical Research (ICMR) with 68 publications, followed by National Institute of Health (NIH) with 32 contributions from the marked period of study.

Publishing patterns and citation performance of manuscripts relating to paediatric cardiology and congenital heart disease: comparison of paediatric and adult cardiology journals.

Manuscripts pertaining to paediatric cardiology and CHD have been published in a variety of different journals. Some of these journals are journals dedicated to paediatric cardiology, while others are focused on adult cardiology. Historically, it has been considered that manuscripts published in journals devoted to adult cardiology have greater citation potential. Our objective was to compare citation performance between manuscripts related to paediatric cardiology and CHD published in paediatric as opposed to adult cardiology journals. We identified manuscripts related to paediatric cardiology and CHD published in five journals of interest during 2014. Of these journals, two were primarily concerned with adult cardiology, while the other three focused on paediatric cardiology. The number of citations for these identified manuscripts was gathered from Google Scholar. We compared the number of citations (median, mean, and 25th, 75th, 90th, and 95th percentiles), the potential for citation, and the h-index for the identified manuscripts. We identified a total of 828 manuscripts related to paediatric cardiology and congenital heart as published in the 5 journals during 2014. Of these, 783 (95%) were published in journals focused on paediatric cardiology, and the remaining 45 (5%) were published in journals focused on adult cardiology. The median number of citations was 41 in the manuscripts published in the journals focused on adult cardiology, as opposed to 7 in journals focused on paediatric cardiology (p < 0.001). The h-index, however, was greater for the journals dedicated to paediatric cardiology (36 versus 27). Approximately one-twentieth of the work relating to paediatric cardiology and CHD is published in journals that focus predominantly on adult cardiology. The median number of citations is greater when manuscripts concerning paediatric cardiology and CHD are published in these journals focused on adult cardiology. The h-index, however, is higher when the manuscripts are published in journals dedicated to paediatric cardiology. While such publications in journals that focus on adult cardiology tend to generate a greater number of citations than those achieved for works published in specialised paediatric cardiology journals, the potential for citation is no different between the journals. Due to the drastically lower number of manuscripts published in journals dedicated to adult cardiology, however, median performance is different.

New Guidance on Cytochrome P450 2C19 Phenotype-based Use of Proton Pump Inhibitors.

Since the approval of the first proton pump inhibitor (PPI) in 1989, our knowledge regarding this class of medications has further developed. An increasing amount of data now supports the association between cytochrome P450 2C19 (CYP2C19) phenotype and PPI safety and efficacy. This includes pediatric studies, such as those published here and in other pediatric journals within the past year. Moreover, the most recent pediatric Helicobacter pylori guidelines stated that using the PPIs that are less dependent on CYP2C19 for inactivation may be preferred for H pylori eradication among populations that are more likely to have rapid clearance of CYP2C19-metabolized PPIs. Conversely, pantoprazole package insert recommends a dose reduction in known pediatric CYP2C19 poor metabolizers (PMs), citing a 6-fold increase in serum concentrations compared with normal metabolizers (NMs). The purpose of this communication is to introduce a recently published Clinical Pharmacogenetics Implementation Consortium (CPIC) guideline for CYP2C19 and PPI dosing.
