Biología en reproducción asistida. Introducción

El autor ha tomado como base de este manuscrito el contenido de la introducción al simposio sobre Fertilidad Asistida publicada en la Revista Peruana de Ginecología y Obstetricia en el año 2012(1). La infertilidad en el Perú y en casi todos los países no es considerada como una enfermedad importante para ser tomada en cuenta como un problema de Salud Pública(2). Según el Instituto Nacional de Estadística e Informática(3), en el Perú debemos ser en el año 2018 alrededor de 32 170 000 de personas. Dentro del crecimiento poblacional, deberían haber 10 292 180 que estuvieran en el rango de 20 a 39 años. Según la Organización Mundial de la Salud, el 15% de ellas, es decir 1 543 827, presentarían problemas de infertilidad.

Importancia de los encuentros de editores de revistas médicas especializadas

En la editorial del número 4 del 2017 se dan a conocer los resultados del encuentro de editores de revistas latinoamericanas de obstetricia y ginecología, Cancún 2017(1), estos eventos permiten el intercambio de experiencias y colaboración(2), y deberían ser replicadas por otras revistas especializadas. En la actualidad de las siete revistas latinoamericanas especializadas en ginecología y obstetricia indizadas en SciELO, seis de ellas se encuentran además indizadas en Scopus con excepción de la Revista Peruana de Ginecología y Obstetricia. Por tanto, la colaboración entre editores y asociaciones que promueven revistas científicas son una fortaleza que podrían apoyar una futura indización.

Recommendations for the use of ECT in pregnancy: literature review and proposed clinical protocol.

Psychiatric disorders are common in pregnancy, affecting 15-29% of pregnant women. Untreated depression has negative health consequences for mother and fetus. Electroconvulsive therapy (ECT) is an effective option for the treatment of severe depression, high suicide risk, catatonia, medication-resistant illness, psychotic agitation, severe physical decline, and other life-threatening conditions. To our knowledge, however, there is no literature that consolidates all the evidence on maternal and fetal risks associated with untreated depression, medications, and ECT then translating it into one cohesive protocol that could serve as a management guide and a source of reassurance to health-care providers involved in such practice. Hoping to facilitate ECT access to perinatal patients, the authors combined their multidisciplinary clinical experience (in perinatal psychiatry, neuropsychiatry and neuromodulation, and anesthesiology) at three different centers in the USA (Brigham and Women's Hospital/Harvard Medical School, The University of Chicago, and Brown University) with a careful and critical literature review and propose guidelines for the administration of ECT in pregnancy. A comprehensive review of the relevant literature regarding both ECT and psychotropic medications in pregnancy was performed, including meta-analyses of randomized controlled trials published in general medicine, anesthesiology, psychiatry, and obstetrics journals and guidelines. The indication and appropriateness of ECT in pregnancy must be carefully weighed against the risks of untreated maternal illness and those of alternative treatment options. The safety of ECT in pregnancy has been documented over the last 50years. The adverse effects in pregnancy are similar to the risks of ECT in any individual. The most common risk to the mother is premature contractions and preterm labor, which occur infrequently and are not clearly caused by ECT. The rates of miscarriages were not significantly different from that of the general population. There have been no associations of ECT with congenital anomalies, either morphologic or behavioral, and no neurocognitive disturbances in the child. ECT is a reasonably safe and effective treatment alternative for management of many psychiatric disorders in pregnant patients. The authors provide recommendations for treatment modifications in pregnancy-based physiologic changes that occur during that period and consolidate them into a protocol that can assist clinicians in improving access and safety of ECT for pregnant patients.

A 15-Year Study of Trends in Authorship by Gender in Two U.S. Obstetrics and Gynecology Journals.

To assess whether there was a significant trend in the number of women as first, last, and coauthors in two U.S. journals of obstetrics and gynecology over the past 15 years and to assess whether this publication rate was commensurate with the percentage of women in the academic specialty. This retrospective study identified the gender of first and last authors of original research articles for the years 2000, 2005, 2010, and 2015. We determined trends in publication rates of authorship and coauthorship by pairing the gender of the first and last authors. We selected a subset of the data to compare the percentages of women authors with MDs at U.S. academic institutions with the nationally reported percentages of women faculty in the specialty. The journals yielded 2,699 articles for analysis. The percentage of articles written by women as first authors increased from 37% in 2000 to 71% in 2015 and, as last authors, increased from 26% in 2000 to 49% in 2015. Paired as coauthors, women published at a rate of 11% in 2000 to 38% in 2015. The analysis of a subset of 1,621 articles showed that as first authors, women published at a rate equal to or above the percentage of women in academics, but as last authors published fewer articles than expected. Women published more articles over time, kept pace or exceeded their faculty percentages as first authors, lagged behind these percentages as last authors, and as coauthors eventually surpassed the publication rate of male coauthors.

An elevated cortisol level is a risk factor for preterm birth and other adverse health conditions in offspring, according to the study authors.iStock/kjekolPTSD-D, which is characterized by altered perception of oneself and the world, is associated with a greater number of lifetime trauma exposures, including a history of childhood maltreatment.“Exposure to early relational trauma that predisposes a person to dissociation and PTSD may affect that individual’s short- and long-term cortisol patterns,” wrote Julia S. Seng, Ph.D., of the University of Michigan and colleagues. The difference between cortisol levels in the women with PTSD-D and the other groups was most apparent in early pregnancy, which is a critical period for fetal development, noted the authors.The study by Seng and colleagues involved 395 women expecting their first child who were divided into four groups: those without trauma, those with trauma but no PTSD, those with lifetime PTSD, and those with PTSD-D (presence of depersonalization and/or derealization consistent with the DSM-5 dissociative subtype definition). The researchers analyzed saliva cortisol specimens collected from these women at three different times on a single day during the first half of their pregnancy. A subsample of 111 women, including women from each of the four cohorts, provided three salivary cortisol specimens per day, 12 times, from early pregnancy to six weeks postpartum. (This sample included 34 women without trauma, 38 with trauma but no PTSD, 31 with PTSD only, and eight with PTSD-D.)In early pregnancy (gestational week 8), the cortisol levels of participants in the PTSD-D group were two times greater in the morning, eight times greater in the afternoon, and 10 times greater at bedtime than the cortisol levels of participants in the non-exposed control group. In late pregnancy (gestational week 32), participants in the PTSD-D group had cortisol levels that were less than two times greater in the morning and 1.5 times greater levels in the afternoon and at bedtime compared with participants in the non-exposed control group. “We don’t know why this group had higher cortisol levels,” Seng told Psychiatric News. “Likely there are other stress-response systems that may be dysregulated by early childhood relational trauma that is often the index trauma for PTSD-D.”These cortisol levels were so high that the researchers considered them “possibly toxic,” but Seng said it isn’t known at what level cortisol sets into motion adverse perinatal outcomes.“Although some women with histories of childhood maltreatment are resilient or recovered by the time they become pregnant, these biological findings indicate that some are very adversely affected psychologically and very stressed during the childbearing year,” the authors wrote. “We can screen and apply a stepped approach to maternity care that includes case-finding and interventions for women with PTSD, posttraumatic depression, and PTSD-D.”The study was funded by a National Institutes of Health grant. ■An abstract of “Gestational and Postnatal Cortisol Profiles of Women With Posttraumatic Stress Disorder and the Dissociative Subtype” can be accessed here. Conflict of interest related to clinical practice is underreported: The case of noninvasive prenatal testing.

Authors of policy statements from the American College of Obstetricians and Gynecologists and from the Society for Maternal-Fetal Medicine do not acknowledge the potential for their clinical income to influence their opinions, or the positions of the societies they represent. These policy statements were published in Obstetrics and Gynecology and the American Journal of Obstetrics and Gynecology, again, without acknowledgment of the potential for conflict of interest. The case of noninvasive prenatal testing, which has threatened the role of maternal-fetal medicine in the practice of prenatal screening and diagnosis, and has significantly reduced the demand for invasive prenatal diagnosis, illustrates the importance of identifying this potential conflict.

Introducción al simposio

El autor ha tomado como base de este manuscrito el contenido de la Presentación a un simposio similar publicado en el año 2009 en la Revista Peruana de Ginecología y Obstetricia (RPGO)(1). El fin último de la medicina, tradicionalmente ha sido el procurar la mejoría de la persona enferma evitando hacer daño, según la máxima dudosamente atribuida a Hipócrates primum non nocere; sin embargo, la Bioética como disciplina no surge hasta la segunda mitad del siglo XX. Para muchos investigadores del tema, el nacimiento del concepto de bioética, aunque no se denominaba con ese término, se produce en Seattle (EE. UU.) al crearse a comienzos de los años sesenta un comité no médico para decidir quiénes tenían preferencia para hemodiálisis. La pregunta de fondo era ¿por qué un avance médico debería crear una discriminación médica? ¿Cómo y quién elegía a los candidatos? La respuesta a estos interrogantes no recaía sobre los médicos, sino sobre una representación de la comunidad(2).
