Treatment of Neer IIb distal clavicular fracture with locking plate under arthroscope

To investigate the feasibility and clinical effect of the treatment of NeerⅡb distal clavicular fracture with locking plate under arthroscopy. Between June 2018 and September 2022, the medical records of 15 patients with NeerⅡb distal clavicular fracture treated with locking plate under arthroscope were retrospectively analyzed, including 9 males and 6 females, aged from 25 to 57 years old with an average of (42.50±7.75) years old, 5 left shoulder, 10 right shoulder, and duration of the disease ranged from 4 to 7 days with an average of (5.16±0.98) d. Visual analog score (VAS), Constant-Murley shoulder joint score scale and Neer standard score were used before operation, 1 month after operation and the last follow-up. All patients successfully completed the treatment of NeerⅡb distal clavicular fracture with locking plate under arthroscopy, with the operation time ranging from 0.3 to1.0 h with an average of (0.55±0.24) h. All patients were healed at stageⅠ. The follow-up time ranged from 6 to 15 months with an average of (9.2±2.8) months. The preoperative VAS score was 7.23±0.67, the Constant-Murley shoulder joint score was (19.57±0.91), and the Neer standard score was (11.27±1.12);The VAS score was (1.56±0.81), the Constant-Murley shoulder joint score was (52.62±1.54), and the Neer standard score was (61.98±2.99) in the first month after operation;At the last follow-up, the VAS was (0.42±0.54), the Constant-Murley shoulder joint score was (91.24±1.97), and the Neer standard score was (93.24±3.38). The difference between VAS, Constant-Murley shoulder joint score and Neer standard score was statistically significant at one month postoperative and preoperative (P<0.05), and the difference between the last follow-up visit and one month postoperative was statistically significant (P<0.05). The treatment of NeerⅡb distal clavicular fracture with locking plate under arthroscope can restore the function of shoulder joint, with definite curative effect and feasibility.

Using at least 20% medial unicompartmental knee arthroplasty is associated with improved patient-reported outcome measures across all knee arthroplasty patients.

To investigate the impact of orthopaedic surgeons' arthroplasty distributions on patient-reported outcome measures (PROMs) following knee arthroplasty, thus addressing the gap in knowledge regarding the optimal distribution of arthroplasties. 2256 knee arthroplasties were included (total knee arthroplasty [TKA] or unicompartmental knee arthroplasty [UKA]). All were conducted at a single centre between August 2016 and August 2022 with a minimum of 1-year follow-up. The Oxford Knee Score (OKS), the Forgotten Joint Score (FJS) and the Activity and Participation Questionnaire (APQ) were assessed preoperatively, and at 3 and 12 months postoperatively. Patients were categorized based on the surgeons' yearly surgeries: (1) TKA only, (2) TKA+ <20% medial UKA, (3) TKA+ ≥20% medial UKA and (4) TKA+ ≥20% medial UKA + lateral UKA + patellofemoral UKA. Linear regression models adjusted for demographic variables and preoperative PROM scores were used to estimate changes in mean PROM scores. Group 4 showed significantly higher improvements in PROM scores at 3 and 12 months compared to Group 1. In the 12-month adjusted analysis, Group 4 had 1.9 points (95% confidence interval [CI]: 1.0-2.8) higher OKS-, 7.0 points (95% CI: 3.9-10.2) higher FJS- and 8.3 points (95% CI: 4.8-11.8) higher APQ-change than Group 1. There were no significant differences between Groups 1 and 2, nor any clinically relevant differences between Groups 3 and 4. Additionally, the percentage of patients who achieved excellent OKS (>41) was significantly higher in Groups 3 + 4 compared to Groups 1 + 2 (p < 0.001). Despite limitations, the findings of this study suggest that utilizing ≥20% medial UKA leads to greater postoperative improvements in PROM across all treated knee arthroplasty patients. Level III.

Clinical and Radiographic Outcomes With Minimum 2-Year Follow-up and Sub-Analysis of Navigation vs Non-Navigation for Hip Resurfacing.

The purpose of this study was to report the short-term clinical outcomes of hip resurfacing with navigation and the impact on accuracy of acetabular implant placement in both the frontal and sagittal planes. Data were retrospectively analyzed for patients who received hip resurfacing between 2010 and 2021. Eligible patients had postoperative radiographs and completed a minimum 2-year follow-up questionnaire for the following patient-reported outcomes: modified Harris Hip Score (mHHS), Harris Hip Score (HHS), Forgotten Joint Score (FJS), visual analog scale (VAS) score, satisfaction, and Hip Disability and Osteoarthritis Outcome Score, Joint Replacement (HOOS-JR). Hips were propensity matched in a 1:1 ratio based on the use of navigation, age, and body mass index. The percentage of hips that met the minimal clinically important difference (MCID) for mHHS and VAS score was noted. Component placement analysis was conducted based on the safe zones defined by Lewinnek and Callanan and the Relative Acetabular Inclination Limit. Seventy-six hips were matched, 38 per group. No differences were observed in patient-reported outcomes or the percentage of hips reaching MCID between the groups. The navigation group was 28.8 and 6.8 times more likely to be within the Callanan and Lewinnek safe zones, respectively. Based on the Relative Acetabular Inclination Limit, the navigation group was 3.1 and 6.4 times more likely to be within the 95% and 99% CI safe zones, respectively. Comparable improvements in patient-reported outcomes were observed in the two groups during a minimum 2-year follow-up. Navigation-assisted surgery enhances the accuracy of acetabular component positioning, with a higher likelihood of cup placement within the safe zones. [Orthopedics. 202x;4x(x):xx-xx.].

UCC: A unified cascade compression framework for vision transformer models

In recent years, Vision Transformer (ViT) and its variants have dominated many computer vision tasks. However, the high computational consumption and training data requirements of ViT make it challenging to be deployed directly on resource-constrained devices and environments. Model compression is an effective approach to accelerate deep learning networks, but existing methods for compressing ViT models are limited in their scopes and struggle to strike a balance between performance and computational cost. In this paper, we propose a novel Unified Cascaded Compression Framework (UCC) to compress ViT in a more precise and efficient manner. Specifically, we first analyze the frequency information within tokens and prune them based on a joint score of their both spatial and spectral characteristics. Subsequently, we propose a similarity-based token aggregation scheme that combines the abundant contextual information contained in all pruned tokens with the host tokens according to their weights. Additionally, we introduce a novel cumulative cascaded pruning strategy that performs bottom-up cascaded pruning of tokens based on cumulative scores, avoiding information loss caused by individual idiosyncrasies of blocks. Finally, we design a novel two-level distillation strategy, incorporating imitation and exploration, to ensure the diversity of knowledge and better performance recovery. Extensive experiments demonstrate that our unified cascaded compression framework outperforms most existing state-of-the-art approaches, compresses the floating-point operations of ViT-Base as well as DeiT-Base models by 22 % and 54.1 %, and improves the recognition accuracy of the models by 3.74 % and 1.89 %, respectively, significantly reducing model computational consumption while enhancing performance, which enables efficient end-to-end training of compact ViT models.

Effect of tibial rotation on knee and ankle function in patients with extra-articular distal tibial fractures after minimally invasive plate osteosynthesis treatment

To investigate the effect of tibial rotation on knee and ankle function in the patients with extra-articular distal tibial fractures after minimally invasive plate osteosynthesis (MIPO) treatment. A retrospective analysis was conducted on 45 patients with extra-articular distal tibial fractures who underwent MIPO between January 2021 and December 2022. There were 20 males and 25 females, aged from 19 to 68 years (mean, 43.6 years). The causes of fractures included falling from heights in 11 cases, traffic accidents in 15 cases, bruising by a heavy object in 7 cases, and falling in 12 cases. The time from injury to hospitalization was 1-6 hours (mean, 3.7 hours). There were 28 cases of simple tibial fractures and 17 cases of tibial fractures combined with ipsilateral fibular fractures; 9 cases of open fractures and 36 cases of closed fractures. According to AO/Orthopaedic Trauma Association (AO/OTA) classification, the distal tibial fractures were classified as type 43A1 in 12 cases, type 43A2 in 17 cases, and type 43A3 in 16 cases. During follow-up, the Lower Limb Function Score (LEFS), American Orthopaedic Foot and Ankle Society (AOFAS) score, and Knee Injury and Osteoarthritis Score (KOOS) were used to evaluate the lower limb, ankle, and knee joint functions, respectively. The range of motion (ROM) of the knee and ankle joints was measured. The Short Form Health Survey (SF-36) was used to assess the quality of life of the patients. The fracture healing was observed by anteroposterior and lateral X-ray films of the ankle joint. The bilateral tibial rotation angles were measured on CT and the differences between the affected and healthy sides were caculated. The difference ≥10° was judged as tibial malrotation. According to whether there was tibial malrotation, the patients were allocated into a normal group and a malrotation group for efficacy comparison. The incisions of all patients healed by first intention without any early complications. All patients were followed up 12-26 months (mean, 18.6 months). Imaging re-examination showed that all fractures healed, with a healing time of 9-14 weeks (mean, 11.2 weeks). At last follow-up, the LEFS score was 60-68 (mean, 62.3); the AOFAS score was 89-97 (mean, 92.6); the KOOS score was 158-164 (mean, 161.3). The ROM of ankle was 40.0°-45.0° (mean, 42.8°) in flexion and 10.5°-22.0° (mean, 17.7°) in extension; the ROM of knee was 130.0°-135.0° (mean, 132.6°) in flexion and -8.8°- -5.0° (mean, -7.1°) in extension. The SF-36 score was 89-93 (mean, 90.7). The absolute value of difference of tibial rotation angle was 2.6°-17.3° (mean, 8.9°) in 45 patients, and the malrotation was observed in 15 patients (33.3%), including 10 cases of internal rotation and 5 cases of external rotation. There was no significant difference ( P>0.05) in the proportion of patients with fibular fractures before operation between the malrotation group and the normal group, as well as in the AOFAS score, KOOS score, LEFS score, SF-36 score, and ROMs of knee and ankle joints at last follow-up. The incidence of tibial malrotation after MIPO treatment for extra-articular distal tibial fractures is relatively high, but it has no significant effect on knee and ankle functions. However, careful manipulation and precise evaluation should be performed during operation to avoid the occurrence of malrotation.

The efficacy of patellar denervation on prognosis and kneeling capacity after unicompartmental knee arthroplasty: a randomized clinical trial.

The aim of this study was to investigate the effect of patellar denervation (PD) on pain, function and ability to kneel after unicompartmental knee arthroplasty (UKA). Patients with medial knee osteoarthritis who underwent UKA were prospectively selected. Patients were randomly divided into PD and non-PD groups based on whether patellar denervation was performed. Clinical assessment was performed using the Hospital for Special Surgery (HSS) knee score, Kujiala score, visual analogue scale (VAS) and forgotten joint score (FJS-12), as well as postoperative complications were recorded. The patients' postoperative self-perception and actual ability to perform different kneeling positions were assessed in the two groups. UKA patients treated with PD achieved better Kujiala scores and FJS-12 scores, reduced anterior knee pain and improved kneeling ability postoperatively, validating the effectiveness of PD in UKA. Perception and actual performance of kneeling remained mismatched in PD patients, but performance during different kneeling activities was generally better than in non-PD patients. Patellar denervation can safely and effectively improve patellofemoral joint function, pain and kneeling ability in the early postoperative period after UKA.

Metabolic syndrome and patient-reported outcome two years after hip and knee arthroplasty.

The influence of metabolic syndrome (MetS) on the outcome after hip and knee arthroplasty is debated. We aimed to investigate the change in patient-reported outcome measure (PROM) scores after hip and knee arthroplasty, comparing patients with and without MetS. From 1 May 2017 to 30 November 2019, a prospective cohort of 2,586 patients undergoing elective unilateral hip and knee arthroplasty was established in Denmark. Data from national registries and a local database were used to determine the presence of MetS. Patients' scores on Oxford Hip Score (OHS) or Oxford Knee Score (OKS), EuroQol five-dimension five-level questionnaire (EQ-5D-5L), University of California, Los Angeles (UCLA) Activity Scale, and Forgotten Joint Score (FJS) at baseline, three, 12, and 24 months after surgery were collected. Primary outcome was the difference between groups from baseline to 12 months in OHS and OKS. Secondary outcomes were scores of OHS and OKS at three and 24 months and EQ-5D-5L, UCLA Activity Scale, and FJS at three, 12, and 24 months after surgery. Generalized linear mixed model was applied, adjusting for age, sex, Charlson Comorbidity Index, and smoking to present marginal mean and associated 95% CIs. A total of 62.3% (1,611/2,586) of the cohort met the criteria for MetS. Both groups showed similar increase in mean OHS (MetS group 22.5 (95% CI 21.8 to 23.1), non-MetS group 22.1 (21.3 to 22.8); p = 0.477) and mean OKS (MetS group 18.0 (17.4 to 18.6), non-MetS group 17.8 (17.0 to 18.7); p = 0.722) at 12 months' follow-up. Between groups, similar improvements were seen for OHS and OKS at three and 24 months postoperatively and for the mean EQ-5D-5L, EuroQol-visual analogue scale (EQ-VAS), UCLA Activity Scale, and FJS at every timepoint. Patients meeting the criteria for MetS obtain the same improvement in PROM scores as individuals without MetS up to 24 months after hip and knee arthroplasty. This is important for the clinician to take into account when assessing and advising patients with MetS.

The Impact of Previous Lumbar Spine Surgery on Primary Total Hip Arthroplasty: Minimum 2-Year Outcomes Controlling for Approach and Technology

BackgroundPrior lumbar spine surgery (LSS) can limit spine mobility, potentially increasing hip motion requirements. This study aimed to assess the influence of LSS on primary total hip arthroplasty (THA). MethodsRetrospective analysis was conducted on patients who underwent THA with prior LSS. Included patients had completed patient reported outcomes (PRO) and visual analog scale (VAS) for pain questionnaires or had a documented revision surgery with a minimum 2-year follow-up. Patients were propensity matched to a benchmark control group of patients who did not have previous spine pathology in a 1:3 ratio, controlling for age at surgery, surgical approach, use of advanced technology, sex, and body mass index (BMI). The analysis included comparisons of hip arthroplasty thresholds and complications. Sub-analyses based on the type of lumbar surgery and type of approach were also conducted. ResultsThere were 244 hips included in the study. The LSS group reported comparatively lower postoperative PRO scores. However, the LSS group experienced a similar magnitude of improvement. The LSS group met patient acceptable symptom state (PASS) threshold for Forgotten Joint Score (FJS) at a significantly lower rate. The LSS had a higher frequency of complications leading to revision THA with a relative risk of 24 and a relative risk of 20.8 for revision THA due to instability. Comparing patients by type of LSS, PROs, the percentage of patients reaching hip arthroplasty thresholds, and the revision THA rates were similar. There was no significant difference in complications considering the type of approach. ConclusionPatients who had prior LSS who underwent primary THA demonstrated equivalent improvements in PROs, but achieved lower overall postoperative scores and met hip arthroplasty thresholds at lower rates. Furthermore, patients undergoing primary THA with prior LSS had a higher risk of complications leading to revision surgery, including a 24-fold relative risk for complications leading to revision and a 20.8-fold relative risk of revision due to instability.

Clinical outcomes and complications of a non-modular dual-mobility acetabular device, minimum 2 years follow-up.

Non-modular dual-mobility (DM) acetabular bearing constructs are becoming more popular due to their potential advantages for addressing range of motion (ROM) and stability. The objective of this post-market clinical study was to assess clinical outcomes, including patient satisfaction, pain and function, survivorship, and component positioning, in patients ⩾2 years after primary THA using non-modular DM acetabular components. This retrospective, multicentre study evaluated complications and functional outcomes in recipients of non-modular DM acetabular constructs at least 2 years post-THA. Our primary outcomes included the dislocation rate and mean Hip disability and Osteoarthritis Outcomes Score (HOOS). Secondary outcomes included radiographic measurements, patient satisfaction, Forgotten Joint Score (FJS), Harris Hip Score (HHS), and survivorship. 316 eligible patients enrolled in this study across 3 sites, including 168 men (53%) and 148 women (47%). The cohort averaged 5.37 years post-THA (range 2.2-10.1 years) at the study visit. The mean FJS, HHS, and HOOS were good to excellent, with values of 82.3, 90.7, and 92.0, respectively. Patient satisfaction averaged 4.68, between "Very Good" (4) and "Excellent" (5). Minimal radiolucent lines (RLL) surrounded the acetabulum. More RLL surrounded the femur, but most lines were less than 1 mm (4.7% of the study population). There were few adverse events (4 events, 1.3%). Three revisions were performed (0.9%), with one patient requiring removal of the acetabular cup. The high mid- to long-term survivorship and low to no dislocation and reoperation rates indicate this acetabular DM device is a viable option for THA patients. The patient metrics reflect satisfaction, alleviated pain, and restored function with a low risk of adverse outcomes.

No Radiographic Evidence of Medial Collateral Ligament Elongation in Valgus Osteoarthritic Knees Enables Treatment with Kinematically Aligned Total Knee Arthroplasty.

When performing caliper-verified kinematically aligned total knee arthroplasty (KA TKA) in the osteoarthritic (OA) knee with valgus deformity, an elongated medial collateral ligament (MCL) could result in a valgus setting of the tibial component. The present study analyzed KA TKA in patients with valgus deformities (i.e., tibiofemoral angle > 10 degrees of valgus) and determined (1) the occurrence of radiographic MCL elongation, (2) the incidence of lateral collateral ligament (LCL) and posterior cruciate ligament (PCL) release and the use of constrained components, and (3) whether the 1-year Forgotten Joint Score (FJS), Oxford Knee Score (OKS), Knee Injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS JR), and Likert satisfaction score were comparable to KA TKAs for OA deformities ≤10 degrees of valgus. One hundred and two consecutive patients who underwent KA TKA by a single surgeon were analyzed radiographically and clinically at a minimum follow-up of 1 year. Radiographic MCL elongation was identified by a greater than 1 degree of valgus orientation of the tibial component relative to the OA tibial joint line. Twenty-six patients had a radiographic anatomic tibiofemoral angle greater than 10 degrees of valgus (range of OA deformity: 11-23 degrees of valgus). Seventy-six had an OA deformity ≤10 degrees of valgus (10-degree valgus to -14-degree varus). No patient had MCL elongation or a ligament release, or required constrained components. The median FJS of 78, OKS of 42, and KOOS JR of 76, and the 85% satisfaction rate of the patients with greater than 10 degrees of OA valgus deformity were not significantly different from those with ≤10 degrees of OA valgus deformity (p ≥ 0.17). Because MCL elongation was not detected in OA deformities up to 23 degrees of valgus, the risk of under-correcting the valgus deformity leading to instability and poor outcome scores is low when performing KA TKA using primary components without releasing the LCL and/or PCL. Age, activity, and native femoral offset are associated with articular noise in ceramic on ceramic total hip arthroplasty (THA) with custom stems.

To report the prevalence of articular noise following total hip arthroplasty (THA) with custom stems and ceramic-on-ceramic (CoC) bearings, to determine the risk factors for articular noise, and the effect of articular noise on quality of life (QoL). A consecutive series of uncemented primary THAs using custom stems implanted between 01/02/ 2014-01/04/2017 were evaluated. The cohort comprised 479 patients (529 hips, 301 males and 228 females), aged 55.9 ± 11.6 with a BMI of 25.8 ± 4.3kg/m2. Postoperative assessment included Oxford hip score (OHS), forgotten joint score (FJS), and a dedicated questionnaire on articular noise. Descriptive statistics were used to summarise the data. Regression analyses were performed to study factors associated with presence of noise. At a minimum follow-up of fiveyears, 431 patients (476 hips) were available for postoperative assessment. OHS was 45.3 ± 6.1, FJS was 83.6 ± 24.7, and 69 patients (71 hips, 15%) reported articular noise. The impact of noise on QoL was 1.4 ± 2.1. Multivariable analyses confirmed that the presence of articular noise was associated with younger age (OR,0.95; 95%CI,0.93-0.97; p < 0.001), smaller native femoral offset (OR, 0.95; 95%CI,0.90-1.00;p = 0.034), as well as intense (OR, 3.15; 95%CI, 1.15-9.79; p = 0.033) and very intense physical activity (OR, 4.71; 95%CI, 1.52-16.15; p = 0.009). The prevalence of articular noise following primary THA with custom stems and CoC bearings was 15%, butthe impact of noise on QoL was minimal for most patients. Younger, highly active patients should be advised of an increased likelihood of noise from CoC THA, particularly if they have low native femoral offset on preoperative imaging.

Magnetic Resonance Imaging of Temporomandibular Joint and Aortic Root Score in Fibrillinopathies.

Background and Objectives: Fibrillinopathies have different phenotypic expression: Marfan-like skeleton features, MLSF; mitral valve prolapse syndrome, MVPS; MASS phenotype, M = mitral valve prolapse, MVP, A = aortic root dilation, S = skeletal features, and the second S = (cutaneous) striae; Marfan Syndrome, MS. The study had the following main objectives: the correlation between disc displacement, DD (a major sign for temporomandibular joint dysfunction, TMDs) on magnetic resonance imaging, MRI, and aortic Z score (a major sign for aortic root dilation) on echocardiography; the predictive value of DD towards aortic Z score. DD had 2 types of severity: DD with reduction (the mild type, DDwR), and DD without reduction (the severe type, DDwoR). Materials and Methods: The type of fibrillinopathy was precised by clinical exam (systemic score), ophthalmic exam (for ectopia lentis), and echocardiography (aortic Z score, MVP). Medical treatment consisted of betablockers, BB (for patients with tachycardia), or angiotensin II receptor blockers, ARB. Surgical treatment was addressed to aortic root aneurysm, and severe mitral regurgitation, MR. DD benefited from dental conservatory treatment or surgical restoration. Results: DD-Z score had a powerful correlation in MASS (Rho = 0.787, p < 0.01), and in MS patients (Rho = 0.819, p < 0.01). For the entire sample, both DDwR-Z score and DDwoR-Z score had a moderate correlation (Rho = 0.469, and respectively 0.669). Furthermore, DD was an important predictor for Z score. DDwoR had a double power of prediction for the Z score compared with DDwR (B coefficient = 1.661 for DDwR and 3.281 for DDwoR). Conclusions: TMDs had a powerful correlation with aortic root dilation in MASS and MS patients from the sample. Likewise, TMDs was a major predictor for aortic root dilation, in the entire sample. In clinical practice, we can utilize an extracardiac finding, TMDs, obtained by a non-invasive technique, MRI, for cardiac severity stratification of fibrillinopathies.

High patient satisfaction with Customized Total Knee Arthroplasty at five year follow-up.

Despite numerous studies demonstrating promising short-term outcomes of Total Knee Arthroplasty (TKA) with Customized Individually Made (CIM) implants, there is a significant lack of data on their mid-term effectiveness. Given the increasing number of TKAs performed annually, the rising demand for CIM implants, and the associated burden of revision surgeries, understanding the mid-term performance of CIM implants is crucial. Therefore, this study aims to report on the mid-term (minimum 5 years) outcomes of TKA using a CIM implant. This retrospective cohort study included a consecutive series of 116 patients who received the ConforMIS® iTotal CR implant between 2015 and 2018. Inclusion criteria were end-stage knee osteoarthritis with coronal deformities below 10° and absence of ligamentous instability. Exclusion criteria included simultaneous bilateral TKA. Patients were followed up at a minimum of five years post-surgery. They completed a questionnaire reporting on satisfaction, pain levels using the Visual Analogue Scale (VAS), current weight, the Oxford Knee Score (OKS), and the Forgotten Joint Score for the knee (FJS-knee). Statistical analysis included descriptive statistics for demographic and clinical variables, and outcomes were reported as means with ranges. The mean follow-up duration was 5.9 ± 0.8 years (range 5-7.4 years). 90% of patients stated they would undergo the same operation again, and 93% were either satisfied or very satisfied. The mean VAS for pain at rest was 2 ± 1.5 (range 0-6) and during exercise was 3 ± 2 (range 0-8). 58 patients (53%) managed to lose weight. The mean OKS was 41 ± 9 points (range 15-48), and the mean FJS-knee was 67 ± 23 points (range 4-100). No severe complications occurred. CIM TKA using the ConforMIS® iTotal CR implant can achieve excellent results with 93% of patients being satisfied or very satisfied at mid-term follow-up of five years. Prospective, randomized, and patient-blinded trials comparing off-the-shelf (OTS) TKAs with CIM implants are necessary to evaluate whether these implants are superior or not.

Seismic performance of earthquake-damaged HSRAC beam-column interior joints rehabilitated by bonding steel plates in the plastic hinge of beams

In order to investigate the seismic performance of earthquake-damaged high-strength recycled aggregate concrete (HSRAC) beam-column interior joints employing ultra-high strength bars (UHSB) in columns, four specimens were designed and manufactured. The research variables included whether the steel tube was encased in the column and whether beams were rehabilitated by bonding steel plates. Among them, the rehabilitated specimens were firstly loaded to 1.0 % drift ratio, and then loaded to failure after rehabilitating the plastic hinge of beams. The test results showed that the beam hinge failure mode occurred in the specimens loaded directly to failure. The damage of columns and joint cores was relatively slight, and the crack width satisfied the repairability limits before 2.0 % drift ratio. The shear failure of the joint core occurred in the rehabilitated specimen without steel tube, as shear deformation and stirrup strain in the joint core increased. The shear bearing capacity of joints increased after steel tubes were encased in columns, and the damage of joint cores was reduced. The bearing capacity and deformation capacity of specimens improved after rehabilitating. Finite element models were established and verified with experiment results. Parameter analysis was conducted for different pre-damage degrees, and the results indicated that as pre-damage degree increased, the bearing capacity of specimens decreased.

Hybrid rehabilitation of 3D RC beam-column joints using UHP-HFRC and FRP wrapping: Experimental evaluation and theoretical analysis

Beam-column joints are crucial elements in reinforced concrete (RC) structures subjected to seismic loading, as they are vulnerable elements prone to significant damage during earthquakes. Due to extensive structural damage, various methods have been developed to strengthen beam-column joints. However, research on rehabilitation and repair methods has been limited, with most studies focusing on 2-D joints and overlooking the confinement effect of corner beams on the joint core. This study aims to investigate novel hybrid rehabilitation systems, including ultra-high performance hybrid fiber-reinforced concrete (UHP-HFRC) and fiber-reinforced polymer (FRP) systems, alongside embedding reinforcement into heavily damaged 3-D beam-column joints subjected to pre-cyclic loading and subsequent cyclic loading. Parameters related to hysteresis response were obtained and discussed, such as ductility, lateral stiffness, dissipated energy, equivalent hysteresis damping, and pinching ratio of the tested specimens. The experimental findings revealed that utilizing an FRP and UHP-HFRC hybrid rehabilitation system enhanced ductility, dissipated energy, and lateral stiffness of heavily damaged joints by 25 %, 73 %, and 81 %, respectively, compared to the undamaged reference specimen. Additionally, a simplified design model was developed to predict the shear resistance of beam-column joints, incorporating principles of strut-and-tie. The theoretical design model was also validated against the experimental database, demonstrating a good accuracy with MV and COV of 0.97 and 13 %, respectively.

Effect of anterior cruciate ligament integrity on the short- and mid-term effectiveness of mobile-bearing medial unicompartmental knee arthroplasty

To investigate the effect of anterior cruciate ligament (ACL) integrity on the short- and mid-term effectiveness of mobile-bearing medial unicompartmental knee arthroplasty (UKA). The clinical data of 128 patients with anteromedial osteoarthritis who underwent mobile-bearing medial UKA between June 2019 and June 2021 was retrospectively analyzed. According to the integrity of ACL structure under direct vision during operation, the patients were divided into normal group (30 cases), synovial defect group (53 cases), and longitudinal split group (45 cases). There was no significant difference in gender, age, body mass index, preoperative knee range of motion (ROM), hip-knee-ankle angle (HKA), Knee Society Score (KSS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and other baseline data among the 3 groups ( P>0.05). The knee ROM, KSS score (including clinical and functional scores), WOMAC score (including pain, stiffness, and function scores) before operation and at last follow-up, the Lysholm score, International Knee Documentation Committee (IKDC) score, and Forgetting Joint Score (FJS-12) at last follow-up were recorded and compared among the 3 groups. Imaging examination was performed to evaluate the wear of cartilage in the lateral tibiofemoral compartment and patellofemoral compartment of the knee joint. HKA and posterior tibial slope (PTS) were measured at last follow-up. All the patients of 3 groups were followed up 22-56 months (mean, 40.4 months). There was no significant difference in follow-up time among the 3 groups ( P>0.05). At last follow-up, imaging examination showed no obvious cartilage wear progression of the lateral tibiofemoral compartment and patellofemoral compartment, and no adverse events such as aseptic loosening of the prosthesis, dislocation of the removable pad, or infection occurred during the follow-up. At last follow-up, knee ROM, HKA, KSS scores, and WOMAC scores of the 3 groups significantly improved when compared with preoperative ones ( P<0.05). There was no significant difference in the changes of the above indicators before and after operation, and also the Lysholm score, IKDC score, FJS-12 score, and PTS among the 3 groups ( P>0.05). The integrity of ACL has no significant effect on the short- and mid-term effectiveness of mobile-bearing medial UKA.

Excellent results of restricted kinematic alignment total knee arthroplasty at a minimum of 10 years of follow-up.

While restricted kinematic alignment (rKA) total knee arthroplasty (TKA) with cemented implants has been shown to provide a similar survivorship rate to mechanical alignment (MA) in the short term, no studies have reported on the long-term survivorship and function. One hundred four consecutive cemented cruciate retaining TKAs implanted using computer navigation and following the rKA principles proposed by Vendittoli were reviewed at a minimum of 10 years after surgery. Implant revisions, reoperations and clinical outcomes were assessed using knee injury and osteoarthritis outcome score (KOOS), forgotten joint score (FJS), patients' satisfaction and joint perception questionnaires. Radiographs were analyzed to identify signs of osteolysis and implant loosening. Implant survivorship was 99.0% at a mean follow-up of 11.3 years (range: 10.3-12.9) with one early revision for instability. Patients perceived their TKA as natural or artificial without limitation in 50.0% of cases, and 95.3% were satisfied or very satisfied with their TKA. The mean FJS was 67.6 (range: 0-100). The mean KOOS were as follows: pain 84.7 (range: 38-100), symptoms 85.5 (range: 46-100), function in daily activities 82.6 (range: 40-100), function in sport and recreation 35.2 (range: 0-100) and quality of life 79.1 (range: 0-100). No radiological evidence of implant aseptic loosening or osteolysis was identified. Cemented TKA implanted with the rKA alignment protocol demonstrated excellent long-term implant survivorship and is a safe alternative to MA to improve patient function and satisfaction. Two-Year Outcomes are Equal in Kellgren-Lawrence Osteoarthritis Grades 1-4 after Total Knee Arthroplasty with Medial-Pivot Implants and Kinematic Alignment.

Introduction While risks exist with total knee arthroplasty (TKA) and survivorship is not indefinite, delaying surgery may lead to further deterioration of the knee joint, functional outcomes, and health status. Most studies report that patients with high-grade osteoarthritis (OA) scores have higher satisfaction and functional scores, cautioning against TKA in those with lower OA grades. This study correlates Kellgren-Lawrence (KL) OA scores with outcomes following kinematically-aligned TKA. Methods Preoperative anteroposterior weight-bearing radiographs of 168 patients enrolled in a prospective trial were scored for KL grade. Patients were segregated into two groups: the low-grade OA group had KL scores of 1 or 2, the high-grade OA group had a KL score of 3 or 4. Outcomes as measured with Forgotten Joint Score (FJS), Knee Society Score (KSS), and Range of motion were compared up to 2-years. Results There were 29 subjects with low-grade KL scores and 127 with high-grade KL scores. There were no differences in BMI, age, or gender distribution. The only statistically significant difference was in the preoperative KSS Pain/Motion score, 60.6 in the low-grade OA group vs. 46.4 in the high-grade group (P&lt;0.008). There were no significant differences between the groups in FJS at one or two-year follow-up. There were no differences in KSS Functional or Composite Scores, or in range of motion (ROM) at pre-op, 1-year or 2-years, and in KSS Pain/Motion at 1 or 2-years. Conclusion There were no significant differences in FJS, KSS, and ROM in patients with low- vs. high-grade OA up to 2-year follow-up. Patients with lower grade OA demonstrated substantial improvements in their FJS, KSS scores, and ROM indistinguishable from the higher-grade OA cohort up to 2-years, suggesting no shorter-term downside to earlier intervention with kinematically-aligned medial-pivot (MP) TKA.

Early Aseptic Tibial Loosening Is a Concern With a Modern Two-Peg Cementless Total Knee Arthroplasty Design

BackgroundWhile early generations of cementless total knee arthroplasty (TKA) had inferior outcomes compared to cemented TKA, modern cementless designs have offered excellent clinical results. The purpose of this study was to compare patient-reported outcome measures (PROMs) and early aseptic revision rates of a specific cementless TKA design featuring a two-pegged tibia to that of its cemented counterpart. MethodsA retrospective case series of all cementless and cemented TKAs utilizing a single design performed at a single, high-volume academic center was performed. Institutional review board approval was obtained. All cases were performed between November 2018 and March 2022. A minimum one-year follow-up was required. Demographics, complications, and reoperation/revision data were collected. Oxford Knee Score and Forgotten Joint Score were collected at one-year follow-up. Radiographic review was performed for cementless TKAs that were revised or had PROMs < one SD (“poor performers”) or had PROMs > one SD (“high performers”) below or above the mean, respectively. ResultsThere were 329 cementless and 349 cemented TKAs included. Mean follow-up was 1.9 and 2.6 years for cementless and cemented cohorts, respectively. There were no statistical PROM differences between the two cohorts. There was no statistical difference in aseptic revision rates between the cohorts (4.0% cementless versus 1.7% cemented, P = 0.078); however, there was a higher rate of tibial aseptic loosening in the cementless cohort (2.7% cementless versus 0% cemented, P = 0.002). The mean time to revision for aseptic tibial loosening was 17.6 months. There was no statistical difference in radiolucencies between “poor performers” and “high performers.” ConclusionsWhen compared to its cemented counterpart, the cementless TKA that was reintroduced in 2018 had similar one-year PROMs but a higher rate of early tibial loosening (2.7 versus 0.0%, P = 0.002). Seismic performance of a novel concrete-filled steel tube column to continuous reinforced concrete beam joint with multiple openings in the core region

This study developed a novel concrete-filled steel tube column to continuous reinforced concrete beam joint with multiple openings in the core region, aiming to eliminate field welding and improve construction efficiency. To evaluate the seismic performance of the joint, six full-scale specimens were tested under cyclic loading, which and a series of numerical simulations were implemented upon refined models. The effects of reinforcement ratio, steel tube thickness, and concrete strength on the failure mode and the mechanical performance degradation of the joint were quantitively evaluated. The results indicated that depending on the beam-column flexural capacity ratio, three typical failure modes, i.e., compression bending failure at the column end, shear failure at the joint core region, and flexural failure at the beam plastic hinge region, were observed. Among them, the flexural failure-dominated joint possessed the best energy dissipation capacity and a high ductility coefficient greater than 3.72. Moreover, increasing the thickness of steel tube within the core region can effectively compensate for the reduction of seismic performance caused by multiple openings, and a preliminary value of at least 2.5 times the thickness of column steel tube is recommended. This novel joint configuration exemplifies a balance between structural integrity, construction efficiency, and mechanical performance, rendering it a feasible alternative for frame structure in seismic-prone regions.
