The Impact of Proper PPE Protocol in A Highly Community Spread of Infectious COVID-19 in Semporna

Introduction: The district of Semporna, Sabah was majorly hit by the third wave surge of COVID-19 in September 2020. At the peak of the crisis, services in Semporna Hospital were paralyzed with 40 healthcare workers (HCW) found to be COVID-19 positive; contributing to 14% of the total manpower.Objectives: Due to the immediate crisis, the main priority was to control the spread of COVID- 19 amongst the Semporna Hospital HCWs. This was important to curb direct infection from handling suspected patients seeking treatment in Semporna Hospital. Based on the hospital capabilities and resources, a comprehensive modified protocol was needed to control the situation.Methods: Semporna Hospital pandemic emergency task force was established with the presence of multi department and specialties. Multiple issues were raised and attended to, especially; issue of preparedness, low PPE stock and Emergency Department infrastructure.Results: The number of Semporna Hospital HCWs infected with COVID-19 was minimal after the initial disaster. Emergency Department infrastructure was improvised, workflow processes modified, HCW protection education prioritized and complete PPE sets were stocked up. All these efforts were under the strict supervision of the infectious control unit. The presence of multiple units of Powered Air-Purifying Respirator (PAPR) completed our adherence to the Ministry of Health (MOH) guidelines in managing the highly infectious level 4 patients for aerosolized generating procedures (AGP).Conclusion: This achievement can be used as a preparedness reference for other non-specialist district hospitals in Malaysia.International Journal of Human and Health Sciences Supplementary Issue: 2021 Page: S21

AAAAI Work Group Report: Trends in Oral Food Challenge Practices Among Allergists in the United States

The oral food challenge (OFC) is the criterion standard for diagnosing food allergy, but prior studies indicate many allergists may not be using OFCs for various reasons. To better understand current OFC trends, practices, and barriers, the American Academy of Allergy Asthma and Immunology (AAAAI) Adverse Reactions to Foods Committee subcommittee updated a 19-item survey (previously administered in 2009) and sent it to AAAAI and American College of Allergy, Asthma, and Immunology (ACAAI) membership. There were a total of 546 respondents who represented approximately a 10% response rate. Among the 546 respondents, compared with 2009, significantly more providers offer OFCs (95% vs 84.5%), offer >10 OFCs per month (17% vs 5.6%), obtain informed consent (82.2% vs 53.6%), and performed OFCs in fellowship (71% vs 45%) (all P < .001). Fellowship OFC training was limited, with 56% performing <10 OFCs in fellowship and 29% performing none. Although 94% see patients <12 months of age, 35.5% do not offer OFCs for early peanut introduction. Although 79% dedicate a supervising medical provider (MD, NP, PA) and 86% have a written OFC protocol, only 60% had a standardized reaction treatment protocol and 56% prepared emergency medications before OFC. Compared with 2009, there was significant worsening of perceived barriers to performing OFCs, including time (65.6% vs 55%), space (55.3% vs 27.1%), staffing (59.6% vs 44.3%), experience (16.9% vs 11.5%), and hospital proximity (10.9% vs 7.9%), though reimbursement as a barrier improved (45.9% vs 53.7%) (all P < .01). Compared with 2009, although more providers offer OFCs, multiple perceived barriers to performing OFCs have worsened. Hesitancy to challenge infants and emergency preparedness issues are emerging potential concerns.

Lymphatic Filariasis in India: A Journey towards Elimination

The journey towards elimination of Lymphatic Filariasis (ELF) in India started with the deliberations in a meeting held at Delhi in 1996 recommending for pilot project of Mass Drug Administration (MDA) with DEC. Global Programme to Eliminate Lymphatic Filariasis (GPELF was launched in 2000 subsequent to World Health Assembly (WHA) resolution in 1997 making India as signatory. ELF campaign was launched on 5th June, 2004 with annual MDA in endemic districts. However, all the endemic districts could not initiate MDA due to logistics and preparedness issues, thus the journey initially experienced challenges of hurried start. Serious Adverse Events (SAE) and poor compliance were reported from many states which were tackled through advocacy and capacity building of health workers and community volunteers. MDA was managed with staggering of dates in different states and strong supervision helped in improving drug compliance. The improved reported drug coverage resulted in decline of microfilaria prevalence in many districts except some districts. India’s significant progress was recognised internationally as approximately 200 of 650 million population at risk of Lymphatic Filariasis (LF) was made free of risk by 2017 by passing Transmission Assessment Survey (TAS) though some districts could not clear TAS. Efforts to improve drug compliance were intensified and to achieve goal faster, MDA with three drug Ivermectin, DEC and Albendazole has been initiated in addition to ascertaining the current status of LF endemicity in non-MDA districts. Based on experience of long journey towards ELF with mix of success and challenges, it is suggested to intensify ELF in a mission mode with priority.

The Effect of Learning Models and Academic Ability on Students’ Scientific Literacy in Disaster Preparedness

Scientific literacy is important for elementary students, especially for disaster preparedness issues. The research purpose was to analyze the effect of Problem Based Learning models and students’ academic abilities on scientific literacy in attitudes, contexts, competencies, and knowledge. The research method used a quasi-experimental with the matching only pretest-posttest control group design. The populations were 4 elementary schools with a disaster preparedness program. Samples were taken by cluster random sampling. The homogeneity test based on the average scores from odd semester 2018/2019 in fourth-grade elementary school for subjects such as civic education, mathematics, Indonesian language, science, and social sciences. The independent variable was thematic with PBL learning model. The moderator variable was a student’s academic ability and the dependent variable was scientific literacy which includes attitude, context, knowledge, and competence. Instruments in this study were attitude questionnaire, context and knowledge test, and science competency observation. Quantitative data were analyzed with IBM SPSS Statistics 25. The research experiments show that (1) PBL model has a significant influence on the student in scientific literacy aspects such as attitudes, contexts, knowledge, and competencies; (2) academic ability does not significantly influence the scientific literacy aspects. Margin of Safety Based Flood Reliability Evaluation of Wastewater Treatment Plants: Part 2- Quantification of Reliability Attributes

In a companion paper (Part 1), the basic concepts are discussed in details. In this paper, the application of the concepts and the proposed methodology in that paper are utilized to set the strategies in order to quantify reliability attributes. The case study is Hunts Point wastewater treatment plant and its sewershed in Bronx, New York City, so that it could function during flood. The strategies are selected and reliability of the Hunts Point plant is estimated before and after using best management practices (BMPs). Therefore, the copula based non-stationary 100–year flood frequency analysis of rainfall and storm surge analyzed in the companion paper (Part 1), are used to obtain the design values of surge and rainfall. A differential evaluation Markov Chain with Bayesian interface is used in that paper for parameter estimation. Because the co-occurrence of surge and rainfall is more critical in coastal areas, the design value resulted from bivariate analysis has been considered in this study. In this Part 2, a multi-criteria decision making (MCDM) approach, which characterized the uncertainty of sub-criteria in load-resistance concept, is applied. The Margin of Safety (MOS) concept is extended to numerically assess a rather non-probabilistic MCDM evaluation of load and resistance to estimate the reliability of the plant. Finally, Effective strategies are selected for flood hazard mitigation to improve the reliability and performance of the plant. The results present an increase of about 46% in reliability after using BMPs. The framework presented in this paper is applicable to other coastal watersheds and could be a platform for addressing some pressing issues in coastal preparedness with impacts on design criteria of coastal infrastructure.

A Text-Driven, Task-Based Approach to the Design of Materials for Teaching Intercultural Communicative Competence

&lt;p&gt;Although a large body of research points to the need of providing English as a Foreign Language (EFL) learners with intercultural communicative competence (ICC), questions remain as to exactly how ICC should be incorporated into the EFL curriculum. These questions arise in part due to issues of already-packed curricula which seem not to leave time for the teaching of ICC and in part due to issues of teacher preparedness. Additionally, questions arise concerning which culture should be taught. Taking into account materials used in the English Language Center (ELC) of Shantou University (Guangdong Province, China), this paper argues that a text-driven, task-based method of materials design allows for the inclusion of ICC across the EFL curriculum. It also argues that such a method lends itself to the teaching of ICC through a consideration of a variety of cultures.&lt;/p&gt;

Social work through collaborative autoethnography

ABSTRACTThis paper discusses a research project involving five MA Social Work Students and one member of Social Work Academic Staff. Using narrative and taking a collaborative autoethnographical approach, this project highlights some of the feelings that students articulated following a 70-day placement experience. Findings include anxiety, powerlessness and frustration, together with growing confidence, recognition of their skills and a deeper understanding of the role of ‘self’ in social work. Raising issues of preparedness for practice placement, this paper has implications for both social work practice and social work education. Autoethnography (AE) is both a method of carrying out research and a methodology, specifically a qualitative methodology linked to ethnography and narrative inquiry. AE results in highly personalized narrative accounts of the researcher’s engagement with specific sociocultural contexts in the pursuit of knowing more about a phenomenon. Applying such a methodology to explore collaboratively issues of student lived experience of placement is a new and innovative use of this method.

Using the Theory of Planned Behavior to Explain Expecting Couples Birth Preparedness Intentions in a Rural Setting: A Cross-Sectional Study from Rukwa, Southern Tanzania

Background. According to the Theory of Planned Behavior, an intention to carry out a certain behavior facilitates action. In the context of planning for birth, certain preparations and planning may better ensure maternal and neonatal survival. Little is known on the predictors of birth preparedness intention among expecting couples. The aim of this study was to determine the predictors of birth preparedness intentions among expecting couples. Methods. A community based cross-sectional study targeting pregnant women and their partners was performed from June until October 2017. A three-stage probability sampling technique was employed to obtain a sample of 546 couples A structured questionnaire based upon the Theory of Planned Behavior was used. The questionnaire explored three main domains of birth preparedness intentions. These three domains included (1) attitudes towards birth preparedness, (2) perceived subjective norms towards birth preparedness, and (3) perceived behavior control towards birth preparedness. Results. The vast majority of study participants had birth preparedness intentions. This included 521 (95.4%) pregnant women and 543 (99.5%) of their male partners. After adjusting for the confounders, the predictors of birth preparedness intentions among pregnant women were attitude (AOR=70.134, 95% CI=12.536-392.360, p&lt;0.001) and perceived behavior control (AOR=7.327, 95% CI=1.545-34.761, p&lt;0.05) which were significant. Among male partners, only attitudes (AOR=31.315, 95% CI=1.497-655.149, p&lt;0.05) influenced the birth preparedness intention. Conclusion. Birth preparedness intention among male partners was higher compared to their female partners. The reason for the difference could be the concern each group puts on the issue of birth preparedness. Among the three domains of intention, attitude and perceived behavior control were statistically significant predictors of birth preparedness intention among pregnant women. Attitude was the only domain which influenced birth preparedness intention among male partners. Therefore, interventional studies are recommended targeting attitudes and perceived behavior control in order to boost birth preparedness intention.

The significance of interfamilial relationships on birth preparedness and complication readiness in Pakistan

BackgroundIn the interests of improving maternal health care and survival, the issue of birth preparedness and complication readiness has been much debated and has remained a priority for the international health community. The provision of birth preparedness and complications readiness is determined by a range of different factors. AimThe main aim of this study is to identify and measure the influence of husbands and other family relationships on birth preparedness and complications readiness in the Khyber Pakhtunkhwa province of Pakistan. MethodsThis study is a cross-sectional exploratory study. Data was collected through a survey questionnaire. Logistic regression and descriptive analysis was used. FindingsAnalysis indicated that the mother-in-law’s role, men’s and women’s level of education and interfamilial relationships are still the most significant factors influencing birth preparedness and complications readiness. Of the respondents, 86% were receiving antenatal care and 76.5% were planning for the birth to take place in state-run hospitals or private obstetric and gynae clinics. ConclusionThe tendency to take up antenatal care in Khyber Pakhtunkhwa can mainly be credited to a mutual understanding between husband and wife and a good relationship between the woman and her mother-in-law. Highlighting the significance of these relationships has implications for ensuring birth preparedness and complications readiness.

Assessment of National Efforts in Emergency Preparedness for Nuclear Terrorism

This summary of the 53rd Annual Meeting of the National Council on Radiation Protection and Measurements (NCRP) captures the highlights of the presentations including the Warren K. Sinclair Keynote Address and the five scientific sessions and wrap-up summary and panel discussion of "Assessment of National Efforts in Emergency Preparedness for Nuclear Terrorism: Is There a Need for Realignment to Close Remaining Gaps?" The issue of radiological emergency preparedness has evolved in the last 20 y from a primarily nuclear power plant focus to a wider, more comprehensive approach that includes response to all types of radiological and nuclear emergencies, including terrorism. The meeting took an introspective look at the advances in radiological emergency preparedness in the last 15 y and focused on four major topic areas: plans and guidance, training and exercising for both the first responder and the first receiver communities, recovery and return, and communication. In each area, the speakers reflected on the current state of that specific area and provided three to five practicable priority actions/initiatives for future work. As a result of the meeting, NCRP has made a commitment to create a committee to write a commentary that will go into more detail in each of the proposed priority areas discussed in the meeting and provide a roadmap for future work.

Disaster Preparedness among Older Adults: Social Support, Community Participation, and Demographic Characteristics

ABSTRACTThis study examined factors associated with disaster preparedness among older adults, with special attention paid to demographic characteristics, social support, and community participation. From a larger study about disaster preparedness of persons with disabilities, a total of 719 adults aged 55 years and older were used for this study. The results of the logistic regressions indicate that individuals who have higher levels of social support and more connections to community organizations are more likely to be prepared for disaster-related emergency situations. In another model, using resource preparedness as the dependent variable, an interaction effect between social support and income was found. Overall, higher income and higher informal support are related to a higher level of resources for disaster preparedness. However, the relationship between income and disaster preparedness is much stronger for those sub-groups receiving more informal support. The findings of this study expand knowledge regarding issues of disaster preparedness among older adults and demonstrates the importance of social resources and networks. This study suggests an intervention model for decision-makers and community leaders in terms of providing information and advice to members of their communities about how to adequately prepare for natural disasters.

Global Dispersant Approvals in Asia Pacific – Current Status and On Going Challenges

ABSTRACT Following the Macondo Incident, the international oil and gas industry spent significant time and effort analyzing lessons learned and implementing key projects to ensure that critical response and preparedness issues that were identified are addressed to improve response capabilities. The Global Dispersant Stockpile (GDS) was established as part of a post-Macondo Joint Industry Project through Oil Spill Response Limited (OSRL), recognizing that delivery of sufficient quantities of dispersant is a key element for a successful dispersant operation, especially during the initial phases of a large scale response to an event such as a subsea well blowout. Taking into account the global approval status and proven effectiveness on a range of crude oils, three key oil dispersants, Finasol® OSR 52 (Total), Corexit® EC9500A (Nalco) and Slickgone® NS (Dasic) were selected for the Global Dispersant Stockpile. A total of 5,000 m3 of these dispersants are now stored and ready to be deployed from five strategically positioned global locations. For example, sizable volumes of two of these products (total volume = 700 m3) are located at OSRL’s response base in Singapore, which can be quickly mobilized to support a response in the Asia Pacific region. An ongoing effort associated with the management of the GDS is to enable the pre-approval of at least one of the three products for countries in the region where spill response may be required. At present, this is not the case in the region for a variety of reasons, e.g., toxicity concerns and biodegradation processes of dispersed oil. A particularly cautious approach by regulatory authorities following the Macondo incident, coupled with a number of other specific regional concerns, has exacerbated the issue of obtaining and maintaining dispersant approvals in the region. The aim of this paper is to identify and discuss the existing regulatory framework governing the dispersant product approval process and dispersant use authorization for countries in Asia Pacific. The paper will detail the present status of regulations related to dispersant use for a number of countries in the region, the potential challenges associated with achieving permissions in countries with no regulations and a discussion of strategies to address identified obstacles. Additionally the activities that are being undertaken to expand regulatory approvals will also be addressed. It is anticipated that a greater understanding of the reasoning behind the GDS will facilitate a positive regulatory perspective and the potential for dispersant pre-approval in the region.

Perceptions of crisis preparedness and motivation: A study among municipal leaders

The need for communities to be prepared for a wide variety of critical events places considerable responsibility on local municipal leaders. However, few studies have examined how these leaders themselves view crisis preparedness issues. The purpose of this study was to examine factors that contribute to three aspects of preparedness among municipal leaders: perceived municipal preparedness, perceived individual preparedness and motivation for preparedness work. Six hypotheses were formulated. The research questions were investigated using data from a questionnaire sent out to all Swedish municipalities (N=290) and four categories of municipal leaders respectively (N=1101). The response rate was 67%. Data were analyzed by linear regression and logistic regression. Different factors predicted the three outcome variables, which indicates different mental concepts. The hypotheses were partly supported and the results are discussed using self-efficacy theory. The findings have implications for understanding perceived preparedness and motivation, and can be used to e.g. develop crisis management exercises.

Approach of social institutions to preparedness for emergency

The third call of the Ministry of Interior of the Czech Republic concerning the support of research, experimental development and innovation, was announced for the period 2013–2015. The Institute of Radiology, Toxicology and Civil Protection of the Faculty of Health and Social Sciences of the University of South Bohemia in Ceske Budejovice submitted an application for research focused on population protection according to population differentiation. The aim of the project was to analyze the current status of evacuation planning from the emergency planning zone according to population differentiation. This article discusses the issue of social institutions preparedness for evacuation from emergency planning zones. Evacuation planning is one of the most important parts of population protection. It has its own specifics and it is necessary to consistently perceive all of the aspects that evacuation includes. Social institutions are one of the important aspects which must be considered. In the emergency planning zones of nuclear power plants there are a total of 10 social institutions.

Hospital influenza pandemic stockpiling needs: A computer simulation.

A severe influenza pandemic could overwhelm hospitals but planning guidance that accounts for the dynamic interrelationships between planning elements is lacking. We developed a methodology to calculate pandemic supply needs based on operational considerations in hospitals and then tested the methodology at Mayo Clinic in Rochester, MN. We upgraded a previously designed computer modeling tool and input carefully researched resource data from the hospital to run 10,000 Monte Carlo simulations using various combinations of variables to determine resource needs across a spectrum of scenarios. Of 10,000 iterations, 1,315 fell within the parameters defined by our simulation design and logical constraints. From these valid iterations, we projected supply requirements by percentile for key supplies, pharmaceuticals, and personal protective equipment requirements needed in a severe pandemic. We projected supplies needs for a range of scenarios that use up to 100% of Mayo Clinic-Rochester's surge capacity of beds and ventilators. The results indicate that there are diminishing patient care benefits for stockpiling on the high side of the range, but that having some stockpile of critical resources, even if it is relatively modest, is most important. We were able to display the probabilities of needing various supply levels across a spectrum of scenarios. The tool could be used to model many other hospital preparedness issues, but validation in other settings is needed.

Effects of Land Use Changes on Airport and Flight Safety; A Study of Wilson Airport, Kenya

-The aviation sector is one of the most important sectors for any country especially its ability to: promote interconnectedness within countries as well as between countries. However, the sector faces many risks caused by various emergencies and accidents associated with the sector, and much so in the precincts of airports. The impact of these accidents causes monumental social, economic and environmental consequences to victims. The exponential growth of the sector in the country calls for special attention by relevant stakeholders to devise strategies to address disaster preparedness issues. This is especially due to the fact that the country has already experienced a number of aviation disasters and mishaps related to the industry. Consequently, this study sought to assess the effects of land use changes on airport and flight safety at Wilson Airport. This study was carried out at Wilson airport located in Nairobi West. The qualitative design was applied in this study to analyze and describe the effects of land use changes on airport and flight safety in a rapidly growing aviation sector. A total of 216 respondents were targeted by the study (including 150 members of the community, 30 aviation regulators, 30 air operators and 6 service providers) out of which 195 responded (including 132 members of the community, 28 aviation regulators, 29 air operators and 6 service provider) giving a response rate of 90%. Primary data was collected by use of questionnaires, interview guide and Focused Group Discussions while Secondary data were collected from written or published records and maps from the Kenya National Bureau of statistics. Quantitative data was analysed by use of descriptive statistics such as frequencies and percentages while qualitative data was analysed using content analysis. The study found that 42 (74%) of the respondents indicated that land use changes have effect on flight safety. The land use changes included: setting up of major shopping malls near the airport, proximity of Uhuru gardens to the airport, adjacent Nairobi national park, Mitumba slums, upcoming high-rise development and existence of motor garages next to the airport. The results from correlation analysis showed that disaster preparedness is positively related with land use changes with a Pearson‟s Correlation Coefficient of r = 0.509 and that at a level of significance of 0.000 meaning that it is statistically significant as p value is less than 0.05. It was recommended that the government should do an urgent operation on land encroachment as this is perceived to be a disaster in the waiting.

Consequently, this study sought to assess the appropriateness and adequacy of the existing physical infrastructure in mitigating aviation risks in the airport. This study was carried out at Wilson airport located in Nairobi West. The qualitative design was applied in this study to analyze and describe the effects of land use changes on airport and flight safety in a rapidly growing aviation sector. The target population for the study comprised of 50 aviation regulators, 50 air operators, 100 service providers and communities living around the Airport. Primary data was collected by use of questionnaires, interview guide and Focused Group Discussions while Secondary data were collected from written or published records and maps from the Kenya National Bureau of statistics. Quantitative data was analysed by use of descriptive statistics such as frequencies and percentages while qualitative data was analysed using content analysis. The study found that 62% of the respondents indicated that Wilson Airport does not have the capacity in terms of space and facilities and that 75% of the respondents indicated Air field capacity was inadequate. The study also found that 63.2% of the respondents indicated thatAirport Visual Aids (such as pavement markings) were inadequate, 57.9% of the respondents indicated Airport Lighting was inadequate, 40.4% of the respondents indicated that runway orientation, length, width, and pavement strength were inadequate and 30 (56%) indicated that taxi ways were inadequate. The results for correlation analysis showed that there is a positive correlation between the disaster preparedness and appropriateness and adequacy of the existing infrastructure with a Pearson‟s Correlation Coefficient of r = 0.769 and a level of significance of 0.000 meaning that is it statistically significant.

Purpose The purpose of this paper is to highlight the risks faced by employers as a result of subjecting vulnerable employees to exploitative business practices that are akin to slavery. Design/methodology/approach The study considers examples of exploitative approaches that employers have recently adopted.
