What Happens 20 to 30 years After Radial Keratotomy? Case Series.

Radial keratotomy (RK) was commonly performed in the 1980s and 1990s. We aimed to clarify the current status of post-RK refractive correction and treatment. We retrospectively reviewed the charts of 70 patients with a history of RK. Of the 70 patients, 44 were identified for clinical outcomes. Refractive or therapeutic intervention (rigid gas-permeable contact lens fit, spectacle prescription, corneal surgery, and use of pilocarpine hydrochloride for photophobia) was possible in 59% of patients with postoperative visual deterioration after RK; in the remaining 41%, therapeutic intervention was not possible. Rigid gas-permeable contact lens fit for corneal irregular astigmatism was the most common refractive intervention and was effective in 36% of cases in the university hospital.

Repeatability and Agreement of 4 Biometers Measuring Corneal Astigmatism in Eyes With Irregular Corneal Astigmatism Component

To investigate the repeatability and agreement of corneal astigmatism measurements in eyes with irregular corneal astigmatism component (ICAC) using four devices: IOLMaster 700 biometer, Lenstar 900 biometer, iTrace, and Pentacam. Prospective cross-sectional reliability analysis. Sixty-four eyes (52 patients) with ICAC were examined three times using the four devices. The eye with ICAC in this study is defined as the cornea has a certain degree of irregular astigmatism (asymmetric and/or skewed bowtie pattern of corneal topography according to corneal topography classification), accompanied with total corneal higher-order aberrations in the 4 mm zone of 0.3 µm or greater. Corneal astigmatism was evaluated using three categories: anterior corneal astigmatism (ACA), posterior corneal astigmatism, and total corneal astigmatism (TCA). The repeatability was determined using the ∆Ast (arithmetic mean of vector differences among three repeated corneal astigmatism measurements). Bland-Altman plots and astigmatism vector analyses were employed to assess agreement. The IOLMaster 700 (∆Ast = 0.27 ± 0.20 D) showcased higher repeatability in ACA measurements compared to iTrace (∆Ast = 0.37 ± 0.38 D, P = .040) and Pentacam (∆Ast = 0.50 ± 0.22 D, P < .001), and paralleled the performance of Lenstar 900 (∆Ast = 0.31 ± 0.26 D, P = .338). The Pentacam (∆Ast = 0.09 ± 0.07 D, P < .001) demonstrated superior repeatability in posterior corneal astigmatism, whereas the IOLMaster 700 (∆Ast = 0.33 ± 0.23 D, P < .001) excelled in TCA. The IOLMaster 700 exhibited good agreement with either Lenstar 900 or iTrace, characterized by narrow 95% limits of agreement and clinically acceptable vector differences. Conversely, vector differences between Pentacam and the other three devices in ACA and TCA measurements were clinically significant, exceeding 0.50 D (all P < .05). In terms of repeatability of corneal astigmatism measurements in eyes with ICAC, the IOLMaster 700 and Lenstar 900 outperformed iTrace and Pentacam. While the IOLMaster 700 can be used interchangeably with either Lenstar 900 or iTrace, the Pentacam is not interchangeable with the other three devices.

Effectiveness of toric intraocular lens implantation for correcting irregular corneal astigmatism in cataract eyes

A retrospective cohort study was conducted to observe the correction effect of Toric intraocular lens (IOL) implantation in cataract eyes with specific types of irregular corneal astigmatism. Thirty-four eyes with either the "asymmetric bow-tie" pattern (Type I) or the "angled bow-tie" pattern (Type II) were included. Corneal topography was assessed using Pentacam HR, and changes in preoperative corneal astigmatism, visual acuity, manifest refraction, and objective visual quality were measured and compared. The average uncorrected distance visual acuity improved significantly from 0.86 ± 0.40 logMAR to 0.22 ± 0.15 logMAR (P < 0.001). Preoperative corneal astigmatism of 2.05 ± 0.90 D was corrected to a postoperative residual astigmatism of 0.78 ± 0.57 D (P < 0.001), with 32% of eyes within 0.50 D. The residual astigmatism prediction errors in Type I and Type II cases were (0.97 ± 0.68 D) and (0.66 ± 0.37 D), respectively (P = 0.100). The mean spherical equivalent prediction error in Type II cases (0.07 ± 0.36 D) was significantly smaller than that in Type I cases (− 0.29 ± 0.52 D) (P = 0.030). This study concludes that Toric IOL implantation effectively corrects specific types of irregular corneal astigmatism in cataract surgery. Eyes with the "angled bow-tie" pattern show higher accuracy in refractive predictions compared to eyes with the "asymmetric bow-tie" pattern.

Prevalence and clinical profile of keratoconus in patients presenting at a provincial hospital in KwaZulu, Natal, South Africa: A case study.

Keratoconus (KC) is a progressive, asymmetrical corneal disease, characterized by stromal thinning that leads to distortion, causing vision loss. The visual loss is secondary to corneal scarring, irregular astigmatism, and myopia. The prevalence of KC has been reported to differ in different parts of the world. The study aimed to determine the prevalence and profile of patients with KC presenting to a provincial hospital in KwaZulu-Natal, South Africa. A retrospective study design was used to review 412 clinical records of patients attending the McCord Provincial Eye Hospital (MPEH) during a five-year period (2016-2020). Data on age, race, refraction, clinical profile, treatment plan, and diagnosis were ascertained. The prevalence of KC in MPEH was found to be 13.7% with a mean age of 24.7±7.94 years. Black African and females had a higher frequency of KC compared to males and other ethnic groups. Most of the patients presented with a severe stage of KC and referral was the most common management. Central corneal thinning and Munson's sign were the most prevalent clinical signs. There was no statistically significant difference between the worse and better eye when comparing the clinical signs. The prevalence and clinical profile of patients with KC in this study was similar to that reported by previous studies and more in Blacks and females. Population based epidemiological studies are needed to determine the prevalence of KC in South Africa to enable early clinical interventions.

Modified technique for corneal laceration repair using amniotic membrane

Background Water-tight closure of lacerated corneal wounds may require a lot of sutures and result in significant corneal scarring and irregular astigmatism. Aim The aim of this study was to describe the technique of amniotic membrane (AM) onlay strip for corneal lacerations repair and to evaluate its outcome. Patients and methods This is a retrospective case series that included eight eyes with traumatic corneal lacerations. A strip of AM was applied to the corneal surface to cover the wound which was then sutured with 10/0 Nylon sutures involving the membrane. The sutures were well-spaced, and the suturing was avoided in the visual axis. Average keratometric (K) reading, keratometric astigmatism (KA), manifest refractive astigmatism (RA) in diopters (D), and spectacle or soft contact lens best corrected visual acuity (BCVA) were assessed 2 months after suture removal. Results Transient postoperative hypotony was encountered in five cases (62.5%). The average K was 44.61±2.81 D, KA 2.20±0.71 D, and RA 1.78±0.73 D (range=1–3 D). The postoperative LogMAR BCVA was 0.39±0.18 (range=0.22–0.70). Conclusions The application of AM inlay strip during repair of corneal lacerations was associated with good visual outcome possibly due to the reduction of wound and suture related corneal astigmatism.

Effects of Custom-Designed Soft Contact Lenses on Irregular Astigmatism Correction in Patients with Keratoconus.

To evaluate the efficacy of YOUSOFT soft contact lenses in correcting irregular astigmatism and prescription results of patients with keratoconus. The retrospective observational study included 55 eyes (mean age, 32.2 ± 10.6 years; 36 men and 6 women) of 42 patients with keratoconus who tried YOUSOFT for rigid gas permeable (RGP) lens intolerance. Average keratometry, corneal astigmatism, and maximum keratometry were 49.4 ± 5.2 diopters (D), 3.7 ± 2.1 D, and 57.3 ± 8.2 D, respectively. Patients were divided into YOUSOFT prescription and non-prescription cases, wherein the prescription rates were calculated. YOUSOFT visual acuity was compared with spectacle-corrected distance visual acuity (CDVA) and RGP lens-CDVA. YOUSOFT was prescribed to 28 out of 42 patients (prescription rate 67%). In the YOUSOFT prescription cases, YOUSOFT-CDVA (logMAR -0.04; 95% confidence interval [CI]: -0.08 to 0.00) was significantly better than spectacle-CDVA (logMAR 0.23; 95% CI: 0.08 to 0.38; P < 0.0001), whereas YOUSOFT-CDVA (logMAR -0.03; 95% CI: -0.08 to 0.03) did not significantly differ from the RGP lens-CDVA (logMAR -0.02; 95% CI: -0.08 to 0.04; P = 0.856). YOUSOFT was effective in correcting irregular corneal astigmatism, suggesting that it is highly effective in patients with RGP lens intolerance.

The influence of pterygium on corneal densitometry evaluated using the Oculus Pentacam system.

To assess the effect of pterygium on corneal densitometry (CD) values. One hundred and nine patients (155 eyes) with primary pterygium were divided into a severe pterygium group (79 eyes) and a mild-to-moderate pterygium group (76 eyes) according to pterygium severity. Among them, 63 patients had monocular pterygium; and 25 patients (38 eyes) underwent pterygium excision combined with conjunctival autograft follow-up. A Pentacam anterior segment analyzer was used to obtain the CD values and corneal morphological parameters, including central corneal thickness (CCT), flat-axis keratometry (K1), steep-axis keratometry (K2), corneal astigmatism, irregular astigmatism, and spherical aberration. CD was subdivided into four concentric radial regions based on corneal diameter and three layers according to depth. CD values at 0-12 mm of the anterior 120 μm layer, 0-10 mm of the center layer and full thickness, and 2-6 mm of the posterior 60 μm layer were significantly higher in eyes affected by pterygium than in the contralateral unaffected eyes (all P < 0.05). CD values were significantly higher in the severe pterygium group than in the mild to moderate pterygium group (all P < 0.05). Corneal astigmatism, irregular astigmatism, K1, K2, CCT, and spherical aberration correlated with CD values in eyes with pterygium (all P < 0.05). CD values at 6-10, 0-12 mm in the anterior 120 μm layer and full thickness, 10-12 and 0-12 mm in the center layer were significantly decreased 1 month after pterygium surgery compared with those before surgery (all P < 0.05). Patients with pterygium had increased CD values, particularly in the anterior and central layers. CD values were correlated with pterygium severity grading and corneal parameters. Pterygium surgery partially reduced the CD values.

Intacs and Topography-Guided Photorefractive Keratectomy for Treatment of Incomplete LASIK Flap-Induced Irregular Corneal Topography: A Case Report

Purpose: To report a case of irregular corneal astigmatism and decreased visual function secondary to excimer ablation over an incomplete laser in situ keratomileusis (LASIK) flap treated with Intacs (CorneaGen) implantation and topography-guided (TG-PRK) and wavefront-optimized (WF-PRK) photorefractive keratectomy. Methods: The patient underwent Intacs implantation, TG-PRK, and WF-PRK enhancement in the right eye, with 4 to 6 months between procedures. Principal outcomes included uncorrected (UDVA) and corrected (CDVA) distance visual acuity, manifest refraction, topographic maximum keratometry, and inferior-superior value. Results: UDVA changed from 20/40 to 20/100 after Intacs implantation, but improved to 20/50 after TG-PRK and 20/25 at 4 months after WF-PRK. CDVA declined from 20/40 to 20/50 after Intacs implantation, but improved to 20/20 after TG-PRK and 20/15 after WF-PRK. Overall, maximum keratometry flattened from 53.20 to 47.00 diopters (D) and inferior-superior value improved from 11.60 to 2.40 D. Conclusions: This case illustrates the successful use of sequential Intacs implantation and TG-PRK after an irregular LASIK ablation to improve irregular astigmatism and loss of visual function. Correction of Asymmetric Bowtie Corneal Astigmatism with a Toric Intraocular Lens: Outcomes and Accuracy of Measurement Modes.

The outcomes of toric intraocular lens (IOL) implantation in correcting asymmetric bowtie corneal astigmatism remain uncertain. The accurate measurement of corneal astigmatism is essential for surgical planning. In this prospective cohort study, patients with asymmetric or symmetric bowtie corneal astigmatism who underwent toric IOL implantation were recruited. Preoperative corneal astigmatism was measured with an IOLMaster and Pentacam (including the simulated keratometry (SimK), total corneal refractive power (TCRP), and wavefront aberration (WFA) modes). At 3 months after surgery, the refractive outcomes and residual astigmatic refractive errors were compared with patients with symmetric bowtie astigmatism. The prediction errors (the differences between the calculated actual corneal astigmatism and the measured corneal astigmatism) were compared among the different measurement modes in the asymmetric group. There were no differences in residual astigmatism between the asymmetric and symmetric groups. However, the mean absolute residual astigmatic refractive error was greater in the asymmetric group than in the symmetric group (0.72 ± 0.42 D vs. 0.53 ± 0.24 D, p = 0.043). In the asymmetric group, the mean absolute prediction errors for the IOLMaster, SimK, TCRP and WFA modes were 0.53 ± 0.40, 0.56 ± 0.47, 0.68 ± 0.52, and 0.43 ± 0.40 D, respectively. The Pentacam WFA mode was the most accurate mode (p < 0.05). The absolute prediction error of the WFA mode was positively correlated with the total corneal irregular astigmatism higher-order aberrations and coma (r = 0.416 and r = 0.473, respectively; both p < 0.05). Our study suggests toric IOL implantation effectively corrected asymmetric bowtie corneal astigmatism. The Pentacam WFA mode may be the most accurate measurement mode, although its accuracy decreased as asymmetry increased.

Analysis of Various Modalities for Intraocular Pressure Measurement in Relation to Keratoconus Severity in 246 Eyes of the Homburg Keratoconus Center.

Intraocular pressure (IOP) measurement may be difficult in keratoconus (KC) due to corneal protrusion and irregular astigmatism. This study aimed to assess which IOP measurement modality is least affected by KC severity. Data from 246 corneas of 246 patients with KC were retrospectively analyzed. KC stages were determined using the Topographic KC (TKC) and ABCD KC classifications derived from Pentacam (Oculus, Germany). IOP was measured using Goldmann applanation tonometry (GAT), Ocular Response Analyzer (ORA, Reichert Instruments, USA), and Corvis ST (CST, Oculus, Germany). Cronbach alpha (CA), analysis of variance with Bonferroni correction, Dunnett T3, and Pearson correlation were performed. Using CA, the reliability of measurements using various modalities increased to 0.764 to 0.943 when excluding IOP Goldmann overall and in each KC stage (TKC and ABCD). Analysis of variance revealed significant differences between TKC and ABCD stages for almost all IOP modalities. The Bonferroni post hoc test showed significant differences between the measured IOP in earlier and advanced KC stages, except for the biomechanically CST-corrected IOP (bIOP). Pearson correlation analysis showed a significant correlation between IOP and thinnest corneal thickness (TCT) for all IOP modalities except bIOP. CST-based bIOP seems to be best suited for IOP measurement in KC because it did not correlate with TCT in contrast to IOP measurements by ORA or GAT. The measurement results from GAT in patients with KC should be interpreted with care and always in view of corneal thickness. As a thumb rule, we suggest to add at least 2 mm Hg to the measured GAT value.

Short-term visual outcome with sclerocorneal contact lens on irregular cornea.

The purpose is to study the change in visual acuity obtained with sclerocorneal contact lens (SCL) in patients with irregular corneal astigmatism. A prospective pre-post observational study was designed to include consecutive consenting patients with irregular corneal astigmatism and best-corrected visual acuity worse than 6/12 and fitted with SCL. Visual acuity was assessed using Snellen charts before fitting SCL and 1 week after the fit and converted to the equivalent logMAR units for analysis. We included 51 eyes of 41 patients with a mean age of 26.31 ± 8.86 of which 32 (78%) were men. Keratoconus was the most common indication (n = 42 eyes, 82.35%), followed by corneal scar (n = 3, 4%), post keratoplasty (n = 2, 4%), high myopia (n = 2, 4%), pellucid marginal degeneration (n = 1, 2%), and aphakia (n = 1, 2%). The mean overall uncorrected visual acuity improved significantly (P < 0.001) with SCL from 1.18 ± 0.34 logMAR (6/120 Snellen's equivalent) to 0.27 ± 0.15 (6/9 Snellen's equivalent) at 1-week post-SCL fitting. The mean overall best spectacle-corrected visual acuity improved from 0.89 ± 0.45 logMAR (6/36 Snellen's equivalent) to 0.26 ± 0.15 (6/9 Snellen's equivalent) at 1-week post-SCL fitting. Sclerocorneal contact lens improves visual acuity significantly in patients with irregular corneal astigmatism and reduces the need for corneal transplantation.

Clinical Use of Mini-Scleral Contact Lens in Ocular Surface Diseases

A scleral contact lens lies on the sclera, not the cornea, which differs from a rigid gas permeable (RGP) contact lens, and has shown successful clinical outcomes in patients unable to tolerate glasses or RGP contact lenses because of a severely deformed cornea. In addition, a scleral lens forms a tear film between the cornea and contact lens that stabilizes the ocular surface and could be a useful therapeutic option. The mini-scleral contact lens is a useful therapeutic option for visual improvement and symptom control in patients with corneal abnormalities such as keratoconus, corneal opacity, ocular surface disease, and irregular corneal astigmatism.

Oxygen in Corneal Collagen Crosslinking to Treat Keratoconus: A Systematic Review and Meta-Analysis.

Keratoconus is a disorder that results in visual loss from increased corneal high-order aberrations and irregular astigmatism and reduces quality of life. The primary treatment for progressive keratoconus is crosslinking (CXL). Recently, it has been suggested that oxygen enhances the type II photodynamic reaction of CXL that is oxygen dependent. Our study investigated the effect of increased oxygen availability in epithelium-on CXL on visual acuity and corneal curvature. We searched PubMed, EMBASE, Medline, Web of Science, and Scopus databases on November 3, 2021. We included studies that reported increased oxygen availability during CXL in patients with keratoconus published within the last 10 years. A meta-analysis on the primary outcomes, maximum keratometry, and corrected distance visual acuity, was conducted. The search yielded 108 publications which were screened and assessed for eligibility. Six studies were included in the systematic review and 5 studies were included in our meta-analysis of the outcomes of increased oxygen availability in accelerated CXL. The meta-analysis on data after 6 months of follow-up found a significant decrease in mean maximum keratometry of 1.2 diopter (95% confidence interval: 0.2-2.3; P =0.02) and an improvement in mean corrected distance visual acuity by 0.08 logMAR (95% confidence interval, 0.02-0.13; P =0.01). There were no serious adverse events reported. Increasing oxygen during epithelium-on CXL improved visual acuity and produced corneal flattening without any serious adverse events in patients with keratoconus. The demarcation line depth was significantly higher with oxygen compared to the control group. Further data are required with a control group and long-term follow-up across a range of CXL protocols for implementation into standard clinical practice.

Corneal irregularity and visual function using anterior segment optical coherence tomography in TGFBI corneal dystrophy

The purpose of this study was to evaluate corneal irregular astigmatism of patients with granular and lattice corneal dystrophy (GCD and LCD). 70 GCD, 35 LCD, and 81 control eyes were included. Anterior and posterior corneal topographic data obtained from anterior segment optical coherence tomography were expanded into four components via Fourier harmonic analysis. These components were compared with healthy eyes and the association between each component and best-corrected visual acuity (BCVA) was investigated. Anterior and posterior components increased in both GCD and LCD eyes. Anterior and posterior components of GCD2, anterior of LCD type 1 (LCD1), posterior of LCD type IIIA (LCD 3A), and type IV (LCD4) significantly increased. BCVA was significantly associated with anterior and posterior components in LCD eyes but not in GCD. The anterior components of LCD1, anterior and posterior of LCD3A, and posterior of LCD4, were positively correlated with BCVA. As conclusions, in GCD eyes, anterior and posterior components differed from those of the control but BCVA was not significantly associated with them. In LCD eyes, the anterior and posterior components increased, and BCVA was significantly associated with the anterior and posterior components.

Fourier Analysis on Irregular Corneal Astigmatism Using Optical Coherence Tomography in Various Severity Stages of Keratoconus

To investigate the diagnostic capability of Fourier indices in detecting clinical or subclinical keratoconus (KC). Prospective cross-sectional study. The study included 126 eyes with clinical KC (50 KC without any corneal scar, 50 KC with anterior corneal scar, and 26 KC with posterior scar having a history of acute corneal hydrops), 50 with topographic KC (without clinical signs), 50 with pre-topographic KC (normal topography without clinical signs), and 50 controls. Corneal tomographic data were obtained using anterior segment optical coherence tomography (OCT). Fourier analysis decomposed dioptric data from both anterior and posterior corneal surface into spherical, regular astigmatism, asymmetry, and higher-order irregularity components. The discriminating ability of the Fourier indices of pre-topographic KC, topographic KC, and clinical KC from controls were assessed after quantitative Fourier analysis of irregular corneal astigmatism. Posterior asymmetry and higher-order irregularity components were significantly greater in pre-topographic KC eyes than those in controls (P < .001 for both), with the highest area under the receiver operating characteristic curve (AUROC) of 0.778 and 0.709, respectively. The same was true for anterior asymmetry, posterior asymmetry, and posterior higher-order irregularity components in topographic KC (AUROC of 0.945, 0.941, and 0.893, respectively), whereas it was >0.948 for all Fourier components in clinical KC. Fourier analysis using OCT can evaluate anterior and posterior corneal irregular astigmatism of various KC stages, from very mild to advanced, including severe cases with corneal scar. Irregular astigmatism indices from the posterior corneal surface showed the highest AUROC values in discriminating early KC stages.

Impact of rigid gas-permeable contact lens on keratometric indices and corneal thickness of keratoconus eyes examined with anterior segment optical coherence tomography.

Purpose/AimDetecting keratoconus (KC) progression helps determine the surgical indication for corneal cross-linking (CXL). This retrospective observational study aimed to examine changes in keratometric indices and corneal thickness in patients with KC who used rigid gas-permeable (RGP) contact lenses.Materials and methodsThis study involved 31 eyes (31 patients) diagnosed with KC. No patient had used RGP or any other type of contact lenses for at least 1 month. Corneal topographic data were obtained using three-dimensional anterior segment optical coherence tomography before and after >1 month of RGP lens use.ResultsThe average and maximum keratometry values changed after using an RGP lens (-1.05 ± 1.92 D, p < 0.01 and -1.65 ± 4.20 D, p = 0.04, respectively); the spherical component of the anterior corneal surface became significantly smaller (p = 0.02). No change was observed in the central or thinnest corneal thickness values. Keratometric changes were greater in eyes with severe KC than in those with moderate KC (p = 0.014).ConclusionsKeratometry and spherical components of the anterior corneal surface values decreased after RGP lens use; keratometric changes were greater in eyes with severe KC than in those with moderate KC. Corneal progression indices, including corneal thickness, posterior keratometry, and irregular astigmatism values, mostly remained unchanged. It is important to consider these findings when evaluating corneal topography of KC and preparing CXL.

Study of the etiology, socio demographic profile and visual outcome of traumatic cataract in Bangalore – Rural

Purpose: To study the Etiology, Socio-Demographic profile and Visual outcome of Traumatic Cataract in Bangalore Rural. Materials and Methods: This cross- sectional study was done on 50 patientswho presented in Department of Ophthalmology from June 2021 to Dec 2021. Relevant clinical history, visual acuity and demographic data at time of presentation and post operatively after 2 months were recorded. All the data was compiled and managed in SSPS. Results: Among 50 patients, majority of patients were young adults between 21-30 years. Males were affected more than females.The major cause was wooden stick trauma. Most of the patients had penetrating eye injury [n=35, 70%] and poor visual acuity at presentation. Conclusion: Due to engagement in outdoor activities, males face increased risk of trauma. Because of other complications like corneal opacity, scarring and irregular astigmatism, visual outcome post operatively was poor.

Hemidivisional vector planning to reduce and regularize irregular astigmatism by laser treatment

PurposeTo demonstrate how hemidivisional vector planning of refractive laser treatments of astigmatism can be used to directly address idiopathic corneal irregular astigmatism that has an asymmetrical, non-orthogonal bow tie topography appearance.DesignCase study.MethodsThe cornea is conceptually divided into two hemidivisions along the flat meridian of the corneal topographic astigmatism (CorT), which means that each hemidivision will approximately correspond to one lobe of the asymmetric, non-orthogonal topographic bow tie. An astigmatism reduction treatment can then be planned separately for each hemidivision using the vector planning technique, based on both its two hemidivisional CorT measures and common manifest refractive cylinder. The remaining irregularity is then regularized, and the junctional zone smoothed across the flat meridian. The final intended treatment thus combines hemidivisional astigmatism reduction and regularization of the corneal astigmatism and spherical refractive error in one treatment application. This could be applied to LASIK, PRK, SMILE, and Transepithelial PRK procedures using Designer Cornea® software.ResultsA theoretical treatment profile is derived from an actual example of a cornea with idiopathic asymmetric non-orthogonal astigmatism. The three steps of the derivation are as follows: (i) astigmatism reduction through the use of the vector planning technique; (ii) regularization, and (iii) smoothing across the hemidivisional midline.ConclusionsHemidivisional vector planning treatments could potentially both reduce and regularize asymmetric non-orthogonal astigmatism. These treatments can be systematically customized to account for qualitative and quantitative differences between the two corneal hemidivisions at the same time as correction of coexistent myopia or hyperopia.

Transepithelial phototherapeutic keratectomy for post-traumatic recurrent corneal erosions.

Purpose:To evaluate the outcomes of transepithelial phototherapeutic keratectomy (transepithelial PTK) for treatment of posttraumatic recurrent corneal erosions.Methods:Twenty-four eyes of 22 patients with posttraumatic recurrent corneal erosions who were treated with transepithelial PTK from April 2018 to July 2020 were included in this retrospective study. The rates of recurrent erosions and complications were evaluated during the follow-up after surgery. Visual acuity and refraction were recorded preoperatively and 1 year after surgery. Total corneal astigmatism, total corneal irregular astigmatism, and total corneal spherical aberrations were recorded using corneal tomography preoperatively and 1 year after surgery.Results:Mean postoperative follow-up was 13 months (range: 12–32 months). None of the patients suffered from any complications or further erosions. Uncorrected distance visual acuity, best-corrected distance visual acuity, spherical equivalent, and cylinder remained stable at 1-year postoperatively compared with the baseline (P = 0.214, P = 0.461, P = 0.084, and P = 0.879, respectively). Moreover, there was no significant difference in total corneal astigmatism, total corneal irregular astigmatism, and total corneal spherical aberrations between baseline and 1-year postoperative visit (P = 0.938, P = 0.136, and P = 0.981, respectively)Conclusion:Transepithelial PTK was an effective treatment for patients with posttraumatic recurrent corneal erosions.

Aberrometric, Keratometric, and Visual Outcomes After Trans-Epithelial Topography-Guided Phototherapeutic Keratectomy for the Treatment of Irregular Corneas.

PurposeTo assess the safety, aberrometric and keratometric changes, and stability of trans-epithelial topography-guided phototherapeutic keratectomy (TE-TG-PTK) with mitomycin C (MMC) using the ALLEGRO Topolyzer platform for the treatment of irregular astigmatism.MethodsThis is a retrospective case series including 57 eyes that underwent TE-TG-PTK + MMC using the ALLEGRO Topolyzer platform for the treatment of irregular astigmatism. CDVA, manifest refraction (MR), keratometry readings, and aberrometry readings were analyzed at 1, 3, 6, and 12 months.ResultsCauses of corneal irregularity included non-infectious leucoma (n=23), infectious leucoma (n=7), adenoviral keratitis (n=20), corneal haze (n=2), post-penetrant keratoplasty (PKP) (n=1), and others (n=4). Overall, 76% of the eyes (n=40) gained lines of vision; patients gained 1, and 2 or more lines of vision in 76%, and 38% of cases, respectively. Only 1 patient (2%) lost 5 lines of vision. Mean preoperative CDVA (LogMAR) was 0.37 ±0.31 and improved to 0.14 ±0.18 (p<0.001) at final follow-up (12 months). CDVA remained unchanged in 10 eyes (21%). No significant changes were observed in mean keratometry (Kmean) and keratometric astigmatism readings. Regarding aberrometry, only changes in coma proved to be significant 6 months after surgery (P<0.01). No intraoperative/postoperative complications were reported.ConclusionAt final follow-up, significant improvements were observed in CDVA and coma. TE-TG-PTK + MMC proved to be an effective and safe procedure for the treatment of corneal irregular astigmatism due to several causes.
