Health promotion policies, perceptions, actions and needs in sports clubs in Ireland

Objective: Policy development is the biggest gap for health-promoting sports clubs. The present study aims to identify Irish sports club’s involvement in health promotion (HP) policy development. Design: Mixed methods concurrent survey design, with quantitative data providing insights into priorities, activities and documentation and qualitative data documenting stakeholders’ perceived needs. Setting: Two hundred and thirty-nine sports clubs in Ireland. Method: The survey measured perceptions of HP, the importance of promoting 10 health topics (e.g. suicide prevention, healthy eating, addictive behaviours) and corresponding actions relevant to HP in club policy documents and future needs. Quantitative data were analysed using multivariate statistics. The policy cycle was used to conduct a deductive analysis of qualitative data on sports clubs’ perceived needs. Results: A high importance score (>70%) was found for the 10 health topics. Sports clubs reported their greatest investment was in physical activity promotion, and the lowest investment was in supporting the participation of disabled people. Up to two thirds of sports clubs had no HP policy. Irish sports clubs’ perceptions of HP showed higher but similar patterns of response relative to sports clubs in other countries. Conclusion: Findings suggest that sports clubs consider HP to be an important part of their remit, but up to 66% have no policy in this regard. The policy cycle analysis helped identify the key tools needed to promote sports clubs’ HP policy development. Practical implications include fostering the inclusion of HP in sports clubs’ development plans by establishing templates and receiving support from sports federations.

Pharmacists’ involvements and barriers in the provision of health promotion services towards noncommunicable diseases: Community-based cross-sectional study in Northwest Ethiopia

BackgroundCommunity drug retail outlets (CDROs) are among the initial healthcare facilities where pharmacists play a crucial role in preventing and managing noncommunicable diseases (NCDs). Therefore, this study assessed pharmacists’ level of involvement and barriers in the provision of health promotion for noncommunicable diseases at CDROs in Northwest Ethiopia.MethodsA community-based multicenter cross-sectional study was conducted among community pharmacists in Northwest Ethiopia from April to June 2022. Data was collected using a self-administered structured questionnaire, and analyzed using the Statistical Package for Social Science (SPSS) version 26. The level of involvement mean score difference among pharmacists was investigated using an independent samples t-test and a one-way ANOVA. Logistic regression analysis was used to examine the association between pharmacists’ level of involvement and other variables. A p-value < 0.05 at a 95% confidence interval (CI) was considered statistically significant.ResultsA total of 285 (94.4%) participants participated in the study out of 302 approached samples. Overall, more than half (58.9%) of the participants showed a high level of involvement in health promotion. Pharmacists who had a degree and/or above (AOR = 0.03, 95% CI: 0.01–0.63; p < 0.001) and served a lower number of clients per day (AOR = 0.19, 95% CI: 0.04–94; p = 0.042) were less likely to have low involvement in health promotion services. Pharmacists who worked fewer hours per day (AOR = 3.65, 95% CI: 1.79–7.48; p = 0.005) were more likely to have low involvement. Lack of an appropriate area in the CDROs (52.1%) and lack of coordination with other healthcare providers (43.6%) were the most reported barriers to the provision of health promotion.ConclusionMost pharmacists were found to have a high level of involvement in health promotion activities. A lack of an appropriate area in the CDROs and a lack of coordination with other healthcare providers were among the most reported barriers. Pharmacists might benefit from training to increase their educational backgrounds, and barriers could be addressed to enhance the pharmacist involvement.

Preconception Care: Intention to Action- An Intervention Based on the Planned Behavior Theory for Maternal and Neonatal Health: A Randomized Clinical Trial

Background and Objectives: Preconception care involves health promotion, screening, and interventions for women and men to reduce the risk factors with the potential effect on their future pregnancies; the present research is aimed at investigating the effect of planned behavior theory on the knowledge, attitude, subjective norms, perceived behavior, self-control, intention, and practices of preconception care. Methods: This is a controlled randomized trial study to examine the impacts on the intentions and conductance of the preconception care among the women referring to a marriage consulting center. To this end, 110 women referring to the marriage counseling centers of Asad Abad city in Iran were selected and then divided into two groups. The educational sessions were held twice a week in four 60-minute sessions for the intervention group, while the control group received no intervention. Wilcoxon and Mann-Whitney and ϰ2 were employed for data analysis at the significance level of 0.05. The reliability of the tools was evaluated using Cronbach Alpha. Findings: The results indicated a significant difference between the two groups in terms of awareness (14.27 ± 1.53 vs. 5.32 ± 2.95), attitudes (45.89 ± 3.52 vs. 36.36 ± 6.77), subjective norms (30.01 ± 3.34 vs. 26.38 ± 4.10), and perceived behavioral control (23.10 ± 2.06 vs. 18.87 ± 4.61) for the intervention and control groups, respectively (P = 0.001). Also, 89% of the intervention group (vs. 4% of the controls) tried to make the record for preconception care. Conclusion: The theory of planned behavior can effectively improve awareness, attitude, and perceived behavioral control in women. This theory also improved the behavioral intention of women toward preconception care. The midwives and physicians are recommended to use this educational method in health centers to motivate the women to take preconception counseling with the help of health providers. Trial registration: This study was registered in the Iranian registry of clinical trials with the code IRCT2015121425529N1 in 2016.04.17

Developing local hospitals for public health excellence

The current study investigated the factors affecting the success of local hospital operations in promoting public health excellence in Thailand. A mixed method approach with focus groups and questionnaires was utilized. The study also depended on descriptive statistics and hierarchical regression analysis. According to the findings, public health providers agreed that the main factors leading to the success of local hospital operations were participation with people in the community, respect, and policy development. Other aspects included the use of devices, tools, medicine or medical supplies, training, and project organization. Meanwhile, the service recipients held the view that any suggestion of information, knowledge, or capability of personnel has an influence on operational guidelines. However, success at higher levels must rely on two things: 1) the location of a local hospital and 2) analysis of the guidelines for developing the services of local hospitals for promoting public health in the future. The study also relied on content analysis. According to the findings, appropriate operating guidelines for local hospitals would comprise policy development for promoting public health, participation by people in the community, and public services by the hospital, including training to increase the potential of personnel and project organization involving health promotion.

Voluntary (NGO) Sector’s Involvement in Health Promotion for Older Population in Europe

&#x0D; Background and objective: The NGO’s (non-governmental organisations’) role in health promotion is very much praised and encouraged. The basic features of this sector, namely social participation, voluntary activity and self-help, are by default considered as beneficial for health promotion for older people. New policy approaches stimulate NGOs involvement in it. This makes questions about this involvement – its role, context and success – increasingly relevant.&#x0D; Method: The research is based on narrative literature review as well as interviews and consultations with informers (practitioners and experts) in selected European countries.&#x0D; Results: Voluntary/NGO sector increases its role as a complimentary or alternative source of health promotion for older people in Europe. Its involvement, roles and functions as well as actual significance, however, depends on a number of factors. It depends on the sustainability of funding, stability of employment, elderly participation rate and recognized availability of services. Those factors are determined by socio-economic and systemic conditions: a country-specific model of third sector, overall social attitudes towards participation and system of governance in health promotion.&#x0D; Conclusion: The sector’s significance is widely acknowledged, however, its impact is often only assumed rather that sufficiently assessed and proven. By its voluntary nature, the sector might not be able to address the problem of health inequalities. Also, various political and economic changes across Europe result in the change of the sector’s performance.&#x0D;

Challenges, opportunities, and potential roles of the private primary care providers in tuberculosis and diabetes mellitus collaborative care and control: a qualitative study

IntroductionThe comorbidity of tuberculosis and diabetes mellitus (TB-DM) is a looming global co-epidemic problem. Despite the Indonesian Government’s ongoing effort to impose regulation for collaborative TB-DM management, the involvement of private primary care providers (PPCs) has not been considered before the COVID-19 pandemic. This study aimed to capture the PPCs’ existing practices and explore their challenges, opportunities, and potential roles in the collaborative TB-DM services and control.MethodsA descriptive qualitative research design was used to collect data. Two Focus Group Discussions (FGDs) were conducted with 13 healthcare workers (HCWs) from different private clinics and eight private/solo general practitioners (GPs) from Yogyakarta City, Indonesia. We triangulated these data with data from FGDs of HCWs community health centers (CHCs) and in-depth interviews of three regional health regulators, five hospitals staff members, and a representative of national health insurance. The discussions were audio-recorded, transcribed verbatim, and subjected to thematic analysis.ResultsPPCs have not been initiated into the implementation of the collaborative TB-DM programme. The themes identified in this study were health system-related barriers, knowledge and perception of HCWs, lack of implementation of bi-directional screening, and needs of multisector role. The potential roles identified for PPCs include involvement in health promotion, bi-directional screening, patient referral, and data reporting according to the TB-DM programme indicators. However, more thorough improvement of PPCs’ capacity and logistic supplies are needed to provide comprehensive TB treatment.ConclusionAlthough PPCs’ involvement in implementing collaborative TB-DM services has yet to be considered, their potential role should not be neglected. Therefore, it is essential to increase their involvement by enhancing their capacity and improving the Public-Private Mix. PPCs’ engagement should be initiated and maintained to ensure the sustainability of the programme.

The involvement in health promotion in the setting of daycare centres - Description of access

BackgroundFor health promotion and prevention in childhood, e.g. overweight, the daycare setting enables particularly early access to the target groups. The aim is to address not only children, but more explicitly also their parents and the wider social environment, and to involve them in the planning and implementation of measures. The project is developing participative and demand-oriented offers in different institutions throughout Germany. Scientific monitoring aims to generate recommendations for action for participatory health research from these various experiences.MethodsAccess takes place via day care centres and family centres. The participatory research design starts with the Search Conference method. With this method, institution-specific needs are identified on a local level and goals are defined that determine the further processes. The participatory procedure is accompanied by interviews and focus group discussions which are analysed according to Mayring.ResultsDue to lockdown, access to the target groups via the institution is difficult. Families and network partners are hardly accessible and heavily burdened. Nevertheless, it has been possible to establish digital participation in some institutions. The evaluations show that parents have a great interest in the health of their children. On the one hand, the results indicate that the current conditions lead to a wait-and-see attitude on the part of the institutions, while at the same time the topic of health is gaining in importance among all those involved.ConclusionsParticipatory methods pose a particular challenge during lockdown and must be adapted to the current situation. Especially when moving participation into the digital space, daycare centres (and similar institutions) play a key role as gatekeepers.Key messages Covid 19 highlights the central role of families and the immediate social environment for children.It is essential to involve them in health promotion to enable children to grow up healthily.

Is there a time and place for health education in chain pharmacies? Perspectives of Polish community pharmacists.

Pharmaceutical practice has evolved significantly from the provision of drugs to a more patient-centred model. However, the ownership structure of pharmacies may impact the level of cognitive services provided by them. The discrepancy may be observed between pharmacists' opinions on how involved they should get and their actual involvement in health promotion and disease prevention. Given the growing market share of pharmacy chains in Europe, this study aimed to investigate the attitudes of pharmacists employed in them towards their role as health educators. It applies Ajzen's theory of planned behaviour to examine pharmacists' perspectives with the use of semi-structured in-depth interviews. A total of 10 semi-structured face-to-face interviews were conducted in May 2017 among Polish pharmacists employed in chains. Obtained results showed that respondents welcomed the possibility of providing health education, but simultaneously they seek physicians' and patients' acceptance for undertaking this task. Competency gaps and unfavourable working conditions may also decrease pharmacists' intention to serve as health educators. Including the role of a health educator to the scope of the pharmacists' practice requires a precise definition of their competencies and responsibilities together with additional training aimed to fill potential gaps in their qualifications. The pharmacists' image as a health educator should also be widely communicated to patients and other healthcare professionals.

MODEL MATEMATIKA MENGENAI KESADARAN MASYARAKAT DALAM PERILAKU HIDUP BERSIH DAN SEHAT DI KOTA SAMARINDA

Samarinda is one of the big city in Kalimantan since it is a capital city of East Kalimantan Province. Its population is about 988,943 in 2015, but only 28.62 percent of the whole households were applying clean and healthy living behavior. To encourage this behavior to the society, it needs some health promotions to be introduced to them. In order to know the pattern of clean and healthy behaving households with respect to time, so this research is aimed to perform a mathematical model of the implementation of clean and healthy living behavior which involves health promotion. Runge-Kutta is the used model to solve this model. The result informs that the rate of change of the households which implement a clean and healthy life, called as good people, increases for all sub-districts in Samarinda City. In contrast, households that do not know and do not behave clean and healthy life, called as bad people, are reduced because of the promotion. Developing a national framework for community involvement in health promotion in Tajikistan

Abstract Background In rural Tajikistan, various approaches have been taken to involve communities in health promotion, but many of them survived only as long as there was outside donor support. Methods In 2017, these approaches were examined and conceptualized, leading to the development of a “Guideline on Partnership with Communities on Health Issues” that was put into practice in 11 rural districts of the country. Results The Guideline sees Primary Health Care (PHC) staff trained as facilitators so as to conduct Participatory Reflection and Action (PRA) sessions with their local communities. Based on health priorities identified through these PRA sessions, and on national health priorities, the Health Promotion Institute chooses annual community activities facilitating behavioral change. Community Health Teams (CHTs) are formed, and the facilitators train them in disease prevention activities, and in how to plan initiatives to promote better health and well-being at village level. In the 5 initial districts, CHTs successfully mobilized community members to take part in activities related to identified health priorities e.g. health actions on Hypertension and Diabetes including free screenings. Furthermore, CHTs started to address determinants of health e.g. by supporting disabled people in their communities, organizing weekly waste removals, and initiating womens' walking groups. Conclusions Engaging communities by forming and capacitating teams, leads them to become a partner to the PHC system, and empowers them to tackle local determinants of health through their own initiatives. The capacity built in the local primary health care and health promotion structures hold potential for an expansion of this community-based approach to health promotion throughout the country. So what? By encouraging a government-recognized approach to community involvement in health, capacity and ownership of local institutions and stakeholders has increased, providing promise for long term sustainability. Assessment of community pharmacist's involvement in health promotion and education activities of patients in Khartoum, 2020

Introduction: Health promotion enables people to gain better control and improve their health and overall well-being. An important aspect of health promotion is community action and participation through health education which encourages socioeconomic and cultural activities and improves environmental determinants of health. Justifications: Sudan national guidelines for good pharmacy practice in community pharmacy 2016 state that community pharmacists should have a role in health promotion in community, so researches must be done to assess achievement in this issue. Objectives: The objective was to assess the extent of the pharmacists' involvement in counseling patients about health promotion and education topics, their preparation to counsel patients in health promotion and education topics, and their perceived success in changing the patients' health behavior. Materials and Methods: An observational cross-sectional study was carried out from March to April 2020 among community pharmacists in Khartoum locality. Results: A total of 150 community pharmacists who participated in the study showed that the most important purpose of consumers' visits to community pharmacy was related to seeking pharmaceutical advice about drug dosage, indication, availability, and side effects which were ranked first, second, third, and fourth, respectively. In the area of the pharmacists' perceptions on their preparedness to counsel patients on health-related behaviors, the perceived preparedness was highest for counseling on taking drugs as prescribed/directed (mean [standard deviation (SD)]; 4.5 [0.9]) and knowledge about drug contents and side effects (mean [SD]; 4.2 [1.1]). The community pharmacists perceived success in helping patients to change their health-related behaviors. Conclusion: The majority of participants have a positive attitude toward counseling the population on health behaviors and indicated their willingness to learn more about health promotion.

Professional football clubs' involvement in health promotion in Spain: an audit of current practices.

BACKGROUND: The implementation of effective community-based health interventions within Spanish football clubs has the potential to positively influence the public health agenda and enable the health care system in Spain to be more successful and sustainable. This paper aims to explore the involvement of Spanish football clubs in health promotion activities, their potential for future involvement and what that would require. METHODS: A mixed methods explanatory sequential design, with a purposive sample of La Liga clubs. Data collection included online questionnaires and phone interviews. Quantitative methods enabled us to describe the number and types of programmes the clubs are currently involved in. Qualitative data was useful to further unpick the processes followed by the clubs in planning and developing health promotion programmes, while identifying any determinants to change. RESULTS: Seventeen clubs completed questionnaires and 11 participated in interviews. Clubs generally support inclusive programmes that target disadvantaged groups. Health-related programmes focus on healthy eating, physical activity and blood donation. Thematic analysis of interviews with 11 representatives of La Liga clubs resulted in three key themes. These related to: (1) Diversity of programmes; (2) (Lack of) evidence-based approaches to intervention design and evaluation; and (3) Contrasting views about a club’s role in health promotion interventions. CONCLUSION: Spanish football clubs have potential to reach into communities that are currently underserved. However, there is limited infrastructure and understanding within the clubs to do this. Nevertheless, there is huge opportunity for organisations with public health responsibility in Spain to implement translational approaches within football-based settings.

Willingness to adopt wearable devices with behavioral and economic incentives by health insurance wellness programs: results of a US cross-sectional survey with multiple consumer health vignettes

BackgroundThe number of health-related wearable devices is growing but it is not clear if Americans are willing to adopt health insurance wellness programs based on wearables and the incentives with which they would be more willing to adopt.MethodsIn this cross-sectional study we used a survey methodology, usage vignettes and a dichotomous scale to examine U.S. residents’ willingness to adopt wearables (WTAW) in six use-cases where it was mandatory to use a wearable device and share the resulting data with a health insurance company. Each use-case was tested also for the influence of additional economic incentives on WTAW.ResultsA total of 997 Americans across 46 states participated in the study. Most of them were 25 to 34 years old (40.22%), 57.27% were female, and 74.52% were white. On average, 69.5% of the respondents were willing to adopt health-insurance use-cases based on wearable devices, though 77.8% of them were concerned about issues related to economic benefits, data privacy and to a lesser extent, technological accuracy. WTAW was 11–18% higher among consumers in use-cases involving health promotion and disease prevention. Furthermore, additional economic incentives combined with wearables increased WTAW overall. Notably, financial incentives involving providing healthcare credits, insurance premium discount, and/or wellness product discounts had particularly greater effectiveness for increasing WTAW in the consumer use-cases involving participation: for health promotion (RR = 1.06 for financial incentive, 95% CI: 1.01–1.11; P = 0.018); for personalized products and services (RR = 1.11 for financial incentive, 95% CI: 1.01–1.21; P = 0.018); and for automated underwriting discount at annual renewal (RR = 1.28 for financial incentive, 95% CI: 1.20–1.37; P < 0.001).ConclusionsUnder the adequate economic, data privacy and technical conditions, 2 out of 3 Americans would be willing to adopt health insurance wellness programs based on wearable devices, particularly if they have benefits related to health promotion and disease prevention, and particularly with financial incentives.

Health information needs assessment among self-help groups and willingness for involvement in health promotion in a rural setting in Puducherry: A mixed-method study.

BACKGROUND:Health promotion is the process of enabling people to increase control over their own health. Community involvement in social and environmental interventions tends to improve people's health and quality of life. Self-help groups (SHGs) primarily focus on microfinancing, thereby acting as a catalyst in bringing backward society to the mainstream. They possess huge potential to influence the health of the community.OBJECTIVE:The objective of the study is to assess the willingness among SHGs for the involvement in health promotion activities and to assess their health information needs.MATERIALS AND METHODS:A community-based, cross-sectional study was conducted during September 2017 in rural Puducherry. All 86 SHGs in four selected villages were covered, and their leaders were interviewed using a structured questionnaire on functioning of SHGs and their health information needs. Willingness for health promotion activities was rated on an interest scale (1–100).RESULTS:Among the 86 heads of SHGs interviewed, 81 SHGs (94.1%) were registered. They were mainly involved in microfinancing. Health information needs expressed were cancer detection (45%), diabetes (60%), hypertension (56%) and vector-borne diseases (63%). When asked to rate their willingness on an interest scale for health promotion activities, nearly 64% showed a great interest (i.e., score > 60). Majority were willing to work for noncommunicable disease-related activities such as provision of drugs (86%) and for screening of various cancers (84%).CONCLUSION:This study has shown that majority of SHG members have expressed willingness for the involvement in health-related activities, thereby can be utilized as an important resource for health promotion in rural areas.

Agencements of Reanimation: Facilitating an Active Old Age through Danish Co-Creation Initiatives

Contemporary healthy and active aging policies coincide with new forms of governance in European welfare states promoting active citizenship, which in Denmark has been termed “co-creation.” As active aging and co-creation policies go hand in hand, new forms of health promotion programs and public-private collaborations emerge. This article centers on two initiatives that target older people in two Danish municipalities: a local COPD-choir in Vordingborg and the local department in Ishøj of the (inter)national Cycling without Age association. The municipalities take on the assignment of reanimating old age through such initiatives, by challenging the institutional barriers of the municipality. The article describes how co-creation is a redistribution of agency in the European welfare states and how older volunteers are called upon to participate in the organization of municipal initiatives, while at the same time these initiatives come with pre-fixed definitions of “good” old age. The article explores the co-creation initiatives as indicative of the agencements (Çalışkan and Callon 2010) of reanimation that endeavor to revive old age and subjectivise older people as active citizens. While such agencement involves health promotion, this article argues that the aim of initiatives is instead to engage older citizens, and hence to facilitate an active old age.

Understanding the community interest of breast cancer in Indonesia: a digital epidemiology study using Google trends

Introduction. Breast cancer was count for 30.5% of all cancers diagnosed in Indonesia. Although preventable, breast cancer is mostly diagnosed in advanced stages and caused leading dead among females. Given the increasing growth of online information seeking behavior, adequate cancer health promotion through virtual setting is needed to tackle the massive increasing burden of breast cancer. Therefore this study aims to explore the community interest in breast cancer using Google Trends. Method. 5 years ( from September 2013 to August 2018) information searches for breast cancer from Google were retrieved in Indonesian language. Data were downloaded at national and sub-region level to examine the pattern and distribution of queries. Results. Sporadic traces of information searches related to breast cancer from Google Trends presented the pattern and distribution of queries. Massive search happened in July 2015 and June 2017 following the died of Indonesian celebrity who suffered from breast cancer. However, the cancer awareness month every October does not impact the number of information searches. Considering the high influence of celebrities, many studies reveal the positive impact of celebrities involvement in health promotion. Celebrity can attract the public attention to health messages and increase the agreement of vaccination and screening for cancer. Thereby, celebrities involvement and availability of qualified breast cancer online information should be increased to win the breast cancer health promotion program in the digital era. Online information related to breast cancer could be disseminated through targetted risk population using Mobile JKN that has been downloaded by 1.5 million members. Conclusion. Google Trends could be potentially used as a novel tool to measure the dynamics of community interest in breast cancer in Indonesia. Therefore, adequate cancer health promotion through a virtual setting is the key to tackling the massive increasing burden of breast cancer in the digital era.

Atividades de promoção da saúde para adolescentes de uma escola no município de Curitiba: relato de experiência

A saúde é um fator essencial para o desenvolvimento do ser humano, desta forma os projetos de promoção à saúde do adolescente buscam o desenvolvimento de hábitos saudáveis no sentido do auto-cuidado. Objetivo: promover à saúde de um grupo de atletas de 11 a 14 anos do Colégio Militar do Paraná, envolvendo temas como prática de exercício físico, alimentação saudável, qualidade do sono e saúde ambiental. Métodos: o estudo foi desenvolvido no período de 2016, consistindo em 4 encontros com duração de 30 minutos, envolvendo práticas de promoção e educação em saúde subdividas em conceitual teórico e prático. Resultados: os estudantes praticavam exercício físico e a maioria acreditava possuir uma alimentação saudável e uma boa qualidade de sono, porém, após a realização das atividades, relataram ter aprendido mais sobre a sua própria saúde. Conclusão: existe a necessidade de programas de promoção à saúde do adolescente dentro das escolas.

Embedding health equity strategically within built environments

Population health approaches are visible among multidisciplinary methods used in urban design and planning, but attention to health equity is not always an explicit focus. Population and Public Health-Saskatoon Health Region recognized the need for frameworks to prioritize, integrate and measure health equity within local built environments. A cross-department healthy built environment (HBE) initiative coordinated activities involving Health Promotion, Environmental Public Health, Public Health Observatory, and Medical Health Officers engaged with municipal, academic and community partners in Saskatoon, Saskatchewan. The HBE team conducted evidence reviews and consulted with partners to identify common health equity issues in built environments and best and leading practices to address them. The HBE team then prioritized and undertook projects to model a health equity approach. Projects included the following: (1) developing a Health Equity in Healthy Built Environment Framework; (2) engaging in a partner campaign highlighting built environment and health equity during a municipal election; (3) producing a Health Equity Impact Assessment (HEIA) report on the City of Saskatoon's growth plan; and (4) developing a monitoring and evaluation framework for health equity outcomes. Other outputs include making new connections between local HBE and poverty reduction efforts and promoting social inclusion guidelines in consultation processes. Within a population health approach to HBE, an explicit focus on health equity can be a catalyst for engaging partners in cross-sectoral action for building inclusive physical and social environments.

Factors That Influence Linkages to HIV Continuum of Care Services: Implications for Multi-Level Interventions.

Worldwide, the human immunodeficiency virus (HIV) continuum of care involves health promotion providers (e.g., social workers and health educators) linking patients to medical personnel who provide HIV testing, primary care, and antiretroviral treatments. Regrettably, these life-saving linkages are not always made consistently and many patients are not retained in care. To design, test and implement effective interventions, we need to first identify key factors that may improve linkage-making. To help close this gap, we used in-depth interviews with 20 providers selected from a sample of 250 participants in a mixed-method longitudinal study conducted in New York City (2012–2017) in order to examine the implementation of HIV services for at-risk populations. Following a sociomedical framework, we identified provider-, interpersonal- and environmental-level factors that influence how providers engage patients in the care continuum by linking them to HIV testing, HIV care, and other support services. These factors occurred in four domains of reference: Providers’ Professional Knowledge Base; Providers’ Interprofessional Collaboration; Providers’ Work-Related Changes; and Best Practices in a Competitive Environment. Of particular importance, our findings show that a competitive environment and a fear of losing patients to other agencies may inhibit providers from engaging in linkage-making. Our results suggest relationships between factors within and across all four domains; we recommend interventions to modify factors in all domains for maximum effect toward improving care continuum linkage-making. Our findings may be applicable in different areas of the globe with high HIV prevalence.
