There are both pharmacologic and practical reasons for recommending intraoperative intraperitoneal chemotherapy. From a pharmacologic point of view, direct installation of selected drugs into the peritoneal cavity results in a large increase in the amount of chemotherapy that reaches abdominal and pelvic surfaces. This pharmacologic advantage is the result of the peritoneal-plasma barrier. Molecules of large hydrophyllic drugs leave the peritoneal cavity more slowly than they are metabolized systemically.
