Direct surgical treatment of intracavernous internal carotid artery aneurysms: Report of four cases

The authors have operated directly on four cases of intracavernous internal carotid artery aneurysms by opening the cavernous sinus. Surgery was performed using a semisitting position, in which the upper half of the patient's body was elevated approximately 20° from the horizontal plane. The superior wall of the cavernous sinus was opened through the pterional approach, and an aneurysmal neck clipping was successfully performed on three patients. A coating of the aneurysm was carried out on one patient. The results obtained were excellent in all cases. The authors believe that the operative technique reported is useful for surgical treatment of intracavernous lesions.

Dynamic Computed Tomography: Intracranial Applications

Rapid sequential computed tomography of the brain after the bolus injection of contrast material provides invaluable information as to the characteristic blood flow of intracranial lesions in a noninvasive manner. Plotted dynamic curves permit accurate diagnosis of particularly difficult cases of infarcts and neoplasms. Dynamic computed tomographic (CT) scanning has become a part of the CT work-up for infarcts, which has allowed their earlier demonstration, detected as areas of hypoperfusion not clearly evident on an initial conventional CT study. Quantitative assessment of vasogenic edema and hypoperfusion are helpful in establishing the diagnosis of infarction and neoplasia. Orbital and parasellar neoplasms can be distinguished accurately from vascular lesions. Dynamic CT studies complemented conventional film screen arteriography in the evaluation of three cases of intracavernous internal carotid artery aneurysm, defining thrombus formation and wall thickness and thus influencing the therapeutic approach. In addition, this modality is useful in differentiating jugular fossa neoplasm from vascular malformation. Bilateral Mycotic Aneurysms of the Intracavernous Carotid Artery

A rare case of bilateral intracavernous internal carotid artery aneurysms is presented. The clinical and laboratory data strongly suggested the mycotic nature of the aneurysms. Internal carotid artery ligation was performed because of clinical evidence of progressive enlargement of the right-sided aneurysm. The left-sided aneurysm was managed medically. It remained asymptomatic and, 14 months later, a control angiogram showed no significant change in its size and configuration.

Direct surgical treatment of bilateral intracavernous internal carotid artery aneurysms

Bilateral intracavernous internal carotid artery aneurysms are described in a 3-year-old child. The etiology was uncertain, although the aneurysms may have been mycotic. As there was clinical and radiological evidence of progressive enlargement of both aneurysms, the more conventional forms of treatment were not applicable. A direct surgical approach was made to both aneurysms with exploration of the intracavernous portion of the internal carotid artery under deep hypothermia and cardiac arrest.
