Intraabdominal follicular dendritic cell sarcoma: a report of three cases and review of the literature.

Follicular dendritic cell sarcoma (FDCS) is a rare neoplasm that originates from follicular dendritic cells in lymphoid follicles. FDCS has been increasingly reported in recent years. However, data on FDCS are mostly based on single case reports or case series and its natural history and standard treatment are not clear. To increase the understanding of this rare disease, we report our experience of three cases of FDCS with an analysis of the morphological and immunophenotypic characteristics, clinical course, treatment options and response to treatment. In addition, we reviewed the literature on FDCS.

患者女,24岁.因发现腹腔肿物20 d于2012年6月29日入院.体检:上腹部可触及一约15 cm×10 cm肿物,质韧,边界不清,无压痛、反跳痛,无移动性浊音.患者曾因停经65 d,阴道流血2d到当地医院就诊,B超检查提示早孕,尿人绒毛膜促性腺激素(HCG)阳性,血常规示白细胞31.17 × 109/L。

Inflammatory pseudotumor-like follicular dendritic cell tumor of the liver with expression of estrogen receptor suggests a pathogenic mechanism: a case report and review of the literature

The term inflammatory pseudotumor-like follicular dendritic cell tumor has been suggested for a follicular dendritic cell tumor of the liver associated with a dense inflammatory infiltrate and Epstein–Barr virus infection. This tumor is less aggressive than usual intraabdominal follicular dendritic cell sarcoma and shows a strong female predominance; however, the pathogenesis is uncertain. We report the case of a 59-year-old woman who presented with a 6-cm mass in the liver. Histological examination showed spindle cells with a storiform pattern and prominent inflammatory infiltrate mimicking inflammatory pseudotumor. Spindle cells expressed follicular dendritic cell markers CD21 and CD35, and were positive for Epstein–Barr virus-encoded RNA and LMP-1 protein. In addition, neoplastic cells expressed estrogen receptor (ER). Fifteen cases of inflammatory pseudotumor-like follicular dendritic cell tumor of the liver including the case we describe here have been reported in the literature. A review of the literature disclosed that normal follicular dendritic cells may express ER; however, extensive testing has not been previously reported in follicular dendritic cell sarcoma. The female predominance and the expression of ER in the case we report are suggestive of a hormonal role in the pathogenesis and therapy of inflammatory pseudotumor-like follicular dendritic cell tumor of the liver.

Intra-abdominal Follicular Dendritic Cell Sarcoma With Marked Pleomorphic Features and Aberrant Expression of Neuroendocrine Markers: Report of a Case With Immunohistochemical Analysis

Follicular dendritic cell sarcoma (FDCS) is a very rare malignant tumor arising most frequently in lymph nodes with only few reports of extranodal locations. We report the case of a 35-year-old man with a large retroperitoneal mass. Histologically the tumor was composed of highly pleomorphic cells exhibiting some uncommon features such as an epithelioid appearance, cystic spaces, and multinucleated cells with morphologic features of emperipolesis. Immunohistochemically the neoplastic cells were immunoreactive for CD21, CD23 and CD35. A previously unreported expression of neuroendocrine markers (Synaptophisyn and Neuron-Specific-Enolase) was present. Ultrastructurally no neuroendocrine secretory granules were detected. FDCS can mimic a wide variety of other malignant tumors, and a correct diagnosis requires exclusion of other neoplasms and immunohistochemical confirmation.

Intra-abdominal follicular dendritic cell sarcoma

Follicular dendritic cell (FDC) sarcoma is a very rare condition. We report here an intra-abdominal FDC sarcoma occurring as a mass, dependent on the celiac and left gastric lymph chains, that was completely excised. Eighteen months after surgery a recurrence at the liver pedicle was detected by a CT-scan and fully resected; in order to prevent another disease relapse postoperative radiotherapy was given.

Paraneoplastic pemphigus associated with follicular dendritic cell sarcoma arising from Castleman’s tumor

Paraneoplastic pemphigus (PNP) is associated with various forms of underlying neoplasia. We report a patient with PNP associated with an intraabdominal follicular dendritic cell sarcoma that developed from a Castleman’s tumor that had preexisted for 40 years. (J Am Acad Dermatol 1999;40:294-7.)
