Update Breast Cancer 2024 Part 1 - Expert Opinion on Advanced Breast Cancer.

Clinical evidence is interpreted based on clinical studies and personal experience which can lead to different interpretations of data. This makes the opinions issued by panels of experts such as the Advanced Breast Cancer Panel which convened in November 2023 for the seventh time (ABC7) particularly important. At the conference, current issues around advanced breast cancer were evaluated by an international team of experts. In 2023 the data on CDK4/6 inhibitors was so extensive that the answers to questions about the sequencing of therapy and the potential use of chemotherapy as an alternative therapy were relatively clear. Moreover, data on antibody drug conjugates which provides a good overview of their uses is available for all molecular subtypes. Some therapeutic settings, including patients with brain metastases or leptomeningeal disease, older patients, locally advanced breast cancer and visceral crises, continue to be particularly important and were discussed in structured sessions. The scientific context of some of the topics discussed at ABC7 is presented and assessed here.

What makes an article a must read in medical education?

BackgroundThe dissemination of published scholarship is intended to bring new evidence and ideas to a wide audience. However, the increasing number of articles makes it challenging to determine where to focus one’s attention. This study describes factors that may influence decisions to read and recommend a medical education article.MethodsAuthors analyzed data collected from March 2021 through September 2022 during a monthly process to identify “Must Read” articles in medical education. An international team of health sciences educators, learners, and researchers voted on titles and abstracts to advance articles to full text review. Full texts were rated using five criteria: relevance, methodology, readability, originality, and whether it addressed a critical issue in medical education. At an end-of-month meeting, 3–4 articles were chosen by consensus as “Must Read” articles. Analyses were used to explore the associations of article characteristics and ratings with Must Read selection.ResultsOver a period of 19 months, 7487 articles from 856 journals were screened, 207 (2.8%) full texts were evaluated, and 62 (0.8%) were chosen as Must Reads. During screening, 3976 articles (53.1%) received no votes. BMC Medical Education had the largest number of articles at screening (n = 1181, 15.8%). Academic Medicine had the largest number as Must Reads (n = 22, 35.5%). In logistic regressions adjusting for the effect of individual reviewers, all rating criteria were independently associated with selection as a Must Read (p < 0.05), with methodology (OR 1.44 (95%CI = 1.23–1.69) and relevance (OR 1.43 (95%CI = 1.20–1.70)) having the highest odds ratios.ConclusionsOver half of the published medical education articles did not appeal to a diverse group of potential readers; this represents a missed opportunity to make an impact and potentially wasted effort. Our findings suggest opportunities to enhance value in the production and dissemination of medical education scholarship.

Mechanisms of Change in Exposure Therapy for Anxiety and Related Disorders: A Research Agenda

Anxiety and related disorders are a significant public-health burden with rising prevalence in the wake of the COVID-19 pandemic. As demand for effective anxiety treatment increases, so too does the need for strategies to bolster treatment outcomes. Research on the mechanisms of exposure therapy, the frontline behavioral treatment, will be critically important for optimizing clinical outcomes. We outline an initial agenda for future research on the mechanisms of change of exposure therapy, developed in collaboration with a large international team of researchers through the Exposure Therapy Consortium. Key questions and recommendations for future research focus on four priority areas: conceptualization, measurement, study design/analysis, and individual/contextual differences. Rising to the challenge of addressing these questions will require coordinated action and availability of centralized tools that can be used across trials, settings, and research groups.

A journey in the world of craniofacial development: From 1968 to the future.

This article is based on my talk at the meeting "3rd Advances in Craniosynostosis: Basic Science to Clinical Practice", held at University College, London, on 25 August 2023. It describes my contribution, together with that of my research team and external collaborators, to the field of craniofacial development. This began with my PhD research on the effects of excess vitamin A in rat embryos, which led to a study of normal as well as abnormal formation of the cranial neural tube. Many techniques for analysing morphogenetic change became available to me over the years: whole embryo culture, scanning and transmission electron microscopy, cell division analysis, immunohistochemistry and biochemical analysis of the extracellular matrix. The molecular revolution of the 1980s, and key collaborations with international research teams, enabled functional interpretation of some of the earlier morphological observations and required a change of experimental species to the mouse. Interactions between the molecular and experimental analysis of craniofacial morphogenesis in my laboratory with specialists in molecular genetics and clinicians brought my research journey near to my original aim: to contribute to a better understanding of the causes of human congenital anomalies.

Stosunki niderlandzko-rosyjskie w kontekście inwazji na Ukrainę

The Kingdom of the Netherlands is the only Western state, a member of the European Union, which has tense relations with Russia comparable to Polish-Russian ones after 2010. They have collapsed because of air disasters on the Russian controlled territories. Although the way they were played by politicians of both countries in a different manner, the final results were similar – close to zero. In the years 2011–2014, bilateral relations were going well, and their climax was reached during the winter Olympics in Sochi. The annexation of Crimea in 2014 worsened them. The Netherlands joined the sanctions announced against Russia. The collapse of the relations of the Kingdom of the Netherlands with Russia took place at the end of May 2018. It was the result of the shooting down of the Malaysia Airlines passenger aircraft (code: MH-17) over the Donetsk region on its route from Amsterdam to Kuala Lumpur route. It caused the death of 298 people on the board. The international investigation team found Russia guilty of this barbarian act. As a result of the trial, the Dutch courts ruled the fault of three people and sentenced them to life imprisonment.Diplomatic relations of the Netherlands with Ukraine were established in 1992. Since the Russian invasion of the Kingdom of the Netherlands, the policy of supporting Ukrainians in the fight against the invader has been consistent. The Dutch government favors a hard approach to Russia. The Netherlands participates in the economic sanctions of the European Union towards this country. The Dutch government offers Ukraine significant humanitarian, military and financial assistance. In 2024 it will reach an amount of around 2 billion Euro. The resignation of the government of Prime Minister Mark Rutte in September 2023 will not change the principles of foreign policy of the state, regardless of the composition of a new cabinet, almost all parties still want to continue the support of Ukraine.

Dějiny výzkumu v jeskyni Švédův stůl v Moravském krasu

This article provides an overview of the published archaeological, geological, anthropological, and paleontological research that has been undertaken at Švédův stůl Cave at Ochoz u Brna in the southern part of the Moravian Karst. Most of these excavations took place from the end of the 19th century to the present and have already been described in the literature; however, to date, there has been no comprehensive review for an English-speaking audience. The authors focus on the excavations of Bohuslav Klíma between 1953–1955 and the discovery of a Neanderthal’s mandible by the student Karl Kubasek in 1905, and the circumstances under which this find entered the collections of the Moravian Museum in Brno. The article also includes, for the first time ever, photographs from the Bohuslav Klíma archive documenting his excavations in the cave in the 1950s. This documentation was also used to place the test pits excavated in front of the cave in 2019 by an international research team in the spatial framework of Klíma’s excavations.

The Birth of a New BIM Standard: From PED 2018 to 2023, New Parameters and Workflows "Going Live" for Everyone

Building Information Modelling (BIM) has become the de facto standard for the digital representation of buildings. However, from the pedestrian dynamics perspective, BIM Industry Foundation Classes (IFC) schema specification do not fully support data properties required for two-way data sharing with pedestrian modelling tools. An international team of academic and industry researchers, supported by buildingSMART International (bSI), is developing an Occupant Movement Analysis (OMA) standard. The project is focused on expanding the IFC schema specification to support workflows for pedestrian simulation tools and is close to completion. So far, multiple process maps and a list of data properties synchronised with several representative pedestrian modelling tools have been produced. This list of data properties was then converted into bSI's recommended flexible and machine interpretable Information Delivery Specification (IDS) format for specifying data exchange requirements and to add clarity. Currently, this is undergoing testing and review by the project team. Once completed, it will be submitted to bSI’s committees for review. Also, to support this work, a prototype open-source Add-in has been developed to demonstrate a two-way integrated data sharing between BIM authoring tools and pedestrian simulation tools. This standard will enhance data sharing between BIM authoring and pedestrian modelling tools by facilitating the capturing of the required data and addressing friction in multiple design iterations and reassessment.

Technical performance differences amongst international netball teams at the Commonwealth Games 2022

ABSTRACT Team and opposition quality are factors that can influence netball performance. Although these effects are present within domestic netball, there is currently no assessment of these at the international level. The aim of this study was to assess performance differences between teams during all competition stages and matches at the 2022 Commonwealth Games. Thirty-six fixtures from all 12 teams were analysed, with performance indicators (PIs) relating to centre passes, turnovers and goal scoring assessed. Teams were split into three groups based on their finishing position in the competition (top 4, middle 4, bottom 4). When assessing team quality differences, significant differences were revealed for all but three PIs between groups (total number of restart and live turnovers and the total number of these combined). The quality of the opponent was also found to affect teams differently, with top-quality teams performing more consistently than lower-quality teams and being able to beat these teams by large winning margins (average of 55 goals and 26 goals versus bottom-quality and middle-quality teams respectively). Therefore, lower-quality teams should focus on implementing clear and effective tactical plans prior to matches, and performance analysis tools should be created for these teams to raise the quality of international netball.

Utilizing digital technologies for rapid damage assessment and reconnaissance: the February 6, 2023 Kahramanmaraş-Türkiye earthquakes (Mw 7.7 and Mw 7.6)

AbstractThis paper presents a comprehensive overview of the rapid damage assessment and reconnaissance efforts following the devastating earthquakes on February 6, 2023, in Türkiye. It specifically focuses on implementing the SiteEye Disaster Plugin, an additional component of SiteEye software developed by i4 Company engineers and Middle East Technical University researchers. This tool played a critical role in managing and analyzing a massive dataset comprising over 28,000 images and videos. The research highlights the plugin’s innovative features, such as offline data collection, georeferenced-based layering, and an integrated damage classification system, significantly improving earthquake impact assessments’ accuracy and efficiency. It also underscores the importance of interdisciplinary collaboration involving national and international teams and the role of open data in disaster management. The findings demonstrate how digital technologies can transform the field of disaster response, offering new approaches for rapid assessment and effective management in the aftermath of seismic events. This research contributes valuable insights into enhancing disaster preparedness and response strategies, particularly in earthquake-prone areas.

Scientific Utility of Selected Latin American Global Geoparks: A Literature-Based Case Study

Global geoparks, i.e., the members of the UNESCO Global Geopark (UGGp) network, possess highly valuable geoheritage resources, which can be used for the purposes of not only education and tourism, but also science. Five examples from four Latin American countries (Chile, Ecuador, Mexico, and Peru) were employed to realize the importance of these global geoparks as facilitators of international research. Journal articles devoted to these geoparks were selected with the major bibliographical databases, and the information from them was analyzed quantitatively. Particularly, the numbers of articles published before and after the UGGp membership, paying significant and marginal attention to geoparks, and published by international teams were calculated; the general themes of the articles were outlined. It was established that the total number of publications grew after the UGGp membership of these geoparks, but not steadily. The established dynamics of publishing are unstable and differ between the geoparks. Geoparks of Mexico (Mixteca Alta) and Peru (Colca y Volcanes de Andagua) are notable examples because they started to facilitate international research before the UGGp membership. Many articles consider geoparks only marginally (nonetheless, there are also articles paying significant attention to geoparks, especially in the cases of two Mexican geoparks). The author teams are often not restricted to Latin America. The research themes are rather diverse (geoheritage, geology, ecology, innovations, society, technology), although the majority of the publications are geoheritage-focused. Of special interest is the Colca y Volcanes de Andagua global geopark, which has been considered in the innovation-focused article. Generally, our results indicate a moderate importance of the considered global geoparks to international research activity and the incomplete exploitation of their scientific potential. Two Mexican geoparks (Comarca Minera and Mixteca Alta) seem to be the most successful in the facilitation of international research. Indeed, the scientific utility of global geoparks should be strengthened.

Do more heads imply better performance? An empirical study of team thought leaders' impact on scientific team performance

Thought leadership plays a crucial role in boosting team performance; thus, teams with more thought leaders may perform better. However, the impact of the number of thought leaders on team performance in a scientific context remains understudied. In this study, we consider the authors of a publication as a scientific team and define the authors responsible for conceptual tasks, i.e., “conceived and designed the experiments” (one of the tasks described in the PLOS contribution statements classification system), as thought leaders. Leveraging more than 140,000 papers from PLOS journals, we examine the relationship between the number of thought leaders and two aspects of team performance (i.e., team impact and team disruptiveness) from both correlational and causal perspectives. The results showed that (1) an inverted U-shaped relationship exists between the number of thought leaders and the team's impact, and (2) teams with more thought leaders tend to produce less disruptive ideas. We also explored the impact of international collaboration, team size, and gender diversity together with the number of thought leaders on team performance and found that (3) international collaboration improves team impact but lowers the disruptiveness of team outputs. This study advances scholarly understanding of thought leadership in scientific teams and provides valuable insights for policymakers and team managers.

Breastfeeding Following Spinal Cord Injury: Consumer Guide for Mothers.

The World Health Organization (WHO) recommends that infants be breastfed exclusively for the first 6 months of age. However, there are few resources available on the effects a spinal cord injury (SCI) can have for breastfeeding mothers. It is difficult to find information to address the unique challenges women with SCI experience when planning or trying to breastfeed. Our international team, including women with SCI, health care providers, and SCI researchers, aims to address the information gap through the creation of this consumer guide. The purpose of this consumer guide is to share the most common issues women with SCI experience during breastfeeding and provide information, practical suggestions, recommendations, and key resources in lay language. General information about breastfeeding is available on the internet, in books, or from friends and health care providers. We do not intend to repeat nor replace general breastfeeding information or medical advice. Breastfeeding for mothers with SCI is complex and requires a team of health care providers with complementary expertise. Such a team may include family physician, obstetrician, physiatrist, neurologist, occupational and physical therapist, lactation consultant, midwife, and psychologist. We hope this consumer guide can serve as a quick reference guide for mothers with SCI planning of trying to breastfeed. This guide will also be helpful to health care providers as an educational tool.

Северный морской путь на Арктических чтениях в Москве

The paper presents an analytical review of the materials of the Arctic Readings which took place in Moscow in February 2024. The author describes the idea and subject of this international academic forum as well as the contents of the presentations. The discussion of the participants of the Arctic Readings was centred on the book prepared and published by an international team of researchers into the history of the Northeast Passage and Northern Sea Route. The article highlights that the theme of the past and resent exploration of the Arctic and Northern Sea Route as well as the understanding of the historical experience and prospects for their development are strategically important both for Russia and for international relations. It is indicated that the Russian-language version of the discussed publication will be useful for specialists and a wide range of interested readers. A wish is expressed that the Arctic Readings become a good tradition in our country.

Training nurses in an international emergency medical team using a serious role-playing game: a retrospective comparative analysis

BackgroundAlthough game-based applications have been used in disaster medicine education, no serious computer games have been designed specifically for training these nurses in an IEMT setting. To address this need, we developed a serious computer game called the IEMTtraining game. In this game, players assume the roles of IEMT nurses, assess patient injuries in a virtual environment, and provide suitable treatment options.MethodsThe design of this study is a retrospective comparative analysis. The research was conducted with 209 nurses in a hospital. The data collection process of this study was conducted at the 2019-2020 academic year. A retrospective comparative analysis was conducted on the pre-, post-, and final test scores of nurses in the IEMT. Additionally, a survey questionnaire was distributed to trainees to gather insights into teaching methods that were subsequently analyzed.ResultsThere was a significant difference in the overall test scores between the two groups, with the game group demonstrating superior performance compared to the control group (odds ratio = 1.363, p value = 0.010). The survey results indicated that the game group exhibited higher learning motivation scores and lower cognitive load compared with the lecture group.ConclusionsThe IEMT training game developed by the instructor team is a promising and effective method for training nurses in disaster rescue within IEMTs. The game equips the trainees with the necessary skills and knowledge to respond effectively to emergencies. It is easily comprehended, enhances knowledge retention and motivation to learn, and reduces cognitive load.

State of open science in cancer research

PurposeThis study has been focused on assessing the Open Science scenario of cancer research during the period 2011–2021, in terms of the derived scientific publications and raw data dissemination.MethodsA cancer search equation was executed in the Science Citation Index-Expanded, collecting the papers signed by at least one Spanish institution. The same search strategy was performed in the Data Citation Index to describe dataset diffusion.Results50,822 papers were recovered, 71% of which belong to first and second quartile journals. 59% of the articles were published in Open Access (OA) journals. The Open Access model and international collaboration positively conditioned the number of citations received. Among the most productive journals stood out Plos One, Cancers, and Clinical and Translational Oncology. 2693 genomics, proteomics and metabolomics datasets were retrieved, being Gene Expression Omnibus the favoured repository.ConclusionsThere has been an increase in oncology publications in Open Access. Most were published in first quartile journals and received higher citations than non-Open Access articles, as well as when oncological investigation was performed between international research teams, being relevant in the context of Open Science. Genetic repositories have been the preferred for sharing oncology datasets. Further investigation of research and data sharing in oncology is needed, supported by stronger Open Science policies, to achieve better data sharing practices among three scientific main pillars: researchers, publishers, and scientific organizations.

Feasibility and acceptability of a novel biomedical device to prevent neonatal hypothermia and augment Kangaroo Mother Care in Kenya: Qualitative analysis of focus group discussions and key Informant Interviews.

Hypothermia is a leading newborn complication, especially among premature and/or low birth weight infants. Within low/middle-income countries where incubators and radiant warmers are often in short supply, leading to gaps in the thermal care chain, neonatal hypothermia underlies high rates of newborn morbidity and mortality. Kangaroo Mother Care/Skin-to-skin care is an effective method for prevention of hypothermia in premature and low birthweight babies but can be very burdensome for families and healthcare providers. Our international multidisciplinary team has developed a prototype for a wearable biomedical device ("NeoWarm") to provide continuous thermal care and augment kangaroo mother care practices in low-resource settings. The objective of this study was to assess the feasibility and acceptability of NeoWarm and to obtain user design feedback for an early prototype from among adult end-users in Western Kenya. We performed key informant interviews (n = 17) among healthcare providers and 5 focus group discussions (FGDs) among 3 groups of adult stakeholders of premature babies, including: (1) parents/family members of premature babies aged 6 weeks or less (3 FGDs); (2) healthcare providers of newborns (e.g., nurses; physicians; 1 FGD); (3) community opinion leaders and stakeholders (e.g., traditional birth attendants; pastors; village elders; 1 FGD). Content and thematic analyses of transcripts indicate that NeoWarm is acceptable and feasible in promoting facility-based kangaroo mother care in the Kenyan setting. Novel findings derived from respondents include (1) the ability of the device to potentially overcome several barriers to traditional kangaroo mother care methods and (2) user-driven encouragement to expand the use case of the device to potentially include community-based kangaroo mother care and neonatal transport. User design feedback obtained during the interviews informed several key design iterations for subsequent prototypes of the device.

The #Enzian classification for ultrasound diagnosis of endometriosis: description and explanation of the classification using our own clinical cases

An extensive deep infiltrating endometriosis usually requires complex and time-consuming surgical treatment, often with multidisciplinary surgery teams forming. The goal of ultrasound is to find and describe in detail, if possible, all endometriosis lesions at the preoperative stage. A structured classification is needed for a detailed description of all foci of pelvic and extrapelvic endometriosis that would be understandable to both the radiologists and the gynecological surgeon. The current version of the #Enzian classification was designed by an international team of scientists from 11 countries with vast experience in the diagnosis and treatment of endometriosis. This classification is based on a description of the location of endometriosis lesions, the depth of invasion into the pelvic organs and tissues, as well as the presence of invasion into adjacent abdominal organs and disruption of their functions. The classification provides continuity between preoperative imaging and surgical assessment of the severity of endometriosis. In this article, we tried to describe and comment in detail on the ultrasound version of this classification, illustrated by our own clinical cases. Using the #Enzian classification provides clinicians with a common “language” to describe endometriosis in a comprehensive and easily reproducible manner. The authors of this article have been using the #Enzian classification in their work for more than 2 years. Based on our existing experience, we consider the #Enzian classification not only convenient and useful, but also meets all the requirements of both a diagnostician and a surgeon.

Managerial challenges in the conditions of digitalisation. A review of the book “Management for digital transformation” (Springer, 2024)

The active processes of digitalisation of socio-economic systems require modifications of managerial activities which should be facilitated by relevant scientific developments. This article is a review of the monograph devoted to peculiarities of management in the conditions of digital transformation and written by the international team of specialists. The work includes 9 main chapters which address very different aspects of the problem and particularly control during digitalisation, technology transfer, talent management, interaction with customers, marketing tools, e-commerce, functioning of virtual teams, inter-organisational collaboration, management of digital waste. Generally, it is found that despite the fact that digitalisation opens new perspectives for managerial activities, it also forms significant difficulties, and its effects are ambivalent or can often be negative. The considered research field seems to be rather poorly studied, and the monograph successfully poses new questions for the subsequent investigations. The reviewed work demonstrates the mosaic composition; nonetheless, it is informative and offers an original view of the problem. It contains a lot of interesting conceptual constructs and is rich in factual materials. Also, the edition is distinguished by a substantial number of terms, and the available subject index facilitates better orientation in the latter. The style is purely academic, but the potential readership seems to be wide. Despite a few minor deficiencies, the monograph, undoubtedly, will be interesting to Russian specialists.

Nursing roles in disaster zones: Experiences and lessons from Turkey's 2023 earthquakes.

Disasters affect human health and well-being globally. Nursing plays a vital role in disaster preparedness and response, ensuring efficient early care coordination and delivering effective field treatment. This study investigates the challenges an Israeli humanitarian delegation encountered during their response to major earthquakes in Turkey in 2023. It explicitly focuses on difficulties in preparation, operations, and collaboration with local teams. The study further analyzes the findings and extracts valuable lessons from the mission. Using a qualitative descriptive design, 22 out of 32 nurses involved in delegation participated in three focus group discussions within two months of returning to Israel. The discussions were recorded, transcribed verbatim, and analyzed thematically. The study followed the COREQ guidelines, ensuring comprehensive reporting and methodological rigor in qualitative research. The study's main findings spanned predeparture preparation, mission challenges in the disaster zone, and postmission lessons, each highlighted by subthemes and participant quotations. A strong sense of mission was evident among the participants, along with frustration at inefficient time management prior to deployment. Many participants noted additional challenges, related to the difficulty of working in multiple languages and across cultures, and the opportunities for resolution. Finally, participants called for better psychological support following the mission. Nurses in disaster zones offer valuable insights to enhance preparation, cross-cultural communication, and postmission implementation. Nurse managers and healthcare policymakers can utilize this study's findings to develop future nursing training programs in disaster-related skills. Additionally, it can help foster collaboration among international healthcare teams.

Comment on: Laparoscopic access in the treatment of palpable cryptorchidism (review)

In issue #1 of volume 28 (2024) of the "Russian Journal of Pediatric Surgery" journal, an international team of authors published the scientific review "Laparoscopic access in the treatment of palpable cryptorchidism (review)" (DOI: https://doi.org/10.17816/ps667). The article analyzed international publications with the results of using laparoscopic access for orchiopexy of palpable testicles. Unfortunately, in all the articles mentioned in the review the follow-up period was 12–18 months. In addition, there is no assessment of gonads state before descending in the analysed articles. It seems that a mechanistic approach was chosen: they used laparoscopic access, mobilised the testis, performed orchiopexy, made sure that there was no atrophy or retention, and assessed the result as positive. This is not sufficient to analyse and objectively evaluate the proposed method in a statistically valid manner. When choosing an orchiopexy method, it is necessary to be guided by reliable long-term results and try not to contrast traditional orchiopexy methods with laparoscopic methods.
