Survey on Physical Activity Among Adolescents in Pune City

The main aim of the study was to assess the physical activity among adolescents of Pune City. For this study total 500 students were randomly selected from Pune city. The subject’s age was ranged in between 13-17 years. For the purpose of collecting the data the researcher used and selected International Physical Activity Questionnaire (IPAQ) is a widely used instrument for assessing physical activity levels in various populations by (Craig et al., 2003). The questionnaire is divided into four domains. These are Leisure-Time Physical Activity: Activities during discretionary time, such as sports, exercise, and recreational activities. Occupational Physical Activity: Activities related to work or employment. Transportation Physical Activity: Activities related to commuting, such as walking or cycling. Household and Garden Physical Activity: Activities related to household chores and gardening. Data were measured by IPAQ questionnaire which is used to calculate the total metabolic equivalent of task (MET) minutes per week. MET is a unit that represents the energy expenditure of an activity relative to resting metabolic rate. MET values are assigned to different levels of physical activity intensity (e.g., walking, moderate-intensity activities, vigorous-intensity activities), and the total MET minutes per week are calculated by multiplying the duration and frequency of each activity. Percentage was used as a statistical tool analyses the data. The result of the study revealed that students 57.6% (288) are less involved in physical activity, where as 23.4% (117) students are moderately involved in physical activity and 19% (95) students are highly involved in physical activity.

Level of physical activity of a male prison population measured with the IPAQ questionnaire.

To describe the level of physical activity of a male prison population measured with the International Physical Activity Questionnaire (IPAQ), correlating years of sentence and age range. Non-experimental cross-sectional and descriptive study. The data was collected through the short version IPAQ questionnaire. The data was analyzed through descriptive statistics such as mean and standard deviation, and an inferential analysis was carried out, with a P ≥ 0.05. The questionnaire was applied to a sample of 100 male prisoners from the city of Talca, Chile. They were grouped firstly into years of sentence and then by age range, obtaining a result from the evaluated population that shows moderate level of physical activity despite the context of confinement, in which walking is the activity that generates more MET's among the sample. There are no significant differences between the evaluated groups. The evaluated population shows a lower level of physical activity than a similar group in other countries, although they show better levels of physical activity when they are compared to the Chilean general public. Walking, which prisoners do every day in the modules and shared spaces in prison, is found to be a protective factor against chronic non-communicable diseases.

Abstract P5-08-12: Lifestyle factors and patient-reported outcome measures using the European Organisation for Research and Treatment of Cancer survivorship questionnaire in early breast cancer patients attending for surveillance imaging

Abstract Background An increasing number of patients survive their early breast cancer (EBC) but may experience longer term impact on their quality of life (QoL) arising from their diagnosis and treatment. Survivors are also at risk of other possibly life-shortening chronic diseases due to concomitant risk factors (smoking, drinking >14 units of alcohol/week, and low exercise levels). Little is known about the lifestyle factors and long-term challenges facing EBC survivors. We report the initial results of a questionnaire (containing patient-reported outcomes measures (PROMS), and lifestyle factor questions) completed by EBC survivors at the Royal Marsden hospital (RMH), UK. Methods We prospectively identified a patient cohort of patients (October 2021 to June 2022) with stage I-III EBC who had completed their curative treatment (endocrine treatment (ET) may be ongoing) and who were attending for their annual surveillance breast imaging at years 1, 2, 4, and >5 years. Patients completed PROMS (using EORTC (European Organisation for Research and Treatment of Cancer) QLQ-SURV100 and -BR23) and questionnaires regarding smoking, alcohol, and exercise levels (via the International Physical Activity Questionnaire (IPAQ)). Patient, clinical, disease and treatment characteristics were recorded and the EORTC and IPAQ questionnaires were analysed according to the EORTC and IPAQ analysis manuals. Results Data are available for 187 women at the time of this analysis (it is anticipated data will be available for 350 at the time of SABCS 2022). Patients were attending for their year 1 (N=66, 35.3%), 2 (N=43, 23%), 4 (N=43), or >5 years (N=35, 18.7%) imaging post-surgery. 62 (86.6%) patients were Eastern Cooperative Oncology Group Performance Status 0-1, and 31 (16.6%) had >2 of other comorbidities. 123 patients (65.8%) were post-, 16 (8.6%) peri-, and 48 (25.7%) premenopausal. 178 (95.2%) underwent primary surgery. Of those patients undergoing surgery, 19 (10.2%) had a mastectomy, and 168 (89.8%) breast conserving surgery. All patients had axillary surgery: 26 (13.9%) axillary dissection, and 158 (84.5%) sentinel lymph node biopsy. 166 patients (88.8%) received adjuvant ET and for most patients (N= 149, 79.7%) this was ongoing at time of analysis. 31.6% (N= 59) had prior exposure to cardiotoxic treatment. Risk groups for other chronic health conditions could be identified: 92 (49.2%) patients had a BMI of >25, 5 (4.2%) were current smokers, and 16 (13.6%) were classified as at-risk drinking. Patients reported that their physical function was affected following their diagnosis and treatment, with 51 (27.3%) having trouble running short distances and 52 (27.8%) taking a long walk carrying a heavy backpack. Nonetheless all patients (N=118 available for IPAQ analysis) had moderate (N=61, 51.7%), or high-level (N= 58, 49.2%) levels of exercise. 85 patients (45.5%) had pain in the past week, and 101 patients (54%) were dissatisfied with the appearance of their body. Most patients (N=152, 81.3%) estimated their overall QoL in the last week as very good (5-7) (scale 1-7, with 7 being excellent). QLQ-BR23 symptom scales were compared between patient groups. Patients who had chemotherapy suffered ongoing from hair loss (52.6% vs 45.2%) than chemo-naïve patients. There were no major differences between groups who received ET and those ET naïve. Conclusions We were able to identify groups of patients at risk for other comorbidities and cancers. We could also highlight groups of EBC survivors with a higher level of QoL concerns. PROMS should be routinely assessed in EBC survivors to better address subgroups at higher risk, inform consultations, and treatment decisions. Physiotherapy and Physical Activity: A Cross-Sectional Survey Exploring Physical Activity Promotion and the Physical Activity among Physiotherapists in Bangalore

Introduction: Physical action is a significant viewpoint in our everyday life. In India, around 36.9% of the population is inactive physically. Physical action has a significant medical advantage related to increasingly ideal natural cardiovascular disease hazards considering youngsters and teenagers’ profiles. Healthy habits are progressively significant in physiotherapists as they are future advisors who dismiss embracing a healthy way of life and are bound to neglect to set up well-being advancement and open doors for their patients. Aim of the Study: The study's objective is to identify the physical activity levels among the Physiotherapist in Bangalore. Material and Method: This study involves 100 Physiotherapists between the ages of 22-50 years of male and female genders in Bangalore. A self-reported physical activity assessment was taken using the IPAQ (International Physical Activity Questionnaire) based on inclusion and exclusion criteria. Outcome Measure: Participants self-reported physical activity using the IPAQ Questionnaire, which comprises 4 generic items (Occupation, leisure time, domestic, transport). Analysis: Interpreting the data using the SPSS V. 20 software. Conclusion: This study shows that 50% of physiotherapists are physically active and 36% are moderate, and 14% are low in physical activity in Bangalore.

Psychological Effects of Motivational Aquatic Resistance Interval Training and Nutritional Education in Older Women.

Several studies have demonstrated the positive effects of physical exercise programs on physical and mental health throughout life. It is necessary to know the factors that contribute to the well-being of older adults in order to achieve healthy aging. The aim of this study was to evaluate the relationship between well-being perception and the use of autonomy supportive coaching behaviours across a motivational aquatic resistance interval training program. Thirty-four women over 65 years of age from the province of Alicante, Spain, participated, and were randomly assigned to: motivational aquatic resistance interval training group (MART; age: 69.6 ± 5.01 years, height: 1.62 ± 7.88 m, weight: 75.3 ± 12.8 kg) and control group (CG; age: 67.7 ± 3.60 years, height: 1.54 ± 5.47 m, weight: 66.9 ± 10.2 kg). The MART program was conducted for 14 weeks, with three training sessions/week. The CG did not perform any physical activity during the study. Perception of autonomy support was assessed through the Autonomy-Supportive Coaching Questionnaire (ASCQ), Psychological needs by the Basic Psychological Needs in Exercise Scale (BPNES), Intrinsic motivation to exercise was assessed through Intrinsic Motivation Inventory (IMI) and Perception of Physical Activity by the International physical activity questionnaire (IPAQ). In MART, compared to CG, significant differences were observed in BPNS, IMI and IPAQ questionnaires used, except in the ASCQ. The differences were significant in all three cases in BPNS (p < 0.05 in autonomy and competence and p = 0.001 in relationship with others), obtaining better scores after intervention than previously. As for the IMI scale, significant differences were also obtained in both subscales. The level of physical activity also improved significantly, with higher scores on the IPAQ after the intervention than before (p < 0.001). In conclusion, when practitioners perceive greater well-being, satisfaction of their basic psychological needs, greater self-selection, volition, and autonomy instead of pressure, demand and control, the result is better behaviour with greater psychological well-being, adherence and consequent health benefits.

MO594POTENTIAL BENEFICIAL EFFECT OF EXTRA VIRGIN OLIVE OIL WITH HIGH MINOR POLAR COMPOUNDS CONTENTS IN NEPHROPATHIC PATIENTS: PRELIMINARY DATA

Abstract Background and Aims Chronic degenerative non-communicable diseases represent one of the main causes of death in the world. Among these, chronic kidney disease (CKD) has an estimated prevalence of 7-12% worldwide. CKD is related to numerous comorbidities, especially these cardiovascular. Furthermore, its progression is characterized by a low-grade inflammatory status and oxidative stress, factors that negatively influence quality of life. In recent years, numerous in vitro and in vivo studies have analysed the beneficial effects of natural bioactive compounds on health. Extra virgin olive oil (EVOO) is rich in minor polar compounds (MPCs), capable of exerting a cardioprotective, anti-inflammatory and antioxidant action. The 98-99% of EVOO total weight is made up by fatty acids, in particular, monounsaturated ones such as oleic acid, and a small percentage 1-2% by MPCs. Among MPCs, hydroxytyrosol, tyrosol, oleacin and oleocanthal are of particular importance for their heathy effects. In our study, we analysed the potential healthy effects of EVOO MPCs in CKD patients. Method The EVOO selected for the study (Ophenoil), was previously characterized for its high content of MPCs through quantitative and qualitative analysis performed using HPLC-DAD-MS and Oxitester. We enrolled 20 CKD patients undergoing conservative therapy (stages I-IV according to K-DIGO guidelines). All enrolled patients have been instructed to assume 40 ml per day of EVOO, for 9 weeks. Laboratory parameters have been collected at two different time-points of the study (at baseline T0 and after 9 weeks T1) (Fig 1). At T0 the patients have been undergo to a detailed medical history. All CKD patients have been unstructured to follow a conventional Mediterranean diet (which adherence is assessed by PREDIMED questionnaire), containing a controlled protein intake according to CKD stage, in which the EVOO Ophenoil represent the main source of plant-based fats. In both time-points of the study, all patients have been undergoing to blood sampling to evaluate renal function, lipid profile, inflammatory parameters (C-reactive protein-CRP, erythrocyte sedimentation rate-ESR, Interleukin(IL)-6, Tumor necrosis factor(TNF)-α) and oxidative stress (Free Oxygen Radicals Test-FORT and Free Oxygen Radical Defence-FORD by CR4000). Moreover, we evaluated the cardiovascular risk indices (Carotid intima-Media Thickness (C-ITM) monitored by eco(color)Doppler ultrasound). Each patient has been interviewed with Quality of Life Assessment Questionnaire (Short-Form 36 items health Survey- SF-36) and International Physical Activity Questionnaire (IPAQ). Results Preliminary data highlight a reduction of the inflammatory state, monitored by CRP (p=0.0299), ESR (p=0.0063), TNF-α (p=0.0001), IL-6 (p=0.0219), an improvement of renal function (azotemia, p=0.0256) and of lipid profile (triglycerides, p=0.0476) after 9 weeks of EVOO assumption (Fig 2). Moreover, we observed a significant improvement of the C-ITM (p=0.0146) and a significant reduction of oxidative stress monitored by FORT (p=0.0144). The results of the PREDIMED and IPAQ questionnaires showed no significant differences. This data indicates that patients have not changed their lifestyle and the results obtained are attributable to EVOO assumption. At the end of the study, there was an improvement of quality of life, in the domains of emotional well-being (p=0.0340) and general health (p=0.0405) of SF-36 questionnaire (Fig 3). Conclusion The consumption of EVOO with high MPCs content would seem to exert an anti-inflammatory (due to high content of oleocanthal) and antioxidant (due to high content of hydroxytyrosol, oleacin and oleuropein aglicone) action in CKD patients. Therefore, once again it is highlighted how the diet therapy plays a pivotal role in the clinical management of CKD patients and that it allows an improvement of their quality of life.

Adherence to the Mediterranean diet, level of physical activity and body dissatisfaction in subjects 16–50 years old in the Region of Murcia, Spain

ObjectiveTo analyze obesity, level of adherence to the Mediterranean diet, physical activity and body dissatisfaction in people aged 16–50 years in the Region of Murcia, Spain. MethodDescriptive and cross-sectional study. A total of 85 people (58 men) aged 16–50 years (28.68 ± 8.846) from 3 sports centers with similar sociodemographic characteristics participated in the study. Anthropometric values were recorded. They were tested for adherence to the Mediterranean diet, International Physical Activity Questionnaire (IPAQ) and test for dissatisfaction with their body (BSQ) and it was statistically related. ResultsIt can be seen that the level of physical activity of both men and women is low, according to the IPAQ questionnaire, the men presented a METs/min per week of 3687.9 (DT = 3137.2) significantly higher than women’s.In body dissatisfaction we see that women’s concern with their bodies (59.3%) is greater than men’s (31%), with a statistically significant difference (p = 0.013). Eating habits and BMI do not have statistically significant differences according to sex.When applying the Mediterranean diet adherence test, it is observed that 95.3% of those with weight overload have a medium or low score, and only 4.7% have a discharge score (p = 0.128).Those with weight overload have a score of 2935.73 MET/min per week (DT = 2633.16) and those without weight overload have a value of 3826.73 (DT = 3164.93, p = 0.268). There are significant differences regarding the values of body dissatisfaction. The level of concern of those with weight overload is 53.5%, compared to those without weight overload who have a level of concern of 26.2% (p = 0.014). ConclusionsWeight overload is associated with body dissatisfaction and a low score in the test of adherence to the Mediterranean diet in people aged 16–50 years. The concern about body image in women was 59.3%.

Riscos ergonômicos e biomecânicos ocupacionais no transporte de pacientes no centro cirúrgico: pesquisa qualiquantitativa de estudo transversal

OBJETIVO: Investigar os riscos ergonômicos e biomecânicos ocupacionais em profissionais da enfermagem no transporte de pacientes, no centro cirúrgico de um hospital público. METODOLOGIA: Estudo de caráter exploratório, observacional e descritivo. Para tanto, utilizou-se o Questionário Internacional da Atividade Física (IPAQ), que analisa diferentes contextos do cotidiano para estimar o tempo semanal gasto em caminhadas, atividades físicas de intensidade moderada, vigorosa e atividades passivas (tempo sentado). O Questionário Nórdico, possibilita a identificação de distúrbios osteomusculares nos 12 meses e 7 dias anteriores à entrevista. Permite também um diagnóstico do posto de trabalho pela sua relação com a prevalência do local e tempo do surgimento dos sintomas dolorosos. Além disso, aplicou-se a metodologia da Análise Ergonômica do Trabalho (AET). RESULTADOS: De acordo com a análise dos dados coletados participaram deste estudo 44 profissionais da enfermagem, 32 (72,7%) eram do gênero feminino e 13 (29,54%) do gênero masculino. Quando analisado a especialidade: 24 (54,5 %) eram auxiliares de enfermagem, técnicos de enfermagem correspondem à 14 (31,8 %) e Enfermeiros 6 (13,6 %). O questionário IPAQ demonstrou que 16 (35%), destes profissionais mantém-se ativos. Por meio da análise ergonômica do trabalho, as variáveis como, tipo de maca, condições de manutenção, tipo de anestesia, idade do paciente, entre outros, interferem em maiores ou menores esforços no transporte de pacientes. CONCLUSÃO: Desse modo, tornam-se necessárias a adoção de medidas preventivas que visem a melhoria do estado de saúde prevenindo complicações musculoesqueléticas no desempenho das atividades. Além de promover boas condições de trabalho aos profissionais no seu ambiente profissional, bem como trazer benefícios ao hospital.

Evaluation of changes in physical activity among participants in a company-based program

To analyze the effectiveness of a company-based physical activity program using the International Physical Activity Questionnaire (IPAQ) questionnaire. In March 2014 we implemented a company-based physical activity program (Mueve-T, by its Spanish acronym) with the objective of increasing awareness of physical activity among employees, and measuring improvements in activity level. To evaluate the effectiveness of the program, we administered the IPAQ Questionnaire, before the program began in 2014 and two years later, in 2016. Descriptive summary statistics included frequencies, percentages and bar charts. To compare pre- and post-intervention results, we used a logistic repeated measures model, adjusted for two populations, with an ordinal polytomous response for IPAQ (low or inactive, moderate and high), applied to the overall study population and further examined by sex, workplace and job. A total of 746 participants responded in 2014 and 563 in 2016. Only 314 respondents completed both surveys. In 2014, 35.4% of the respondents reported a low level of activity, moderate was 39.5% and highwas, 25.2%. In 2016, 36.3% reported a low level of activity, moderate was 39.8% and a high level was indicated by 23.9%. Overall, there were no significant differences in physical activity levels between the pre- and post-intervention period, among those who participated in both surveys. Likewise, there were no significant differences when this result was examined by sex, although employees sales or commercial tasks has lower levels of physical activity as compared to other centers or jobs. The results did not find this intervention program to be effective in increasing levels of physical activity. The differences found among the sales and commercial task employees could have been due to differences in the effect on awareness achieved by companywide information dissemination (e.g., newsletters, talks, intranet notes, etc.) as compared to specific individual activities (e.g., (yoga classes, zumba, cycling, walks, etc.). A longer evaluation period is needed to confirm these findings.

Mobile health, physical activity, and obesity: Subanalysis of a randomized controlled trial.

Background:Recent studies on physical activity were analyzed by randomizing participants into either the intervention or control group. It is necessary to classify each intervention and control groups according to physical activity using the International Physical Activity Questionnaire (IPAQ).Methods:This was a pilot project for SmartCare Services. The intervention group received obesity management services using a smartphone for 24 weeks, while the control group did not receive the interventions. Six anthropometric indices were analyzed: weight, body mass index (BMI), waist circumference, body fat, systolic blood pressure (SBP), and diastolic blood pressure (DBP). Five laboratory tests, including fasting blood sugar (FBS), glycosylated hemoglobin (HbA1c), high-density lipoprotein cholesterol (HDL-C), total cholesterol (TC), and triglycerides (TGs), were also assessed. The final 324 participants were categorized using the IPAQ questionnaire, and anthropometric indices and laboratory tests were analyzed for within-group and between-group changes from baseline to final visit.Results:Statistically significant decreases in the intervention group compared with the control group were observed in terms of insufficient activity (IA) (−1.6 ± 3.03 vs −0.1 ± 1.94 kg) and moderate activity (MA) (−2.5 ± 3.81 vs −0.3 ± 2.24 kg) for weight, IA (−0.7 ± 1.14 vs −0.2 ± 0.93 kg/m2) and MA (−0.9 ± 1.30 vs −0.2 ± 0.86 kg/m2) for BMI, and health-enhancing physical activity (HEPA) (−1.6 ± 3.69% vs −0.1 ± 3.15%) for body fat. For HbA1c, HEPA in the intervention group showed significant decreases (−0.2 ± 0.67 vs 0.0 ± 0.34 mg/dL) compared with the control group.Conclusion:Anthropometric indices and laboratory test results were improved in the smartphone-based intervention group. Especially, improvement of metabolic components in the group with more active physical activity was remarkable.

The impact of physical activity measured by the International Physical Activity questionnaire on the prevalence of stress urinary incontinence in young women

ObjectiveThe aim of this study was to determine the prevalence of SUI symptoms in sportswomen (with a high intensity of physical activity) and non-sportswomen (with a low intensity of physical activity), according to the estimated intensity of physical activity in metabolic equivalents using the IPAQ questionnaire. Another goal was to identify relationships between SUI symptoms, intensity of physical activity, and quality of life. Study designA total of 1005 participants were enrolled into the study. We used the International Consultation on Incontinence Questionnaire (ICIQ-UI SF), the Overactive Bladder Questionnaire (OAB-q), the Urinary Incontinence Quality of Life scale (I-QoL) and the International Physical Activity Questionnaire (IPAQ). ResultsOut of the 1005 participants, the final sample of 557 women is the result of the adoption of exclusion criteria. The sample consisted of 557 women (270 sportswomen and 287 non-sportswomen) with an average age of 20.9 ± 2.8 years. The ICIQ-UI SF confirmed slight urinary leakage in 33 (6.14%) sportswomen and 11 (2.04%) non-sportswomen. The risk of reporting SUI was higher in the sportswomen group (odds ratio: 3.49; 95% CI: 1.727–7.064, p < 0.001). Significant positive correlation was observed between SUI (assessed by ICIQ-UI SF) and high intensity physical activity (in metabolic equivalents (r = 0.242, p < 0.01). Significant negative correlation was observed between SUI and quality of life (I-QoL) (r = −0.648, p < 0.001). OAB symptoms were not present in the monitored groups. (OAB – q – SS in sportwomen were 4.3 ± 5.4, in non-sportwomen 4.5 ± 4.9, p = 0.265). ConclusionsSportswomen with high-intensity physical activities in metabolic equivalents measured by the IPAQ have a greater chance of reporting SUI than non-sportswomen, resulting in a negative impact on quality of life.

Physical Activity Questionnaires Do Not Accurately Estimate Fitness in Older Women.

The objective was to compare the performance of the International Physical Activity Questionnaire (IPAQ) and Baecke questionnaire to estimate maximal oxygen uptake (VO2max) in healthy older women. One hundred healthy women aged 60years and older answered the IPAQ and Baecke questionnaires and underwent a cardiopulmonary exercise test. The 6-min walk (6MWT), timed up and go (TUG), and handgrip strength test (HST) were also performed. Mean age and body mass index were 68.5 ± 6.3years and 27 ± 4.7kg/m2, respectively. No significant correlation was seen between VO2max, IPAQ and Baecke questionnaires. IPAQ did not correlate with any functional tests while Baecke correlated weakly with the 6MWT and TUG test. VO2max correlated significantly with the 6MWT (r = .38; p = .0001), HST (r = .34; p = .001), and TUG (r = -.41; p = .0001). In a multivariate regression model, TUG was the best estimator for VO2max (R2 = .217; p = .0001). In conclusion, IPAQ and Baecke questionnaires do not associate significantly with VO2max in older women.

Use of the IPAQ questionnaire in the form of a mobile application in monitoring physical activity of patients with cardiovascular diseases.

Systematic, dynamic physical effort brings about tremendous health benefits, not only in young individuals, but as a form of primary prevention, and primarily as secondary prevention of cardiovascular system diseases. The research project was conducted in the Rehabilitation Centre at the Witold Chodźko Institute of Rural Health (IMW) in Lublin, Poland, and covered a group of 927 ambulatory patients. From among the study group, two subgroups were distinguished (Groups 1 and 2). Selection of patients was targeted, based on the criterion of the existing diagnosis of cardiovascular diseases (Group 1; N=53), and the lack of these diseases (Group 2; N=53). The daily level of physical activity was assessed using the monitoring application (Instrument for Activity Measurement IMW) - using a short version of the International Physical Activity Questionnaire (IPAQ), and alternatively analyzing the data from a smartphone accelerometer. The total level of weekly physical activity in Group 1 was 4532.88±3611.6 MET-min/week, and was considerably higher, compared to the control group (3142.58±2536.9 MET-min/week). Also, the values of weekly caloric cost of physical activity (KAF kcalxweek -1 ) showed higher values (6935.77±5957.4), compared to the control group (4274±3344.9). The group of cardiologic patients (Group 1) showed a higher level of activity, especially within the range of effort on an intensive level, and low level - going for walks, walking(MET1, MET3), which was also translated into the caloric cost of these levels of physical activity (KAF1, KAF3). The results of the study indicated a significantly higher level of physical activity in the group of patients with concomitant cardiovascular diseases, compared to the control group. The overestimation of the level of physical activity perceived by patients seems to be an important problem, especially in the control group.

201. PHYSICAL ACTIVITY LEVELS IN FEMALE RHEUMATOID ARTHRITIS PATIENTS ON LONG TERM ANTI-TUMOUR NECROSIS FACTOR THERAPY COMPARED TO PATIENTS WITH ACTIVE RHEUMATOID DISEASE AND HEALTHY CONTROLS

Background: Anti-TNF therapy has revolutionised the management of rheumatoid arthritis (RA) with rapid and sustained improvements in pain, function and quality of life. However, we do not know how this impacts upon habitual daily physical activity and whether treated patients attain activity levels seen in healthy controls. This study aimed to compare the physical activity levels of patients whose RA was well controlled on long-term anti-TNF therapy to RA patients with active arthritis and non-RA controls. Methods: Participants were patients on anti-TNF for more than two years (tRA) with DAS 3.2 (aRA) and healthy controls (C), matched for age and BMI. Physical activity was assessed using the Actigraph GT3x+ accelerometer, worn throughout waking hours for seven days to determine time spent in light activity, moderate to vigorous physical activity (MVPA) and sedentary time. The International Physical Activity Questionnaire (IPAQ) was also completed. Groups were compared using analysis of variance with Bonferroni post hoc tests; Kruskal-Wallis or Mann-Whitney U- test as appropriate. Results: RA disease duration was significantly greater in tRA than aRA. Groups did not differ significantly in age, height, weight or body mass index (Table). Daily step count was significantly lower in aRA than tRA and C. Sedentary time (as a proportion of wear time) was significantly greater in aRA than tRA, whilst the reverse was true for light activity time. MVPA time was significantly lower in both RA groups than in controls. IPAQ questionnaires demonstrated significant differences between groups, with substantially higher values in C than RA groups in total METs and MET-minutes per week in domestic and garden, leisure, walking activities as well as total moderate and vigorous activities. RA patients had lower moderate to vigorous activity time than controls, regardless of treatment. aRA had lower light activity time, and more sedentary time, than tRA Conclusion: Moderate to vigorous physical activity should be promoted in all RA patients as even those with well controlled disease exhibit a deficit in comparison to controls

A Comparison of Self-Reported and Objective Physical Activity Measures in Young Australian Women.

BackgroundThe evidence for beneficial effects of recommended levels of physical activity is overwhelming. However, 70% of Australians fail to meet these levels. In particular, physical activity participation by women falls sharply between ages 16 to 25 years. Further information about physical activity measures in young women is needed. Self-administered questionnaires are often used to measure physical activity given their ease of application, but known limitations, including recall bias, compromise the accuracy of data. Alternatives such as objective measures are commonly used to overcome this problem, but are more costly and time consuming.ObjectiveTo compare the output between the Modified Active Australia Survey (MAAS), the International Physical Activity Questionnaire (IPAQ), and an objective physical activity measure—the SenseWear Armband (SWA)—to evaluate the test-retest reliability of the MAAS and to determine the acceptability of the SWA among young women.MethodsYoung women from Victoria, Australia, aged 18 to 25 years who had participated in previous studies via Facebook advertising were recruited. Participants completed the two physical activity questionnaires online, immediately before and after wearing the armband for 7 consecutive days. Data from the SWA was blocked into 10-minute activity times. Follow-up IPAQ, MAAS, and SWA data were analyzed by comparing the total continuous and categorical activity scores, while concurrent validity of IPAQ and MAAS were analyzed by comparing follow-up scores. Test-retest reliability of MAAS was analyzed by comparing MAAS total physical activity scores at baseline and follow-up. Participants provided feedback in the follow-up questionnaire about their experience of wearing the armband to determine acceptability of the SWA. Data analyses included graphical (ie, Bland-Altman plot, scatterplot) and analytical (ie, canonical correlation, kappa statistic) methods to determine agreement between MAAS, IPAQ, and SWA data.ResultsA total of 58 participants returned complete data. Comparisons between the MAAS and IPAQ questionnaires (n=52) showed moderate agreement for both categorical (kappa=.48, P<.001) and continuous data (r=.69, P<.001). Overall, the IPAQ tended to give higher scores. No significant correlation was observed between SWA and IPAQ or MAAS continuous data, for both minute-by-minute and blocked SWA data. The SWA tended to record lower scores than the questionnaires, suggesting participants tended to overreport their amount of physical activity. The test-retest analysis of MAAS showed moderate agreement for continuous outcomes (r=.44, P=.001). However, poor agreement was seen for categorical outcomes. The acceptability of the SWA to participants was high.ConclusionsModerate agreement between the MAAS and IPAQ and moderate reliability of the MAAS indicates that the MAAS may be a suitable alternative to the IPAQ to assess total physical activity in young women, due to its shorter length and consequently lower participant burden. The SWA, and likely other monitoring devices, have the advantage over questionnaires of avoiding overreporting of self-reported physical activity, while being highly acceptable to participants.

The role of health care experience and consumer information efficacy in shaping privacy and security perceptions of medical records: national consumer survey results.

BackgroundProviders’ adoption of electronic health records (EHRs) is increasing and consumers have expressed concerns about the potential effects of EHRs on privacy and security. Yet, we lack a comprehensive understanding regarding factors that affect individuals’ perceptions regarding the privacy and security of their medical information.ObjectiveThe aim of this study was to describe national perceptions regarding the privacy and security of medical records and identify a comprehensive set of factors associated with these perceptions.MethodsUsing a nationally representative 2011-2012 survey, we reported on adults’ perceptions regarding privacy and security of medical records and sharing of health information between providers, and whether adults withheld information from a health care provider due to privacy or security concerns. We used multivariable models to examine the association between these outcomes and sociodemographic characteristics, health and health care experience, information efficacy, and technology-related variables.ResultsApproximately one-quarter of American adults (weighted n=235,217,323; unweighted n=3959) indicated they were very confident (n=989) and approximately half indicated they were somewhat confident (n=1597) in the privacy of their medical records; we found similar results regarding adults’ confidence in the security of medical records (very confident: n=828; somewhat confident: n=1742). In all, 12.33% (520/3904) withheld information from a health care provider and 59.06% (2100/3459) expressed concerns about the security of both faxed and electronic health information. Adjusting for other characteristics, adults who reported higher quality of care had significantly greater confidence in the privacy and security of their medical records and were less likely to withhold information from their health care provider due to privacy or security concerns. Adults with higher information efficacy had significantly greater confidence in the privacy and security of medical records and less concern about sharing of health information by both fax and electronic means. Individuals’ perceptions of whether their providers use an EHR was not associated with any privacy or security outcomes.ConclusionsAlthough most adults are confident in the privacy and security of their medical records, many express concerns regarding sharing of information between providers; a minority report withholding information from their providers due to privacy and security concerns. Whether individuals thought their provider was using an EHR was not associated with negative privacy/security perceptions or withholding, suggesting the transition to EHRs is not associated with negative perceptions regarding the privacy and security of medical information. However, monitoring to see how this evolves will be important. Given that positive health care experiences and higher information efficacy were associated with more favorable perceptions of privacy and security, efforts should continue to encourage providers to secure medical records, provide patients with a “meaningful choice” in how their data are shared, and enable individuals to access information they need to manage their care.

Health Promotion in Older Chinese

Aging, in conjunction with decreasing physical activity, is associated with a range of health problems. Simple, low-maintenance, population-based means of promoting activity to counteract the age-associated decline are required. We therefore assessed the effect of pedometry and buddy support to increase physical activity. We undertook a clustered randomized trial (HKCTR-346) of 24 community centers involving 399 older Chinese participants (≥ 60 yr). Centers were randomly allocated to 1) pedometry and buddy, 2) pedometry and no buddy, 3) no pedometry and buddy, and 4) no pedometry and no buddy with a 2 × 2 factorial design. The trial simultaneously tested the individual and combined effects of the interventions. The intervention groups also received monthly organized group activities to provide encouragement and support. Outcome measures were assessed at 6 and 12 months, including physical fitness and activity and cardiovascular disease risk factors (anthropometry and blood pressure). From the 24 centers, 356 volunteers (89.2%) completed the study. Those receiving the interventions had higher mean physical activity levels at 12 months of 1820 (95% confidence interval (CI) = 1360-2290) and 1260 (95% CI = 780-1740) MET·min·wk(-1), respectively relative to the decrease in the control groups. The buddy peer support intervention significantly improved mean aerobic fitness (12% [95% CI = 4%-21%]) and reduced both body fat (-0.6% [95% CI = -1.1% to 0.0%]) and time to complete the 2.5-m get-up-and-go test (-0.27 [95% CI = -0.53 to -0.01] s). No other improvements in the cardiovascular disease risk factors were observed. The combination of motivational tools was no better than the individual interventions. Both motivational interventions increased physical activity levels, and the buddy style improved fitness. These tools could be useful adjuncts in the prevention of obesity and age-related complications.

Abstract 381: Favorable Effects of Habitual Physical Activity on Vascular Function: Ikaria Study

Background: Physical activity (PA) has substantial vascular and cardiac health benefits and can ameliorate cardiac risk. Measurement of endothelial function is well validated in large population studies as strong predictor of adverse cardiovascular outcomes. In the present study we evaluated the effect of habitual physical activity on vascular function in residents of Ikaria Island. The inhabitants of this island show increased longevity and high prevalence of healthy aging. Methods: The study was conducted on a subgroup population of IKARIA study consisted of 327 subjects (155 men) aged 40-91 years, permanent inhabitants of Ikaria Island. Endothelial function was evaluated by ultrasound measurement of flow-mediated-dilatation (FMD). We evaluated PA using the shortened version of the self-reported International Physical Activity Questionnaire (IPAQ). Overall the study sample was divided in three groups according to the categorical score achieved in IPAQ questionnaire: low PA (n=75), moderate PA (n=200) and vigorous PA (n=48). Subjects in the low PA group were recorded as physical inactive and the rest as physical active. Results: Mean FMD (5.79±3.19%) was inversely associated with age (r=-0.242, p&lt;0.001). Inactive subjects had significantly lower FMD compared to active subjects (4.95±3.02% vs. 6.06±3.23%, p=0.008). Multiple linear regression analysis after adjustment for several known confounders (age, diabetes mellitus, hypertension, smoking, hypercholesterolemia, cardiovascular disease, body mass index) revealed significant differences in FMD values between moderate vs. low PA group [b=1.24, 95%CI: (0.28, 2.20), p=0.011] whereas a trend was evident between vigorous and low PA group [b=1.11, 95%CI: (-0.16, 2.38), p=0.089]. Moreover subjects walking for more than 10 minutes less than two times weakly had significantly lower FMD compared with subjects walking three to five days weakly or almost every day (5.13±2.82% vs. 6.95±3.84% vs. 6.00±3.23%, p=0.005). Conclusions: The present study revealed another cardioprotective mechanism of habitual PA through beneficial modification of endothelium function.

Background The World Health Report 2002 estimated that sedentary is one of the main 10 causes of morbidity and mortality and the proportion of people whose health is at risk due to sedentary life is approximately 60%. Objective To assess the relationship of the physical activity level with cardiovascular risk factors and metabolic syndrome in 61 healthy men. Methods The short version of the International Physical Activity Questionnaire (IPAQ) recommended by the World Health Organization was used as a valid measure to estimate two categories of physical activity, such as: low level (insufficient and sedentary) and vigorous level (moderate and very active). Cardiovascular risk factors and metabolic syndrome were defined according to criteria of the National Cholesterol Education Program of the United States and International Diabetes Federation respectively. Serum levels of C-reactive protein and ferritin were also measured, and insulin sensitivity was estimated by the Homeostatic Model Assessment-Insulin Resistance (HOMA-IR). Results Mean age of study population was 47.1 ± 6.9 years. 17 participants (28%) had metabolic syndrome. There were no differences between the categories of low and vigorous level physical activity, or relationship with total physical activity (MET × week). No association was seen between low levels of physical activity and metabolic syndrome criteria. Conclusions The high prevalence of physical inactivity found in study participants using the IPAQ questionnaire was not associated to cardiovascular risk factors and metabolic syndrome.
