Compliance with CPR quality guidelines and survival after 30 days following out-of-hospital cardiac arrest. A retrospective study.

Our study assessed the quality of cardiopulmonary resuscitation (CPR) given by emergency medical services in Southern Ostrobothnia Finland, as is advised in the international guidelines. The goal was to evaluate the current quality of CPR given to patients who suffered an out-of-hospital cardiac arrest and to examine possible measures for improving emergency medical services. A retrospective study was conducted on out-of-hospital cardiac arrest patients in Southern Ostrobothnia, Finland, during a three-year period. Confounding caused by each patient's individual medical history was addressed by calculating Charlson Comorbidity Index (CCI), a score describing individual's risk for death in 10 years. The Utstein analysis and the CPR metrics were acquired from the medical records hospital district in question and analysed in an orderly manner using SPSS. Descriptive statistics are presented as mean (SD) and median [IQR]. We found that of the 349 patients, 144 (41%) received ROSC, 96 (28%) survived to the hospital and 51 (15%) survived for at least 30 days. CPR metrics data were available for 181 patients. CCIs were 3.0 versus 5.0 (p = .157) for the ones who did and those who did not survive at least 30 days. Correspondingly, following metrics were as follows: Mean compression depth was 5.1 (1.3) versus 5.6 (0.8) cm (p = .088), median 28 [18;40] versus 40 [26;54]% of the compressions were in target depth (p = .015) and median compression rate was 113 [109;119] versus 112 [108;120] min-1 (p = .757). The median no-flow fraction was 5.1 [2.8;7.1] versus 3.7 [2.5;5.5] s (p = .073). Ventricular fibrillation (OR 8.74, 95% CI 2.89-26.43, p < .001), public location (OR 3.163, 95% CI 1.03-9.69, p = .044) and compression rate of 100-110/min (OR 7.923, 95% CI 2.11-29.82, p = .002) were related to survival. Patients who suffered out-of-hospital cardiac arrest in Southern Ostrobothnia received CPR that met the international CPR quality target values. The proportion of unintentional pauses during CPR was low and the 30-day survival rate exceeded the international average.

Quality Evaluation Algorithm for Chest Compressions Based on OpenPose Model

Aiming at the problems of the low evaluation efficiency of the existing traditional cardiopulmonary resuscitation (CPR) training mode and the considerable development of machine vision technology, a quality evaluation algorithm for chest compressions (CCs) based on the OpenPose human pose estimation (HPE) model is proposed. Firstly, five evaluation criteria are proposed based on major international CPR guidelines along with our experimental study on elbow straightness. Then, the OpenPose network is applied to obtain the coordinates of the key points of the human skeleton. The algorithm subsequently calculates the geometric angles and displacement of the selected joint key points using the detected coordinates. Finally, it determines whether the compression posture is standard, and it calculates the depth, frequency, position and chest rebound, which are the critical evaluation metrics of CCs. Experimental results show that the average accuracy of network behavior detection reaches 94.85%, and detection speed reaches 25 fps.

Design of a portable external cardiac compression device

Cardiopulmonary resuscitation (CPR) guidelines emphasize that external chest compressions should be started as soon as possible when CPR is performed in patients with cardiac arrest. Moreover, those guidelines stress on fast and hard compressions to make the chest fully rebound and minimize non-pressing time. Current mechanical recovery device has several problems such as displacement of the pressing position, high price, difficult to move, and easy dislocation of piston. Because of the physical loss of high-intensity unarmed CPR, the depth and frequency of external chest compression will decrease with the extension of CPR time, leading to CPR failure. Besides, there are other problems caused by non-professional staff, such as the deviation of compression position, the inaccuracy of compression depth and the unsatisfactory rebound of the chest wall. Based on the above factors, the medical staff from the intensive care unit of the Eighth Medical Center of the Chinese People's Liberation Army General Hospital designed a portable external chest cardiac compressor based on international CPR guidelines which obtained the National Utility Model Patent of China (ZL 2018 2 1173254.3). The portable external chest cardiac compressor is composed of a positioning sucker, elastic body, mounting shell, and pressing components. Rapid and accurate compression positioning, visible compression depth and full chest rebound can be achieved. This device is mobile, easy to operate, and suitable for a broad crowd and various occasions.

COVID-19 and resuscitation: La tournée of traditional Chinese medicine?

BACKGROUNDAs it has been established in previous publications of the author, the current extra-hospital statistics referring to cardiopulmonary resuscitation (CPR) are far from being minimally satisfactory (14%-17% success). Since the appearance of acquired immune deficiency syndrome, its application has been increasingly undermined as other subsequent pandemics (H1N1, Ebola, coronavirus disease 2019) seriously infringing lay rescuers intervention during classical CPR steps (mouth-to-mouth ventilation), forcing to modify vital support protocols. Both KI-1 Yong quan and PC-9 Zhong chong alternative rescue maneuvers could come to aid those victims of impending death situation due to both cardiac arrest or stroke, upgrading current survival rates of said unfortunate patients.AIMTo validate a complementary resuscitation maneuver originated in Chinese Medicine knowledge, carefully integrated into international CPR protocols [World Journal of Critical Care Medicine (WJCCM), August 2013].METHODSThe model to verify its statistical validity of quoted research was the Retrospective Cohort Study, which redeems the “semiotic paradigm” that gave rise to medical semiotics. Its value strives in the differential detail if the deceased patients are considered the control group instead of the patients that may be deceased. Thus, combining the semiotic paradigm with the Retrospective Cohort Study allows us to manage the collateral potential lethal effects of the random process in cases of extreme emergencies.RESULTSThe statistic results provided by the methodological analysis of this work were previously published in WJCCM August 2013, ISSN 2220-3141). In a total of 89 patients in which the Yong quan maneuver was tested, 75 survived and 14 died. In order to compare this data with the percentages of survivors in the other maneuvers, we stipulate the assumption that if 89 patients are the 100% of the sample, how many patients would survive if the survival rate is 6.4% in CPR, 30% in defibrillation and 48% in CPR + defibrillation. By this way we obtained the approximate values of patients that would survive when applying these classical resuscitation maneuvers. Then we obtained the format of the tables to perform the exact Fisher test with the help of a statistical processor; the consequent result in a valuation of P < 0.0001 was considered "extremely statistically significant".CONCLUSIONThe author herein provides a methodological–statistical analysis of such contribution which does not imply any cost at all and could even help prevent the withdrawal of classical CPR practices.

The Impact of Cardiopulmonary Resuscitation Guidelines Updates Against Out-of-Hospital Shockable Cardiac Arrest: A Population-Based Cohort Study

Background: International cardiopulmonary resuscitation (CPR) guidelines based on the best available evidence from resuscitation science have been updated since 2000. However, few nationwide population-based studies have been investigated comprehensive efficacy of those updates for out-of-hospital cardiac arrest (OHCA). Methods: From the All-Japan Utstein Registry between 2005 and 2015, we enrolled 73 578 adults who had witnessed shockable (ventricular fibrillation or pulseless ventricular tachycardia) OHCA. We examined temporal trends over eleven years (2005 on the Guidelines 2000 era, from 2006 to 2010 on the consensus on CPR and emergency cardiovascular care science with treatment recommendations [CoSTR] 2005 era, and from 2011 to 2015 on the CoSTR 2010 era). The primary end point was favorable neurological outcome (Cerebral Performance Category of 1 or 2) at 30 days after OHCA. Findings: Of 73 578 study patients, 61,080 (83%) were witnessed by bystander and 12 498 (17%) by emergency medical services (EMS) responder. After adjustment for Utstein variables, odds ratios for favorable 30-day neurological outcome increased year by year (reference, 2005): the bystander-witnessed group, 1∙19 (95% CI 1∙03-1∙37) in 2006, 2∙40 (95% CI 2∙10-2∙74) in 2010, 2∙45 (95% CI 2∙14-2∙80) in 2011 and 3∙02 (95% CI 2∙63-3∙46) in 2015; the EMS responder-witnessed group, 1∙27 (95% CI 1∙01-1∙60) in 2007, 1∙28 (95% CI 1∙02-1∙60) in 2010, 1∙54 (95% CI 1∙23-1∙94) in 2011 and 1∙58 (95% CI 1∙26-1∙99) in 2015. Similar results were seen in the subgroups receiving EMS-dispatcher CPR instruction, bystander chest compression-only CPR, early defibrillation, biphasic waveform defibrillators or advanced airway management. However, there was no evidence for any benefit from the addition of epinephrine. Interpretation: The international CPR guidelines updates for shockable OHCA had improved neurologically intact survival year by year. However, epinephrine is a topic for the future. Resuscitation Care in Out-Of-Hospital Cardiac Arrest: a Survey of the Emergency Medical Services Personnel in the Republic of Crimea

Introduction. The choice of the ways for optimizing management of out-of-hospital cardiac arrest by emergency medical services (EMS) should be based on the evidence from existing practice of resuscitation care. The study was aimed at evaluating personal experience of cardiopulmonary resuscitation (CPR) performance, timing of last training in resuscitation and level of self-perceived knowledge and skills of CPR among the employees of the EMS of the Republic of Crimea, as well as to assess their beliefs on significance of individual CPR procedures in terms of influence on success of resuscitation and to study their perceptions on factors that may have positive or negative impact on effectiveness of resuscitation care.Material and methods. An anonymous survey of the EMS personnel of the Republic of Crimea was conducted in July–December 2019. The questionnaire was utilized to collect demographic data, information on experience of conducting CPR for the last year, on a current practice of CPR debriefings and timing of last CPR training. The participants assessed their knowledge and skills of CPR and their perception of influence of early implementation of individual CPR procedures on success of resuscitation on a 5-point Likert scale, and indicated factors, that may limit or enhance the effectiveness of resuscitation.Results. A total of 251 specialist of the EMS participated in the survey, including 63 physicians, 129 feldshers and 4 nurses of ambulance teams, 45 dispatchers and 5 senior officers of the EMS stations, which corresponds to 28.8% of physicians, 13.0% of feldsher/nursing staff and 17.4% of dispatcher specialists of the republic. Percentage of male respondents was 35.5%, mean age — 41.5 years, mean duration of EMS work experience — 15.5 years. The mean declared number of cases of CPR conducted in the last 12 months was 2.2 (range: 0–20), and 23.5% specialists of the ambulance teams have not participated in any CPR attempt over the last year. About 60.2% respondents were trained in CPR within the last year. When compared to other specialists of ambulance teams, feldshers who work alone (without a coworker), were significantly less commonly performing a real-life resuscitation (p=0.008) or receiving training in CPR (p=0.020). Dispatchers were least covered with resuscitation training (only 33.3% were trained within the last year) and had the lowest level of CPR knowledge and skills. 25.9% of the specialists had an erroneous judgement that early drug administration and/or tracheal intubation have higher influence on success of resuscitation when compared to chest compressions and/or defibrillation. The level of knowledge and skills of CPR depend on the timing of last training (p&lt;0.001).Conclusions. The experience of participation in real-life attempts of CPR is severely limited for considerable part of the EMS employees, and many specialists have not been trained in resuscitation for a long time. The perceptions of the EMS specialists regarding the importance of individual CPR procedures are not always compliant with the statements of current international CPR guidelines. In order to enhance the effectiveness of resuscitation care in the Republic of Crimea, it is advisable to monitor the coverage of the EMS employees with resuscitation training and the rates of their participation in resuscitation attempts, as well as to perform additional targeted training in CPR, in particular, using alternative methods of education.

Meta-analysis of the success rate of heartbeat recovery in patients with prehospital cardiac arrest in the past 40\u2009years in China

BackgroundSystematic evaluation of the successful heartbeat recovery rate (HRR) in patients during the platinum ten minutes after cardiac arrest.MethodsThe databases of CNKI (January 1979–March 2019), Chongqing VIP (January 1989–March 2019), Wanfang (January 1990–March 2019) and Web of Science (1900-May 2020) were searched. To collect the clinical data of patients with cardiac arrest before hospitalization and analyze the cardiopulmonary resuscitation (CPR) at different times. Literature selection and data extraction were carried out by two researchers independently, and the meta package of R software (version 3. 61) was used for analysis.ResultsA total of 116 papers met the inclusion criteria, including 37,181 patients. Of these patients, 3367 had their heartbeats successfully restored. The results showed a high degree of heterogeneity (χ2 = 6999.21, P < 0.01, I2 = 97.6%). The meta-analysis was conducted using a random-effects model. The combined effect size was 0.199 (0.157–0.250). (1) According to the five CPR groups (International Cardiopulmonary Resuscitation Guide 2000, 2005, 2010, 2015 and other versions), the HRR of other versions [0.264 (0.176–0.375)] was higher than the International Cardiopulmonary Resuscitation 2005 edition [0.121 (0.092–0.158)]. (2) The rescue time was divided into the 0 to ≤5 min group, the 5 to ≤10 min group, the 10 to ≤15 min group, and the > 15 min group. The HRR were 0.417 (0.341–0.496), 0.143 (0.104–0.193), 0.049 (0.034–0.069), and 0.022 (0.009–0.051), respectively. The HRR was higher in the 0 to ≤5 min group than in the 5 to ≤10 min group, the 10 to ≤15 min group and the > 15 min group. There was no difference between the 10 to ≤15 min group and the > 15 min group. (3) When the groups were stratified with the cutoff of 10 min, the ≤10 min group HRR [0.250 (0.202–0.306)] was higher than the > 10 min group rate [0.041 (0.029–0.057)]. (4) The HRR of the telephone guidance group was [0.273 (0.227–0.325)] lower than that of the 0 to ≤5 min group [0.429 (0.347–0.516)] but higher than that of the 5 to ≤10 min group, the 10 to ≤15 min group, and the > 15 min group. (5) The HRR of the witness group [0.325 (0.216–0.458)] was not different from that of the 0 to ≤5 min group, but it was higher than those of the 5 to ≤10 min group, the 10 to ≤15 min group and the > 15 min group. (6) There was no significant difference HRR between the witnessed group, the telephone guidance group and the ≤10 min group.Conclusions(1) The HRR is time-sensitive, and early rescue can improve it. (2) CPR performed within the platinum ten minutes must be executed by the public, and other forces are auxiliary. (3) The concept of peri-cardiac arrest period (PCAP) should be established and improved to guide CPR.

Regurgitation and pulmonary aspiration during cardio-pulmonary resuscitation (CPR) with a laryngeal tube: A pilot crossover human cadaver study

BackgroundHigh-quality chest compressions are imperative for Cardio-Pulmonary-Resuscitation (CPR). International CPR guidelines advocate, that chest compressions should not be interrupted for ventilation once a patient’s trachea is intubated or a supraglottic-airway-device positioned. Supraglottic-airway-devices offer limited protection against pulmonary aspiration. Simultaneous chest compressions and positive pressure ventilation both increase intrathoracic pressure and potentially enhances the risk of pulmonary aspiration. The hypothesis was, that regurgitation and pulmonary aspiration is more common during continuous versus interrupted chest compressions in human cadavers ventilated with a laryngeal tube airway.MethodsTwenty suitable cadavers were included, and were positioned supine, the stomach was emptied, 500 ml of methylene-blue-solution instilled and laryngeal tube inserted. Cadavers were randomly assigned to: 1) continuous chest compressions; or, 2) interrupted chest compressions for ventilation breaths. After 14 minutes of the initial designated CPR strategy, pulmonary aspiration was assessed with a flexible bronchoscope. The methylene-blue-solution was replaced by 500 ml barium-sulfate radiopaque suspension. 14 minutes of CPR with the second designated ventilation strategy was performed. Pulmonary aspiration was then assessed with a conventional chest X-ray.ResultsTwo cadavers were excluded for technical reasons, leaving 18 cadavers for statistical analysis. Pulmonary aspiration was observed in 9 (50%) cadavers with continuous chest compressions, and 7 (39%) with interrupted chest compressions (P = 0.75).ConclusionOur pilot study indicate, that incidence of pulmonary aspiration is generally high in patients undergoing CPR when a laryngeal tube is used for ventilation. Our study was not powered to identify potentially important differences in regurgitation or aspiration between ongoing vs. interrupted chest compression. Our results nonetheless suggest that interrupted chest compressions might better protect against pulmonary aspiration when a laryngeal tube is used for ventilation.

How Ventilation Is Delivered During Cardiopulmonary Resuscitation: An International Survey.

Recommendations regarding ventilation during cardiopulmonary resuscitation (CPR) are based on a low level of scientific evidence. We hypothesized that practices about ventilation during CPR might be heterogeneous and may differ worldwide. To address this question, we surveyed physicians from several countries on their practices during CPR. We used a Web-based opinion survey. Links to the survey were sent by e-mail newsletters and displayed on the Web sites of medical societies involved in CPR practice from December 2013 to March 2014. 1,328 surveys were opened, and 548 were completed (41%). Responses came from 54 countries, but 64% came from 6 countries. Responders were mostly physicians (89%). From this group, 97% declared following specific CPR guidelines. Regarding practices, 28% declared always or frequently adopting only continuous chest compressions without additional ventilation. With regard to mechanical chest compression devices, 38% responded that such devices were available to them; when used, 28% declared always or frequently experiencing problems with ventilation such as frequent alarms. During bag-mask ventilation in intubated patients, 18% declared stopping chest compression during insufflation, and 39% applied > 10 breaths/min, which conflicts with international CPR guidelines. When a ventilator was used, the volume controlled mode was the most common strategy cited, but there was heterogeneity regarding ventilator settings for PEEP, trigger, FIO2 , and breathing frequency. SpO2 and end-tidal CO2 were the 2 most monitored variables cited. Physicians indicated heterogeneous practices that often differ significantly from international CPR guidelines. This may reflect the low level of evidence and a lack of detailed recommendations concerning ventilation during CPR.

Improving Temporal Trends in Survival and Neurological Outcomes After Out-of-Hospital Cardiac Arrest.

Considerable effort has gone into improving outcomes from out-of-hospital cardiac arrest (OHCA). Studies suggest that survival is improving; however, prior studies had insufficient data to pursue the relationship between markers of guideline compliance and temporal trends. The objective of the study was to evaluate trends in OHCA survival over an 8-year period that included the implementation of the 2005 and 2010 international cardiopulmonary resuscitation (CPR) guidelines. This was a population-based cohort study of all consecutive treated OHCA patients of presumed cardiac cause between 2006 and 2013 in the City of Toronto, Canada, and surrounding regions. Temporal changes were measured by χ2 trend test. The association between year of the OHCA and survival was evaluated using logistic regression and joinpoint analysis. A total of 23 619 patients with OHCA met study inclusion criteria. During the study period, survival to hospital discharge doubled (4.8% in 2006 to 9.4% in 2013; P<0.0001), and survival with good neurological outcome increased (6.2% in 2010 to 8.5% in 2013; P=0.005). Improvements occurred in the rates of bystander CPR and automated external defibrillator application, high-quality CPR metrics, and in-hospital targeted temperature management. After adjusting for the Utstein variables, survival to hospital discharge (odds ratio, 1.12; 95% confidence interval, 1.09-1.15) and survival with good neurological outcome (odds ratio, 1.13; 95% confidence interval, 1.05-1.22) increased with each year of study. Survival after OHCA has improved over time. This trend was associated with improved rates of bystander CPR, automated external defibrillator use, high-quality CPR metrics, and in-hospital targeted temperature management. The results suggest that multiple factors, each improving over time, may have contributed to the observed increase in survival.

Recent Trend in Therapeutic Hypothermia and Early-Onset Pneumonia in Cardiac Arrest

The ultimate goal of cardiopulmonary resuscitation (CPR) is restoring spontaneous circulation and minimizing neurologic deficits. Since two human studies presented improved neurologic outcome and reduced mortality after cardiac arrest in 2002,[1,2] therapeutic hypothermia (TH) has been recommended consistently in the international CPR guidelines for post-cardiac arrest care.[3-5] TH improves the neurologic outcome due to attenuation of the inflammatory response in the brain.[1] On the other hand, TH can have systemic adverse effect such as high infection rate. Geurts et al.[6] emphasized in a meta-analysis of 23 studies that TH was a risk factor of both pneumonia and sepsis after return of spontaneous circulation in cardiac arrest patients. Even if TH is not used, infection is more common in post-cardiac arrest care,[7] and pneumonia is the most common type of infection in out-of-hospital cardiac arrest (OHCA).[8] The cause of this high incidence of pneumonia in OHCA is that factors such as loss of airway protection, changed mental status, pulmonary contusion by chest compression, emergent airway access, and mechanical ventilation increase the risk of pulmonary infection.[7] Some studies have suggested that post-resuscitation pneumonia could be divided into early-onset and late-onset pneumonia according to onset time and prevalent pathogens; however, the onset time varied from three to seven days depending on the study.[8-11] Perbet et al.[11] reported that TH was an independent risk factor of early-onset pneumonia (EOP), which prolonged mechanical ventilation support and intensive care unit (ICU) stay with unchanged neurologic outcome and ICU mortality in a retrospective and large (641 patients) cohort study. Therefore, intensivists should be aware of management of EOP while conducting HT. Prophylactic antibiotics decreased the incidence of EOP in comatose patients with a variety of causes such as head trauma, intracranial hemorrhage, stroke, or cardiac arrest.[12] In a recently published study, prophylactic antibiotics reduced the incidence of pneumonia in cardiac arrest survivors undergoing TH, but they did not reduce patient mortality.[13] However, the researchers did not distinguish earlyor late-onset pneumonia. Kim et al.[14] have reported that prophylactic antibiotics in OHCA patients undergoing TH does not reduce the incidence of EOP. The authors insisted that the study evaluated the effect of prophylactic antibiotics on EOP for the first time. Evaluation of figure model training on improvement of the cardiopulmonary resuscitation skills in new medical recruits

Objective To study the effect of new figure pattern on the cardiopulmonary resuscitation (CPR) training for the new medical staff.Methods The keys of 2010 CPR were changed into figure pattern,and totals of 172 new medical staff were received the new training.They were assessed with questionnaire before and after the training on first-aid knowledge and skills.Results The average score of operation of new medical staff was significantly lower than that after the training [(56.88 ± 8.926) vs (96.20 ± 2.89),t =23.35,P ＜0.05],as well as the knowledge awareness rate (52％ vs 92.6％).Conclusions Compared to the traditional training model,the new training model made medical staff pay attention on every details and key data,and make sure the precise of practice in position,depth,dynamics and frequency. Out-of-Hospital Pediatric Cardiorespiratory Arrest in Galicia

Pediatric out-of-hospital cardiorespiratory arrest (CRA) is a rare event but has a high mortality and morbidity among survivors. In 2005, an international consensus on science and treatment recommendations has been released, with the aim of improving the assistance of patients who had and, eventually, increasing survival without neurologic sequelae. Our objective was to assess the impact of the 2005 guidelines on the initial prehospital assistance of children with out-of-hospital CRA in a community with scattered population. This is a prospective observational study following the Utstein-style guidelines of pediatric CRA in 2 periods: group 1 (pre-2005), from July 2002 to February 2005 (32 months); and group 2 (post-2005), from January 2007 to December 2008 (24 months). Patients aged from 0 months to 16 years who had an out-of-hospital respiratory or cardiac arrest were included in the study. There were 31 patients (84% cardiac) who had CRA in group 1 and 21 patients (62% cardiac) who had CRA in group 2 (P = 0.073). Both groups were comparable in age, sex, CRA cause, place of CRA incident, management of airway, fluid administrations, and defibrillation attempts. A significant increment in the number of bystander cardiopulmonary resuscitation (CPR) was observed in group 2 (13 [62%] vs 7 [29%], P = 0.004). The intraosseous access was more frequently used in the post-2005 group (8 [38%] vs 5 [16%], P = 0.021). In group 2, a higher percentage of patients received more than 1 adrenaline dose (95% vs 61%, P = 0.006), were treated with bicarbonate (7 [33%] vs 3 [10%], P = 0.045), and were not treated with atropine (5 [24%] vs 17 [55%], P = 0.020). Survival to hospital admission, sustained return of spontaneous circulation, and survival to hospital discharge were comparable in both groups. In cases of pediatric out-of-hospital CRA in a community with scattered population, after the introduction of the 2005 international CPR recommendations, there was an increase in bystander CPR and changes in immediate treatment were detected. However, these changes did not result in a significant outcome improvement.

(P2-3) Analysis of Chest Compression Rate and Its Affect on the Quality of Chest Compressions

BackgroundIn the last 50 years of modern-era cardiopulmonary resuscitation (CPR), survival rates remain dismal, worldwide. International CPR guidelines recommended a compression rate of at least 100 per minute. There is little evidence documenting if and to what extent high compression rates affect the quality of chest compressions.ObjectivesAn objective of this study was to evaluate the effect mean compression rate (MCR) had on the overall quality of chest compressions. Investigators hypothesized that MCRs &gt; 110 would result in a smaller percentage of adequate: compressions (PAC); depth (PAD); and recoil (PAR).MethodsIn this observational pilot study, basic life support providers were recruited from prehospital and in-hospital settings to provide 10 minutes of continuous chest compressions, based on the 2005 American Heart Association guidelines. An adequate compression was defined as a compression that was &gt; 35 mm, had full recoil, and correct hand position. Data were recorded using the Laerdal PC Skill reporting System.ResultsNinety four (91.3%) of 103 participants completed 10 minutes of compressions. Rescuers represented a variety of backgrounds, average age of 35.5 ± 11.0 years. Fifty eight (56.2%) rescuers had performed CPR in the last two years, and 54 (52.4%) practiced prehospital EMS. Providers that did not complete the entire 10 minutes tended to have a higher MCR than those completing 10 minutes, 114.2 ± 19.3 vs. 105.8 ± 15.4 respectively. Within the first two minutes, rescuers with a MCR &gt; 110 delivered 45% of their compressions adequately, compared to 60% when a rescuer's MCR was &lt; 110. This initial disparity was primarily due to decreased PAR, not decreased PAD. After 2 minutes, higher MCRs correlated with decreased PAC, due to decreased PAD.ConclusionsData indicates a higher MCR results in decreased PAC, PAD, and PAR, likely attributed to increased rescuer fatigue.

New cardiopulmonary resuscitation guidelines 2010: Managing the newly born in delivery room

Most newborns are born vigorous and do not require neonatal resuscitation. However, about 10% of newborns require some type of resuscitative assistance at birth. Although the vast majority will require just assisted lung aeration, about 1% requires major interventions such as intubation, chest compressions, or medications. Recently, new evidence has prompted modifications in the international cardiopulmonary resuscitation (CPR) guidelines for both neonatal, paediatric and adult patients. Perinatal and neonatal health care providers must be aware of these changes in order to provide the most appropriate and evidence-based emergency interventions for newborns in the delivery room. The aim of this article is to provide an overview of the main recommended changes in neonatal resuscitation at birth, according to the publication of the international Liaison Committee on Resuscitation (ILCOR) in the CoSTR document (based on evidence of sciences) and the new 2010 guidelines released by the European Resuscitation Council (ERC), the American Heart Association (AHA), and the American Academy of Pediatrics (AAP).

Quality of Cardiopulmonary Resuscitation During In-Hospital Cardiac Arrest

The survival benefit of well-performed cardiopulmonary resuscitation (CPR) is well-documented, but little objective data exist regarding actual CPR quality during cardiac arrest. Recent studies have challenged the notion that CPR is uniformly performed according to established international guidelines. To measure multiple parameters of in-hospital CPR quality and to determine compliance with published American Heart Association and international guidelines. A prospective observational study of 67 patients who experienced in-hospital cardiac arrest at the University of Chicago Hospitals, Chicago, Ill, between December 11, 2002, and April 5, 2004. Using a monitor/defibrillator with novel additional sensing capabilities, the parameters of CPR quality including chest compression rate, compression depth, ventilation rate, and the fraction of arrest time without chest compressions (no-flow fraction) were recorded. Adherence to American Heart Association and international CPR guidelines. Analysis of the first 5 minutes of each resuscitation by 30-second segments revealed that chest compression rates were less than 90/min in 28.1% of segments. Compression depth was too shallow (defined as <38 mm) for 37.4% of compressions. Ventilation rates were high, with 60.9% of segments containing a rate of more than 20/min. Additionally, the mean (SD) no-flow fraction was 0.24 (0.18). A 10-second pause each minute of arrest would yield a no-flow fraction of 0.17. A total of 27 patients (40.3%) achieved return of spontaneous circulation and 7 (10.4%) were discharged from the hospital. In this study of in-hospital cardiac arrest, the quality of multiple parameters of CPR was inconsistent and often did not meet published guideline recommendations, even when performed by well-trained hospital staff. The importance of high-quality CPR suggests the need for rescuer feedback and monitoring of CPR quality during resuscitation efforts.

Arginine vasopressin during cardiopulmonary resuscitation and vasodilatory shock: current experience and future perspectives

Epinephrine use during cardiopulmonary resuscitation (CPR) is controversial because of its receptor-mediated adverse effects such as increased myocardial oxygen consumption, ventricular arrhythmias, ventilation-perfusion defect, postresuscitation myocardial dysfunction, ventricular arrhythmias, and cardiac failure. In the CPR laboratory, vasopressin improved vital organ blood flow, cerebral oxygen delivery, resuscitability, and neurologic recovery more than did epinephrine. In patients with out-of-hospital ventricular fibrillation, a larger proportion of patients treated with vasopressin survived 24 hours than did patients treated with epinephrine. Currently, a large trial of out-of-hospital cardiac arrest patients being treated with vasopressin versus epinephrine is ongoing in Germany, Austria, and Switzerland. The new international CPR guidelines recommend 40 U vasopressin intravenously, and 1 mg epinephrine intravenously, as equally effective for the treatment of adult patients in ventricular fibrillation; however, no recommendation for vasopressin has been made to date for adult patients with asystole and pulseless electrical activity, or in children, because of lack of clinical data. When adrenergic vasopressors were unable to maintain arterial blood pressure in patients with vasodilatory shock, continuous infusions of vasopressin (0.04-0.10 U/min) stabilized cardiocirculatory parameters and even ensured weaning from catecholamines.
