ABSTRACT: The incidence of cranial cruciate ligament rupture (CCLR) in dogs is high, which is considered the main arthropathy in the species. Once diagnosed, surgical stabilization is recommended and different treatments are categorized as intracapsular, extracapsular, and osteotomies. There is still no consensus regarding the most optimal method of stabilization, and some studies have attempted to create or improve existing techniques, making them more effective. This study presented an intra-articular stifle stabilization technique using a synthetic polyester implant using 32 anatomical specimens from canine cadavers, which were separated by weight into two groups. The drawer movement was analyzed at three timepoints: intact cranial cruciate ligament (CCL), dissected ligament, and after surgical stabilization using the proposed technique. Results showed a mean cranial displacement of the tibia relative to the femur of 0.61 ± 0.08 millimeters before dissection (mm), 2.61 ± 0.08 mm after dissection, and 0.68 ± 0.08 mm after surgical stabilization (P < 0.01). In conclusion, the intra-articular stabilization technique with polyester thread was effective in stabilizing ex vivo dog stifles after CCL dissection at the immediate postoperative period.

Ex vivo biomechanical evaluation of extracapsular stabilization with quasi-isometric points in canine cranial cruciate ligament-deficient stifles.

Cranial cruciate ligament (CCL) disease is one of the most common causes of lameness in dogs. The extracapsular stabilization (ECS) utilizing bone anchors and monofilament nylon leader was an alternative treatment for CCL-deficient (CCLD) dogs. However, the biomechanical response of the canine stifle to such a surgical repair strategy in conjunction with the use of recently reported quasi-isometric anchoring points remains unclear. The objectives of the study were to evaluate the mobility and stability of CCL-intact, CCLD, and CCLD stifles repaired with ECS at two different pairs of quasi-isometric points (quasi-IPs). Twelve stifle specimens from 7 dogs underwent mobility and stability tests under 4 different conditions, namely, CCL-intact, CCLD, and ECS-repaired at 2 different pairs of quasi-IPs (referred to as ECS-IP1 and ECS-IP2). The mobility tests evaluated 6 degrees-of-freedom stifle kinematics during flexion and extension. The stability tests involved cranial drawer and tibial internal rotation (IR) tests at various stifle opening angles and quantifying the cranial tibial translation (CTT) and tibial IR angles under constantly applied loadings. The ECS repaired at quasi-IPs was shown to restore cranial instability of the stifles with averaged CTT magnitudes < 1.4mm. During the tibial IR test, the ECS treatments resulted in significantly less tibial IR compared to those in intact CCL stifles. The mobility tests showed similar results. The 2 chosen pairs of quasi-IPs were shown to effectively correct the excessive CTT caused by CCLD stifles, whereas the excessive tibial external rotation in comparison to those of intact stifles should be considered for its subsequent influence on joint alignment and the contact pressure applied to the stifle joint.

Meniscal Load and Load Distribution in the Canine Stifle after Modified Tibial Tuberosity Advancement with 9 mm and 12 mm Cranialization of the Tibial Tuberosity in Different Standing Angles.

The aim of this study was to investigate the kinetic and kinematic changes in the stifle after a modified tibial tuberosity advancement (TTA) with 9 and 12 mm cranialization of the tibial tuberosity in different standing angles. Biomechanical ex vivo study using seven unpaired canine cadaver hindlimbs. Sham TTA surgery was performed. Motion sensors were fixed to the tibia and the femur for kinematic data acquisition. Pressure mapping sensors were placed between femur and both menisci. Thirty percent body weight was applied to the limbs with the stifle in 135 or 145 degrees of extension. Each knee was tested in 135 degrees with intact cranial cruciate ligament (CCL) and deficient CCL with 12 mm cranialization of the tibial tuberosity in 135 and 145 degrees of extension. The last two tests were repeated with 9 mm. Transection of the CCL altered kinematics and kinetics. Tibial tuberosity advancement with 12 mm cranialization sufficiently restored stifle kinematics in 135 and 145 degrees but 9 mm TTA failed to do so in 135 degrees. The same effects were seen for internal rotation of the tibia. After TTA, a significant reduction in the force acting on both menisci was detected. Tibial tuberosity advancement could restore stifle kinematics and meniscal kinetics after transection of the CCL ex vivo in the present study. Tibial tuberosity advancement reduced the contact force ratio on both menisci significantly. No changes of peak pressure and peak pressure location occurred following TTA under any of the tested experimental settings. Increased stifle extension (145 degrees) might lead to more stability, contradictory to biomechanical theory.

The Effect of Femur Positioning on Measurement of Tibial Plateau Angle: An In Vitro Study

Five canine cadaveric pelvic limbs with intact cranial cruciate ligaments were used to quantify the effect of variation in limb positioning on the radiographic measurement of the tibial plateau angle (TPA) with reference to the degree of femoral condyle superimposition. Intra-osseous pin placement and a custom jig design allowed the controlled three-dimensional manipulation of limbs. Medio-lateral digital radiographic projections were taken with perfect femoral hemicondylar superimposition to establish a "reference" TPA (difference in position = 0 mm), and subsequently in varying degrees of supination/pronation and abduction/adduction. The lack of femoral hemicondylar superimposition for each radiograph was quantified using a tangential line technique with reference to the long tibial axis. A total of 176 radiographs were each assessed by three observers. "True" TPA was measured and it ranged within 17-25° across all limbs assessed. Variation in femoral condylar positioning ranged from -13 mm to +13 mm proximo-distally, and -11 mm to +11 mm cranio-caudally. Moreover, 3 mm non-superimposition of the femoral condyles produced 90.6% of measurements with 1° difference between measured and "true" TPA, and a sensitivity of 97.9% for a 2° difference. Further reduction in femoral condylar superimposition to 4 mm reduced the frequency of 1° difference between measured and "true" TPA to 84.9%, and to 94.8% for a 2° difference. In conclusion, measurement of TPA in large breed dogs from radiographs with greater than 3 mm variation in femoral condylar superimposition should be interpreted with caution.

Stifle hyperextension identified in a dog with an excessively negative tibial plateau angle and a grossly intact cranial cruciate ligament following inappropriate cranial closing wedge ostectomy

AbstractA cross breed dog was examined due to an abnormal right pelvic limb gait following cranial closing wedge ostectomy surgery. Gait evaluation revealed stifle hyperextension during the stance phase of the gait. Manipulation of the stifle revealed no cranial or caudal drawer and no cranial tibial thrust. Radiographs showed complete healing of the proximal tibial osteotomy, with a tibial plateau angle (TPA) of –14°. Computed tomography showed 20° of external tibial torsion on the right pelvic limb. A mini‐medial stifle arthrotomy revealed no gross damage to the cruciate ligaments and intact menisci. A cranial opening wedge osteotomy of 17° was performed and the torsional alignment corrected, after which the tibia was stabilised with orthogonal plates and an intramedullary pin. The postoperative radiographs confirmed a satisfactory TPA of 3° and normal tibial alignment. At 6 months follow‐up, a video record revealed normal right pelvic limb gait.

Evaluation of Meniscal Load and Load Distribution in the Canine Stifle after Tibial Plateau Levelling Osteotomy with Postoperative Tibia Plateau Angles of 6 and 1 Degrees.

Biomechanical ex vivo study using seven unpaired canine cadaver hindlimbs from adult Retrievers.Hinge plates were applied and a sham TPLO surgery was performed. Motion sensors were fixed to the tibia and the femur for kinematic data acquisition. Pressure mapping sensors were placed between femur and both menisci. Thirty per cent bodyweight was applied to the limbs with the stifle in 135 degrees of extension. Each knee was tested with intact cranial cruciate ligament (CCL), deficient CCL, 6 degrees TPLO and 1degree TPLO. Transection of the CCL altered kinematics and kinetics. However, comparing the intact with both TPLO set-ups, no changes in kinematics were detected. After 1 degree TPLO, a significant reduction in the force acting on both menisci was detected (p = 0.006). Tibial plateau levelling osteotomy restores stifle kinematics and meniscal kinetics after transection of the CCL ex vivo. The contact force on both menisci is reduced significantly after TPLO with a TPA of 1 degree. Increased stifle flexion might lead to caudal tibial motion.

Biomechanical evaluation of bovine stifles stabilized with an innovative braided superelastic nitinol prosthesis after transection of the cranial cruciate ligament.

To determine the stability bovine stifles stabilized with nylon or nitinol superelastic prostheses after transection of the cranial cruciate ligament (CCL). Ex vivo study. Stifles (n=15) harvested from adult bovine cadavers. The stifles were randomly assigned pairwise to a ligament reconstruction technique (n=5): (1) and (2) Hamilton's technique using a prosthesis made of 24 nitinol strands (0.39 mm) braided at 40°or single 600-lb test nylon implant, and (3) nitinol prosthesis placed in femoral and tibial bone tunnels (bone-to-bone). Craniocaudal tibial translation at ±2000 N was applied to the tibia, and mediolateral angular displacement via measured under torsional tibial loading at ±60 Nm on three occasions: intact CCL, transected, and stabilized. Outcomes were evaluated with a mixed effect linear model for repeated measures. Bone-to-bone using nitinol was the only repair that decreased tibial translation after CCL transection (p= .001) with a 23% change magnitude compared with intact CCL. Hamilton was the only stabilization reestablishing angular displacement, similar to intact CCL (p=.109 and .134 for nitinol and nylon). Bone-to-bone nitinol stabilization decreased angular displacement after CCL-transection with an 8% change magnitude (p=.040) without returning to normal values. CCL replacement with nylon did restore joint stability. Nitinol prostheses passed through single femoral and tibial bone tunnels (bone-to-bone) were the only techniques reducing tibial translation. Bone-to-bone stabilization with a nitinol prosthesis may be considered as an alternative to nylon for CCL replacement in cattle. These results provide evidence to justify clinical evaluation in cattle undergoing CCL replacement.

Effect of the Centre of Rotation in Tibial Plateau Levelling Osteotomy on Quadriceps Tensile Force: An Ex Vivo Study in Canine Cadavers.

The aim of this study was to determine the effect of the centre of rotation in tibial plateau levelling osteotomy (TPLO) on the tensile force of the quadriceps. Tibial plateau levelling osteotomy was performed on the left pelvic limbs from 20 normal adult Beagle cadavers. To replicate the tensile force of the quadriceps, gastrocnemius and stifle flexor muscles, these muscles were replaced with wires. The tensile force of each wire, cranial tibial displacement and internal tibial rotation were measured under the following conditions: intact cranial cruciate ligament, transected cranial cruciate ligament, ideally centred osteotomy TPLO (ICO group) and distally centred osteotomy TPLO (DCO group). The ratios of the tensile forces for the wires divided by the vertical force were used for analyses. The mean intact and post-TPLO tibial plateau angles (TPA) in the ICO group were 30.3° ± 1.9° and 6.1° ± 1.6°, respectively, and those in the DCO group were 29.8° ± 2.4° and 6.8° ± 0.9°, respectively. The mean quadriceps tensile force after TPLO was significantly greater in the DCO group (3.9 ± 0.3) than the ICO group (3.3 ± 0.4) (p = 0.006). Both groups exhibited tibial caudal displacement after TPLO. The tensile force of the quadriceps muscles changed in accordance with the centre of the osteotomy in TPLO. The DCO group had increased tensile force, which may cause patellar ligament thickening after TPLO. Setting the postoperative TPA at 6° may cause excessive rotation in patients with a normal tensile force of the stifle flexor muscles.

Influence of tibial plateau leveling osteotomy and tibial tuberosity advancement on passive laxity of the cranial cruciate deficient stifle in dogs.

To evaluate the effect of tibial plateau leveling osteotomy (TPLO) and tibial tuberosity transposition (TTA) on craniocaudal stability of the cranial cruciate ligament (CCL) deficient stifle at 3 joint angles. Ex vivo biomechanical study. Eight stifles from 4 dogs. One of the hind limbs was randomly assigned to undergo TPLO or TTA, with the contralateral limb assigned to the alternate procedure. Each specimen was tested with the stifle at 150°, 135°, and 120° of flexion. Cranial translation of the tibia relative to the femur was measured under cranial loads ranging from 0.2 to 2.0 kg with a single motion tracking sensor (Nest of Birds) fixed to the tibia. Each stifle was tested before and after transection of the CCL and after performing the assigned procedure. Tibial plateau leveling osteotomy and TTA caused a decrease in the cranial translation of the tibia relative to the femur, especially at greater angles of flexion. Tibial plateau leveling osteotomy caused a greater decrease in tibial cranial translation and was associated with a degree of translation that did not differ from joints with intact CCL, regardless of the degree of stifle flexion. Tibial plateau leveling osteotomy and TTA caused an angle dependent decrease in craniocaudal translation of the tibia in the CCL deficient stifle. Only TPLO restored translations similar to those of the intact joint at all angles tested. Tibial plateau leveling osteotomy and TTA provide passive stability to the stifle. Tibial plateau leveling osteotomy seems more effective than TTA at restoring craniocaudal stability of the stifle.

Histologic assessment of ligament vascularity and synovitis in dogs with cranial cruciate ligament disease.

To assess the relationship between histologic degeneration of cranial cruciate ligaments (CCLs) and severity of synovitis and ligament vascularity. CCL and synovium from 59 stifle joints (53 dogs). CCL and synovium specimens were obtained from stifle joints of juvenile (15 joints; 12 dogs) and adult (25 joints; 22 dogs) dogs with intact CCLs and dogs with CCL rupture (rCCL; 19 joints; 19 dogs). Vascular density and degenerative changes of the CCL core region and severity of synovitis were semiquantitatively evaluated. Relationships were analyzed by use of a random effects model to account for correlated specimens. Mean ± SD modified Bonar scores (scale, 0 to 9) of adults (4.85 ± 0.44) and dogs with rCCL (5.69 ± 0.49) were significantly higher than scores of juveniles (1.13 ± 0.55). Vascularity scores (scale, 0 to 3) were significantly higher for juveniles (3.00 ± 0.24) than for adults (1.53 ± 0.27) and dogs with rCCL (0.78 ± 0.23). Synovitis scores were not significantly different among groups. There was a significant negative relationship between modified Bonar scores and vascularity scores for juveniles and adults and for adults and dogs with rCCL when controlling for age, but there was not a significant relationship between modified Bonar scores and synovitis scores. There was a significant relationship between modified Bonar scores and body weight of adults. Poor blood supply to the core region could be an important underlying condition for spontaneous degeneration of the CCL in at-risk dogs.

Ex vivo preliminary investigation of radiographic quantitative assessment of cranial tibial displacement at varying degrees of canine stifle flexion with or without an intact cranial cruciate ligament

BackgroundThe presence of cranial tibial subluxation can aid in the detection of joint instability as a result of CrCL injury. Detection of cranial tibial subluxation has been described using the tibial compression test (TCT) and cranial drawer test (CDT); however, diagnosis of CrCL insufficiency by assessing cranial subluxation motion of the tibia is subjective and difficult to quantify accurately. The aim of this study was to investigate a measurement technique to assess the degree of cranial tibial displacement relative to the femoral condyles on mediolateral projection stifle radiographs at varying degrees of stifle flexion (90°, 110°, and 135°) in CrCL intact, partially, and completely transected conditions. Radiographic measurements included: CrCL length and intercondylar distance (ICD), defined as the distance between the tibial mechanical axis (TMA) and the femoral condylar axis (FCA). The influence of CrCL status, stifle flexion angle, and measurement type on measured distance was evaluated. The relationship between CrCL length and ICD measurement was also assessed.ResultsComplete transection of the CrCL resulted in significant cranial tibial displacement. Stifle flexion angle affected ICD, but not CrCL length. Normalized measured CrCL length and ICD were significantly different; however, no differences existed between the change in distance detected by CrCL length and ICD measurements as CrCL transection status changed. Correlation coefficients detected a significant positive correlation between measured CrCL and ICD.ConclusionThe ICD measurement technique was able to quantify tibial displacement at various stifle flexion angles in the intact and completely transected CrCL conditions. The ICD measurement was more affected by stifle flexion angle than was the CrCL length.

Cranial cruciate ligament structure in relation to the tibial plateau slope and intercondylar notch width in dogs.

Cranial cruciate ligament (CCL) rupture is one of the most common orthopedic conditions in dogs. The pathogenesis of CCL rupture is not fully described and remains to be elucidated fully. Several hypotheses have been proposed to explain the etiology of these changes. The objective of this study was to investigate structural changes in the CCL in relation to the tibial plateau angle (TPA) and the intercondylar notch (ICN) width in dogs. Fifty-five skeletally mature dogs were included in this study. ICN width and TPA measurements were obtained from intact CCL stifles. Samples of the CCL, caudal cruciate ligament (CaCL), and femoral head ligament (FHL) were harvested and stained for routine histological and immunohistochemical analysis. Microscopic changes in the ligaments were observed and were found to correlate with the TPA and ICN width values. The degree of structural changes within the CCL was observed to correlate with an increasing TPA and a narrowing ICN width. Changes in the CCL are likely to be caused by excessive forces acting through the ligament in stifles with a high TPA. Chondroid metaplasia of the CCL is an adaptation to abnormal mechanics within the stifle joint caused by altered bone morphology.

Cruciate ligament degeneration and stifle joint synovitis in 56 dogs with intact cranial cruciate ligaments: Correlation of histological findings and numbers and phenotypes of inflammatory cells with age, body weight and breed

The majority of dogs with ruptured cranial cruciate ligaments (crCLs) have inflammatory changes of the stifle joint suggesting that synovitis is an important factor which is involved in the development of degenerative ligament changes. Detailed information is not available concerning the possible occurrence of inflammatory changes in the stifle joint synovium of dogs with macroscopically intact crCLs and its correlation with presence and severity of degenerative changes of the crCLs. Therefore, the purpose of this study was to examine post mortem tissue samples of 56 dogs with non-ruptured crCLs histologically for the presence of inflammatory and degenerative changes of the stifle joint synovium and cruciate ligaments, respectively. In 25/56 (44.6%) dogs, histology showed that both lymphoplasmacytic synovitis and degenerative alterations of the crCLs were present. In these dogs, there was a significant positive statistical correlation between the severity of synovitis and degenerative crCL lesions. The results suggest that synovitis in the stifle joints of dogs with non-ruptured crCLs is a frequent event and probably is involved in the development of degenerative lesions occurring in canine crCLs. Also, the severity of crCL degeneration in these 25 dogs was significantly correlated with their age and body weight. In 2/56 (3.6%) cases, only lymphoplasmacytic synovitis was found in the absence of degenerative crCL lesions. In 15/56 (26.8%) dogs, only degenerative lesions of the crCLs without synovitis were present. Statistically, a significant correlation was found between the severity of degenerative alterations and age and body weight of these dogs. Phenotyping of inflammatory cells by immunohistochemistry showed that the synovium of dogs which histologically had lymphoplasmacytic synovitis was infiltrated with CD3+ T lymphocytes, CD79+ B lymphocytes, major histocompatibility class II antigen (MHC class II)+ cells and macrophages expressing CD163 or S100/A8/S100A9 (calprotectin), while tartrate-resistant acid phosphatase (TRAP)+ cells were absent. Quantification and statistical evaluation of inflammatory cell types in the inflamed synovium revealed that the numbers of lymphocytic cells and macrophages were significantly correlated with the severity of synovitis. These findings indicate that, besides T and B lymphocytes, both pro- and anti-inflammatory macrophages play a role in the development of degenerative crCL alterations.

Joint stability after canine cranial cruciate ligament graft reconstruction varies among femoral fixation sites.

To quantify stability in cranial cruciate ligament (CrCL) deficient canine stifles with hamstring grafts affixed at 3 femoral locations. Canine stifle motion study using a multi-cohort, repeated measures design. 27 canine cadaver stifles. Hamstring grafts (HG) were affixed at the gracilis-semitendinosus insertion and on the lateral femur (1) proximal trochlear ridge (TR), (2) craniodistal to fabella (F), or (3) condyle center (CC). Total, cranial, and caudal tibial translation and total, medial, and lateral angular displacement, with and without translational load, were quantified with the CrCL intact, transected, and reconstructed. Angular displacement was quantified from points on the distal femur and proximal tibia. Graft strain was calculated from tissue displacement measured at joint angles of 30°, 60°, 90°, and 120°. Tibial translation was lowest in F constructs, which also achieved the least difference in tibial translation from intact stifles. Tibial translation was lower in intact stifles than in CrCL transected or reconstructed stifles. Less angular displacement of the proximal tibia was detected in the medial than in the lateral direction, and tibial displacement was lower in the cranial than the caudal direction. Angular displacement was lowest in the F treatment group. F constructs had the lowest graft strain at joint angles greater than 30°. Femoral fixation of a canine hamstring graft craniodistal to the lateral fabella conferred the best joint stability and lowest graft strain in vitro. No fixation method restored joint stability of the intact CrCL.

Partial Cranial Cruciate Ligament Tears Treated with Stem Cell and Platelet-Rich Plasma Combination Therapy in 36 Dogs: A Retrospective Study.

To evaluate outcomes in 36 dogs with a partial cranial cruciate ligament (CCL) tear treated with autologous bone marrow aspirate concentrate (BMAC) or adipose-derived progenitor cells (ADPC) with platelet-rich plasma (PRP) combination. Medical records of client-owned dogs diagnosed with an early partial (≤50%) tear of the craniomedial band of the CCL that was treated with BMAC-PRP or ADPC-PRP were reviewed from 2010 to 2015. Signalment, medical history, physical and orthopedic examination, objective temporospatial gait analyses, radiographs, day 0 and day 90 diagnostic arthroscopy findings, treatment, and outcome were among the data collected. A functional owner questionnaire, including the validated Helsinki chronic pain index (HCPI), was sent to owners whose dog was known to not have had a tibial plateau leveling osteotomy (TPLO). Statistical analysis was performed on data, where significance was established at p < 0.05. Stifle arthroscopy findings at 90 days posttreatment were available on 13 of the 36 dogs. In nine dogs, a fully intact CCL with marked neovascularization and a normal fiber pattern was found with all previous regions of disruption healed. One dog revealed significant improvement and received an additional injection. The remaining three dogs had a >50% CCL tear, and a TPLO was performed. Four additional dogs were known to have had a TPLO performed elsewhere. Baseline and day 90 posttreatment objective gait analyses were available on 11 of the 36 dogs. A significant difference was found between the treated limb total pressure index percent (TPI%) at day 0 and day 90 (p = 0.0124), and between the treated limb and contralateral limb TPI% at day 0 (p = 0.0003). No significant difference was found between the treated limb and contralateral limb TPI% at day 90 (p = 0.7466). Twelve questionnaires were returned, of which eight were performance/sporting dogs. Seven of the eight had returned to sport; the remaining dog had just begun a return to sport conditioning program 6 months posttreatment. All 12 respondents believed that their dog had an excellent or very good quality of life and rated their dog's procedural outcome as excellent or good. The use of BMAC-PRP and ADPC-PRP shows promise for the treatment of early partial CCL tears in dogs. Further studies are needed and should be randomized, blinded, and controlled.

Evaluation of surface blood flow in intact and ruptured canine cruciate ligaments using laser Doppler flowmetry.

To evaluate the usefulness of laser Doppler flowmetry (LDF) to measure surface blood flow in canine cruciate ligaments, compare measurements in different sites of intact and partially ruptured canine cranial cruciate ligaments (CrCL) and intact caudal cruciate ligaments (CaCL), and investigate any association between surface blood flow in partially ruptured CrCL and synovitis or duration of clinical signs. Case-controlled clinical study. Sixteen dogs with partially ruptured CrCL and five dogs with intact CrCL. Blood cell flux (BCF) readings during three measurement cycles using LDF at two sites in each ligament (mid-substance and the distal portion of the CrCL, and mid-substance and the proximal portion of the CaCL) were recorded. Synovial changes were graded grossly and histologically using the Osteoarthritis Research Society International histopathology scoring system. The within-run coefficients of variation (CV) for a single BCF measurement cycle were 12.2% and 12.7% in the ruptured and intact CrCL groups, respectively. The between-run CV for three measurement cycles was 20.8% and 14.8%, respectively. The intraclass correlation coefficient (ICC, absolute agreement) was 0.66 for a single measurement cycle and 0.86 for the average of three cycles. No difference in average BCF readings was found between any two sites in either group, but BCF readings in both CrCL sites were significantly higher in the ruptured CrCL group than the intact CrCL group. No associations between BCF and synovial grades or duration of lameness were identified. Laser Doppler flowmetry can be used to assess surface blood flow in intact and partially ruptured canine cruciate ligaments with acceptable precision. Using this method, surface blood flow appears greater in partially ruptured canine CrCL than intact CrCL. Further studies are required to determine if this is a sequela of trauma or synovitis.

Aetiology and pathogenesis of cranial cruciate ligament rupture in cats by histological examination.

Objectives The aim of this study was to examine histologically intact and ruptured cranial cruciate ligaments in cats, in order to evaluate whether degeneration is a prerequisite for rupture. Methods We performed a histological examination of 50 intact and 19 ruptured cranial cruciate ligaments in cadaver or client-owned cats, respectively, using light microscopy. Cats with stifle pathology were further divided into five age groups in order to investigate the relationship of changes in the ligament with lifespan. Cats with ruptured cranial cruciate ligaments were divided into two groups according to medical history (with presumed history of trauma or without any known history of trauma) in order to investigate the relationship of ligament rupture with a traumatic event. Data from 200 healthy cats were selected randomly and reviewed to make a statistical comparison of cats with and without cranial cruciate ligament rupture (reference group). Results On histological examination, the intact cranial cruciate ligaments showed basic parallel arrangement of the collagen fibres, with no relation to age. While cats of a more advanced age showed fibrocartilage in the middle of the cranial cruciate ligament - a likely physiological reaction to compression forces over the lifespan - degenerative changes within the fibrocartilage were absent in all cases, regardless of age or rupture status. Cats suffering from cranial cruciate ligament rupture without history of trauma were significantly older than cats in the reference group. Conclusions and relevance This study showed that differentiation of fibrocartilage in the middle of the cranial cruciate ligament is likely a physiological reaction to compressive forces and not a degenerative change associated with greater risk of rupture in advanced age. This finding in cats is distinct from the known decrease in differentiation of fibrocartilage in dogs with cranial cruciate ligament rupture. Furthermore, the histological examination of cats revealed no other signs of degeneration in the cranial cruciate ligaments. Thus, degeneration is likely not an aetiological factor for cranial cruciate ligament rupture in cats.

Mechanical strength of four allograft fixation techniques for ruptured cranial cruciate ligament repair in dogs.

To test ex vivo mechanical properties of 4 allograft fixation techniques for cranial cruciate ligament (CCL) replacement. 30 stifle joints from canine cadavers. CCL-deficient stifle joints repaired by 1 of 4 techniques (n = 6/group) and CCL-intact stifle joints (control group; 6) were mechanically tested. Three repair techniques involved a patella-patella ligament segment (PPL) allograft: a tibial and femoral interference screw (PPL-2S), a femoral interference screw and the patella seated in a tapering bone tunnel in the tibia (PPL-1S), or addition of a suture and a bone anchor to the PPL-1S (PPL-SL). The fourth technique involved a deep digital flexor tendon (DDFT) allograft secured with transverse femoral fixation and stabilized with a tibial interference screw and 2 spiked washers on the tibia (DDFT-TF). The tibia was axially loaded at a joint angle of 135°. Loads to induce 3, 5, and 10 mm of femoral-tibia translation; stiffness; and load at ultimate failure with the corresponding displacement were calculated. Group means were compared with a multivariate ANOVA. Mean ± SD load for the intact (control) CCL was 520.0 ± 51.3 N and did not differ significantly from the load needed to induce 3 mm of femoral-tibial translation for fixation techniques PPL-SL (422.4 ± 46.3 N) and DDFT-TF (654.2 ± 117.7 N). Results for the DDFT-TF were similar to those of the intact CCL for all outcome measures. The DDFT-TF yielded mechanical properties similar to those of intact CCLs and may be a viable technique to test in vivo.

Treatment of cranial cruciate ligament rupture in the feline stifle. Biomechanical comparison of a standard fabella-tibial suture and lateral sutures placed between quasi-isometric points.

To determine whether a lateral suture placed with bone anchors between quasi-isometric points in a cat is superior to a standard fabella-tibial suture for the stabilization of cranial cruciate ligament (CrCL) rupture compared to an intact stifle joint. Biomechanical cadaveric study. Six stifle joints with intact cruciate ligaments from three skeletally mature cats were placed in a loading mounting set and tested with axial loads of 20N and 60N at three different joint angles (75°,130° and 160°). The procedure was repeated with a transected CrCL; a stabilized stifle joint after a combination of three lateral suture techniques (fabella-tibial suture technique [SFT]; femoro-tibial suture technique 1 [FTS-1] and femoro-tibial suture technique 2 [FTS-2]). Radiographic examination of the relative position of the tibia to the fixed femur was compared. Stabilization of the stifle joint with lateral sutures had comparable stability to the intact specimens in the cranio-caudal direction (p = 0.2) but not in the proximo-distal direction for the SFT (p = 0.04) and FTS-2 technique (p = 0.03). There was no significant difference between the three stabilization techniques (p >0.05). Lateral sutures placed with bone anchors at quasi-isometric points performed better than SFT and FTS-2 in stabilizing the feline stifle after CrCL rupture in the proximo-distal plane. Biomechanical stability in the cranio-caudal plane after placement of a lateral suture across the feline stifle was similar to the intact CrCL.

Ability of the Tightrope® and Percutaneous Lateral Fabellar Suture Techniques to Control Cranial Tibial Translation

To compare the ability of the Tightrope® (TR) cranial cruciate ligament (CCL) technique, percutaneous lateral fabella suture (pLFS) technique, and normal CCL to control cranial tibial translation (CTT). In vitro biomechanical study. Cadaveric canine pelvic limbs (n = 18 pairs). Six small animal surgical residents (1 pair each) and a Diplomate of the American College of Veterinary Surgeons (10 pairs) performed TR and pLFS techniques on paired limbs. Two intact limb pairs served as controls. Limbs were assessed by palpation, radiographs, and dissection before mechanical testing of resistance to CTT. Forces resisted during displacement were compared between groups with a mixed ANOVA and post hoc tests. With 5 mm of displacement, the pLFS resisted 72 ± 45 N and the TR resisted 66 ± 48 N of load. The intact CCL resisted 400 ± 35 N. The intact CCL resisted displacement significantly more than either surgical technique. TR and pLFS had similar ability to resist CTT but neither restored the biomechanical properties of an intact CCL.
