Comparative Study of Injection Triamcinolone and Hyaluronidase Combination with Injection Platelet Rich Plasma (PRP) in Management of Oral Submucous Fibrosis (OSMF).

This study aimed to compare the results of injection triamcinolone and hyaluronidase combination with injection Platelet Rich Plasma (PRP) in management of Oral Submucous Fibrosis (OSMF). Present study was carried out in randomly divided two groups of 30 patients each of OSMF who all are presented with chief complain of reduced mouth opening. Group A patients were given 1ml of injection triamcinolone and hyaluronidase combination. Group B patients received 1ml of injection Platelet Rich Plasma. Both injections were given intralesionally once a week for 6weeks. Results of ANOVA shows significant better results in improving mouth opening in group B patients receiving injection Platelet Rich Plasma as a treatment. In Group A, patients shows improvement in Maximum interincisal distance (MIID) of mean 6.51 ± 1.02mm as compared to the patients in group B shows improvement in MIID of mean 9.53 ± 1.06mm (p value < 0.05). Treatment of OSMF with injection Platelet Rich Plasma is a novel method and found to be more efficient than treatmentwith injection triamcinolone and hyaluronidase combination.

Enhanced Bioactive Potential of Functionalized Injectable Platelet-Rich Plasma

Injectable platelet-rich fibrin (iPRF) is a frequently used platelet concentrate used for various medical purposes both in veterinary and human medicine due to the regenerative potential of hard and soft tissues, and also because of its antimicrobial effectiveness. This in vitro study was carried out to assess the cumulative antimicrobial and antibiofilm effect of iPRF functionalized with a multifunctional glycoprotein, human lactoferrin (Lf). Thus, the ability to potentiate cell proliferation was tested on keratinocytes and evaluated by the CCK8 test. The combinations of iPRF and Lf induced an increase in the proliferation rate after 24 h. The average cell viability of treated cultures (all nine variants) was 102.87% ± 1.00, and the growth tendency was maintained even at 48 h. The highest proliferation rate was observed in cultures treated with 7% iPRF in combination with 50 µg/mL of Lf, with an average viability of 102.40% ± 0.80. The antibacterial and antibiofilm activity of iPRF, of human lactoferrin and their combination were tested by agar-well diffusion (Kirby-Bauer assay), broth microdilution, and crystal violet assay against five reference bacterial strains. iPRF showed antimicrobial and antibiofilm potential, but with variations depending on the tested bacterial strain. The global analysis of the results indicates an increased antimicrobial potential at the highest concentration of Lf mixed with iPRF. The study findings confirmed the hypothesized enhanced bioactive properties of functionalized iPRF against both Gram-positive and Gram-negative biofilm-producing bacteria. These findings could be further applied, but additional studies are needed to evaluate the mechanisms that are involved in these specific bioactive properties.

Comparison of topical 0.03% tacrolimus and homologous injectable platelet-rich plasma in the treatment of keratoconjunctivitis sicca in dogs.

Keratoconjunctivitis sicca (KCS) is predominantly an immune-mediated chronic inflammatory ocular disease that is commonly diagnosed in dogs. This study aimed to compare the conventional use of topical immunosuppressant tacrolimus 0.03% eye drops and a new therapy injectable homologous platelet-rich plasma (HPRP) into the third eyelid gland and inferior and superior palpebral conjunctiva of dogs with KCS. A total of 66 eyes from 33 dogs were evaluated. The eyes were divided into three equal groups: Negative control group, tacrolimus group (TG), and homologous platelet-rich plasma group (HPRPG). The animals were evaluated using the Schirmer's tear test-1 (STT-1), osmolarity test (OT), strip meniscometry test (SMT), tear film break-up test (TBUT), fluorescein test, lissamine green test (LGT), and cytological and histopathological analyses. In TG, there was a significant increase (p < 0.05) in the STT-1 and SMT values, and goblet cell count in the palpebral conjunctiva by the end of the study. In HPRPG, 36% (four dogs) received three applications, 55% (six dogs) received two applications, and 9% (one dog) received one application before the initial ocular signs improved. There was a significant decrease (p < 0.05) in the lymphocyte and neutrophil counts of the palpebral conjunctiva in HPRPG than in TG. Both groups showed equivalent improvements in TBUT, OT, and LGT values. Tacrolimus 0.03% eye drops were more efficient than HPRP in increasing tear production and the number of goblet cells. However, injectable HPRP was more efficient than tacrolimus in decreasing the number of conjunctival inflammatory cells. Treatment with injectable HPRP requires an average of two to three applications, is safe and feasible, and can be used as a cheaper alternative or as an adjuvant to conventional treatment with topical immunosuppressants.

Arthrocentesis, Injectable Platelet-Rich Plasma and Combination of Both Protocols of Temporomandibular Joint Disorders Management: A Single-Blinded Randomized Clinical Trial.

Introduction Osteoarthritis is one of the most common disorders of the temporomandibular joint (TMJ). The complex etiopathogenesis of the temporomandibular disorders (TMDs), and the variability of symptoms make it difficult to adopt standardized therapeutic protocols. Recently, platelet-rich plasma (PRP) injections have been applied into the TMJ in patients with TMJ osteoarthritis. On the other hand, arthrocentesis has received a widespread acceptance, as a minimally-invasive surgical procedure for TMDs. This study aimed to assess and compare the effect of each of these protocols (arthrocentesis, PRP injection, combination of them) in the management of TMJ osteoarthritis. Methods A single-blinded randomized clinical trial was conducted on a sample of 33 participants with limited mouth opening, pain on function and joint sounds due to TMJ osteoarthritis. Participants were assigned randomly into three groups: Arthrocentesis with PRP group; PRP group; Arthrocentesis group. Maximum mouth opening (MMO), pain and joint sounds were re-evaluated during a six-month observation period. Intra- and inter-group comparisons regarding these variables were performed. Results Participants in all study groups showed statistically significant improvement in terms of MMO and pain during the six-month postoperative period (p<0.05), and showed no significant improvements regarding joint sounds (p>0.05). Pain evaluations showed better outcomes in arthrocentesis with PRP group in comparison to arthrocentesis or injectable PRP alone after six months of follow-up (p<0.05). There were no significant differences in the outcomes of MMO and joint sounds evaluations between the three groups after six months. Conclusions Within the limitations of this study, it can be concluded that the three assessed treatment protocols were effective in improving limited mouth opening and pain in patients with TMJ osteoarthritis. A combination of TMJ arthrocentesis and PRP intra-articular injections showed the best outcomes regarding pain symptoms. None of the tested treatment protocols showed improvement in terms of articular sounds.

Scalable and high-throughput production of an injectable platelet-rich plasma (PRP)/cell-laden microcarrier/hydrogel composite system for hair follicle tissue engineering

BackgroundTissue engineering of hair follicles (HFs) has enormous potential for hair loss treatment. However, certain challenges remain, including weakening of the dermal papilla cell (DPC) viability, proliferation, and HF inducibility, as well as the associated inefficient and tedious preparation process required to generate extracellular matrix (ECM)-mimicking substrates for biomolecules or cells. Herein, we utilized gelatin methacryloyl (GelMA) and chitosan hydrogels to prepare scalable, monodispersed, and diameter-controllable interpenetrating network GelMA/chitosan-microcarriers (IGMs) loaded with platelet-rich plasma (PRP) and seeded with DPCs, on a high-throughput microfluidic chip.ResultsThe ECM-mimicking hydrogels used for IGMs exhibited surface nano-topography and high porosity. Mass production of IGMs with distinct and precise diameters was achieved by adjusting the oil and aqueous phase flow rate ratio. Moreover, IGMs exhibited appropriate swelling and sustained growth factor release to facilitate a relatively long hair growth phase. DPCs seeded on PRP-loaded IGMs exhibited good viability (> 90%), adhesion, spreading, and proliferative properties (1.2-fold greater than control group). Importantly, PRP-loaded IGMs presented a higher hair inducibility of DPCs in vitro compared to the control and IGMs group (p < 0.05). Furthermore, DPC/PRP-laden IGMs were effectively mixed with epidermal cell (EPC)-laden GelMA to form a PRP-loaded DPC/EPC co-cultured hydrogel system (DECHS), which was subcutaneously injected into the hypodermis of nude mice. The PRP-loaded DECHS generated significantly more HFs (~ 35 per site) and novel vessels (~ 12 per site) than the other groups (p < 0.05 for each).ConclusionTaken together, these results illustrate that, based on high-throughput microfluidics, we obtained scalable and controllable production of ECM-mimicking IGMs and DECHS, which simulate an effective micro- and macro-environment to promote DPC bioactivity and hair regeneration, thus representing a potential new strategy for HF tissue engineering.

Comparative Study on the Effect of Injectable Platelet Rich Plasma versus its Topical Application in the Treatment of Thermal Burn in Adult Male Albino Rat: Histological and Immunohistochemical Study

Background: Burn is one of the major health problems and several reports have focused on platelet rich plasma role in treatment of soft tissue lesions.Aim:This work aimed to study the histological effect of injectable platelet rich plasma versus its topical application on skin layers (epidermis and dermis) after thermal burn induction in adult male albino rats.Material and Methods: 60 adult male albino rats were divided into three groups; blood donors group, control and experimental group. The latter was subdivided into subgroups I and II. Subgroup II was subdivided into 3 subgroups. Subgroup IIA: animals were exposed to contact thermal burn without treatment. Subgroup IIB: animals were injected subcutaneously with 0.3 ml of autologous PRP. Subgroup IIC: autologous topical PRP was applied. Skin specimens were obtained at days 7, 14 and 28 post burn and involved in histological and immunohistochemical studies. Results: The results of this study revealed that, treatment with PRP resulted in early and enhanced regeneration of skin burn injury. This was evidenced by light microscopic examination and morphometric studies. The PRP treated subgroups were manifested with early re-epithelialization, collagen fibers regeneration and enhanced neovascularization with restoration of normal histological features of skin tissue. Injectable PRP provides slightly better results regarding collagen deposition, fibroblasts infiltration with significant increase in angiogenesis when compared with topical PRP causing high vascular density in wounds.Conclusion: Early interference by PRP injection or topical application enhances wound healing in second degree burn rat model. More future researches are recommended to assess the efficacy of combined injectable and topical PRP in treating burns.

Efficacy of injectable platelet-rich plasma in reducing alveolar bone resorption following rapid maxillary expansion: A cone-beam computed tomography assessment in a randomized split-mouth controlled trial.

To evaluate the effectiveness of platelet-rich plasma (PRP) with its growth factors in minimizing the side effects of rapid maxillary expansion (RME) on the periodontal tissue of anchoring teeth using cone-beam computed tomography (CBCT). A randomized, split-mouth clinical trial was conducted on 18 patients aged 12-16 years (14 ± 1.65) with a skeletal maxillary constriction who underwent RME using a Hyrax appliance. The sample was randomly divided into two groups: intervention and control sides. PRP was prepared and injected on the buccal aspect of supporting teeth in the intervention group. High-resolution CBCT imaging (H-CBCT) was carried out preoperatively (T0) and after 3 months of retention (T1) to study the buccal bone plate thickness (BBPT) and buccal bone crest level (BBCL) of anchoring teeth. Changes induced by expansion were evaluated using paired sample t-test (P < .05). Results showed that there was no significant difference in BBPT and BBCL between the two groups after RME (P > .05). The prevalence of dehiscence and fenestrations was increased at (T1) in both groups and the percentage was higher in the PRP group. RME induced vertical and horizontal bone loss. PRP did not minimize alveolar defects after RME.

Synovial Membrane–Derived Mesenchymal Stem Cells Supported by Platelet-Rich Plasma Can Repair Osteochondral Defects in a Rabbit Model

The aim of this study was to determine the in vivo effectiveness of synovial membrane-derived mesenchymal stem cell (SDSC)-encapsulated injectable platelet-rich plasma (PRP) gel in the repair of damaged articular cartilage in the rabbit. An osteochondral defect was created in the trochlear groove of the rabbit femur, and the defects were divided into 3 treatment groups: untreated control group, PRP group, and PRP-SDSC group. After 4, 12, and 24 weeks, the tissue specimens were assessed by macroscopic examination and histologic evaluation and stained immunohistochemically for type II collagen and proliferating cell nuclear antigen. In addition, total glycosaminoglycan content was determined at 24 weeks. Rabbit PRP contained a high concentration of platelets and high concentration of growth factors compared with those in whole blood. Twenty-four weeks after transplantation, there was fibrous tissue in the control group. In both the PRP group and the PRP-SDSC group, the defects were repaired with hyaline cartilage and exhibited significantly higher safranin O staining, type II collagen immunostaining, glycosaminoglycan content, cumulative histologic scores, and number of proliferating cell nuclear antigen-positive cells. However, incomplete bone regeneration and irregular cartilage surface integration were observed in the PRP group. Our results indicate that SDSC-embedded PRP gel could successfully resurface the defect with cartilage and restore the subchondral bone in the rabbit model. This study indicates that in an animal model, the application of PRP and SDSC in combination for the treatment of local cartilage defects appears promising; however, PRP-SDSC products might be more or less appropriate to treat different types of tissues and pathologies. The clinical efficacy of PRP remains under debate. Therefore further research is needed at both the basic science and clinical levels.
