A STUDY ON BACTERIOLOGICAL PROFILE OF SURGICAL SITE INFECTION AND THEIR ANTIMICROBIAL SUSCEPTIBILITY PATTERN AT A TERTIARY CARE HOSPITAL IN TIRUPATI

Objective: Surgical Site Infections (SSIs) are defined as infections apparent within 30days of an operative procedure and most often between the 5th and 10th postoperative days. It constitutes a major public health problem worldwide; It is one of the most common causes of nosocomial infection. They are responsible for increasing the treatment cost, length of hospital stays and significant morbidity and mortality. Despite the technical advances in infection control and surgical practices, SSI still continue to be a major problem, even in hospitals with most modern facilities. The present study conducted to isolate and identify microbes from pus samples collected from patients who were suspected for SSI and to determine their antimicrobial susceptibility profiles in a tertiary care hospital. Methods: This is a cross-sectional study conducted in the Department of Microbiology, Sri Venkateswara Medical College, Tirupati for a period of 6 mo. A total of 390 various clinical samples were collected and processed. Isolates were tested for antibiotic susceptibility by a Kirby-Bauer disk diffusion method. Results: Out of 390 pus/wound discharge samples processed, 132 (34%) samples were culture positive, among which 107 (81%) were Gram-negative bacilli and 25 (19%) were Gram-positive cocci. Male to female ratio was 2:1. Most common age group affected was>50yrs. Predominant isolates were Klebsiella pneumoniae (73.47%) followed by Pseudomonas aeruginosa (26.53%) and Staphylococcus aureus (17.4%). Gram-negative bacilli showed maximum susceptibility to Imipenam, Piperacillin-Tazobactam and Amikacin and Gram-positive cocci showed 100% to Linezolid, Vancomycin. Conclusion: SSI remains to be a significant challenge for the surgeon’s inspite of sterile surgical techniques and prophylactic use of antibiotics. Hospital infection control measures like hand hygiene, strict adherence to pre-operative measures, rational use of antibiotics and establishing active surveillance can reduce the prevalence of SSIs.

Prevalence and Antimicrobial Susceptibility Pattern of MRSA amongst Patients from an Indian Tertiary Care Hospital: An Eye Opener

Staphylococcus aureus (S. aureus) is a very common human pathogenic microorganism that can cause a variety of infectious diseases, including skin and soft tissue infections, endocarditis, osteomyelitis, bacteremia, and lethal pneumonia. About one-third of the common population is colonized with S. aureus. MRSA is a formidable pathogen known to cause high mortality & morbidity, that poses a significant threat to public health worldwide. Presence of MRSA strains, resistant to multiple antibiotics especially in hospital stay, has complicated the management of infections caused by this bacterium. The aim of this study was to shed light on the prevalence and antimicrobial sensitivity pattern of MRSA among patients in a tertiary care center located in Faridabad, Haryana. This cross-sectional observational study was conducted in the Department of Microbiology, ESIC Medical College & Hospital, a 510 bedded tertiary care teaching hospital in Faridabad, Haryana, India. All wound samples including pus, exudates, wound swab and tissue samples received for aerobic culture and antimicrobial sensitivity from various clinical departments from January 2019 to July 2019 were included in this study. A total of 747 samples were received from January 2019-July 2019. Mean age of this study population was found to be 50.7 ± 14.8 years. Out of 747 samples, 226 (30.25%) were culture positive. Among the S. aureus isolates, methicillin resistance was seen amongst 39 (58.2%). Antibiotic Susceptibility results of S. aureus showed 100% resistance to Penicillin along with 100% resistance to Fluoroquinolones in both MRSA and MSSA. High prevalence of MRSA amongst patients highlights the importance of continued surveillance and implementation of antimicrobial stewardship program to control the menace of antimicrobial resistance. Strict adherence to Infection Control practices its regular follow up to assess the effectiveness of any hospital infection control measures taken is the key.

BACTERIOLOGICAL PROFILE AND ANTIMICROBIAL SUSCEPTIBILITY PATTERN OF SURGICAL SITE INFECTIONS IN A TEACHING HOSPITAL

Objective: Surgical site infections (SSI) are defined as infections occurring within 30 or 90 d after surgical operation or within 1 y, if an implant is left in place after procedure and affecting either incision or deep tissues at the operation site. SSI is one of the quality indicators of the healthcare system. Due to advent of newer antibiotics and emergence of multidrug-resistant bacteria, SSI requires reappraisal of the microbiological flora and in vitro antibiotic sensitivity pattern. The aim of the present study is to determine the incidence etiology of SSI, and determine their Antibiogram. Methods: Cross-sectional study conducted for a period of three months from January 2024 to March 2024. A total of fifty swabs/pus specimens from various types of surgical sites suspected to be infected on clinical grounds were collected from the post-operative wards of surgical departments and processed in Microbiology laboratory. Isolation and identification of bacterial agents were done as per standard protocols. Antimicrobial susceptibility testing was performed by Kirby-Bauer’s disc diffusion method. Results: Out of 50 samples, 32 were culture-positive. 87.50% showed monomicrobial growth and 12.50% showed polymicrobial growth. Analysis of bacterial profile shows Staphylococcus aureus (27.77%) was the predominant isolate, followed by Pseudomonas aeruginosa (19.44%) and Escherichia coli (19.44%). Antibiogram of gram-positive isolates showed sensitivity to Teicoplanin, Vancomycin, and linezolid, whereas gram-negative isolates were sensitive to Meropenem, Piperacillin-tazobactam and levofloxacin. Conclusion: Status of SSI is the main quality indicators of hospital infection control and prevention measures. Continuous surveillance on etiology and antibiogram of SSI is necessary to monitor antimicrobial resistance and guide in empirical treatment.

Carbapenem-Resistant Klebsiella pneumoniae Bacteremia: Counterbalance between the Endemic Load and the Infection Control Program in a Hospital

Carbapenem-resistant Klebsiella pneumoniae (CRKP) remains a significant public health threat, given the associated increased healthcare burden and mortality rate. The objective of the current study was to investigate the association between the incidence of CRKP bacteremia, antibiotic consumption, and infection control measures in a tertiary-care hospital spanning the years 2013–2018. The analyzed indices included the incidence of CRKP bacteremia, antibiotic consumption, the use of hand hygiene solutions, and isolation rates of multidrug-resistant (MDR) carriers. In the total hospital, the incidence of CRKP bacteremia exhibited an absolute decrease during the study period, although this decrease did not reach statistical significance. Antibiotics used to treat CRKP infections, including carbapenems, colistin, tigecycline, and fosfomycin, as well as all classes of antibiotics, correlated positively with an increased incidence of CRKP bacteremia. On the contrary, increased use of scrub disinfectant solutions correlated negatively with a decreased incidence of CRKP bacteremia (IRR: 0.74, 95%CI: 0.59–0.93, p-value: 0.008) in the Adults ICU. Additionally, increased isolation rates of MDR carrier patients correlated negatively with a decreased incidence of CRKP bacteremia (IRR: 0.35, 95%CI: 0.13–0.97, p-value: 0.044). In conclusion, the implementation of multimodal infection control measures in our hospital contributed to the containment of CRKP, particularly in specific hospital sectors. However, the study suggests the need for additional strategies to overcome the endemic plateau.

Emerging challenges in antimicrobial resistance: implications for pathogenic microorganisms, novel antibiotics, and their impact on sustainability.

Overuse of antibiotics is accelerating the antimicrobial resistance among pathogenic microbes which is a growing public health challenge at the global level. Higher resistance causes severe infections, high complications, longer stays at hospitals and even increased mortality rates. Antimicrobial resistance (AMR) has a significant impact on national economies and their health systems, as it affects the productivity of patients or caregivers due to prolonged hospital stays with high economic costs. The main factor of AMR includes improper and excessive use of antimicrobials; lack of access to clean water, sanitation, and hygiene for humans and animals; poor infection prevention and control measures in hospitals; poor access to medicines and vaccines; lack of awareness and knowledge; and irregularities with legislation. AMR represents a global public health problem, for which epidemiological surveillance systems have been established, aiming to promote collaborations directed at the well-being of human and animal health and the balance of the ecosystem. MDR bacteria such as E. coli, Staphylococcus aureus, Pseudomonas aeruginosa, Enterococcus spp., Acinetobacter spp., and Klebsiella pneumonia can even cause death. These microorganisms use a variety of antibiotic resistance mechanisms, such as the development of drug-deactivating targets, alterations in antibiotic targets, or a decrease in intracellular antibiotic concentration, to render themselves resistant to numerous antibiotics. In context, the United Nations issued the Sustainable Development Goals (SDGs) in 2015 to serve as a worldwide blueprint for a better, more equal, and more sustainable existence on our planet. The SDGs place antimicrobial resistance (AMR) in the context of global public health and socioeconomic issues; also, the continued growth of AMR may hinder the achievement of numerous SDGs. In this review, we discuss the role of environmental pollution in the rise of AMR, different mechanisms underlying the antibiotic resistance, the threats posed by pathogenic microbes, novel antibiotics, strategies such as One Health to combat AMR, and the impact of resistance on sustainability and sustainable development goals.

Perceptions and attitudes of inpatients toward infection control measures in Erbil public hospitals

Background and objective: Infection control refers to the measures taken in hospitals and other healthcare facilities to limit and control infections. The primary purpose of infection control is to reduce infection rates. Inpatients must have a perception and attitude about healthcare-associated infections. The aim of this study was to determine the perceptions and attitudes of inpatients and their related factors regarding infection control measures in Erbil public hospitals. Methods: This cross-sectional study was carried out in Erbil public hospitals from September 1st, 2021, to September 1st, 2022). It included 400 patients admitted for different reasons in Rizgary Teaching Hospital, Erbil Teaching Hospital, and Maternity Teaching Hospital. Data were collected during the period (December 15th, 2021) to (March 29th, 2022). Data was gathered through a direct interview using the questionnaire format. Results: The highest percentage of inpatients were females (67.8%), and 60.3% had sufficient income. A high number of inpatients (43.5%) were admitted to the Rizgary teaching hospital. More than half (52.5%) of the inpatients believed they would get an infection if admitted to the hospital. Concerning being aware of healthcare-associated infections, only 16.0% of inpatients were aware of healthcare-associated infections. Most (83.0%) of the inpatients thought that hand washing reduced the chance of infection. Conclusion: The present study indicated that the perceptions and attitudes of inpatients regarding infection control issues in hospitals are low; accordingly, more efforts are needed to empower patients about infection control measures in hospitals.

Economic burden attributable to hospital-acquired infections among tumor patients from a large regional cancer center in Southern China

To evaluate the economic loss of hospital-acquired infections (HAIs) among tumor patients so as to help policymakers to allocate health care resources and address the issue. We conducted a retrospective, 1:1 matched case-control study in a large region cancer hospital between January 1 and December 31, 2022. The economic burden was estimated as the median of the 1:1 pair differences of various hospitalization fees and hospital length of stay (LOS). In this study of 329 matched pairs, the patients with HAIs incurred higher hospitalization cost (ie, $16,927) and experienced longer hospital LOS (ie, 22 days), compared to the non-HAI groups. The extra hospitalization cost and the prolonged hospital LOS caused by HAIs were $4,919 and 9 days, respectively. Accordingly, the direct nonmedical economic loss attributable to HAI was approximately $478 to 835 per case. Furthermore, the increment of hospitalization costs varied by sites of infection, types of tumors, and stratum of age. HAIs lead to the increment of direct economic burden and hospital LOS in tumor patients. Our findings highlight the importance of implementing effective infection control measures in hospitals to reduce the financial burden on tumor patients.

Study on risk factors of mycobacterium tuberculosis infection among health workers in medical institutions

Objective: To understand the infection status of mycobacterium tuberculosis among health workers in tuberculosis designated medical institutions and explore the risk factors of infection. Methods: From September 2021 to June 2022, a questionnaire survey was conducted among health workers in relevant departments of 4 tuberculosis designated medical institutions by cluster stratified sampling, including the implementation of hospital infection control measures in medical institutions and occupational exposure of medical staff to mycobacterium tuberculosis. Peripheral blood interferon gamma release assays (IGRAs) and lung imaging examination were performed to determine the mycobacterium tuberculosis infection. Factors with statistical significance in univariate analysis were included in multivariate logistic regression to analyze the risk factors of mycobacterium tuberculosis infection. Results: A total of 657 people completed the lung imaging examination and questionnaire, of which 654 people had peripheral blood IGRAs detection, and the latent infection rate of tuberculosis was 39.45% (258/654) . Univariate analysis showed that age, sex, marital status, economic income, occupational category, professional title, length of service, and other variables had statistical significances in tuberculosis latent infection (P<0.05) . In terms of personal health status, there were statistically significant differences in the distribution of health workers in terms of their tuberculosis history, tuberculosis history of their immediate family members, previous tuberculin skin test (TST) (P<0.05) . Multivariate analysis showed that there were four risk factors related to tuberculosis, including professional title (X(1)) , years of tuberculosis related works (X(2)) , tuberculosis history (X(3)) and previous TST (X(4)) . The regression equation of the probability of tuberculosis among health workers was y=-1.920+0.246X(1)+0.046X(2)+1.231X(3)+0.478X(4). Conclusion: The latent infection rate of tuberculosis among health workers in tuberculosis designated medical institutions is high. It is necessary to strengthen the management of infection control, carry out regular screening, enhance the self-protection awareness of health workers, and reduce their exposure to mycobacterium tuberculosis and infection risk.

To assess the impact of training about hospital infection control measures related to hemodialysis services on the knowledge of healthcare providers (HCPs) at the teaching institution of Haryana.

Adequate knowledge among nursing professionals toward hospital infection control measures related to hemodialysis (HD) services is essential to decrease infection among patients. This study assessed nursing professionals' knowledge about hospital infection control measures related to HD services and the effect of training interventions. It was a single group, a pre-post-interventional study conducted on nursing professionals of a single apex medical college of Haryana. A pretested questionnaire consisting of two parts was used as a study tool. Various statistical tests such as paired t-test and Pearson's correlation were used. The pre-intervention group had a lower mean knowledge score (mean knowledge = 90.2786, strongly disagree [SD] = 15.52682, standard error [SE] = 1.09518) and then the post-intervention mean knowledge score (mean knowledge = 137.5622, SD = 9.72252, SE = 0.68577). The training program or educational intervention proved very useful for enhancing nursing professional's knowledge about hospital infection control measures related to HD services.

MICROBIOLOGICAL PROFILE OF HIGH TOUCH AREAS OF CRITICAL AND NON-CRITICAL UNITS IN TEACHING HOSPITAL OF MADHYA PRADESH, INDIA

In the current scenario, Nosocomial infections (NI) due to environmental contamination in shared settings of health care, are a serious public health issue affecting millions of people every year worldwide. Therefore, this study aims to create a microbiological profile of high touch areas of Critical and Non-Critical units in Teaching Hospital of Madhya Pradesh. This Cross-sectional Study deals with bacterial (aerobic and anaerobic) surveillance and its antimicrobial susceptibility testing which will aid in enlisting antimicrobial-resistant bacteria and guide us in the use of antimicrobials judiciously. A total of 128 samples were taken from Critical care units and non-Critical units from eight surface locations classified into categories of immediate patient environment and commonly used equipment. These samples were then processed for isolation and identification by standard methods. Out of 128 samples, 28 (21.8%) samples were culture positive for aerobic only and none for anaerobic bacteria. Among which more culture positive samples were isolated from non-Critical units 53.5%, topping the list of contamination were General Medicine ward and Gynecology ward (17.8 %). The majority of isolated microorganisms were Staphylococcus aureus of which 38.4% were MRSA. It showed maximum resistance for Ampicillin (AMP) and was most sensitive to Gentamicin (GEN). Present study highlights that control of Staphylococcus aureus infection (including MRSA) in hospitals is essential. It can be achieved by proper implementation of hospital infection control measures and regular surveillance activity.

Assessing the Impact of Training on Healthcare Providers' Adherence to Infection Control Measures in Hemodialysis Services.

Background and objective Developing and implementing nursing interventions to educate nurses on infection control procedures in hemodialysis units is of utmost importance and offers significant benefits in enhancing the quality of care. This study aimed to assessthe impact of training on nursing professionals' practices of hospital infection control measures inhemodialysis services. The research also intended to explore the potential association between these practices and various sociodemographic variables. Materials and methods Thiswas a single-group, pre- and post-interventional study carried out in Haryana State, India. A pretested questionnaire consisting of 29 statements, the responses of which were measured on a 5-point Likert scale, was used as the study tool. Descriptive and statistical tests like paired-t-test were used to analyze the data. Results The practices section of the questionnaire comprised 29 statements, the responses to which were measured on a five-point Likert scale. The scoring ranged from 5 ("strongly agree", i.e., positive practice) to 1 ("strongly disagree", i.e., negative practice). The maximum achievable score was 145 and the minimum achievable score was 9. The pre-test group (i.e., before training intervention) had a mean practice score of 115.0945 [standard deviation (SD)=9.34, standard error of the mean (SE)=0.66]. However, the post-test group (i.e. after training intervention) had a mean score of 135.26 (SD=8.34, SE=0.59). The study found that structured training significantly increased the mean practice score (t=-33.70, p=0.001). In addition, the study also highlighted the significant association of mean practice scores with various demographic variables among the pre-test and post-test groups. The improvement in mean practice scores among the post-test group after the structured training program reveals that such interventions will ultimately lead to a decrease in central line-associated bloodstream infections (CLABSIs) among hemodialysis patients. Conclusions Our findings showed that the educational intervention led to significant improvements in the practices of the participants.

A longitudinal environmental surveillance study for SARS-CoV-2 from the emergency department of a teaching hospital in Hong Kong

Understanding factors associated with SARS-CoV-2 exposure risk in the hospital setting may help improve infection control measures for prevention. To monitor SARS-CoV-2 exposure risk among healthcare workers and to identify risk factors associated with SARS-CoV-2 detection. Surface and air samples were collected longitudinally over 14 months spanning 2020-2022 at the Emergency Department (ED) of a teaching hospital in Hong Kong. SARS-CoV-2 viral RNA was detected by real-time reverse-transcription polymerase chain reaction. Ecological factors associated with SARS-CoV-2 detection were analysed by logistic regression. A sero-epidemiological study was conducted in January to April 2021 to monitor SARS-CoV-2 seroprevalence. A questionnaire was used to collect information on job nature and use of personal protective equipment (PPE) of the participants. SARS-CoV-2 RNA was detected at low frequencies from surfaces (0.7%, n=2562) and air samples (1.6%, n=128). Crowding was identified as the main risk factor, as weekly ED attendance (OR=1.002, p=0.04) and sampling after peak-hours of ED attendance (OR=5.216, p=0.03) were associated with the detection of SARS-CoV-2 viral RNA from surfaces. The low exposure risk was corroborated by the zero seropositive rate among 281 participants by April 2021. Crowding may introduce SARS-CoV-2 into ED through increased attendances. Multiple factors may have contributed to the low contamination of SARS-CoV-2 at the ED, including hospital infection control measures for screening ED attendees, high PPE compliance among healthcare workers, and various public health and social measures implemented to reduce community transmission in Hong Kong where a dynamic zero COVID-19 policy was adopted.

Financial impact of nosocomial infections on surgical patients in an eastern Chinese hospital: a propensity score matching study

This study, conducted at Zhejiang Taizhou Hospital, China, aimed to examine the financial impact of nosocomial infections on surgical patients. A retrospective case-control study using propensity score matching was conducted over a 9-month period from January to September 2022. The study included 729 surgical patients with nosocomial infections and 2187 matched controls without infections. Medical expenses, length of hospitalization and total economic burden were compared between the two groups. The rate of nosocomial infections in surgical cases was 2.66%. The median hospitalization cost for patients with nosocomial infections was US$8220, compared with US$3294 for controls. The overall additional medical expenditure attributable to nosocomial infections amounted to US$4908. Notable median differences were observed between cases with nosocomial infections and controls in terms of total hospitalization cost, nursing services, medication, treatment, materials, test fees and blood transfusion fees. In each age group, medical costs for patients with nosocomial infections were more than twice those of controls. Additionally, hospital stays for surgical patients with nosocomial infections were, on average, 13 days longer compared with controls. These findings highlight the importance of implementing effective infection control measures in hospitals to reduce the financial burden on patients and the healthcare system.

Updates on the Status of Carbapenem-Resistant Enterobacterales in Lebanon.

Carbapenem-resistant Enterobacterales (CRE) pathogens have been increasingly isolated and reported in Lebanon. Several studies have been published over the last two decades about the CRE situation in the country. However, compared to the worldwide data, those studies are scarce and mostly restricted to single center studies. In this review, we aim to present a comprehensive and reliable report illustrating the current situation regarding CRE in Lebanon. Variable studies have shown an increasing pattern of carbapenem resistance in Enterobacterales since the first reports of CRE isolates in 2007 and 2008. Escherichia coli and Klebsiella pneumoniae were the most detected ones. The OXA-48 class D carbapenemases were the most prevalent carbapenemases among CRE isolates. Moreover, the emergence of other carbapenemases like the NDM class B carbapenemase has been noticed. Strict infection control measures in hospitals, including the identification of CRE carriers, are needed in Lebanese hospitals since carriage is a potential risk for the spread of CRE in healthcare settings. The dissemination of CRE in the community is noticed and attributed to multiple causes, such as the refugee crisis, water contamination, and antimicrobial misuse. In conclusion, strict infection control measures in healthcare settings, in addition to accurate antimicrobial stewardship program implementation, are urgently needed.

Whole-genome sequencing of Candida haemulonii species complex from Brazil and the United States: Genetic diversity and antifungal susceptibility.

Candida haemulonii complex species can be multidrug-resistant and cause infections such as candidemia. This study determined the genetic relationship between isolates from Brazil and the United States through whole-genome sequencing and performed antifungal susceptibility testing to investigate drug resistance. Contrary to what is widely described, most isolates were susceptible to azoles. However, an atypical susceptibility profile was found in 50% of Candida pseudohaemulonii strains, including resistance to the three echinocandins. Isolates from both countries formed distinct clusters with wide genetic diversity. Isolates from three hospitals in Brazil were clonal and involved in candidemia cases, pointing to the importance of improving hospital infection control measures and molecular identification.

Infection control knowledge and practices among radiographers at government hospitals in the Gaza Strip-Palestine: A cross-sectional study

IntroductionInfections acquired in hospitals are a major concern for patients, professionals and policymakers. They have an impact on the morbidity and mortality rates, length of stay, and microbial resistance. Radiology departments are at high risk for nosocomial infections, and radiographers must adhere to infection control protocols to avoid contracting illnesses and the spread of pathogens. The purpose of this study was to assess radiographers' knowledge and practise of infection control and standard precautions in the Gaza Strip-Palestine government hospitals and to identify the factors that impede radiographers' implementation of infection control. MethodsA hospital-based descriptive cross-sectional study design was applied. A self-administered questionnaire survey with 24 items was developed and distributed to examine radiographers' knowledge and practise of nosocomial infections control and standard precautions from September 2019 to February 2020. Descriptive and inferential statistics were generated using SPSS version 20. ResultsWith a response rate of 86.6%, 73 males and 37 females out of a total of 127 radiographers participated in this study. The vast majority of radiographers, 86 (78.2%), have never been trained in infection control. Total levels of knowledge and practise were 74.4% and 65.2%, respectively which corresponded to moderate levels. Age had a statistically significant impact on both knowledge and practise scores (P = 0.002 and p = 0.019, respectively). In addition, the differences between radiographers' years of work experience and their ratings on knowledge and practise were statistically significant (P = 0.001 and p = 0.011, respectively). A heavy workload, insufficient time, and a lack of training were the main barriers to implementing infection control measures in hospitals. ConclusionPalestinian radiographers reported a moderate level of knowledge and practise of infection control. The majority of radiographers have never had formal infection control training. Implications for practiceThis paper has highlighted the need for a continuing education and training programme for practising radiographers to improve their performance in infection control measures.

Impact of the COVID-19 pandemic on hematopoietic stem cell transplant programmes in Singapore.

Hematopoietic stem cell transplantation (HSCT) has been performed in Singapore since 1985. Currently, more than 100 transplants are performed annually across the public and private sectors. In 2020, the COVID-19 pandemic resulted in unprecedented disruptions to global healthcare systems, and Singapore was no exception. In particular, the field of HSCT faced additional, unique challenges aside from those borne by the healthcare system at large, and appropriate measures were necessary to ensure that HSCT remained available to patients who needed it. The expert opinions of six hematologists from various institutions across Singapore were gathered through individual interviews and summarized. This was supplemented by a literature review on bone marrow donation and HSCT in Singapore. In Singapore, the COVID-19 pandemic has had significant implications for HSCT, ranging from the implementation of additional infection control measures in hospitals to an accelerated rise in haploidentical transplants. Further research is required to better understand and quantify these impacts, improve existing processes, and investigate the effects of COVID-19 and its treatment modalities on patients with HSCT.

Bacterial Profile and Their Antimicrobial Susceptibility Pattern of Isolates Recovered from Intensive Care Unit Environments at Wachemo University Nigist Ellen Mohammed Memorial Comprehensive Specialized Hospital, Southern Ethiopia.

Bacterial contamination of indoor hospitals, especially in intensive care units is a serious health hazard in the world with a high morbidity and mortality rates. Particularly, multidrug-resistant bacteria can cross-contaminate medical devices, inanimate surfaces, health care providers, and patients in the intensive care unit. This study was aimed to assess the bacterial profile and their antimicrobial susceptibility patterns of bacterial isolates from intensive care unit at WUNEMMCSH (Wachemo University Nigest Ellen Mohammed Memorial Compressive Specialized Hospital), Southern Ethiopia. A hospital-based cross sectional study was conducted on 180 intensive care unit environmental samples at WUNEMMCSH from August 1, 2022, to October 30, 2022. In this study, a total of 180 swab samples were collected from medical devices, inanimate surfaces, patients, and health care providers from the intensive care unit by using sterile cotton-tipped swabs moistened with normal saline. Then, bacterial isolates were identified using the standard culture method, Gram stain, and biochemical tests. Antimicrobial susceptibility tests for bacterial isolates were performed by using the Kirby Bauer disk diffusion method. Data were entered into EpiData Version 4.6 cleanup and exported to SPSS V25 for analysis. A total of 180 swab samples were processed from intensive care unit environments, and 143 (79.4%) were found to have been contaminated by at least one potential pathogenic bacterial isolate. A total of two hundred and thirty-eight bacteria were isolated. Of these, the predominant bacteria were coagulase-negative Staphylococci 72 (30.3%), S. aureus 61 (25.6%), E. coli 41 (17.2%), and K. pneumoniae 30 (12.6%). Seventy (49%) out of all swabbed samples were contaminated with mixed isolates. In the antimicrobial susceptibility tests, 19 (86%) Gram-positive bacteria and 25 (76%) Gram-negative bacterial isolates were susceptible to nitrofurantoin, respectively. Vancomycin was sensitive to 83% of Gram-positive isolates. Gram-positive and Gram-negative isolates from irrespective sources showed multidrug resistance in 56.4% and 76.2%, respectively. The inanimate hospital environments, medical device, health care providers, and patients in ICU rooms of WUNEMMCSH (Wachemo University Nigest Ellen Mohammed Memorial Comprehensive Specialized Hospital) were colonized with 143 (79.4%) of potential pathogenic bacterial isolate, which can cause nosocomial infections with high rates of morbidity and mortality among patients. The frequencies of multidrug-resistant 159 (66.8%) bacterial pathogens were alarmingly high. Therefore, to reduce the risk of bacterial contamination and MDR, strict adherence to hospital infection prevention and control measures should be enforced. These measures include regularly performing hand hygiene, periodic disinfection, and sterilization of medical equipment.

Knowledge Of Clinical And Paraclinical Personnel In Rural Dental Hospital About Infection Control Regimen

Background : Infection control includes protocol to prevent infection in dental clinical setting. Knowledge of dental health worker plays an important role in infection control in dental set up. Aim : To evaluate the knowledge of clinical and paraclinical personnel about infection control regimen. Methodology : Present study included 300 participants, out of which 260 were clinical and 40 were paraclinical personnel. Questionnaire included 8 questions based on the knowledge related to hand hygiene, disinfection, sterilization an needle stick injury. Likert scale was used for assessing their responses. Results: There is a significant difference between the clinical and para-clinical personnel regarding Knowledge about infection control practices. Conclusion : This study shows that knowledge of clinical and paraclinical personnel about the infection control measures in dental hospital is limited with definite scope for improvement.
