Laparoscopic Excision of Urachal Cyst

The treatment of choice for persistent urachal anomaly is radical surgical excision intended to prevent complications, notably malignancy in a retained remnant. We report a case of the radical laparoscopic excision of an infected urachal cyst. This effective alternative to an open technique has the advantages inherent in minimally invasive surgery.

Management of urachal anomalies in children and adults

During a period of fifteen years, 9 children and 5 adults with a variety of urachal anomalies were treated. Infected urachal cysts were more common in children whereas adults more frequently had infected urachal sinuses. After careful physical examination, a preoperative diagnosis could be made in most patients, with ultrasound examination decisive in doubtful cases. Whenever feasible, complete excision of the umbilicovesical tract is performed, but in very ill patients, a staged treatment becomes necessary. The preoperative injection of methylene blue is helpful in the identification of communicating tracts, all of which should be removed. All affected children should undergo investigation for associated genitourinary anomalies.

LOCALIZED PERITONITIS DUE TO PERFORATION OF INFECTED URACHAL CYST DIAGNOSED BY ULTRASONOGRAPHY

Acute peritonitis due to a perforation of infected urachal cyst is rare and 9 cases including this case have been reported in Japan. A 41-year-old male was seen at the hospital because of lower abdominal pain with a low grade fever. A 3 cm paintful tumor was palpated approximately on the median of the lower abdomen but no peritoneal irritating sign was noted. Ultrasonography (US) revealed a possible tumor arising in the urachal cyst localized in the extraabdomen beneath the rectus abdominis muscle. CT, however, pointed out a low density area under the rectus muscle but it was unclear whether the mass was localized in the intraabdomen or extraabdomen. In the course of conservative treatment, peritoneal irritating sign and fever appeared. US was performed again, and acute peritonitis due to perforation of the infected urachal cyst was suspected. Total urachectomy was carried out. Postoperative course was uneventful and the patient was discharged from the hospital.Generally infected urachal cyst is treated by antibiotics followed by radical operation of an one-step approach. During this preoperative period US would be very useful to observe the infected urachal cyst with a rare possibility of perforation into the abdomen.

The sonographic appearances of urachal anomalies are illustrated by six patients with persistent urachus (n = 1), infected persistent urachus (n = 1), urachal sinus (n = 1), infected urachal cyst (n = 2) and urachal diverticulum (n = 1). The anomalies were diagnosed sonographically in every case. Symptomatic patients were treated surgically.

Infected urachal cysts present with protean clinical manifestations. Diagnosis is frequently difficult in spite of the use of modern genitourinary radiologic imaging techniques. Patients with intra-abdominal or pelvic symptoms are frequently misdiagnosed. Surgical excision is the treatment of choice for infected urachal cysts.

Advances in the Management of Infected Urachal Cysts

Persistent urachal remnants are uncommon congenital anomalies. Unless an umbilical fistula exists, infection may be the first indication of this abnormality. Five children received initial treatment for this problem at the Children's Hospital of Buffalo during a 20-year period, 1964 to 1984, and a sixth was seen secondarily. There were four boys and two girls; their ages ranged from 8 months to 9 years. Lower abdominal mass with fever and local tenderness were the most common presenting signs. Ultrasound was the most accurate study, correctly diagnosing the cyst in both patients so examined. Incision and drainage alone was performed in one patient. The other five were managed with antibiotic therapy and complete excision as the primary procedure. Cultures were obtained in five patients and were positive in four, growing Staphylococcus aureus in three and Escherichia coli in one. Significant genitourinary abnormalities were discovered in four of the five patients evaluated. It is concluded that the previously recommended therapy of incision and drainage followed by delayed resection was developed in the preantibiotic era to minimize the mortality from sepsis and the morbidity from recurrence. Our experience indicates that the use of appropriate antibiotics followed promptly by complete cyst excision as a primary procedure is both possible and safe in most cases. Furthermore, the large number of associated genitourinary abnormalities suggests that a complete work-up for these conditions should be performed.

Peritonitis due to Intraperitoneal Perforation of Infected Urachal Cysts

Acute bacterial peritonitis due to intraperitoneal perforation of an infected urachal cyst represents a potentially lethal complication of the rare anomalies of the fetal urachus. We report four cases of this disease, including what we believe is the first to have been correctly diagnosed prior to operation. All four patients underwent emergency laparotomy and curative excision of the urachal remnant. Review of our four cases plus seven previously reported cases disclosed the perforated urachal cyst to be predominantly a disease of young males who initially have acute peritonitis but often without specific indication of a urachal anomaly. Bacteriologic examination has identified a wide variety of infecting organisms. Umbilical sinography or abdominal ultrasonography may allow accurate preoperative diagnosis. Broad-spectrum antibiotic coverage and laparotomy are indicated and excision of the entire urachal remnant is consistently curative.

Infected urachal cysts.

Diseases of the urachus are uncommon clinical entities, evidenced by the relatively small number of case reports in the literature. Since 1550 approximately 400 cases of urachal disease have been reported, of which more than 200 were of infections. However, many of these cases are poorly documented or unproved. Thus, in his recent review, Sterling⁴was able to collect only 26 proved cases of infected urachal cyst with onset of symptoms after the age of 5 years. It is the purpose of the present paper to review the embryology, pathogenesis, and clinical features of infected urachus cysts and to add six cases to the literature. <h3>Embryology</h3> In order to achieve adequate insight into urachal disease, brief reference must be made to the embryology of this structure.
