Inclusion of Oral Health in Disabled Patients. Legal Perspective

Background: People with disabilities face obstacles to living full and healthy lives. Purpose: To identify, organize and analyze documents that account for the conditions and health care and the rights of people with disabilities in the United States of Mexico. Methods: A search of references was conducted in different databases using terms such as: oral health, human health, dentistry, medical practice, disability, and rights of people with disabilities. Documents that account for aspects such as standards and institutions that are related to health, particularly the health and rights of people with disabilities, were selected. Conclusions: Disability has evolved both in the concept and in the measures and mechanisms of care in the Mexican legal system. There are documents and legislation that address the issue. However, there are omissions and deficiencies that should be corrected to guarantee full and equitable access to health care for people with disabilities.

Advancing oral health policy through persuasive messaging and effective research measures.

Oral health is often excluded from broad health policy discussions, addressed only in a policy silo. There is a paucity of research to guide policymaking as it relates to oral health. In response, AcademyHealth's Oral Health Interest Group organized a meeting during the 2019 AcademyHealth Annual Research Meeting to promote transdisciplinary dialog on the current state of oral health policy and the steps necessary to improve the oral health of Americans. This article summarizes the two main themes that emerged from the proceedings of the Oral Health Interest Group meeting: a) recommendations for advocating the inclusion of oral health in policy discussions and b) critical research topics and measures needed for effective oral health policies in the future.

Prevention and management of periodontal diseases and dental caries in the older adults.

As a result of aging populations, in the future, dental practitioners will be caring for more older adults than ever before. These older adults, especially in developed countries, will demand a greater number of dental services, driven by increased tooth retention and an expectation of excellent oral healthcare throughout the life course. Further, the global rise in the prevalence and incidence of chronic diseases will increase the risk and/or severity of oral diseases and add a layer of complexity to the management of oral diseases in older adults. More older adults will be at a higher risk of periodontal disease and root caries as a result of reduced tooth loss and edentulism. This article reviews information on periodontitis and root caries,oral diseases which reflect the cumulative risk of the individual, and which are best addressed through prevention. Oral healthcare providers must embrace the concept of lifelong emphasis on prevention, as well as participation as active members of a healthcare team which provides healthcare for older adults in various settings (eg, hospital/clinic-based care, community-based settings, and long-term care facilities). National guidelines that address oral health are being considered by some countries, and if these are implemented they will increase the accessibility to oral health for older adults. In parallel to this, revisions of existing older adult insurance schemes (eg, the inclusion of routine oral healthcare in the US Medicare program) would promote the maintenance of a functional dentition that is pain-free and conducive to general health. The opportunity exists to implement a holistic approach to oral health that will align oral health with general health and emphasize that true health can only be achieved with the inclusion of oral health.

Health invest: A venture that seldom fails

India has a vibrant culture of customs and beliefs, yet the perceptions on health care remains consistent. Health-care delivery systems hinge onto the general health problems, unfortunately, overlooking the oral health. Most often, people seek dental care, but only in emergencies. Inclusion of oral health in the general health policies would initiate in the people of our country, a habit of regular dental visits. The dentists through these routine screening visits, could then not only promote preventive measures but also detect disease at the onset itself, halting its further progress. The awareness regarding oral health thus generated, could reach out to even rural India. This, in turn, improves the well-being of the society. This short communication aims at highlighting the need of oral health policies in India that would pave a way for a more healthy India.

Salud oral en el Ecuador. Perspectiva desde la salud pública y la bioética

El estado ecuatoriano consagra la salud como un derecho funadamental a través de su consitución, para garantizar este derecho ha implementado múltiples estrategias de salud pública a través del Ministerio de Salud. Sin embargo, la inclusión de la salud bucal en estas políticas es una deuda sanitaria pues poco ha sido articulada en estas iniciativas. El presente artículo pretende hacer un resumen de las políticas en salud pública odontológica consideradas por los autores como un eje importante en la cobertura de servicios de la salud en el país. Además considerar que posiblemente la ausencia de información actualizada sobre el estado de salud bucal de los ecuatorianos, contribuye directamente en falta de implementación de nuevas intervenciones basadas en evidencia científica que apoye la toma de decisiones informadas.

Dental providers and pharmacists: a call for enhanced interprofessional collaboration

Reports concerning medication discrepancies in dental records indicate that the concept of interprofessional collaboration between the dental team and pharmacists should be considered at all educational levels in dentistry and pharmacy. Inclusion of oral health as a therapeutic area in didactic pharmacy curricula is needed. Early exposure of dental students and student pharmacists to collaborative practices through interprofessional educational experiences may create a higher degree of awareness of the role of each profession and the potential to improve patient outcomes. Furthermore, efforts are needed to develop a systematic approach for medication review and reconciliation in dental practice to obtain accurate medication lists, potentially by utilising health information technology.

Managing Oral Health: Knowledge and Perceptions of California Advanced Practice Registered Nurses

In 2000, the United States (U.S.) Surgeon General's Report identified dental disease as a silent epidemic and linked an individual's oral health to their overall health and well-being [1]. Shortly thereafter, a public-private partnership, A National Call to Action to Promote Oral Health, was developed for the purpose of integrating efforts to facilitate improvement of the nation's health through oral health activities [2]. Subsequent reports, as well as the inclusion of oral health as one of the Healthy People 2020 Leading Health Indicators continue to affirm the significance of oral health as an important population health matter [3-5]. Although oral health has improved for many over the past decade, oral diseases remain prevalent across the country, particularly among vulnerable and underserved populations, including poor children, racial and ethnic minorities, and older adults.

Reviewing public policy for promoting population oral health in Victoria, Australia (2007-12).

Government policy and planning set the direction for community decisions related to resource allocation, infrastructure, services, programs, workforce and social environments. The aim ofthe present study was to examine the policy and planning context for oral health promotion in Victoria, Australia, over the period 2007-12. Key Victorian policies and plans related to oral health promotion in place during the 2007-12 planning cycle were identified through online searching, and content analysis was performed. Inclusion of oral health (and oral health-related) promotion initiatives was assessed within the goals, objectives and strategies sections of each plan. Six of the 223 public health plans analysed (3%) included oral health 'goals' (including one plan representing nine agencies). Oral health was an 'objective' in 10 documents. Fifty-six plan objectives, and 70 plan strategies related to oral health or healthy eating for young children. Oral health was included in municipal plans (44%) more frequently than the other plans examined. There is a policy opportunity to address oral health at a community level, and to implement population approaches aligned with the Ottawa Charter that address the social determinants of health.

Saúde bucal na rede de atenção e processo de regionalização

The scope of this article is to outline the construction of the regionalized health network, focusing on oral health care and identifying the problems and the progress in its implementation. The context was the proposed national network model in the policies that underpin the Unified Health System. This is a descriptive-exploratory study with a qualitative approach methodologically supported by Grounded Theory. Interviews were conducted with managers, health professionals and teachers graduated in Dentistry, forming three sample groups. The core category was named "Enhancing the interaction of oral health care in the health network in the regionalization process." In the proposed theoretical model, the enhancement of interaction arises from conditions such as: the characterization of oral health care in the regionalized health network; the identification of difficulties in the implementation process; and the analysis of the operation of the oral health care network. The inclusion of oral health in the network is at an early stage, sometimes detached from the set of relations that constitutes the health network, which is also being consolidated. Evidence gathered in this study indicates the structuring of the regionalized health care network as a prerequisite to raise oral health care and assistance to a new level.

Development of an Interprofessional Diabetes and Oral Hygiene Education Program for Youth with Type 2 Diabetes Mellitus

BackgroundThe Diabetes Education and Resource for Children and Adolescents (DER-CA), Children's Hospital, Winnipeg, Manitoba and the University of Manitoba, School of Dental Hygiene (SDH/UM) developed an oral hygiene component to add to the education and surveillance of youth with type 2 diabetes mellitus and their families. ObjectiveThis article describes the 4-year experience of this interprofessional collaboration and the effective “fit” for the dental hygiene screening and education as part of the DER-CA diabetes care, education and support. MethodThe iterative process of the interprofessional collaboration, pilot run and implementation of the curriculum is described. ResultsThe partnership has resulted in the development of a dental hygiene student experiential interprofessional education program, the inclusion of oral health with diabetes education for the youth of the DER-CA, and the development of a collaborative relationship between the DER-CA and SDH/UM. Four graduating classes of dental hygiene students (n=104) have participated in the program. Over 280 youth (6 to 18 years of age) with type 2 diabetes have had oral health screening and education. ConclusionsThe interprofessional collaboration between the DER-CA and SDH/UM is a successful innovative partnership informed by the evidence-base in interprofessional education and type 2 diabetes and oral health that integrated an oral hygiene component in the surveillance and education to youth with type 2 diabetes.

Constructing public oral health policies in Brazil: issues for reflection

This paper addresses the construction of public oral health policies in Brazil by reviewing the available literature. It includes a discussion of the social responses given by the Brazilian State to oral health policies and the relationship of these responses with the ideological oral health movements that have developed globally, and that have specifically influenced oral health policies in Brazil. The influence of these movements has affected a series of hegemonic practices originating from both Market Dentistry and Preventive and Social Dentistry in Brazil. Among the state activities that have been set into motion, the following stand out: the drafting of a law to regulate the fluoridation of the public water supply, and the fluoridation of commercial toothpaste in Brazil; epidemiological surveys to analyze the status of the Brazilian population's oral health; the inclusion of oral health in the Family Health Strategy (Estratégia de Saúde da Família - ESF); the drawing up of the National Oral Health Policy, Smiling Brazil (Brasil Sorridente). From the literature consulted, the progressive expansion of state intervention in oral health policies is observed. However, there remains a preponderance of hegemonic "dental" practices reproduced in the Unified Public Health Service (Sistema Único de Saúde - SUS) and the Family Health Strategy.

Processo de trabalho em saúde bucal: em busca de diferentes olhares para compreender e transformar a realidade

In 2000, the Ministry of Health has established financial support and included Oral Health Teams in the Family Health Strategy, which is recognized as an approach to reorganize the Primary Care Services in the Brazilian Health System. The professionals are inserted in a new context that confronts their conceptions and practices. The aim of this qualitative study was to analyze the working method in oral health at the Family Health Strategy (FHS) in a southern city of Brazil. It was performed by semi-structured interviews applied to health professionals in five primary care unities where Oral Health and Family Health teams work with a multiprofessional sample of 22 individuals. Thematic analysis of categories was used to analyze data which showed five thematic points: inclusion of Oral Health in FHS, structural aspects of the services, professional concepts about the health-disease process, working method, and improvement strategies. The research contributes to the comprehension of the working method in Oral Health in FHS and indicates some possible ways to modify practices.
