SIGMOID VOLVULUS COMPLICATING CAESAREAN SECTION – REVIEW & BRIEF CASE REPORT

Sigmoid volvulus in pregnancy is a very rare clinical situation responsible for high rates of mortality and morbidity in both mother and fetus. It is due to the gravid uterus which itself clouds the clinical picture of sigmoid volvulus.A long and redundant sigmoid colon compressed by enlarged gravid uterus, might explain increased incidence of sigmoid volvulus during pregnancy especially during third trimester. This article describes a case with 36 weeks of gestation where sigmoid volvulus was encountered during caesarean section.

Incidence of Sigmoid Volvulus in Northern Uganda. An Observational Study

Background: Sigmoid volvulus (SV) has a considerable geographical variation in its incidence. The purpose of this study was to determine the incidence of SV in Northern Uganda.Methods:A two year retrospective and one year prospective study in 19 hospitals in Northern Uganda from January 2010 to December 2012 was conducted to determine the incidence of sigmoid volvulus. All patients’ records with a diagnosis of sigmoid volvulus were included in the study. Ethical approval was obtained from the IRB Gulu University and Uganda National Council for Science and Technology (UNCS&T). Data analysis was conducted using STATA/IC version 12.1.Results:The incidence of SV in Northern Uganda was 251.8 per 100,000 surgical population in 2 years. Cases were least observed from May to November and most cases were seen in the dry season from December to April. Conclusions:The incidence of SV in Northern Uganda was 251.8 per 100,000 surgical population in 2 years. The proportion of bowel obstructions due to sigmoid volvulus in Northern Uganda was 23.4% and similarly comparable with the proportion found in other African countries and higher than those indeveloped countries. Sigmoid volvulus in the high altitude of the Andes. Review of 230 cases.

Sigmoid volvulus (SV) is uncommon in the United States. Little has been published in the English literature about the high incidence of SV among rural areas of the Bolivian and Peruvian Andes at 13,000 feet above sea level. A review of 230 cases of SV in a Bolivian hospital is presented. SV accounted for 79 percent of all intestinal obstructions. Nonoperative reduction was attempted in all patients except those with peritonitis. Nonoperative reduction alone was performed in 31 percent of the patients, and 69 percent underwent surgical intervention, 66 percent as an emergency and 3 percent electively. Surgical treatment consisted of sigmoidectomy and primary anastomosis (50 percent), Hartmann's procedure (12 percent), and operative detorsion with sigmoid plication (38 percent). Overall mortality was 13.5 percent. Fifty-seven of the surgically treated patients developed significant complications. The etiology of SV is unclear. High altitude, along with other etiologic factors, may play an important role in SV. To our knowledge, this series represents the highest incidence of SV in bowel obstruction.

Sigmoid volvulus in Kaduna, Nigeria

Worldwide regional variations exist in the incidence of sigmoid volvulus. This article describes the experience with sigmoid volvulus at the Ahmadu Bello University Teaching Hospital in Kaduna, Nigeria. Sixty of 121 patients who had large-bowel obstruction over a 15-year period had sigmoid volvulus. Fifty-seven (95 percent) had gangrenous bowel. The mortality rate was 16.7 percent. The clinical presentation and the treatment modalities offered are discussed.

Surgical management of volvulus of the sigmoid.

As the population ages, sigmoid volvulus appears to be occurring more frequently. This entity has always been common in the Eastern European countries, accounting for 30% to 50% of all intestinal obstructions.1Varying series in the American literature have placed the incidence of sigmoid volvulus between 1% and 8% of intestinal obstructions.2-5There has been an increase in the number of reported case series in recent years.6,7 The exact cause of this condition is unknown, but it is predisposed by several anatomic features, the most important being a redundant sigmoid colon, either congenital or acquired. A high residue diet with a large amount of roughage and chronic constipa-tion may be factors contributing to the redundancy. A second feature leading to volvulus is an afferent and an efferent limb that lie close to each other while its mesocolon is both long and freely movable.
