Chinese expert consensus on the diagnosis and management of infectious endophthalmitis after ophthalmic surgery (2022)

Infectious endophthalmitis after ophthalmic surgery refers to postoperative endophthalmitis associated with pathogen infection. The incidence of infectious endophthalmitis after ophthalmic surgery has been reduced, but there are still many controversial issues in its diagnosis, treatment and prevention. The Chinese Vitreo-Retina Society of Chinese Medical Association, together with domestic experts in cataract, trauma and glaucoma, has synthesized all the current research evidences available at home and abroad and reached consensus opinions after careful discussion, in order to provide reference for preventing and managing infectious endophthalmitis after ophthalmic surgery safely and effectively, and thus to improve its cure rate and patients' visual function.

Intraocular lens removal or not during vitrectomy for acute infectious endophthalmitis after cataract surgery.

At present, the incidence of infectious endophthalmitis after cataract surgery has been significantly reduced, but it is still a serious complication. Removal or not of the intraocular lens (IOL) during vitrectomy in cases with a moderate or severe inflammation is controversial. In order to call upon more discussion, we publish the article entitled "Timely vitrectomy without intraocular lens removal for acute endophthalmitis after cataract surgery" written by Guo et al in this issue. With recent advanced vitrectomy techniques, and critical measures for management of risk factors related to occurrence of infection, IOL remaining during timely vitrectomy for acute endophthalmitis can possibly be safe and effective in selected cases.

INCIDENCE AND RISK FACTORS OF POSTOPERATIVE ENDOPHTHALMITIS AFTER PRIMARY SURGICAL REPAIR COMBINED WITH INTRAOCULAR FOREIGN BODY REMOVAL.

To analyze the incidence and risk factors of postoperative endophthalmitis after primary surgical repair and intraocular foreign body (IOFB) removal within 24 hours of injury. The records of all patients treated surgically for open globe injury and IOFB removal at the Eye Hospital of Shandong First Medical University between January 1, 2015, and June 30, 2020, were retrospectively reviewed. Variables included time from injury to operation, cause of injury, details of surgical repair, and follow-up. The incidence and risk factors of endophthalmitis after IOFB removal were studied. During 5 years, 99 patients with IOFB were reviewed. Of these, 19 patients were diagnosed with endophthalmitis on admission, and 5 were suspected of having endophthalmitis during operation. Fifty-four cases had no clinical signs of endophthalmitis on admission and during operation and were treated with operation within 24 hours after the injury. Two patients (2 of 54; 3.70%) developed endophthalmitis after IOFB removal, and the causative agent in both cases was Bacillus cereus. The incidence of infectious endophthalmitis after primary surgical repair combined with IOFB removal (≤24 hours) was 3.70% in patients who received a series of standard treatments, and B. cereus infection might be a risk factor.

The prevention of infectious endophthalmitis after intraocular foreign body injury in Tibet high altitude district

Objective To evaluate the efficacy of early intravitreal Vancomycin injection for prevention of infectious endophthalmitis after intraocular foreign body injury. Methods One hundred and five eyes of 105 cases with intraocular foreign body injury were divided into two groups. Fifty-five cases were recruited in research group, before suturing the eyeball wound, the foreign body was attracted with magnet firstly, then intravitreal Vancomycin(1 mg) injection was performed and the broad spectrum antibiotics was used after the operation. Some cases failed to attract the foreign body were transferred to another hospital for vitrectomy. In control group, a similar treatment except for intravitreal Vancomycin injection was carried out in 50 cases. The incidences of infectious endophthalmitis between the two groups were compared. Results After operation, infectious endophthalmitis occurred in 3 cases(5.45%) in research group (55 cases). infectious endophthalmitis occurred in 9 cases(18.00%) in control group (50 cases). The difference in incidence of infectious endophthalmitis between the two groups was statistically significant (χ2=4.072, P<0.05). Conclusion Early intravitreal Vancomycin injection can reduce the incidence of incidence of infectious endophthalmitis after intraocular foreign body injury. Infectious Endophthalmitis Following Sutureless Cataract Surgery-Reply

During the past 10 years, we have treated three patients with infections associated with sutureless cataract surgery, and this increase prompted us to report our findings. As mentioned in the original article, the incidence of infectious endophthalmitis following sutureless surgery vs sutured incisions is unknown. Have other postoperative complications associated with sutureless surgery increased? To date, no reports are available regarding these data. Drs Williams, Gills, Grabow, Martin, Ernest, and McFarland suggested an infectious endophthalmitis incidence of 0.015% (one of 6795) following sutureless cataract surgery. We are encouraged that these authors are reviewing their series of 27181 patients to report both positive results in terms of rapid visual rehabilitation and reduced astigmatism in addition to complications, including filtering blebs, significant hyphemas, iris prolapse, flat chambers, and infectious endophthalmitis, as suggested by Dr Colloton.
