Slow Stroke Back Massage Therapy to Reduce Acute Pain in Elderly with Hypertension: A Case Study

Background: Hypertension causes increased vascularization of blood vessels to the brain. This condition will affect the pressure on the nerve fibers in the neck muscles, so that the patient will feel discomfort in the head and nape. The incidence of acute pain in hypertensive patients at the Babelan I Health Center in 2022, namely patients with hypertension who experience moderate acute pain (scale 4-6 NRS) is as many as 16 people (64%). Purpose: Knowing the application of SSBM therapy to reducing acute pain in the elderly with hypertension at Melati room in UPT PSTW (nursing home) Jember. Method: This final scientific work uses the case report method which describes the case of Mrs. J with acute pain nursing problems. Nursing care was provided comprehensively for 6 sessions with 10 minutes for each meeting from 29 September to 05 October 2022. Results: After being given nursing care with SSBM intervention, it was found that there was a decrease in the client's pain level in terms of decreased pain complaints, the average pain scale was from 4.7 NRS to 3.8 NRS, the average systolic blood pressure was 151.7 mmHg to 141 .7 mm Hg, and the average blood pressure was 96.7 mm Hg to 87.5 mm Hg. Conclusion: 6 sessions of SSBM therapy with a duration of 10 minutes can have a positive effect on acute pain problems. Health workers are expected to use SSBM therapy in combination with other non-pharmacological therapies to reduce scale pain.

The Value of Pain Sensitivity Questionnaire in Predicting Postoperative Pain in Living Kidney Donors: A Prospective Observational Study.

This study aimed to investigate the value of the Pain Sensitivity Questionnaire (PSQ) for the prediction of postoperative pain and the relationship between pain sensitivity and postoperative pain in kidney donors undergoing living-related kidney transplantation. A total of 148 kidney donors were selected and the preoperative pain sensitivity questionnaire was administered the day before surgery. Kidney donors were assigned to low PSQ group (PSQ < 6.5, n = 76) or high PSQ group (PSQ ≥ 6.5, n = 72). The primary endpoint was the number of patient-controlled analgesia (PCA). Other outcomes included: the incidence of acute pain, flurbiprofen axetil remediation rate, the incidence of chronic pain, neuropathic pain assessment scale (Douleur Neuropathique 4 Questions, DN4), visual analog scale (VAS) at rest after surgery as well as the correlation between PSQ and QST (Quantitative Sensory Testing). The low PSQ group had a significantly lower number of PCA than high PSQ group (P < 0.0001). The incidence of acute pain was 75% in low PSQ group and 100% in high PSQ group (P < 0.0001). Furthermore, flurbiprofen axetil remediation rate was lower in low PSQ group than that in high PSQ group (P = 0.042). The incidence of chronic pain was significantly lower in low PSQ group than in high PSQ group (6.6% vs 61.1%, P < 0.001). Moreover, DN4 was significantly lower in low PSQ group than that in high PSQ group (P < 0.001). The PSQ-mean was significantly negatively correlated with QST in kidney donors. VAS at rest for the low PSQ group were lower than those of the high PSQ group. The PSQ was found to be associated with the intensity or postoperative pain and might be used to screen patients prior to living-kidney transplantation.

Transabdominal preperitoneal versus hybrid procedures for treating irreducible inguinal hernias: A retrospective controlled study

BackgroundInguinal hernia is a common condition; however, irreducible inguinal hernias are rare. In this retrospective study, two laparoscopic procedures for irreducible inguinal hernia were compared. Materials and methodsThe study cohort comprised 88 patients who had undergone laparoscopic repair of primary irreducible inguinal hernias between 1 June 2011 and 31 December 2019. The patients were retrospectively divided into Hybrid (Group H) and Standard Transabdominal Preperitoneal (TAPP) Groups (Group S). Patient characteristics, hernia details, and intraoperative and postoperative complications were compared between study groups. ResultsThere were no significant differences between the two groups in baseline characteristics, including age, sex, body mass index, hernia type, operation time, hospital stay, cost, and duration of follow-up. No recurrence or surgical site infection occurred in either group. There were no significant differences between the two groups in incidence of spermatic vessel injury (0% vs. 2.04%, P = 0.370), vas deferens injury (0% vs. 6.12%, P = 0.116), epigastric vessels injury (0% vs. 4.08%, P = 0.202), scrotal hematoma (7.69% vs. 2.04%, P = 0.206), dysuria (5.13% vs. 8.16%, P = 0.575), fever (17.95% vs. 16.32%, P = 0.841), seroma (25.64% vs. 32.65%, P = 0.474), chronic pain (0% vs. 2.04%, P = 0.370), sensation of a foreign body (2.56% vs. 2.04%, P = 0.870), or pain on ejaculation (0% vs. 2.04%, P = 0.370). The incidence of acute pain was higher in Group H than in Group S (43.59% vs. 8.16%, P = 0.000). ConclusionThe hybrid TAPP procedure is a safe and feasible means of treating irreducible inguinal hernias. Though it is associated with a higher incidence of postoperative acute pain than the standard TAPP procedure, it may have advantages in avoiding injuries to the vas deferens and spermatic vessels.

A randomised controlled study on the effects of hernial sac stump fenestration on ultrasound seroma prevention in laparoscopic Type III inguinal hernia repair.

Background:The incidence of ultrasound seromas has significantly increased after large hernial sac surgery. Several methods are available for preventing ultrasound seromas, but the clinical results are poor. It has also been demonstrated that hernial sac stump fenestration during laparoscopic incisional hernia repair surgery can significantly decrease the incidence of ultrasound seromas.Materials and Methods:Ninety patients aged 18–75 years who were treated in our hospital for primary Type III indirect inguinal hernia from March 2017 to March 2018 were randomised to a preventive fenestration group and a control group. All patients underwent transabdominal preperitoneal repair. The number of ultrasound seromas in the inguinal regions and ultrasound seroma volume on day 6 and months 1 and 3 after surgery in the two groups were compared. The secondary outcomes included length of surgery, urinary retention, acute pain, chronic pain, length of hospitalisation, recurrence rate and other complications.Results:There were no significant differences in demographic characteristics. Ultrasound seroma incidence and ultrasound seroma volume on day 6 and months 1 and 3 after surgery were significantly lower in the preventive fenestration group than that in the control group. There were no significant differences in the length of hospitalisation or incidence of acute pain or urinary retention between the two groups.Conclusions:Hernial sac stump fenestration after hernial sac transection in inguinal hernia repair surgery is a simple method that can effectively reduce post-operative ultrasound seromas.

In Vivo Evaluation of Painful Symptomatology after Endodontic Treatment Performed Using Two Different Irrigation Needle Insertion Depths

Objective The main purpose of this study was to evaluate pain symptoms in patients after endodontic treatment performed in a single session in teeth with vitality or pulp necrosis, comparing two depths of insertion of the NaviTip irrigation needle. Materials and Methods One-hundred upper uniradicular teeth were selected and divided into four groups ( n = 25), namely Bio group 1 (live pulp/1 mm from the foramen), Bio group 3 (live pulp/3 mm from the foramen), Necro group 1 (pulp necrosis/1 mm from the foramen), and Necro group 3 (pulp necrosis/3 mm from the foramen). All canals were instrumented with Wave One Gold System. Irrigation was performed using 2.5% NaOCl. The teeth were filled using the single-cone technique with AH Plus sealer using a McSpadden compactor. After treatment, patients answered a questionnaire with a visual analog scale scored from 0 to 10 at 1, 3, and 7 days after treatment. Statistical Analysis Data were analyzed using Mann–Whitney U, Wilcoxon, and Friedman tests. Results There was a decrease in average pain levels at the three time points for both vital and necrotic teeth ( p < 0.05). There were no significant differences in postoperative pain levels comparing needle depth, or vitality and pulp necrosis ( p > 0.05). The percentage of mild pain increased over time and moderate pain decreased, regardless of pulp condition. There was no incidence of acute pain at any time. Conclusions Post-treatment endodontic pain levels in upper uniradicular teeth with or without pulp vitality resulted in similar pain scores, regardless of the depth of insertion of the irrigation needle in relation to the apical foramen.

A randomized study on laparoscopic total extraperitoneal inguinal hernia repair with hernia sac transection vs complete sac reduction.

The management of the hernia sac is the core step of laparoscopic indirect inguinal hernioplasty. In this study, a randomized controlled trial was conducted to evaluate the clinical effect of laparoscopic total extraperitoneal inguinal herniorrhaphy (TEP) with hernia sac transection and complete sac reduction. From May 2017 to May 2018, 70 patients aged 18-75 with primary indirect inguinal hernia in our hospital were randomly divided into a transected sac group (TS) and a completely reduced sac group (RS). The primary outcomes compared between the two groups were the hernia sac treatment time, operation time, and the incidence of seroma formation after operation. Secondary outcomes included acute pain, chronic pain (pain which lasted for at least 3months), hospital stay, recurrence rate, and other complications. There were no significant differences in baseline demographic characteristics including age, sex, hernia type, size of hernia defect, and follow-up time between the two groups. The times required for hernia sac separation and operation in the TS group were significantly shorter than in the control group (6.1 ± 2.3 vs. 25 ± 5.8min for hernia sac separation, p < 0.01; and 35.10 ± 3.50 vs. 54.40 ± 4.20min for operation, p < 0.01). There was no significant difference in hospitalization time, incidence of acute pain, seroma formation, or urinary retention between the two groups. During the 12-month follow-up period, chronic pain, early recurrence, and other complications were not found in either group. Both the hernia sac transection technique and the complete sac reduction technique are safe and reliable. The hernia sac transection technique is a simple and effective hernia sac treatment method. It reduces the operation time and does not increase seroma or acute or chronic pain. It is valuable and likely to be popularly applied.

Is birthing pain the trigger of postpartum depression? A commentary on “Childbirth pain and postpartum depression”

In 2007 the International Association for the Study of Pain (IASP) launched the campaign “Real Women, Real Pain”; 2007e2008 was declared as the “Global Year Against Pain” inWomen. By then it was recognized that 90% of women experience severe/unbearable labor pain, and that one very real consequence to labor pain was psychological: “Long-term emotional stress with potential adverse consequences on maternal mental health and family relationships” [1]. One year later Eisenach and his colleagues published their seminal data on the association between acute pain and postpartum depression [2]. Eisenach conducted a multicentre study across North America andWestern Europe, recruiting 1288 women after vaginal and cesarean delivery (CD). The reported combined incidence of acute pain 36 h after deliverywas 10.6%, and persistent pain at 8 weeks was 9.8%. Multivariable regression revealed a threefold increased risk of postpartum depression (PPD) in the presence of acute pain. Rates of persistent pain after cesarean delivery (PPCD) are subject to debate [3]. At the lower end, Eisenach et al. reports PPCD as 1.8% at 6months decreasing to 0.3% at 12months [4]. At the other end of the spectrum, 20% of PPCD is reported 2 years after CD for breech lie [5]. Other studies report rates between 6 and 18% beyond 6 months postpartum [6e8]. Multicentric observational study of pain after the use of a self-gripping lightweight mesh

Investigation in the field of inguinal hernia surgery is now focused on postoperative pain. The extended use of lightweight meshes and alternative methods of fixation may play a relevant role in the reduction of pain. In this study, a new self-gripping lightweight polypropylene mesh is tested. A multicentric, observational study was scheduled to prospectively evaluate this new mesh. Ten centers agreed to participate. Only primary, type 1 or 2 uncomplicated hernias in adults were included. The mesh was placed as a Lichtenstein procedure without any fixation. A complete pain questionnaire was followed at 1week, and at 1, 3 and 6months postoperatively. The principal goal of the study was to evaluate maximum pain score at 6months. Pain was assessed by a visual analog scale. A total of 256 patients were operated. Mean operative time was 35.6min; 76.2% of patients were operated in an ambulatory setting. There were a few postoperative complications: 2 wound infections, 17 seromas, 21 hematomas, 6 orchitis. The incidence of acute pain was 27.3% at week 1 and 7.5% at month 1. The incidence of chronic pain was 3.6% at month 3 and 2.8% at month 6. No recurrences or long-term complications were observed. This self-gripping mesh can be used safely in type 1 and 2 primary, uncomplicated inguinal hernia with minimal morbidity and most patients under ambulatory setting. The registered incidence of chronic pain is lower than 3%.

Gender is a confounding factor in pain trials: women report more pain than men after arthroscopic surgery

A gender difference in the incidence of acute pain may be a confounder in analgesic trials. We have tested the hypothesis that the incidence of acute pain after knee arthroscopic procedures is greater in women than men. We performed three RCTs on intra-articular analgesics in which no postoperative analgesia was given until the need for such treatment was documented by scoring moderate-to-severe pain on a verbal rating scale (VRS 0–4; n=219), and a 0–100 mm visual analogue pain scale (VAS) within 2 h postoperatively. All trials were performed with an intra-articular catheter technique. The design allowed us to study the natural course of pain after arthroscopic surgery until analgesia was required. Women reported more pain of at least moderate intensity than men (84 vs 57%; P<0.0001), indicating that being female is a risk factor for early postoperative pain (RR 1.47, 95% confidence interval from 1.23 to 1.74). The VAS score corresponding to moderate and severe pain is similar in men and women. Only short acting anaesthetics were given in order to minimise carry-over effects. Since previous trials on arthroscopic analgesics neither measured baseline pain nor stratified for gender, a difference between treatment groups could result from an uneven distribution regarding these factors. Our findings have major implications for the interpretation of previously published trials on intra-articular analgesia.
