Objective: To study of cardioprotective efficacy of indapamid in account with C344T polymorphism of CYP11B2 gene in Uzbek hypertensive patients. Design and method: The study included 34 male and female Uzbek patients with stage I-II AH (ESH/ESC 2013) with an average age of 47.24 ± 9.5 years. Mean duration of arterial hypertension was 5.44 ± 4.1 years. M- and B-mode echocardiographic examination was performed. PCR was performed. Indapamid prescribed as monotherapy for 12 weeks. Results were expressed as mean ± SD. Results: After genotyping patients were divided in to two groups: T allele carriers – 39 (57.4%) and C allele carriers – 39(57.4%). Carriers of T- and C-allele showed a significant decrease of LVMI: before treatment - 126.22 ± 26.8 g/m2, after treatment - 109.38 ± 24.72 g/m2 (p < 0.02) in T allele carriers and before treatment - 126.37 ± 30.13 g/m2, after treatment - 110.27 ± 27.61 g/m2 (p < 0.05) in C allele carriers. There were significant decrease simultaneously of interventricular septal thickness (IVST) and posterior wall thickness (PWT) only in T allele carriers. Thus, significant decrease of IVST was: before treatment - 1.04 ± 0.13 sm, after treatment - 0.97 ± 0.12 sm (p < 0.02) and significant decrease of PWT was: before treatment - 0.89 ± 0.16 sm, after treatment - 0.83 ± 0.15 sm (p < 0.001) in T allele carriers. But, significant decrease only IVST was noted in C-allele carriers: before treatment - 1.04 ± 0.13 sm, after treatment - 0.95 ± 0.12 sm (p < 0.001). Whereas, PWT was also decreased in C-allele carriers: 0.88 ± 0.19 sm, after treatment - 0.82 ± 0.17 sm, but not reliably (p > 0.05). The significant improvement of diastolic function of left ventricular was revealed in T allele carriers. Thus, positive dynamic of phase of izometric filling was noted in T allele carriers: before treatment - 0.096 ± 0.02 sec, after treatment - 0.088 ± 0.02 sec (p < 0.02) compared in C-allele carriers: 0.094 ± 0.02 sec, after treatment - 0.083 ± 0.01 sec, but not reliably (p > 0.05). Conclusions: Thus, the analysis in groups with different allele carriage was revealed some advantages in cardioprotective indapamid therapy in patients - T allele carriers compared with C allele carriers of C344T polymorphism of CYP11B2 gene in Uzbek hypertensive patients.
